Analysis of the Influencing Factors of Hospitalization Costs for Hysteroscopic Surgery under the Diagnosis Related Group Payment Model
[Journal Article]CHEN Guang, WANG Zhidan, ZENG Yuanling-Shenzhen Journal of Integrated Traditional Chinese and Western Medicine2025, No.22

Abstract:Objective To analyze the key factors influencing the total hospitalization costs of hysteroscopic surgery under the diagnosis related groups(DRG)payment model, and to provide a basis for optimizing cost control and promoting the rational allocation of resources. Methods A retrospective collection of data from 3903 patients who underwent hysteroscopic surgery at a hospital in Guangzhou from June 2021 to June 2025 was conducted. The total hospitalization cost was taken as the dependent variable, and age, postoperative diagnosis, and surgical methods were taken as independent variables for univariate analysis and multiple linear regression analysis. Results The average age of the 3903 patients was(41.8±10.4)years.Univariate analysis showed that there were statistically significant differences in costs among different ages, different diseases, and different surgical methods(P<0.001).Multiple linear regression(adjusted R²=0.542)indicated that the surgical method was the strongest predictor of cost, with the cost of hysteroscopic endometrial/myoma resection increasing by 153.2%compared to the reference group.Compared with patients under 45 years old, those aged 45 to 60 years had a significant 6.9%reduction in costs.Specific postoperative diagnoses(such as ectopic pregnancy and intrauterine adhesions)were also independent influencing factors for costs(P<0.05).Conclusion Under the DRG payment background, the total hospitalization cost of hysteroscopic surgery is mainly driven by the complexity of the surgical method, and the age of the patient and postoperative diagnosis also have significant influences. It is suggested that in the DRG grouping and weight setting, the complexity of the surgery and patient characteristics should be considered more precisely to establish a fairer and more reasonable compensation mechanism.

Construction of a Risk Prediction Model for Early Miscarriage after IVF-ET in Infertile Patients with Ovarian Endometriomas

Abstract:Objective To analyze the influencing factors of early miscarriage after in vitro fertilization-embryo transfer(IVF-ET)in infertile patients with ovarian endometriomas(OEM)and to construct a risk prediction model. Methods The clinical data of 142 infertile patients with OEM who were treated at Luoyang Central Hospital from January 2022 to February 2025 were retrospectively analyzed. All patients successfully conceived after IVF-ET and were divided into an occurrence group(20 cases)and a non-occurrence group(122 cases)based on whether early miscarriage occurred. The clinical data of the two groups were compared to analyze the influencing factors of early miscarriage after IVF-ET.On this basis,a risk prediction model was constructed and its predictive efficacy was evaluated. Results There were statistically significant differences in pre-assisted reproductive technology body mass index(BMI),proportion of operations different uterine cavity histories, basal estradiol level, and β-human chorionic gonadotropin(β-HCG)level 14 days after embryo transfer between the two groups(P<0.05).Multivariate Logistic regression analysis showed that pre-assisted reproductive technology BMI<18.5 kg·m-2,pre-assisted reproductive technology BMI>24.0 kg·m-2,history of uterine cavity operation>1 time, and decreased β-HCG level 14 days after embryo transfer were risk factors for early miscarriage after IVF-ET in infertile patients with OEM(P<0.05).Based on the above indicators,a risk prediction model for early miscarriage after IVF-ET in infertile patients with OEM was constructed. The calibration curve indicated that the model had good discrimination, and the area under the curve(AUC)was 0.918,95%CI:0.869-0.966.Conclusion Pre-assisted reproductive technology BMI<18.5 kg·m-2,pre-assisted reproductive technology BMI>24.0 kg·m-2,history of uterine cavity operation>1 time, and decreased β-HCG level 14 days after embryo transfer are risk factors for early miscarriage after IVF-ET in infertile patients with OEM.The nomogram model constructed based on these factors has high value in predicting early miscarriage after IVF-ET in infertile patients with OEM.

Mechanism of Curcumin Inhibits the Proliferation and Metastasis of MCF-7 Cells through the PI3K/AKT/mTOR Pathway

Abstract:Objective To observe the effects of curcumin on the proliferation, migration and metastasis of human breast cancer MCF-7 cells and explore the mechanism of the PI3K/AKT/mTOR signaling pathway. Methods The proliferation activity of human breast cancer MCF-7 cells was detected by cell counting kit-8(CCK-8)assay after intervention with different concentrations of curcumin. The invasion ability of MCF-7 cells was evaluated by Transwell assay. The phosphorylation status of p-AKT and p-mTOR in the signaling pathway of human breast cancer MCF-7 cells was analyzed by Western blotting. Results Compared with the blank control group, the proliferation and invasion inhibition ability of human breast cancer MCF-7 cells in different concentrations of curcumin groups were significantly reduced, and the differences were statistically significant(P<0.05).Compared with the blank control group, curcumin could up-regulate the expression of E-cadherin in human breast cancer MCF-7 cells,down-regulate the expression of N-cadherin and Vimentin and inhibit the phosphorylation levels of p-AKT and p-mTOR(P<0.05),and this effect showed a significant dose-time dependent relationship. Conclusion Curcumin can inhibit the growth of human breast cancer MCF-7 cells, weaken the invasion ability and increase the apoptosis rate of cells, and the mechanism may be related to the inhibition of the PI3K/AKT/mTOR signaling pathway by curcumin.

Abstract:Objective Given the alarming increase in the detection rate of suspected scoliosis among teenagers in China with grade, this study aims to evaluate the satisfaction and actual demand for spinal health science popularization education in schools. Methods From October 2024 to June 2025,a cross-sectional survey was conducted among students from 10 primary and secondary schools who participated in the spinal health science popularization activities organized by the spinal health center of Shenzhen Second People's Hospital.A total of 600 questionnaires were distributed, and 582 valid questionnaires were retrieved(response rate:97.00%),and the data were analyzed from the aspects of basic information, satisfaction and acceptance of science popularization lectures, preferences for forms and content demands, behavioral intentions, and multiple logistic regression. Results The study found that the overall acceptance and satisfaction with spinal health science popularization education were high(92.10%"very welcome",88.32%"very understandable").Regarding the preference for lecture forms, there were significant statistical differences among students of different grades(P<0.001).83.19%of primary school students preferred more intuitive and interesting forms such as"interactive games"and"video teaching",while 76.54%of middle and high school students preferred more interactive and analytical forms such as"group discussions"and"case analyses".89.00%of the students expressed that they were"very willing"to share the knowledge they learned about spinal health with their families or friends,Among them, the proportion of primary school students was 92.33%,significantly higher than that of junior and senior high school students, which was 84.36%(P<0.01).86.77%of the students indicated that they paid more attention to their spinal health after participating in this science popularization activity. The results of multiple logistic regression analysis showed that the understandability of the content, the interactivity of the form, and the grade level were independent influencing factors for students'willingness to share knowledge(P<0.05).Conclusion This study confirmed that the grade-based spinal health science popularization strategy is highly feasible and effective.Future science popularization efforts should continue to promote differentiated education based on student needs, enhance the interactivity and practicality of the content, and build a more precise and efficient spinal health education system.

Analysis of Medication Patterns for Treating Thyroid Nodules Based on Real-World Data

Abstract:Objective To explore the medication patterns of Professor Leng Jinhong in treating thyroid nodules(TN)based on real-world deta. Methods The medical records of patients treated by Professor Leng Jinhong at the expert outpatient department of Liaoning University of Traditional Chinese Medicine from January 2022 to October 2024 were used.According to the inclusion and exclusion criteria,306 cases were included.Excel,IBM SPSS 26.0,and SPSS Modeler 18.0 were used for statistical analysis. Results A total of 121 traditional Chinese medicines were involved. The top six frequently used drugs were Poria cocos,Bupleurum,Oyster shell,Honey-fried Glycyrrhiza,Atractylodes, and Peony bark. From the perspective of drug properties and flarors,cold-natured drugs were used most frequently among the four natures, and sweet-tasting drugs were used most frequently among the five flavors.Cluster analysis identified five groups of drug combinations, and the association rule results showed that Poria cocos-Bupleurum was the most frequent drug pair and had the highest confidence level, indicating a significant synergistic effect in the treatment of TN.Conclusion The medication patterns of Professor Leng in treating TN can be summarized as being guided by the"One Qi Circulation"theory of Huang Yuanyu, with the treatment approach focusing on"regulating the liver as the main point and simultaneously regulating the liver, spleen, and kidney."This study further substantiates the scientific connotation of the"Qi and Water Co-treatment"Poria cocos-Bupleurum drug pair.

Experimental Study on the Regulation of Hypoxia/Reoxygenation-Induced Angiogenesis in Human Brain Microvascular Endothelial Cells by LncRNA-MEG3 via the miRNA-141-3p/Notch2 Axis
[Journal Article]LIU Guangsheng, JIANG Yehan, ZHONG Juanli et al.-Shenzhen Journal of Integrated Traditional Chinese and Western Medicine2025, No.21

Abstract:Objective An experimental study was conducted on the regulation of hypoxia/reoxygenation-induced angiogenesis in human brain microvascular endothelial cells by long non-coding RNA(LncRNA)-maternally expressed gene 3(MEG3)through the microRNA-141-3p/Notch homolog 2(Notch2)axis,to explore whether MEG3 affects the angiogenic capacity of hypoxia/reoxygenation human brain microvascular endothelial cells through the miRNA-141-3p/Notch2 axis. Methods An oxygen-glucose deprivation/reoxygenation(OGD/R)model was established to simulate the in vitro environment of ischemic stroke.Cell viability was detected by the Cell Counting Kit-8(CCK-8)assay,apoptosis was analyzed by flow cytometry, and the expression levels of genes and proteins were detected by real-time fluorescence quantitative polymerase chain reaction(RT-qPCR)and Western blot, respectively.Cell migration and angiogenic ability were evaluated by Transwell and tube formation assays, respectively. The interaction between MEG3,poly-pyrimidine tract-binding protein 1(PTBP1),and Notch2 was analyzed by techniques such as RNA immunoprecipitation(RIP),RNA pull-down,and fluorescence in situ hybridization(FISH).In addition,a mouse middle cerebral artery occlusion(MCAO)model was established for in vivo validation. Results OGD/R treatment significantly upregulated the expression level of MEG3 in HBMECs,accompanied by decreased cell viability, increased apoptosis, and weakened migration and angiogenic ability.Knockdown of MEG3 reversed the above OGD/R-induced cell damage.Mechanistically,MEG3 was mainly located in the cytoplasm and directly bound to RNA-binding protein PTBP1,further regulating the stability of Notch2 messenger RNA(mRNA).Under OGD/R conditions,MEG3 enhanced Notch2 expression through PTBP1,thereby inhibiting the migration and angiogenesis of HBMECs.In vivo experiments further confirmed that silencing MEG3 significantly reduced the infarct area, improved neurological function scores, and reduced apoptosis. Conclusion MEG3 enhances the mRNA stability of Notch2 by binding to PTBP1,thereby inhibiting the migration and angiogenesis of HBMECs and promoting the development of ischemic stroke.Targeting the MEG3/PTBP1/Notch2 axis may provide a new potential target for the treatment of ischemic stroke.
