Construction of A Discharge Readiness Assessment Index System for Stroke Patients under A Multidisciplinary Collaboration Model

Abstract:Objective To construct a personalized discharge readiness assessment system for stroke patients based on the Omaha System,in order to implement targeted peri-discharge management and improve the quality of life of patients.Methods Through systematic literature review,a draft of the"Discharge Readiness Assessment System for Stroke Patients"covering basic framework and personalized indicators was formed.Fifteen experts from the fields of neurology and neurosurgery nursing,neurology medical care,health management,neurorehabilitation,and nursing education were selected to conduct two rounds of Delphi consultation using the Delphi method.Expert opinions were analyzed and discussed,and the draft was modified to determine the final personalized assessment system.Results The expert authority scores in the two rounds of consultation were 0.94 and 0.96,respectively,with a response rate of 100%.The Kendall's coefficient of concordance was 0.24 and 0.21(P<0.05),respectively.The final"Discharge Readiness Assessment System for Stroke Patients"includes 4 first-level indicators(physiological assessment,psychosocial assessment,health-related behavior assessment,and environmental assessment),11 second-level indicators,and 37 third-level indicators.The importance scores of each indicator range from 4.0 to 5.0,and the coefficient of variation ranges from 0.00 to 0.23.Conclusion The"Discharge Readiness Assessment System for Stroke Patients"constructed based on the Omaha System can accurately screen the personalized needs of stroke patients from a physiological-psychological-social perspective,providing a basis for targeted peri-discharge management of stroke patients.
