Retrospective Study on Intraoperative Autologous Blood Transfusion in Aortic Dissection Surgery

Abstract:Objective To retrospectively study and analyze the application effect of intraoperative autologous blood transfusion in elective aortic dissection surgery.Methods The general data and preoperative and postoperative clinical laboratory indicators of 104 patients who underwent elective aortic dissection surgery during hospitalization at The First Affiliated Hospital of Naval Medical University,Chinese People's Liberation Army from January 2018 to December 2022 were retrospectively analyzed.The effect of intraoperative autologous blood transfusion on reducing the amount of allogeneic blood transfusion during surgery was observed,and the correlation between the amount of allogeneic blood transfusion and the amount of autologous blood recovery was analyzed by multiple linear regression.Results The average intraoperative blood loss of the patients was(1910.45±976.56)mL,the amount of autologous blood transfusion was(790.01±526.03)mL,the amount of allogeneic blood used was(680.77±659.81)mL,and the cardiopulmonary bypass time was(147.49±44.31)min.The hemoglobin(Hb)level of the postoperative patients was lower than that before the operation,the hematocrit(Hct)was smaller than that before the operation,the platelet(PLT)count was less than that before the operation,and the activated partial thromboplastin time(APTT)and prothrombin time(PT)were longer than those before the operation.The differences were all statistically significant(P<0.05).Multiple linear regression analysis showed that the amount of allogeneic blood transfusion during surgery was positively correlated with intraoperative blood loss(β=0.810,P<0.001)and negatively correlated with the amount of autologous blood recovery(β=-0.460,P<0.001).Conclusion Intraoperative autologous blood transfusion is an important protective factor for reducing the amount of allogeneic blood transfusion in elective aortic dissection surgery,which is of great significance for saving blood resources and blood protection.
