Practical experience on internet-based ROPES model combining diversified teaching methods in the standardized residency training of ultrasound medicine

Abstract:Objective The ROPES(review,overview,presentation,exercise,summary)model was integrated with diversified teaching methods to explore a new teaching mode of clinical thinking course for standardized residency training in the ultrasound medicine department and to improve the quality of training.Methods Taking the clinical thinking teaching course in the standardized training of ultrasound residents as an example,this study expounded the practice process of this new teaching model in the training of clinical thinking ability of residents.The teaching effect was evaluated through classroom performance,stage assessment,year-end assessment and questionnaire survey.Results The total score of resident physicians'assessment is quite high,at(85.60±6.52)points.Residents were particularly satisfied with the improvements in ultrasound theoretical level,learning interest,self-learning ability,clinical thinking ability,teamwork ability,communication and expression ability,and relationship deepening between teachers and students.Conclusion The ROPES model based on internet combined with diversified teaching methods can stimulate the learning enthusiasm of residents,improve clinical thinking ability,and improve the teaching ability of the teachers,which is worthy promoting.

Analysis on the effect of graduation theoretical simulation examination and targeted examination guidance for stomatology residents in a general hospital

Abstract:Objective To analyze the results of simulation examination and the passing rate of the graduation theoretical examination in 2022-2024,so as To evaluate the effect of targeted examination guidance,and provide evidence for improving the training program and the training level.Methods We analyzed the general information,the results of simulation examination and graduation theoretical examination of 82 residents who participated in the graduation theoretical examination in 2022-2024 by retrospective analysis.Furthermore,we analyzed the objective factors affecting the results of the first simulation examination,the differences between the results of three simulation examinations,and the trend of the passing rates each year.Results There were no significant correlations of the passing rates of the first simulation examination in 2022-2024 with the age,gender,personnel type,fresh/former graduate,highest education,or whether passing the graduation theoretical examination(P>0.05).There were significant differences between the scores of the three simulation examinations in each year(P<0.001),which showed a significant upward trend.The passing rates of both the annual simulation examination and the graduation theoretical examination exhibited upward trends,and the passing rates of the graduation examination were stable at more than 95%in the past three years.Conclusion Stratifying residents into groups according to the results of the first simulation examination and providing targeted guidance can effectively improve the theoretical examination results of residents,which powerfully ensures the passing rate of the graduation examination.

Abstract:Doctor-patient communication is an indispensable part of medical practice,and its quality directly affects the therapeutic effect and patient satisfaction.The Balint group,as a psychotherapy and educational tool,has been widely applied in medical education in recent years,aiming to enhance the communication skills of resident physicians.This article reviews and analyzes the existing literature to understand the theoretical basis of the Balint group and its current applications in medical education,to explore its specific implementation and mechanisms in cultivating doctor-patient communication skills,to analyze the challenges faced in the current medical education training model,and to forecast future development trends,hoping to provide references for further optimizing the standardized training of resident physicians.

Status investigation on doctor-patient communication and analysis on training demands among internal medicine residents from the perspective of medical humanities education

Abstract:Objective By investigating the current status of doctor-patient communication among internal medicine residents during clinical training,this study aimed to clarify residents'current performance and training needs,and to provide reference for optimizing doctor-patient communication training programs and medical humanities curriculum design for internal medicine residents.Methods A cluster sampling method was used to distribute questionnaires to 304 internal medicine residents at West China Hospital of Sichuan University from 2022 to 2024.The survey included demographic information,awareness of the importance of doctor-patient communication,current communication practices,and training needs.Data were analyzed using STATA 18,with descriptive statistical analysis for demographic data,t-tests and analysis of variance for comparing communication skills across different demographic groups,correlation analysis for examining relationships between influencing factors and communication skills,and multiple linear regression for identifying factors affecting communication practices and training needs.Results 98.69%of residents had received medical humanities education and had patient communication experience,but scored relatively low in awareness of communication importance,practical performance,and training needs.Residents without relevant training courses or communication experience demonstrated higher practical abilities and stronger training needs.Higher awareness of communication importance was associated with better communication practices and greater demand for training.Conclusion Enhancing clinical doctor-patient communication competence is a key component of standardized residency training.Further development of doctor-patient communication training courses based on shared decision-making is warranted.

Effectiveness and challenges of robot assisted and simulated medical education in the teaching of vertebral augmentation surgery

Abstract:Objective To explore the application of the combination of robot assisted surgery technology and simulated medical education in the practical teaching of percutaneous vertebroplasty teaching and whether it could help resident physicians master the technique and improve their learning interest more quickly and effectively.Methods From April 2020 to April 2024,according to the different years of standardized residency training,they were divided into two groups with 60 in each.The observation group used robot assisted technology combined with simulated medical teaching,and operated in practice.The control group used medical simulators to practice traditional teaching.Both groups underwent three rounds of practice exercises.After each round of operation,medical simulators underwent computed tomography scans to evaluate the puncture channel and bone cement leakage.Results After three rounds of training,all resident physicians fully mastered the percutaneous kyphoplasty surgical procedure.In the observation group,55 mastered the operation in the first round,4 in the second round,and 1 in the third round.In the control group,45 mastered the operation in the first round,9 in the second round,and 6 in the third round.The difference in learning outcomes between the two groups was statistically significant(P=0.041).There were statistically significant differences(P<0.05)in the scores of teaching satisfaction and interest between the two groups.Conclusion The combination of robot assisted surgery technology and simulated medical education in orthopedic standardized residency training can shorten the learning cycle,and improve their interest and learning acceptance,thus enables them to quickly master the technical points of percutaneous kyphoplasty.

Abstract:With the acceleration of the specialization process of stomatology,technical iteration is faster and patient demand is rising,thus higher requirements are put forward for the fine ability of young doctors,such as micromanipulation and multidisciplinary cooperation.But the current education system is difficult to match the refined training needs of specialized talents.In face of the structural conflict between homogeneous training and specialty differentiation,and the lack of adaptability between static assessment and dynamic development of clinical ability,the three-dimensional training system of"competency-oriented+hierarchical progressive+dynamic evaluation"is proposed.The curriculum module is reconstructed through post competency model,the differentiated curriculum group is constructed based on the oral sub-college,and the digital document system recording young doctors'ability growth is introduced.This system breaks through the traditional"pan-specialization"training paradigm and has dual values in promoting the growth of young doctors and promoting the iteration of medical education after graduation.

Application of BOPPPS combined with CBL integrated with curriculum ideology and politics in standardized residency training

Abstract:Objective This study explored the teaching effectiveness of integrating curriculum ideology and politics into the combination of case-based learning(CBL)and the lesson plan model of bridge-in,objective,pre-assessment,participatory learning,post-assessment,summary(BOPPPS)in the standardized residency training.Methods A total of 40 residents who underwent standardized training in the department of thoracic surgery at the Central Hospital Affiliated to Shandong First Medical University from January 2019 to September 2024 were retrospectively selected as the research subjects and divided into two groups with 20 residents in each group.The observation group adopted the teaching and training mode of BOPPPS combined with CBL integrated with curriculum ideology and politics,while the control group adopted the traditional teaching mode.After class,the theoretical examination scores of the two groups and their evaluations of the effectiveness of their respective teaching modes were compared.Results The theoretical examination score of the observation group was(85.15±7.65)points,which was higher than(79.55±8.76)points of the control group,and the difference was statistically significant(P<0.05).In terms of teaching evaluation,the scores of the observation group in teacher-student interaction,cultivation of problem-analysis ability,and cultivation of innovative thinking expansion were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The application of BOPPPS combined with CBL integrated with curriculum ideology and politics can improve the theoretical knowledge level of residents and their satisfaction with teaching.

Application of the cooperative learning method in standardized residency training for neurology residents

Abstract:Objective To investigate the application of cooperative learning in the neurology teaching of standardized residency training.Methods This retrospective cohort study selected the second-year residents rotating in the Neurology Department of Zhengzhou Central Hospital from September 2022 to July 2024.The observation group consisted of 38 residents from August 2023 to July 2024,who were taught using a cooperative learning method.The control group was comprised of 35 residents from September 2022 to July 2023,who received traditional instruction.Teaching effectiveness was assessed using the mini-clinical evaluation exercise(Mini-CEX),the clinical skills examinations,and the theoretical examinations.The interpersonal ability assessment questionnaire was used to evaluate the residents'interpersonal skills.Results The observation group scored higher than the control group in both the theoretical examination(86.90±4.65 vs.79.61±7.65)and the clinical skills examination(87.10±3.82 vs.84.50±3.53),with statistically significant differences(P<0.01).The Mini-CEX scores and teaching satisfaction were also significantly higher in the observation group than in the control group(all P<0.05).All five dimensions of interpersonal ability(proactive communication,appropriate refusal,self-disclosure,conflict management,and emotional support)in the observation group showed significant improvement compared with their baseline levels upon entering the department(all P<0.01).Conclusion The cooperative learning method demonstrates significant effectiveness in the standardized residency training program of our Department of Neurology.It can not only improve the residents'clinical and theoretical performance but also enhance their interpersonal skills.

Application of an integrated curriculum based on a self-designed Wetlab simulation model in eyelid surgery training for resident physicians

Abstract:Objective To promote the teaching of eyelid surgery for resident physicians through a self-designed simulation model and an integrated curriculum.Methods An eyelid tumor disease model was created by subcutaneous injection of ointment into the Porcine orbit model's eyelid from March to April 2024,Zhongshan Ophthalmic Center,Sun Yat-sen University.Based on this,an integrated curriculum consisting of"online video pre-study,on-site guidance,video practice review,and actual case operation"was designed and implemented.A questionnaire survey was conducted to collect students'evaluation of the operation model,and their practice video recordings were assessed.Results Students highly rated the fidelity of the operation model.Nearly all participants believed that the model aided in clinical surgical training and recommended it for routine eyelid surgery training.Review of practice videos revealed that tissue separation and tumor excision were steps requiring focused practice by trainees.With increased practice,there were significant improvements in the standardization and proficiency of operations compared with baseline levels.In the early stages of practice,the duration of operations gradually decreased,while it stabilized in the later stages,indicating that students could complete surgeries more smoothly after familiarizing themselves with the procedural flow.Some students were able to independently perform chalazion surgery after the course.Conclusion The integrated curriculum based on a self-designed simulation model can lower the barrier to surgery learning,provide opportunities for standardized surgical teaching,enhancing trainees'surgical skills from multiple dimensions.

Exploration and practice of the EPAs evaluation system based on job competency for the stratified training model of geriatric medical talents

Abstract:Objective To explore the application effect of the EPAs evaluation system based on"job competency"in the stratified training model of geriatric medical talents.Methods Trainees of the Geriatric Medicine Talent Training Program at the First Affiliated Hospital of Zhejiang University School of Medicine in 2023 were selected as the observation group,and received stratified training under the EPAs evaluation system based on job competency.The trainees of the 2022 program served as the control group,using the traditional training program.Diversified evaluation methods were used to compare the theoretical scores,questionnaire results.Results The average scores of the observation group in theoretical and skill assessments were significantly higher than those of the control group(all P<0.05).Under the EPAs evaluation system,the confidence levels of the observation group significantly increased compared with those before the training,and their training satisfaction scores were also significantly higher than those of the control group(all P<0.05).In the evaluation of each competency dimension,the average scores of the observation group were all higher than those of the control group(all P<0.05).Conclusion The stratified training model under the EPAs evaluation system based on job competency can effectively enhance the theoretical knowledge and practical skills of geriatric medical talent trainees,improve training satisfaction,and promote the comprehensive enhancement of trainees'job competencies.

Practical exploration of professional orientation in standardized residency training for emergency department based on a digital intelligence platform

Abstract:Objective To explore the significance of the emergency department professional orientation in helping resident physicians quickly adapt to the environment and obtain good training quality.Methods This study explored the improvement of the connotation and quality of the professional orientation for our Emergency Department in 2023 to 2024 through the digital teaching management platform at Zhongshan Hospital,Fudan University.Results Through analyzing the evaluation feedback of the professional orientation over the past two years and understanding the training effect,it was found that after multiple reform measures,the evaluation of resident physicians on the professional orientation for Emergency Department was comprehensively improved.Conclusion It is suggested to further strengthen the construction of practical courses in training,cultivate doctor-patient communication skills,and explore diversified teaching methods.Continuous attention should be paid to the evaluation and feedback of training effectiveness,so as to lay a solid foundation for improving the standardized training quality of resident physicians and cultivating high-quality emergency department physicians.

Application of situational simulation teaching of anesthesia crisis management in standardized residency training

Abstract:Objective This study investigated the utilization and impact of anesthesia crisis management situational simulation teaching within the standardized training framework for resident physicians.Methods This study examined the use and effectiveness of crisis management situational simulation teaching in the standardized training of 58 resident physicians in the Department of Anesthesiology at the First Affiliated Hospital of Yangtze University.Participants were randomly allocated into two groups:a control group(n=29)that received traditional teaching methods,and a observation group(n=29)that underwent situational simulation teaching.The comparative analysis was centered on theoretical examination scores,clinical competence assessment scores,satisfaction levels with teaching quality and learning effectiveness,as well as satisfaction with the quality of the teaching curriculum.Results Following the teaching intervention,the observation group demonstrated significantly higher scores in both theoretical knowledge assessments and clinical competency assessments for eight anesthesia crisis events than the control group.The observation group also showed significant improvements in non-technical skills compared with the control group.The satisfaction rate regarding the quality of the teaching curriculum was significantly higher in the observation group(96.55%)than in the control group(51.72%).The above differences were statistically significant(all P<0.05).Conclusion The application of anesthesia crisis management situational simulation teaching in standardized residency training can significantly enhance the theoretical knowledge and clinical competency in anesthesia crisis management,as well as the satisfaction with teaching quality and learning outcomes.

Primary exploration of the teaching mode of treatment for surgical acute abdomen simulation based on virtual reality

Abstract:Objective This study aimed to explore the teaching effect of treatment for surgical acute abdomen simulation based on virtual reality.Methods Clinical data of acute abdomen from the Third Affiliated Hospital of Sun Yat-sen University were collected and prepared for lessons in team work.Teachers acted as standard patients and family members,and clinical scenes were simulated.The residents collected medical history,performed physical examination,analyzed auxiliary examination results,gave diagnosis and treatment plan,and completed the operation on the VR surgical trainer.The scores for VR coordination ability,organization protection,equipment operation,and procedure proficiency of the residents were recorded and analyzed.After training,a questionnaire was applied to residents for training feedback.Results After training,the scores of doctor-patient communication[(61.7±2.4)points vs.(70.7±0.9)points vs.(78.3±2.4)points,P=0.001)and VR operation[(48.3±6.1)points vs.(76.2±8.4)points,P=0.010]were significantly improved.The results of questionnaire survey showed that all the residents hoped to strengthen doctor-patient communication and VR operation training.Conclusion The teaching mode of treatment for surgical acute abdomen simulation based on VR can improve clinical ability and operation ability of surgical residents,making them attracted by the course.

Construction and effectiveness of simulation courses in anesthesia crisis resource management based on cognitive stratification of resident physicians

Abstract:Objective To explore the construction and effectiveness of simulation courses in anesthetic crisis resource management(ACRM)based on cognitive stratification of resident physicians.Methods A total of 64 resident physicians trained in the Anesthesiology Department of our hospital from January 2021 to January 2024 were selected.Among them,25 residents from January 2021 to July 2022 were assigned to the traditional group,and 39 residents from August 2022 to January 2024 were assigned to the simulation group.The traditional group received conventional standardized training,while the simulation group underwent hierarchical progressive ACRM case-based simulation teaching.The assessment scores,non-technical skills performance,and teaching satisfaction were compared between the two groups.Results The theoretical scores,operational scores,and total scores of the simulation group were significantly higher than those of the traditional group(all P<0.05).The simulation group had higher scores in all dimensions of non-technical skills performance than the traditional group(all P<0.05).Although the simulation group had higher teaching satisfaction scores in all aspects compared with the traditional group,only the satisfaction with clinical thinking showed a significant difference(P<0.05).Conclusion The hierarchical anesthesia crisis resource management simulation course constructed based on the cognitive level of resident physicians can effectively improve their theoretical and operational skill scores,and enhance their non-technical skills and teaching satisfaction.

Application of problem-based learning based bedside teaching mode in the standardized residency training of medical imaging physicians

Abstract:Objective To analyze the application effect of the PBL based bedside teaching mode in standardized residency training(referred to as residency training)in the radiology department.Methods A total of 132 residents from Department of Radiology,the First Affiliated Hospital of Zhengzhou University were selected and randomly divided into a control group(n=66)and an observation group(n=66)using a random number table.The observation group adopted the PBL based bedside teaching mode,while the control group received conventional teaching methods.The exit examination scores(theoretical foundation and practical skills)and teaching effects were compared between the two groups.Results The theoretical foundation and practical skills scores of the observation group were higher than those of the control group,and the improvement rates of various dimensions of teaching effectiveness in the observation group were also better than those in the control group,with statistical significance(all P<0.05).Conclusion Compared with conventional teaching mode,PBL based bedside teaching mode can significantly improve the theoretical foundation and practical skills of residents in radiology during their residency training.Moreover,the teaching effect are superior.

Application of ChatGPT-generated MCQs in department rotation exit exams for radiology residents

Abstract:Objective This study aimed to investigate the potential application of ChatGPT in assessing the department exit exam of radiology residents.Methods We employed the platform designed by OpenAI,which was trained through GPT Builder to generate case-based multiple-choice questions(MCQs),to output personalized case-based MCQs tailored to the different clinical specialties of resident physicians.A total of 33 resident physicians from the Department of Radiology and Internal Medicine at the First Affiliated Hospital of Shantou University Medical College participated in the exam.Feedback evaluations on the examination papers were collected through questionnaires completed by resident physicians and 10 teachers.Subjective and objective evaluations were conducted on the examination papers.Results The difficulty coefficient of the examination paper was rated as"easy"according to both subjective and objective assessments,meeting expected standards.Objective assessment revealed that the level of difficulty for the internal medicine group examination was 0.89,while the difficulty level for the radiology group examination was 0.87.However,the discrimination and the degree of confidence exhibited relatively low values,with a discrimination coefficient of 0.25 for the internal medicine group and 0.28 for the radiology group,as well as a reliability coefficient of 0.39 for the internal medicine group and 0.22 for the radiology group.Both resident physicians and teachers provided high multidimensional evaluation scores for the examination papers,primarily falling within"neutral"and"satisfied"levels.Conclusion Utilizing ChatGPT to generate personalized multiple-choice questions for the department exit exams of radiology residents is feasible and holds broad prospects as well as significant practical value.

Effectiveness of standardized residency training for medical masters and non-medical masters in military hospital bases

Abstract:Objective To explore the influencing factors for the effectiveness of standardized residency training in military hospital bases and compare medical masters and non-medical masters.Methods The questionnaire was compiled using grounded theory,and SPSS was used for statistical analysis.Results The dimensions contained in the background evaluation,management evaluation and process evaluation all had significant impact on the outcome evaluation,and for the ratings given by instructors,there was a significant difference only in the dimension of professional ability between the medical masters and non-medical masters(P<0.05).Conclusion It is necessary to strengthen the policy and resource inclination of the bases,to demonstrate the development of military residency management manual,and to explore the management and use policy of students graduated from military residency training bases.

Abstract:Objective This study aimed to assess the satisfaction of resident physicians undergoing standardized residency training at a tertiary hospital in Guangdong,to explore its influencing factors and to propose countermeasures.Methods 437 resident physicians were selected from 23 professional bases of a tertiary hospital in Guangdong,who were enrolled in the 2021-2023 cohorts,as the subjects of a questionnaire survey.The training satisfaction and its influencing factors were analyzed.Results The overall satisfaction score for the training was 3.88(3.46,4.12),with a satisfaction rate of 92.91%(406/437).The satisfaction scores for teaching faculty,training effectiveness,process management,basic conditions,and support measures were 4.00(4.00,4.80),4.00(3.50,4.00),4.00(3.33,4.00),4.00(3.25,4.00),and 3.60(3.00,4.00),respectively,with satisfaction rates of 97.25%,96.34%,93.82%,93.59%and 84.44%,respectively.The professional base,the highest educational background and the weekly working hours were the influencing factors of the satisfaction of the resident physicians(all P<0.05).Conclusion Resident physicians have a high level of overall satisfaction with the training bases,and generally acknowledge the current conditions.Further strengthening the faculty team for residency training and advancing reforms in support systems for residents would help improve their satisfaction.
