Construction and validation of risk prediction model for postoperative complications in elderly patients with hip fractures and diabetesAbstract:Objective To explore the influencing factors of postoperative complications in elderly patients with hip fractures with dia-betes,and to construct a risk prediction model. Methods A total of 360 elderly patients with hip fractures and diabetes admitted to a hospital from November 2019 to October 2022 were retrospectively selected as research subjects. They were divided into the modeling group(n=252)and the validation group(n=108)according to a ratio of 7∶3.Patients in the modeling group were divided into the normal group and the complication group based on whether complications occurred after the operation.Logistic regression analysis was used to analyze the influen-cing factors of postoperative complications in the patients of the modeling group and establish a risk prediction model. The receiver operating characteristic curve was used to evaluate the model.Additionally,108 elderly patients with hip fractures combined with diabetes in the valida-tion group were selected for external validation of the model. Results Among the 252 patients in the modeling group,97 cases had postopera-tive complications, with an incidence rate of 38.49%.Age,intraoperative blood loss, preoperative fasting blood glucose, albumin, and intraoper-ative blood glucose fluctuation were independent risk factors for postoperative complications in elderly patients with hip fractures combined with diabetes(P<0.05).The results showed that the area under the receiver operating characteristic curve(AUC)in the modeling group was 0.918,the sensitivity was 0.804,and the specificity was 0.877.The AUC in the validation group was 0.932,the sensitivity was 0.773,and the specificity was 0.937.The calibration curve showed that there was good consistency between the predicted probability and the actual probabili-ty of the nomogram model. Conclusion The incidence of postoperative complications in elderly patients with hip fractures complicated with diabetes needs to be reduced.Age,intraoperative blood loss, preoperative fasting blood glucose, albumin, and intraoperative blood glucose fluc-tuation are closely related to the occurrence of postoperative complications. The risk prediction model constructed based on the above influen-cing factors has good predictive efficacy and can provide theoretical references for clinical diagnosis and treatment.

Mediating effect of nutritional risk between oral frailty and frailty in elderly inpatientsAbstract:Objective To explore the mediating role of nutritional risk between oral frailty and frailty in elderly inpatients. Methods A survey was conducted among a total of 382 elderly patients admitted to a tertiary grade A hospital in Wuhan, using the general information questionnaire, the Oral Frailty Index-8,the Nutritional Risk Screening 2002 and the Frailty Screening Scale. Results The oral frailty score of elderly inpatients was 4.00(2.00,6.00),the nutritional risk score was 2.00(1.00,3.00),and the frailty score was 3.00(2.00,4.00).Frailty occurred in 62.57%of the patients.Oral frailty, nutritional risk, and frailty were positively correlated with each other(r1=0.337,r2=0.367,r3=0.332,P<0.001).Nutritional risk played a partial mediating role between oral frailty and frailty in elderly inpatients, accounting for 19.47%of the total effect. Conclusion The incidence of frailty in elderly inpatients is relatively high.Nutritional risk is a mediating variable between oral frailty and frailty in elderly inpatients, suggesting that nursing staff can weaken the negative impact of oral frailty on frailty by im-proving the nutritional status of elderly inpatients,so as to reduce or delay the occurrence of frailty.
Application of eye tracking technology in screening for cognitive impairment:an integrative reviewAbstract:Objective To understand the current research status of eye tracking for the assessment and screening of cognitive impair-ment,and to provide reference and guidance for the future development of eye tracking-based assessment or screening tools. Methods Ac-cording to the integrated review framework proposed by Whittemore relevant studies were searched in The Cochrane Library,PubMed,Em-base,Web of Science,PsycINFO,CNKI,Wanfang and VIP databases. The literature search was conducted from the database inception to Janu-ary 20,2025.Literature screening was carried out according to the inclusion criteria, data, including the eye-tracking tasks, screening perform-ance,was extracted and synthesized. Results Ultimately,18 literatures were included in the study, and the cognitive dimensions were mainly executive function and memory function.Eye movement tracking tasks include scanning, stable tracking, visual search, and exploratory eye traj-ectory movement examination. The screening duration includes short duration(3 to 5 minutes)and long duration(20 to 60 minutes). The con-struction methods of the screening model included in the study were index result, regression models and machine learning. The AUC range of screening performance was 0.620~0.975,which was related to the eye movement task and the model construction method. Conclusion The included studies primarily assesed single-damain cognitive function. The screening performance is related to the screening tasks and model construction methods. It is suggested that future research can further analyze the effectiveness of different eye movement tasks and indi-cators,integrate different tasks, and construct screening tools that can evaluate overall cognitive function to screen for cognitive impairment more effectively.
A mixed evidence-based study of current status of subcutaneous injection of prefilled anticoagulant in adult surgical inpa-tientsAbstract:Objective To investigate the current situation and influencing factors of subcutaneous hemorrhage caused by subcutaneous injection of anticoagulants in surgery, and to explore the obstacles to the implementation of evidence based practice. Methods A parallel mixed-methods design was adopted for the study. From January to August 2024,a convenience sampling method was used to investigate the occurrence of subcutaneous bleeding in patients receiving subcutaneous anticoagulant injections in the surgical department of a tertiary grade A hospital in Beijing. In December 2024,nine clinical nurses were selected for interviews to explore the barriers encountered during the imple-mentation of evidence-based practices. Results The incidence of subcutaneous bleeding after pre-filled anticoagulant injection in 755 patients was 35.36%.The risk of subcutaneous hemorrhage in vascular surgery patients was 47.00%that of orthopedic patients(OR=0.470,P=0.003).The possibility of severe subcutaneous hemorrhage in female patients was 1.495 times that of male patients(OR=1.495,P=0.015).The interview results showed that the difficulty in controlling the injection rate and dwell time, the lack of clarity regarding procedures for specific situations and popula-tions,and nurses'existing habits and inconvenience in using tools were the three factors hindering the implementation of evidence. Conclusion For adult surgical inpatients,pre-filled anticoagulant injections carry a risk of subcutaneous hemorrhage. It is necessary to optimize the injection process based on risk stratification, refine operational guidelines for special populations, and enhance procedural standardization with informa-tion technology tools. This will reduce the incidence of subcutaneous hemorrhage and promote continuous quality improvement in nursing care.
