The Application Effectiveness of a Language Cognitive Rehabilitation System Combined with NMES Therapy in Children with Cerebral Palsy and Language DisordersAbstract:Objective:To analyze the therapeutic efficacy of a language-cognitive rehabilitation system combined with neuromuscular electrical stimulation(NMES)treatment in children with cerebral palsy and language disorders.Methods:A total of 116 children with cerebral palsy and language disorders admitted to Shaoguan Second People's Hospital between May 2022 and May 2024 were selected as study subjects.Using a random number table,they were divided into a control group(n=58)and an observation group(n=58).The control group received treatment with a language cognitive rehabilitation system.The observation group received the same treatment as the control group supplemented with NMES.The efficacy,language abilities,and Gesell developmental scores of children in both groups were compared before and after treatmen.Results:The efficacy rate in the observation group was higher than that in the control group,with a statistically significant difference(t=0.705,P<0.05).After intervention,the scores for symbolic form and content of instructions,communication attitude,operational tasks,and memory and imitation in the observation group were all higher than those in the control group,with statistically significant differences(t=8.819,9.263,8.140,9.040;P<0.05).After intervention,the scores for adaptive behavior,language,and personal-social functioning in the observation group were significantly higher than those in the control group,with statistically significant differences(t=3.049,4.331,4.724;P<0.05).Conclusion:The combined application of language cognitive rehabilitation systems and NMES therapy demonstrates favorable outcomes in children with cerebral palsy and language disorders,effectively enhancing their language abilities and elevating their intellectual levels.
Correlation of Self-management with Mood Disorders and Adult Attachment in Young and Middle-aged Hemodialy-sis PatientsAbstract:Objective:To discuss the correlation of self-management,emotional disorders and adult attachment in young and middle-aged hemodialysis patients.Methods:Selecting 207 middle-aged and young hemodialysis patients from the First Affiliated Hospital of Nanchang University from March 2023 to December 2023,Investigate patients'self-management,emotional disorders,and adult attachment,and analyze the correlation between self-management,emotional disorders,and adult attachment.Results:Total score of self-management scale(HDSMI)of young and middle-aged hemodialysis patients(47.75±5.69),the average score was the lowest,meal score of partnership dimension is the highest;the Zung Anxiety Self-assessment Scale(SAS)score(58.41±3.61),Zung Depression Self-assessment Scale(SDS)score(59.31±2.08);attachment avoidance(65.13±15.27)score in the intimate relationship experience scale(ECR)of young and middle-aged hemodialysis patients,attachment anxiety(63.89±11.34)score;the total score and dimensions of HDSMI were negatively correlated with SAS,SDS scores,attachment avoidance,and attachment anxiety(P<0.05).Conclusion:Young and middle-aged hemodialysis patients self management level is medium,remains to be improved,and anxiety depression and adult attachment also needs to ease,patients with self management by cultural level,medical expenses payment,dialysis time,so the low level of education,medical expenses more and long dialysis patients need to strengthen the guidance and support.The lower the degree of emotional disorder and the lower the level of adult attachment,the higher the level of self-management.Medical workers need to actively take intervention measures to reduce the negative psychological state of patients,improve adult attachment,and help improve the level of self-management of patients.
Clinical Application of Bamboo Ring Ginger Moxibustion Combined with Acupoint Plaster Application in Patients with Unstable Angina Pectoris Due to Cold-Induced Heart Meridian Obstruction SyndromeAbstract:Objective:To explore the application effect of bamboo ring ginger moxibustion combined with acupoint application therapy in patients with unstable angina pectoris of coagulated cold in heart vessel.Methods:123 patients with unstable angina pectoris of coagulated cold in heart vessel in a tertiary A hospital of traditional Chinese medicine in Zhengzhou from April 2023 to June 2024 were selected as the research subjects.They were randomly divided into group A(bamboo ring ginger moxibustion combined with acupoint application therapy),group B(bamboo ring ginger moxibustion)and group C(acupoint application therapy).The TCM syndrome scores,efficacy index,frequency and duration of the attack of angina pectoris,nitroglycerin dosage and seattle angina questionnaire(SAQ)scores were compared among the 3 groups.Results:The TCM syndrome scores,efficacy rate of TCM syndrome,frequency and duration of the attack of angina pectoris,nitroglycerin dosage and SAQ scores in the 3 groups were significantly improved after treatment,with statistically significant differences(P<0.05).The TCM syndrome scores,frequency and duration of the attack of angina pectoris and SAQ scores in group A were better than those in group B and group C,with statistically significant differences(P<0.05).The duration of the attack of angina pectoris and SAQ scores in group B were better than those in group C,with statistically significant differences(P<0.05).Conclusion:Bamboo ring ginger moxibustion combined with acupoint application therapy can effectively alleviate patients'symptoms of unstable angina pectoris of coagulated cold in heart vessel,reduce angina attacks and enhance the quality of life.
Clinical Observation of Micro-Video-Guided Respiratory Control Technique in Non-Invasive Ventilation for Patients with Chronic Obstructive Pulmonary Disease and Acute Exacerbation Complicated by Heart FailureAbstract:Objective:To investigate the clinical value of microvideo-assisted respiratory control techniques in non-invasive ventilation for patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated by cardiac insufficiency.Methods:A total of 90 patients with AECOPD complicated by cardiac insufficiency admitted to Shicheng County People's Hospital,Jiangxi Province,from March 2022 to January 2024 were enrolled.Using a random number table,they were divided into group A(basic treatment),group B(non-invasive ventilation),and group C(micro-video+breath-controlled non-invasive ventilation),with 30 patients in each group.After one week of treatment,the efficacy,blood gas parameters,cardiac function indicators,and incidence of adverse emotions were compared among the three groups.Results:The total effective rate in group C was higher than that in groups A and B,while the incidence of adverse emotions was lower than in groups A and B,with statistically significant differences in both cases(P<0.05).After treatment,partial arterial oxygen tressure(PaO2),left ventrialclar ejection fraction(LVEF)increased,while patrial arterial carbon dioxide pressure(PaCO2),left ventriaulor end-systolic diameter(LVEDS)decreased in all three groups,and the changes were most pronounced in group C,with statistically significant differences in all cases(P<0.05).Conclusion:Microvideo combined with breath-controlled techniques in noninvasive ventilator therapy for acute exacerbation of chronic obstructive pulmonary disease with cardiac insufficiency improves treatment efficacy,enhances blood gas parameters and cardiac function,and reduces the incidence of adverse emotional states.
Advances in the Genomics of CholangiocarcinomaAbstract:Cholangiocarcinoma(CCA)is a malignant tumor with poor prognosis,and its morbidity and mortality are increasing year by year.Most patients have already developed to the advanced stage at the time of initial diagnosis.With the development of molecular diagnosis technology and the discovery of new biomarkers,it provides an attractive opportunity for early diagnosis of CCA patients.It summarizes the latest research progress of biomarkers in the genome of CCA in recent years is reviewed,which is expected to provide guidance and reference for the early clinical diagnosis and prognosis of CCA.
Clinical Observation of Bronchoscopic Alveolar Lavage Combined with Inhaled Acetylcysteine in the Treatment of Bronchiectasis with Pulmonary InfectionAbstract:Objective:To observe the efficacy of combined bronchoalveolar lavage(BAL)and inhaled acetylcysteine in treating bronchiectasis with pulmonary infection.Methods:A total of 60 patients with bronchiectasis complicated by pulmonary infection admitted to Shangrao People's Hospital from January 2022 to January 2024 were enrolled in this study.They were randomly divided into group A and group B,with 30 patients in each group.Group A received BAL combined with inhaled acetylcysteine therapy on top of standard treatment,while group B received BAL therapy on top of standard treatment.The clinical efficacy of both groups after treatment,as well as changes in lung function and inflammatory factor levels before and after treatment,were observed.Results:Compared with group B,the total effective rate of clinical efficacy in group A after treatment was significantly higher,with a statistically significant difference(P<0.05).After treatment,both groups showed significant increases in forced vital capacity(FVC),percentage of predicted forced expiratory volume in one second(FEV1),and FEV1/FVC ratio compared to pre-treatment levels,and compared to group B,group A demonstrated higher levels of these indicators,with all differences being statistically significant(P<0.05).After treatment,both groups showed significant reductions in serum interleukin(IL)-6,procalcitonin(PCT),and C-reactive protein(CRP)levels compared to pre-treatment levels,and the levels in group A were lower than those in group B,with all differences being statistically significant(P<0.05).Conclusion:BAL combined with inhaled acetylcysteine demonstrates proven efficacy in treating patients with bronchiectasis complicated by pulmonary infection,facilitating recovery of pulmonary function and alleviating systemic inflammatory responses.
Clinical Features and Chest CT Diagnosis of Severe Pneumonia in Children Requiring Invasive VentilationAbstract:Objective:To analyze the clinical characteristics,laboratory examination and chest CT findings of severe pneumonia in children requiring invasive ventilation.Methods:Clinical data and chest CT imaging data of children with severe pneumonia from 2018 to 2022 were analyzed retrospectively.All children were divided into invasive ventilation group(n=138)and general group(n=344)according to whether invasive ventilation was needed.The clinical features,laboratory indexes,chest CT features and treatment prognosis of the two groups were compared.Results:Among the 482 children with severe pneumonia,and 59.4%of the children with severe pneumonia who needed mechanical ventilation were less than 1 year old.The proportion of basic heart diseases,bronchopulmonary diseases,anemia and malnutrition in the mechanical ventilation group was higher than that in the control group.The proportions of cytomegalovirus and Staphylococcus aureus detected and creatine kinase-myocardial band(CK-MB)level in the mechanical ventilation group were higher than those in the control group.The proportions of lung compactification,atelectasis,pleural effusion and cardiac image enlargement on chest CT in the mechanical ventilation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Children with severe pneumonia requiring mechanical ventilation have certain clinical characteristics and imaging findings.Preventing underlying heart and lung diseases,improving anemia and malnutrition,and early identification of children prone to respiratory failure have great significance in reducing the incidence of severe pneumonia,increasing the cure rate and reducing the fatality rate.
Status and Influencing Factors of Hospital Discharge Readiness in Patients with Chronic Viral Hepatitis BAbstract:Objective:To investigate the status quo and influencing factors of hospital discharge readiness in patients with chronic viral hepatitis B(CHB).Methods:A total of 98 patients with CHB were selected by convenience sampling method,all of whom were hospitalized in infection department of China Pingmei Shenma Group General Hospital from January 2022 to June 2023.The influencing factors of discharge readiness of CHB patients and were investigated.Results:The total score of discharge readiness of CHB patients was(160.09±18.44)points;univariate analysis showed that the discharge readiness of CHB patients was related to residence,per capita monthly family income and education level(P<0.05).There were positive correlation between self-efficacy,quality of discharge guidance and total score of discharge readiness in CHB patients(P<0.05).Multivariate analysis showed that residence,quality of discharge guidance,per capita monthly family income,self-efficacy and education level were all high risk factors affecting the discharge readiness of CHB patients(P<0.05).Conclusion:The discharge readiness of CHB patients is affected by residence,discharge guidance quality,family per capita monthly income,self-efficacy and education level.
Analysis and Intervention of Related Factors of Self-Care Ability in Peritoneal Dialysis PatientsAbstract:Objective:To investigate the related factors of self-care ability in patients with peritoneal dialysis(PD)and explore the intervention measures.Methods:A total of 96 patients receiving PD treatment in Luoyang Central Hospital from March 2021 to March 2022 were selected.General data questionnaire,self-management ability scale,self-efficacy scale and social support rating scale(SSRS)were used to conduct questionnaire survey,and relevant scales were used to evaluate self-care ability,self-efficacy and social support.The correlation and influencing factors were analyzed.Results:The total score of self-care ability in PD patients was(125.62±11.58)points.Univariate analysis showed that age,education,residence and dialysis age were related to self-care ability of PD patients(P<0.05).Pearson correlation analysis showed that social support score and self-efficacy score in PD patients were positively correlated with self-care ability(P<0.05).Multivariate linear analysis showed that age,education,residence,dialysis age,social support and self-efficacy were all risk factors affecting the self-care ability of PD patients(P<0.05).Conclusion:The self-care ability of PD patients needs to be improved,and the self-care ability of PD patients is better in those who are older,less educated,not living alone,shorter dialysis age,and lower social support and self-efficacy.Intervention strategies based on relevant factors can improve the self-care ability of PD patients.
Analysis of the Application of Cold Precipitation Coagulation Factors in Yinchuan AreaAbstract:Objective:To understand the clinical application of cold precipitation coagulation factor in medical institutions in Yinchuan area,analyze the existing problems,and provide effective references for the rational formulation of inventory levels and clinical blood guidance.Methods:Statistical analysis was conducted on the preparation of cold precipitated coagulation factors in Ningxia Blood Center from 2019 to 2023,and the clinical use of cold precipitated coagulation factors in 37 blood hospitals in Yinchuan area.Results:37 medical institutions in the Yinchuan area had used 111 886 units of cold precipitation coagulation factor in the past 5 years,with a supply trend increasing year by year.The proportion of usage in tertiary hospitals reached 97.6%,while the usage in secondary hospitals,private and military hospitals remained at a relatively low level.Conclusion:The usage of cold precipitation coagulation factors in clinical blood institutions in Yinchuan region has been increasing year by year from 2019 to 2023,with a significant increase in tertiary hospitals(P<0.05).In today's world where tight and balanced blood supply has become the norm,it is necessary to reasonably balance the amount of cold precipitation and new plasma preparation,develop better inventory management models,and effectively alleviate blood supply pressure.
Study on the Efficacy of Acupuncture Combined with Holistic Care for Postpartum Uterine ContractionsAbstract:Objective:To investigate the effects of acupuncture combined with holistic care on pain management and uterine recovery in women experiencing postpartum uterine contractions.Method:A total of 186 postpartum women with uterine contraction pain admitted to Zhumadian Traditional Chinese Medicine Hospital between March 2021 and March 2023 were selected as study subjects.They were randomly divided into a control group(n=93)and an observation group(n=93).The control group received holistic nursing care,while the observation group received holistic nursing care supplemented with acupuncture therapy.The degree of uterine contraction pain,postpartum hemorrhage,uterine recovery,psychological state,sleep quality,and nursing satisfaction were compared between the two groups.Results:After the intervention,the psychological state scores,sleep quality scores and nursing satisfaction of the observation group were significantly better than those of the control group,with statistically significant differences(P<0.05).Postpartum,the bleeding volume in the observation group was significantly lower than that in the control group,with statistically significant differences(P<0.05).At 3 days postpartum,the fundal height in the observation group was significantly superior to that in the control group,with a statistically significant difference(P<0.05).At7 days postpartum,the sum of the three uterine dimensions in the observation group was significantly superior to that in the control group,with a statistically significant difference(P<0.05).Conclusion:Postpartum uterine contraction pain can be alleviated in mothers through acupuncture combined with holistic care,which promotes uterine recovery and significantly improves negative emotions,thereby enhancing nursing satisfaction.
Long-term Efficacy of Precise Biliary Surgery for Gallbladder Cancer and Its Effects on Serum NLR,CA19-9,and LDHAbstract:Objective:To analyze the long-term efficacy of precision biliary surgery technology in patients with gallbladder cancer,and explore the impact of this technology on serum neutrophil-to-lymphocyte ratio(NLR),carbohydrate antigen 19-9(CA19-9),and lactate dehydrogenase(LDH).Methods:20 patients who underwent traditional surgical treatment between October 2017 and April 2019 were selected as the control group,and 23 patients who underwent precision biliary surgery between May 2019 and April 2021 were selected as the experimental group.The clinical data of the two groups were retrospectively analyzed to compare the surgical outcomes,long-term survival rates,and serum NLR,CA19-9,LDH.Results:The no residual tumor(R0)resection rate of the patients in the experimental group was higher than that of the control group,the microscopic residual tumor(R1)and macroscopic residual tumor(R2)resection rate was lower than that of the control group,and the number of lymph node dissections was more than that of the control group,the differences were statistically significant(P<0.05)the serum NLR,CA19-9,and LDH levels on 7th day after surgery of the experimental group was lower,and the differences were statistically significant(P<0.05);the 1-year and 3-year survival rates of patients in the experimental group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion:The number of lymph nodes removed during precise biliary surgery for gallbladder cancer is relatively high,which can improve postoperative prognosis and long-term survival rates.
Clinical Application Research of Traditional Chinese Medicine Staging Therapy Assisted in Vitro Fertilization Embryo Transfer Based on the Theory of"Heterozygous Treatment"Abstract:Objective:Observing the clinical effect of traditional Chinese medicine staging therapy based on the theory of"heterozygous treatment"in assisting in vitro fertilization-embryo transfer(IVF-ET)treatment of infertility.Methods:Selecting 85 female infertility patients admitted to The Second People's Hospital of Henan Province from January 2021 to February 2023.They were divided into a conventional group(n=42)and a traditional Chinese medicine group(n=43)using a computer randomization method.The conventional group received IVF-ET treatment,while the traditional Chinese medicine group received traditional Chinese medicine staging therapy based on the"heterozygous treatment"theory to assist IVF-ET treatment.The hormone levels,uterine blood flow parameters,and pregnancy outcomes of the two groups of patients were compared.Results:Under different treatment plans,the follicle stimulating hormone(FSH),luteinizing hormone(LH),anti Mullerian hormone(AMH),and number of antral follicles(AFC)in the traditional Chinese medicine group were all higher than those in the conventional group,with statistical significance(P<0.05);the uterine artery pulsatility index(PI),uterine artery resistance index(RI),whole blood high shear viscosity,and whole blood low shear viscosity in the traditional Chinese medicine group were all lower than those in the conventional group,with statistical significances(P<0.05);the clinical pregnancy rate in the traditional Chinese medicine group was higher than the conventional group,and the incidence of adverse pregnancy outcomes was lower than the conventional group,with statistically significant differences(P<0.05).Conclusion:Based on the theory of"heterozygous treatment",traditional Chinese medicine staging therapy assisted with IVF-ET can improve hormone levels in patients with infertility,and has positive significance in improving uterine blood flow and pregnancy outcomes.
The Effect of In-Situ Simulation Teaching Method in the First Aid Clinical Teaching Process of Nursing Students under the Ideological and Political Background of the SubjectAbstract:Objective:To explore the effect of in-situ simulation teaching method under the background of ideological and political research in the clinical teaching process of first-aid nursing students.Methods:A total of 110 nursing students who practiced in Deyang People's Hospital from January to June 2024 were selected as study objects and divided into group A and Group B by coin toss,with 55 cases in each group.Group A implemented the conventional teaching method,group B implemented the in-situ simulation teaching method under the ideological and political background of the subject on the basis of group A.The two groups were compared in terms of empathy ability[assessed by the Chinese version of Jefferson Empathy Scale-Nursing Students Edition(JSE-NS)],professional identity,test scores(theoretical test scores,skill operation test scores),and humanistic care ability[assessed by the humanistic Care Ability Scale(CAI)].Results:After intervention,JSE-NS scores and total scores in group B were higher than those in group A,the differences were statistically significant(t=21.694,7.817,12.318,19.794;P<0.001).After intervention,compared with group A,group B had higher scores in all dimensions of professional identity,the differences were statistically significant(t=5.912,5.825,6.102,10.539,6.497,7.625;P<0.001).After intervention,the scores of theory test and skill operation test in group B were higher than those in group A,the differences were statistically significant(t=11.311,10.362;P<0.001).After intervention,CAI scores and total scores in group B were higher than those in group A,the differences were statistically significant(t=6.998,7.214,14.777,13.259;P<0.001).Conclusion:The in-situ simulation teaching method can effectively improve the empathy ability,professional identity,examination scores and humanistic care ability of first-aid nursing students under the background of ideological and political research in the clinical teaching process.
The Impact of the ACE-Star Evidence-Based Nursing Model on Postoperative Recovery and Gastrointestinal Recovery in Patients Undergoing Radical Gastrectomy for Gastric CancerAbstract:Objective:To explore the influence of American Academic Center for Evidence-based Practice(ACE)-Star evidence-based nursing model on postoperative rehabilitation and gastrointestinal recovery of patients with gastric cancer undergoing radical surgery.Methods:The clinical data of 46 patients who underwent radical gastrectomy in Jiujiang First People's Hospital from January to December,2023 were retrospectively analyzed.They were divided into two groups with 23 patients in each group according to different nursing schemes.Conventional nursing was applied in the control group,and ACE-Star evidence-based nursing was applied in the observation group.The postoperative rehabilitation,gastrointestinal recovery and complications of the two groups were compared.Results:The anal exhaust time,defecation time,bowel sounds recovery time and fluid intake time in the observation group were shorter than those in the control group,with statistically significant differences(P<0.05).After nursing,the levels of motilin(MTL)and gastrin(GAS)in both groups increased,while the level of vasoactive intestinal peptide(VIP)decreased,and the level of observation group changed more than that of control group,the incidence of complications in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion:The implementation of ACE-Star evidence-based nursing model can promote the postoperative rehabilitation of patients undergoing radical gastrectomy,accelerate the recovery of gastrointestinal function and reduce complications.
To Investigate the Application Effect of ECPR and CCPR in Patients with Emergency Respiratory Cardiac Arrest Based on Blood Oxygen Saturation and Complication RateAbstract:Objective:To investigate the effects of external cardiopulmonary resuscitation(ECPR)and traditional cardiopulmonary resuscitation(CCPR)in patients with emergency respiratory cardiac arrest based on blood oxygen saturation and complication rate.Methods:The data of 60 patients with emergency respiratory cardiac arrest in Luoyang Central Hospital Affiliated to Zhengzhou University from April 2022 to April 2024 were retrospectively analyzed.30 patients treated with CCPR were classified as CCPR group by 1∶1 pairing principle,and 30 patients treated with ECPR after failing CCPR treatment were classified as ECPR group.Comparing the treatment conditions of the two groups[success rate of cardiopulmonary resuscitation(CPR),success rate of restoring spontaneous circulation(ROSC rate)],oxygen metabolism indexes(oxygen load,oxygen consumption,arterial oxygen content),blood gas analysis indexes(oxygen saturation,oxygenation index,blood lactic acid),hemodynamics[coronary perfusion pressure(CPP),total ejection fraction(GEF),cardiac output volume(CO),mean arterial pressure(MAP)]and the incidence of complications.Results:The success rate of CPR in ECPR group was 63.33%(19/30),higher than that in CCPR group,33.33%(10/30),with statistically significant difference(P<0.05).The ROSC rate in ECPR group was 66.67%(20/30)higher than that in CCPR group,40.00%(12/30),with statistically significant differences(P<0.05).Blood oxygen saturation,oxygenation index,oxygen load,oxygen consumption and arterial oxygen content in ECPR group were higher than those in CCPR group at 30 min after resuscitation,and blood lactic acid was lower than that in CCPR group,with statistically significant differences(P<0.05).The 30 min CPP,GEF,CO and MAP of ECPR group were higher than those of CCPR group,with statistically significant differences(P<0.05).The complication rate of ECPR group was 10.00%(3/30)compared with CCPR group 20.00%(6/30),and the difference was not statistically significant(P>0.05).Conclusion:Compared with CCPR,ECPR has a better therapeutic effect,which is helpful to regulate oxygen metabolism,improve hemodynamics,and promote the recovery of blood gas analysis indexes,and has a certain safety.
Application of Graded Nursing Guided by Glasgow Brachford Scoring System in Patients Undergoing Upper Gastrointestinal Bleeding SurgeryAbstract:Objective:Observation of the application effects of graded nursing guided by Glasgow Bradford Scale(GBS)on patients undergoing upper gastrointestinal bleeding surgery.Methods:A total of 120 patients with upper gastrointestinal bleeding admitted to the First People's Hospital of Shangqiu from January 2022 to January 2024 were selected as the study subjects,and divided into the conventional group and GBS group,with 60 cases in each group.The patients in the conventional group were given peroperative care,while the GBS patients were given Graded Nursing Guided by GBS.The hemostasis status,psychological state,occurrence of adverse outcomes,and nursing satisfaction between the two groups of patients were compared.Results:Compared with the conventional group,the hemostasis time,rebleeding rate,and hospitalization time of the GBS group were lower,and the difference was statistically significant(P<0.05).The General Self-Efficacy Scale(GSES)and Connor Davidson resilience scale(CD-RISC)scores of the GBS group were higher than those of the conventional group,and the Patient Health Questionnaire-9(PHQ-9)and Generalized Anxiety Scale(GAD-7)were lower than those of the conventional group,and the difference was statistically significant(P<0.05).During the follow-up period,the total incidence of adverse prognosis in the GBS group was lower than that of the conventional group,and the difference was statistically significant(P<0.05).After the follow up,the nursing skill score,nursing attitude score,accident handling score,communication ability score,and overall satisfaction score of the GBS group were higher than those of the conventional group,and the differences were statistically significant(P<0.05).Conclusion:Graded nursing under GBS guidance can enhance the hemostatic effect of patients with upper gastrointestinal bleeding,improve their psychological state,reduce the risk of adverse prognosis,and increase nursing satisfaction,all of which have positive significance.
Exploration of Related Factors for Poor Postoperative Prognosis in Patients with Aneurysmal Subarachnoid HemorrhageAbstract:Objective:To explore the factors related to poor prognosis after aneurysm clipping surgery in patients with aneurysmal subarachnoid hemorrhage(aSAH).Methods:A total of 185 patients with aSAH admitted to Anyang Regional Hospital from January 2020 to January 2023 were selected.According to the modified Rankin Scale(mRS)score,the 185 patients were divided into a good prognosis group(mRS score≤2 points)and a poor prognosis group(mRS score>2 points).The age,gender,and other data of the 185 patients were collected to analyze the relevant factors for poor prognosis after treatment.Results:Among 185 patients with aSAH who underwent aneurysm clipping surgery,a total of 11 cases had poor prognosis,accounting for 5.95%;logistic regression analysis showed that aged>60,history of hypertension,intracranial hematoma,postoperative rebleeding,postoperative pulmonary infection,postoperative cerebral infarction were independent risk factors for poor prognosis of aSAH patients after aneurysm clipping surgery(P<0.05).Conclusion:Patients with aSAH have a higher risk of poor prognosis after undergoing aneurysm clipping surgery,age>60 years old,history of hypertension,intracranial hematoma,postoperative rebleeding,postoperative pulmonary infection,and postoperative cerebral infarction are risk factors for poor prognosis in patients with aSAH treated with aneurysm clipping surgery.
Application of Roux-en-Y Gastric Bypass Surgery and Laparoscopic Sleeve Gastrectomy in Patients undergoing Weight Loss SurgeryAbstract:Objective:To observe the application effects of Roux-en-Y gastric bypass surgery(LRYGB)and laparoscopic sleeve gastrectomy(LSG)on patients undergoing weight loss surgery.Methods:As a prospective study,it selected 110 patients who underwent laparoscopic sleeve gastrectomy in the First People's Hospital of Nanyang from October 2021 to October 2023.The computer randomization method was used to generate a random sequence,and the enrolled patients were divided into the LRYGB group(55 cases)and the LSG group(55 cases).The postoperative follow-up was conducted for six months to compare the surgical situation,incidence of complications,digestive function,and weight loss effect of the two groups of patients.Results:Under different surgical plans,the LSG group had significantly lower surgical time,intraoperative blood loss,postoperative recovery of exhaust time,recovery of eating time,mobilization time,and hospitalization time compared to the LRYGB group,and the difference was statistically significant(P<0.05).The incidence of complications in the LSG group was lower than that in the LRYGB group,and the difference was statistically significant(P<0.05).Gastrin-17(GAS-17)in the LSG group was higher than that in the LRYGB group at 3 and 6 months of follow-up,and the pepsinogen Ⅰ/Ⅱ ratio(PGR)was higher than that of LRYGB group,and the difference was statistically significant(P<0.05).As of the end of follow-up,there was no statistically significant difference in BMI index,waist circumference,hip circumference,and percentage of excess weight loss(EWL)between the LSG group and the LRYGB group(P>0.05).Conclusion:The weight loss effects of LSG and LRYGB are comparable,but LSG could accelerate the postoperative recovery process of patients undergoing weight loss surgery,and has a positive significance in reducing the risk of surgical complications and alleviating digestive function damage.
Clinical Value Analysis of Serum Enzymes,HBV-DNA Load and Five Indicators of Hepatitis B in Patients with Chronic HBV InfectionAbstract:Objective:To investigate the clinical value of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutathione reductase(GR),HBV-DNA load and five hepatitis B indicators in patients with chronic hepatitis B virus(HBV)infection.Methods:A total of 167 patients with chronic hepatitis B(CHB)admitted to General Hospital of Southern Theater Command of the People's Liberation Army of China from January 2020 to March 2024 were selected as the research objects,while the control group included 40 healthy patients;leading to 48 patients as the cirrhosis group and 119 non-cirrhosis group.The levels of serum ALT,AST,GR,AST/ALT ratio,HBV-DNA load and hepatitis B five indexes in hepatitis B patients were retrospectively analyzed.Results:Compared with the non-cirrhosis group,the levels of serum ALT,AST,GR,the proportion of high HBV-DNA load(>1.0×105),and AST/ALT were all higher in the cirrhosis group;non-cirrhotic patients were prone to develop into cirrhotic patients with increasing age,and AST/ALT>1 was more common in men,the differences were statistically significant(P<0.05),but there was no significant difference in the positive rate of hepatitis B e antigen(HBeAg)and hepatitis B surface antigen(HBsAg)between the two groups(P>0.05).There was no significant difference in serum ALT,AST,GR levels and AST/ALT ratio between HBsAg(+)and HBsAg(-)patients(P>0.05).There were significant differences in serum ALT,AST,and GR levels among patients with different HBV-DNA load(P<0.05),and with the increase of HBV-DNA load,the levels of ALT,AST and GR showed an upward trend.There was no significant difference in the AST/ALT and positive rate of HBeAg among patients with different HBV-DNA load(P>0.05).Serum GR,ALT and AST levels of CHB patients were higher than those of the control group,and the rate of HBV in the group was higher in males than in females,the differences were statistically significant(P<0.05).The receiver operating characteristic(ROC)curves showed that serum GR combined with ALT and AST had high diagnostic efficacy for the detection of CHB and hepatitis B cirrhosis,the differences were statistically significant(P<0.05).Conclusion:The serum ALT,AST,GR,HBV-DNA load and five indicators of hepatitis B in CHB patients have certain application value in judging chronic hepatitis B cirrhosis,the severity of liver damage in CHB patients and improves the diagnosis level of liver cirrhosis.
