Application of narrative nursing combined with risk management in the prognosis of postoperative lung cancer patientsAbstract:Objective To analyze the application of narrative nursing combined with risk management to the prognosis of postoperative lung cancer patients.Methods A total of 240 postoperative lung cancer patients admitted from January 2022 to January 2024 were included in this study.They were randomly divided into two groups using a random number table,with 120 cases per group.Patients in the control group received routine nursing care,while those in the study group received narrative nursing combined with risk management.Psychological status scores,hope scale scores,postoperative pain scores,sleep disorder scores,social support level scores,and quality of life scores before and after the intervention were compared between the two groups.Postoperative recovery indicators and complications were also analyzed.Results After the intervention,psychological status(depression and anxiety),postoperative pain,and sleep disorder scores significantly decreased compared to pre-intervention values,which were significantly lower in the study group than the control group(P<0.05).Hope levels,social support levels,and quality of life scores significantly increased after the intervention in both groups,which were significantly higher in the study group than the control group(P<0.05).The duration of postoperative spontaneous sputum discharge,significant reduction in pain,and length of hospital stay were significantly shorter in the study group than the control group(P<0.05).The complication rate in the study group was significantly lower than the control group(P<0.05).Conclusion Narrative nursing combined with risk management is superior in improving the psychological status,reducing pain and sleep disturbances,and enhancing social support levels and quality of life.This can be an important component in the comprehensive nursing care of postoperative lung cancer patients that deserves clinical application.
Risk prediction model establishment of adverse prognosis in acute epiglottitis and clinical value analysisAbstract:Objective To build a risk prediction model of adverse prognosis in patients with acute epiglottitis and to formulate targeted intervention strategies.Methods From August 2021 to January 2023,188 patients with acute epiglottitis treated were selected.According to the prognosis,they were divided into the mild disease group(n=128)and severe disease group(n=60).Univariate and multivariate logistic analyses were performed to construct the early risk warning model,and Hosmer-lemeshow test was used to test the goodness of fit of the model.Results Univariate and multivariate logistic regression analyses showed significant differences in gender,onset time,smoking,partial pressure of oxygen in arterial blood(PaO2)between groups(P<0.05),and the odds ratio(OR)was greater than 1.The probability model of adverse prognosis in patients with acute epiglottitis was as follows:P=1/[1+exp(59.311-1.337X1-0.242 X2-0.762X3-0.795X4)].The goodness of fit test results showed χ2=4.174,and P=0.841.Conclusion Male gender,nocturnal onset,smoking,and PaO2 reduction are the risk factors for poor prognosis in patients with acute epiglottitis.The risk prediction model of poor prognosis in patients with acute epiglottitis constructed based on this has high clinical predictive value,which can guide clinical practice to carry out early intervention and prevention to improve the prognosis.
Research progress on the fecal microbiota transplantation in the treatment of depression and anxiety disordersAbstract:Depression is a mental disorder characterized by significant and persistent low mood.Anxiety disorder is a type of neurosis characterized by excessive worry.At present,conventional antidepressants have very limited effects,and their side effects and withdrawal reactions seriously affect the quality of life.Fecal microbiota transplantation(FMT)can rebuild the intestinal microenvironment and effectively relieve the symptoms of mood disorders.This article systematically and comprehensively reviewed the research progress of FMT in the treatment of depression and anxiety disorders,providing new ideas for the treatment of depression and anxiety disorders,and references for the further development of FMT in the treatment of mood disorders.
Expression levels of AXIN1,ARID1A and AKT2 in thyroid cancer tissue and their correlations with clinical pathological features and prognosisAbstract:Objective To detect the expression levels of axis inhibition protein 1(AXIN1),AT rich interaction domain 1A(ARID1A),and protein kinase B(AKT2)in thyroid cancer(TC)tissue,and to identify their correlations with clinical pathological features and prognosis of TC.Methods A total of 102 patients confirmed as TC and treated from January 2020 to December 2020 were recruited.Cancer tissue and adjacent tissue specimens were collected,and the 3-year follow-up was conducted.Immunohistochemical staining was applied to detect the protein expressions of AXIN1,ARID1A,and AKT2 proteins in tissues.Multivariate Cox regression was applied to analyze the factors influencing the prognosis of TC patients.Kaplan-Meier method was applied to analyze the correlation of expression levels of AXIN1,ARID1A,and AKT2 in TC tissues with the prognosis.Results The positive expression rate of AXIN1,ARID1A,and AKT2 in TC tissues was 27.45%,40.20%,and 68.63%,respectively.Compared with adjacent tissues,the positive expression rate of AXIN1 and ARID1A in TC tissues was significantly lower(P<0.05),while the positive expression rate of AKT2 was significantly higher(P<0.05).The positive expression rates of AXIN1 and ARID1A in TC patients with lymph node metastasis were significantly lower than those without lymph node metastasis(P<0.05),and the positive expression rate of AKT2 was significantly higher than those without lymph node metastasis(P<0.05).The 3-year survival rate of TC patients with positive AXIN1 was significantly higher than those with negative AXIN1(24/28,85.71%vs 48/74,64.86%,χ2=4.253,P<0.05).The 3-year survival rate of TC patients with positive ARID1A was significantly higher than those with negative ARID1A(34/41,82.93%vs 38/61,62.30%,χ2=5.027,P<0.05).The 3-year survival rate of TC patients with positive AKT2 was significantly lower than those with negative AKT2(44/70,62.86%vs 28/32,87.50%,χ2=6.424,P<0.05).AXIN1 and ARID1A were independent protective factors for the death in TC patients(P<0.05),while AKT2 was an independent risk factor(P<0.05).Conclusion The expression levels of AXIN1 and ARID1A in TC tissues are downregulated,while the expression level of AKT2 is upregulated.They are related to the pathological features and prognosis of TC.AXIN1 and ARID1A are independent protective factors for death in TC patients,while AKT2 is an independent risk factor.
A survey on the anxiety,depression and insomnia of adult residents with chronic diseases in Hebei ProvinceAbstract:Objective To understand the current situation of anxiety,depression and insomnia among adult residents with chronic diseases in Hebei Province,and to analyze the risk factors affecting the current situation.Methods Using the data of main indicators in the 2021 Hebei Province Resident Mental Health Promotion Action,a total of 1,717 residents aged 18 years and above with chronic diseases were selected as the research objects.The Generalized Anxiety Disorder 7-item scale,the Patient Health Questionnaire 9-item scale,and the Insomnia Severity Index were used to evaluate the anxiety,depression,and insomnia,respectively.Chi-square test was used for difference analysis,and logistic regression analysis was used for analyzing influencing factors.Results Among the 1,717 respondents,120(7.0%)suffered from anxiety,170(9.9%)suffered from depression,and 506(29.5%)suffered from insomnia.Logistic regression analysis showed that the age and marital status were influencing factors for chronic diseases combined with anxiety(P<0.05).Age,marital status,occupation,and drinking were influencing factors for chronic diseases combined with depression(P<0.05).Marital status,occupation,drinking,gender,and number of chronic diseases were influencing factors for chronic diseases combined with insomnia(P<0.05).Younger patients were prone to anxiety and depression;unmarried patients were prone to anxiety,depression and insomnia;non-farmers and drinkers were prone to depression and insomnia;and females and patients with multiple chronic diseases were prone to insomnia.Conclusion The residents with chronic diseases in Hebei Province who have anxiety,depression and insomnia are very common.More attention should be paid to key populations such as younger age,unmarried,non-farmers,alcoholics,women,and those with multiple chronic diseases.
Effect of Atractylone on intestinal barrier injury in inflammatory bowel disease rats by regulating the MLCK/MLC signaling pathwayAbstract:Objective To investigate the effect of Atractylone on intestinal barrier injury in inflammatory bowel disease(IBD)rats by regulating the MLCK/MLC signaling pathway.Methods A total of 50 rats were randomly divided into CK group(control group),Model group(IBD modeling),low-dose and high-dose Atractylone groups(IBD modeling plus Atractylone),and ML-7 group(IBD modeling plus MLCK inhibitor),with 10 rats per group.The change in rat body weight was measured.The disease activity index(DAI)was graded.The length of the rat colon was measured.Enzyme linked immunosorbent assay(ELISA)was applied to detect serum levels of tumour necrosis factor α(TNF-α),?interleukin-β(IL-β),IL-10,endotoxin(ET),and D-lactic acid(D-LAC).Commercial reagent kits were applied to detect catalase(CAT),superoxide dismutase(SOD),and malondialdehyde(MDA)levels.H&E staining was applied to score pathological damage in colon tissue.Western blot was applied to detect the protein expressions of tight junction protein(ZO-1),Occludin,Claudin-1,MLCK,p-MLC,and MLC in colon tissue.Results Compared with the CK group,rats in the Model group had significant colon tissue damage,higher DAI,significant higher serum TNF-α,IL-β,ET and D-LAC,MDA content in colon tissue and protein levels of MLCK and p-MLC/MLC,but significant lower body weight,colon length,serum IL-10,CAT and SOD activities in colon tissue and protein levels of ZO-1,Occludin and Claudin-1(P<0.05).Compared with the Model group,rats in the low-dose and high-dose Atractylone and ML-7 groups after treatment had significant alleviated colon tissue damage,significant lower DAI,serum TNF-α,IL-β,ET and D-LAC,MDA content in colon tissue and protein levels of MLCK and p-MLC/MLC,but significant higher body weight,colon length,serum IL-10,CAT and SOD activities in colon tissue and protein levels of ZO-1,Occludin and Claudin-1(P<0.05).Indicators were similar between high-dose Atractylone and ML-7 groups(P>0.05).Conclusion Atractylone can reduce inflammatory response and intestinal barrier damage in IBD rats by inhibiting the MLCK/MLC signaling pathway.
Effects of oral drip of multi-oil fat emulsion on gut microbiota and neurointelligence development of very low birth weight premature infantsAbstract:Objective To observe and analyze the effects of oral drip of multi-oil fat emulsion on gut microbiota and neurointelligence development of very low birth weight premature infants.Methods Eighty patients with MPP who were treated in Dingxi People's Hospital from October 2023 to September 2024 were included as the study subjects.Eighty very low birth weight preterm infants were divided into two groups using a random number table:40 in the SMOF(soybean oil,medium-chain triglycerides,olive oil,and fish oil)group and 40 in the medium-chain triglycerides/lon-chain triglycerides(MCT/LCT)group.After the completion of the treatment,the rehabilitation indexes,liver function,blood lipid index level,the Preterm Infant Oral Feeding Readiness Assessment(PIOFRA)score,gut microbiota,and neurointelligence development of the two groups were compared.Results The muscle tone recovery time,total enteral nutrition time,fetal excretion time and hospital stay time of SMOF group were significantly shorter than the MCT/LCT group(P<0.05).After treatment,aspartate aminotransferase(AST),albumin(ALB)and globulin levels in the SMOF group were significantly higher than the MCT/LCT group,and gamma-glutamyl transferase(γ-GT),total bile acids(TBA),direct bilirubin(DBIL),total bilirubin(TBIL),alanine aminotransferase(ALT),alkaline phosphatase(ALP),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),triglycerides(TG)and total cholesterol(TC)levels were significantly lower than the MCT/LCT group(P<0.05).After additional treatment,the ability to maintain physiological stability during feeding,preterm state,swallowing ability,sucking ability,passive muscle tone,coarse motor tone,active muscle tone,fine motor score,psychomotor development index,intelligence development index,Lactobacillus and Bifidobacterium in SMOF group were all significantly higher than the MCT/LCT group after treatment.The Escherichia coli and incidences of complications were significantly lower than the MCT/LCT group(P<0.05).Conclusion SMOF has good tolerance and safety for very low birth weight preterm infants.Compared with MCT/LCT,oral drip of multi-oil fat emulsion can promote the neurointelligent development of very low birth weight premature infants,and the use effect is better.
Application of a multiple disciplinary teamwork based on an intelligent interactive system on elderly patients with dysphagiaAbstract:Objective To explore the application effect of a multiple disciplinary teamwork based on an intelligent interactive system in the management of elderly patients with dysphagia.Methods In the prospective randomized controlled trial,119 elderly hospitalized patients in the geriatric department of a Grade A hospital from August 2023 to July 2024 were recruited,and they were assigned to control group(routine nursing,n=59)and intervention group(nurse-led multiple disciplinary teamwork based on an intelligent interactive system,n=60),comparing between groups the screening rate of aspiration risk,kubota drinking water test score,aspiration rate,swallowing function,quality of life and nutritional status.Results The screening rate of aspiration risk,drinking water test score,swallowing function,quality of life,and nutritional status scores were significantly better than those of the control group(P<0.05),and the aspiration rate was significantly lower than that of the control group(P<0.05).Conclusion The nurse-led multidisciplinary disciplinary teamwork based on intelligent interaction system can significantly increase the screening rate of aspiration risk;significantly improve swallowing function,life quality and nutritional status,reduce the aspiration rate and decrease the occurrence of adverse events.
Effect of motivation theory combined with enhanced recovery after surgery-based nursing on serum inflammatory factors,thyroid hormones,and immune function in patients undergoing endoscopic thyroid surgery through a non-inflatable axillary approachAbstract:Objective To explore the effects of motivation theory combined with enhanced recovery after surgery(ERAS)-based nursing on serum inflammatory factors,thyroid hormones,and immune function in patients undergoing endoscopic thyroid surgery through a non-inflatable axillary approach.Methods Clinical data of 98 patients who underwent endoscopic thyroid surgery in a non-inflatable axillary approach in Jiangsu Province(Suqian)Hospital(Suqian First Hospital)from April 2022 to April 2024 were retrospectively analyzed.They were randomly divided into the study group(motivation theory combined with ERAS-based nursing,49 cases)and control group(conventional care intervention,49 cases)according to the treatment method.Changes in perioperative indicators,serum inflammatory factor levels,thyroid hormone levels,and immune function indicators were observed and compared between the two groups.Results Before interventions,there were no significant differences in the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),parathyroid hormone(PTH),thyroid stimulating hormone(TSH),thyroxine(T4),total T lymphocytes(CD+3),helper T lymphocytes(CD+4),and cytotoxic T lymphocytes(CD+8)between the two groups(P>0.05).After interventions,patients in the study group had significantly lower/less intraoperative blood loss,postoperative off-bed activity time,drainage volume,length of hospital stay,TNF-α,IL-6,hs-CRP,TSH,and CD+8 levels,and higher PTH,T4,CD+3,and CD+4 levels(P<0.05).Conclusion Motivation theory combined with ERAS-based nursing can effectively improve perioperative indicators,control inflammatory reactions,and enhance thyroid and immune function in patients undergoing endoscopic thyroid surgery through a non-inflatable axillary approach.
Effect of full-course concurrent dose-escalated intensity-modulated radiotherapy combined with S-1 on serum CA19-9 and CEA in patients with esophageal cancerAbstract:Objective To evaluate the effect of adding S-1(tegafur/gimeracil/oteracil)to full-course concurrent dose-escalated intensity-modulated radiotherapy(IMRT)on the serum carbohydrate antigen 19-9(CA19-9)and carcinoembryonic antigen(CEA)levels in patients with esophageal cancer.Methods In this randomized study,106 patients treated between October 2020 and April 2024 were assigned to receive either full-course concurrent doseescalated IMRT alone(control;n=53)or the same IMRT regimen plus concurrent S-1(study;n=53).Dosimetric parameters for organs at risk(cardiac V30,maximum spinal cord dose,and mean doses to the right and left lungs)were recorded during IMRT.Postoperative gastrointestinal function was assessed using a hospital-developed gastrointestinal dysfunction scale.Laboratory indices included white blood cell(WBC),hemoglobin(HGB),platelet(PLT),alanine aminotransferase(ALT),aspartate aminotransferase(AST),ALT/AST ratio,and tumor markers CA19-9 and CEA.Overall response rate(ORR)and adverse events were compared between groups.Results Compared with IMRT alone,IMRT plus S-1 was associated with lower cardiac V30,maximum spinal cord dose,and reduced mean lung doses bilaterally(all P<0.05).Scores for abdominal pain,upper abdominal distension,and nausea/vomiting were significantly lower(P<0.05).WBC,HGB,PLT,CA19-9,and CEA levels significantly decreased,whereas ALT,AST,and the ALT/AST ratio significantly increased(all P<0.05).The ORR was higher in the combination group(P<0.05).Although adverse events were numerically fewer with combination therapy,the difference was not statistically significant(P>0.05).Conclusion In patients with esophageal cancer,adding S-1 to full-course concurrent doseescalated IMRT was associated with more favorable dosimetric parameters to organs at risk,improved symptom control,decreases in CA19-9 and CEA,and higher response rates,without a significant increase in toxicity.
Improvements of the small airway function and inflammatory factor levels in patients with chest tightness variant asthmaAbstract:Objective To explore the therapeutic effect on improving small airway function and inflammatory factors in patients with chest tightness variant asthma.Methods A total of 46 patients with chest tightness variant asthma diagnosed and treated in the outpatient department of our hospital from January 2021 to December 2023 were selected.They were randomly divided into the control group and study group,with 23 cases in each group.Patients in the control group were given oral Montelukast sodium tablet,10 mg t.i.d.before sleeping,and those in the study group were given Zhichuanling oral liquid,10 mL daily,combined with oral Montelukast sodium tablet,10 mg t.i.d.All interventions were given for 2 weeks.Serological indicators,lung function indicators,fractional concentration of exhaled nitric oxide(FeNO),revised-asthma control questionnaire(rACQ)score,and adverse events of the two groups were recorded.Results Post-treatment eosinophil count(EOS),tumor necrosis factor-α(TNF-α),macrophage inflammatory protein-1α(MIP-1α),interleukin-4(IL-4),FeNO,and rACQ scores were all significantly lower than pre-treatment in both groups(P<0.05).Small airway function was significantly improved in both groups,including maximum expiratory flow at 50%forced vital capacity(MEF50),maximum expiratory flow at 25%forced vital capacity(MEF25),and maximum mid-expiratory flow curve(MMEF75/25),which were significantly improved in the study group than the control group(P<0.05).The total effective rate of the study group on the 3rd,7th and 14th days after treatment was significantly higher than that of the control group(P<0.05),which was more pronounced within the first 1 week.Conclusion Zhichuanling oral liquid combined with Montelukast sodium effectively treats chest tightness variant asthma,showing obvious improvement in small airway function and inflammatory factor levels.This combination is taken conveniently with a high safety.
Effect of symptom self-assessment-guided psychological counseling on improving negative psychology and optimizing quality of life in young cancer patientsAbstract:Objective To explore the effect of symptom self-assessment-guided psychological counseling on the negative psychological state and quality of life of young cancer patients.Methods A total of 70 young cancer patients from April 2022 to April 2023 were selected as the research subjects.The 70 patients were randomly divided into the control group and observation group using a random drawing method,with 35 cases per group.Patients in both groups received routine cancer management plans,while those in the observation group were additionally managed by the symptom self-assessment-guided psychological counseling.Psychological status,fatigue symptoms,quality of life,and other indicators were compared.Results Compared with pre-intervention values,cancer-related fatigue and stress perception scores after intervention significantly decreased in both groups,which were significantly lower in the observation group than the control group(P<0.05).The self-care ability score significantly increased in both groups,which were significantly higher in the observation group than the control group(P<0.05).Compared with pre-intervention values,depression scores and cancer recurrence fear scores after interventions significantly decreased in both groups,which were significantly lower in the observation group than the control group(P<0.05).Assessed by the hope level questionnaire,scores of the attitudes of taking positive action(12.34±2.15 vs 9.48±2.07),positive attitudes towards reality and future(12.87±2.16 vs 10.13±2.12)and attitudes of maintaining close relationships with others(12.59±2.08 vs 10.66±2.13)were significantly higher in the observation group than the control group,suggesting a significantly higher hope level in the observation group(P<0.05).Compared with pre-intervention values,scores of quality of life significantly increased in both groups,which were significantly higher in the observation group than the control group(P<0.05).Conclusion The symptom self-assessment-guided psychological counseling program can alleviate the negative psychological state of young cancer patients and improve their quality of life.
Expression of lncRNA WDFY3-AS2 in gastric cancer and its effect on biological behavior of gastric cancer cellsAbstract:Objective To test the expression of long non-coding RNA(lncRNA)WDFY3-AS2 in gastric cancer tissues and their adjacent normal tissues,and to investigate its effect on the biological behavior of gastric cancer cells.Methods Forty gastric cancer patients hospitalized in the Gastrointestinal Surgery from January 2020 to December 2023 were enrolled.Gastric cancer and adjacent normal tissues were intraoperatively harvested to detect the expression of lncRNA WDFY3-AS2 by real-time reverse transcription fluorescent quantitative polymerase chain reaction(RT-qPCR).The gastric cancer cell line BGC-823 with low expression of lncRNA WDFY3-AS2 was selected for subsequent experiments.Cells were divided into the control group and the lncRNA WDFY3-AS2 overexpression group.Proliferation was detected by cell counting kit-8(CCK-8)assay and colony formation assay.Cell invasion and migration ability were detected by Transwell and cell scratch assays.Results LncRNA WDFY3-AS2 was significantly lowly expressed in gastric cancer tissue than adjacent normal tissue(P<0.05).LncRNA WDFY3-AS2 was significantly lowly expressed in gastric cancer cell lines BGC-823 and SGC-7901 than the normal gastric mucosal epithelial cell line(P<0.05).Overexpression of lncRNA WDFY3-AS2 significantly inhibited cell viability,colony formation ability,migratory cell number and wound healing rate than the control group(P<0.05).Conclusion LncRNA WDFY3-AS2 may be used as a promising therapeutic target for gastric cancer.
Lumbar bone marrow fat and water content measured by MRI in patients with type 2 diabetes mellitusAbstract:Objective To compare lumbar bone marrow fat and water content between patients with type 2 diabetes mellitus(T2DM)and nondiabetic controls.Methods Forty-five patients with T2DM who were admitted to the Department of Endocrinology from December 2020 to December 2021 were selected as the research group,and forty-six nondiabetic individuals were selected as the control group at the same time,and the changes in lumbar bone marrow fat and water content quantified by magnetic resonance imaging(MRI)were compared between the two groups.Results There were no significant differences between groups in sex,age,disease duration,body mass index(BMI),or blood pressure(P>0.05).Fasting blood glucose,glycated hemoglobin(HbA1c),and insulin resistance index were significantly higher in the diabetic group than in controls(P<0.01).Lipid profiles,and measures of body fat and muscle content did not differ significantly between groups(P>0.05).Fat signal values measured in lumbar levels L1-L3 were significantly higher in diabetic patients than in nondiabetic subjects(P<0.05).Water signal values measured in lumbar levels L1-L4 were significantly lower in diabetic patients compared with controls(P<0.05).Conclusion Patients with T2DM exhibit increased lumbar bone marrow fat signal and decreased water signal compared with nondiabetic individuals.
Research progress on posterior root injury of meniscusAbstract:The tear of the posterior root of the meniscus causes the loss of circumferential stress in the meniscus,resulting in a decrease in the tibiofemoral contact area and an increase in the tibiofemoral contact pressure.This leads to secondary damage to the articular cartilage caused by meniscus protrusions,thereafter accelerating the progression of knee joint osteonecrosis and osteoarthritis.Therefore,an early diagnosis of posterior root tear of the meniscus is particularly important.In recent years,the importance of preserving meniscus function has once again been concerned.Posterior root tear used to be considered as an irreparable tearing,but it is currently being attempted for repair and has achieved good results.This study aims to review previous relevant literatures and summarize and analyze the etiology,biomechanical changes,clinical diagnosis,comprehensive treatment strategies,and postoperative rehabilitation of posterior meniscus root injury,in order to provide references for clinical decision-making.
Effect of early-pregnancy ultrasound indices on uterine receptivity at 6 months after treatment in women with Cesarean scar pregnancyAbstract:Objective To evaluate the effect of early-pregnancy ultrasound indices on uterine receptivity at 6 months after treatment in women with Cesarean scar pregnancy(CSP).Methods A total of 127 women with CSP admitted from January 2020 to December 2024 were enrolled.Based on the uterine receptivity score at 6 months after treatment,patients were divided into an abnormal group(n=37)and a normal group(n=90).Demographic characteristics and early-pregnancy ultrasound indices were compared between groups.Receiver operating characteristic(ROC)curve analysis assessed the predictive value of two quantitative Doppler indices—trophoblastic blood flow resistance index(RI)and uterine artery pulsatility index(PI)—for poor uterine receptivity at 6 months.Multivariable logistic regression was used to evaluate the independent effects of early-pregnancy ultrasound indices on uterine receptivity.Results Significant between-group differences were observed in age,interpregnancy interval,number of prior CSPs,history of uterine cavity surgery,scar myometrial thickness,gestational sac position and shape,thickness of the gestational sac-bladder myometrial layer,trophoblastic blood flow RI,and uterine artery PI(all P<0.05).ROC analysis showed that trophoblastic blood flow RI and uterine artery PI had good predictive value for poor uterine receptivity,with area under the curve(AUC)values of 0.84 and 0.82,respectively;optimal cutoffs were≤0.43 for RI and≤2.35 for PI.Multivariable logistic regression identified older age and prior uterine cavity surgery as risk factors for impaired uterine receptivity at 6 months,while scar myometrial thickness≥3.0 mm,higher trophoblastic blood flow RI,and higher uterine artery PI were protective factors.Conclusion Among early-pregnancy ultrasound indices,trophoblastic blood flow RI and uterine artery PI provide useful pre-assessment information for predicting poor uterine receptivity at 6 months after treatment in women with CSP.Increased scar myometrial thickness(≥3.0 mm),higher trophoblastic RI,and higher uterine artery PI have protective effects on uterine receptivity.
Correlation of protein levels of p-4E-BP1 and GLUT1 with the proliferation and invasion of ovarian cancerAbstract:Objective To investigate the correlation of protein levels of phosphorylated eukaryotic initiating factor 4E binding protein 1(p-4E-BP1)and glucose transporter 1(GLUT1)with the proliferation and invasion of ovarian cancer.Methods A total of 116 ovarian cancer patients diagnosed and treated from January 2019 to July 2020 were selected.Cancer tissue and normal adjacent tissues were intraoperatively collected.Western blot was used to detect protein levels of p-4E-BP1 and GLUT1 in ovarian cancer tissues and normal adjacent tissues.Real-time fluorescence quantitative polymerase chain reaction(RT-PCR)was used to detect the mRNA levels of proliferation-related genes(c-myelocytomatosis oncogene product[c-Myc],Cyclin D1)and invasion-related genes(ITGA2[Integrin Subunit Alpha 2]and CTHRC1[Collagen Triple Helix Repeat Containing 1]).Pearson correlation was used to analyze the correlation between the expression levels of p-4E-BP1 and GLUT1 in ovarian cancer tissue with the expression levels of proliferation and invasion-related genes.The correlation of p-4E-BP1 and GLUT1 levels in ovarian cancer tissue with clinical pathological characteristics was analyzed.Log-Rank test and Kaplan-Meier curve were used to illustrate the 3-year survival of ovarian cancer.Results Compared with normal adjacent tissue,p-4E-BP1 and GLUT1 in ovarian cancer tissue were significantly upregulated(P<0.05).Compared with normal adjacent tissue,the mRNA levels of Cyclin D1,c-Myc,CTHRC1,and ITGA2 in ovarian cancer tissue were significantly higher(P<0.05).Expression levels of p-4E-BP1 and GLUT1 in ovarian cancer tissue were positively correlated with those of Cyclin D1,c-Myc,CTHRC1,and ITGA2 in ovarian cancer tissue(P<0.05).The expression levels of p-4E-BP1 and GLUT1 in patients with FIGO stage Ⅲ-Ⅳ,lymph node metastasis,infiltration depth T3-T4,poorly differentiated tumors were significantly higher than those with FIGO stageⅠ-Ⅱ,no lymph node metastasis,infiltration depth T1-T2,and moderately well differentiated tumors(P<0.05).The 3-year overall survival of ovarian cancer patients with high expressions of p-4E-BP1 and GLUT1 was significantly lower than that of the low expression groups(P<0.05).Conclusion p-4E-BP1 and GLUT1 are upregulated in ovarian cancer tissue,which are related to abnormal expression of proliferation and invasion-related genes,advanced FIGO staging,lymph node metastasis,increased invasion depth,low tumor differentiation,and reduced 3-year overall survival.
Application of the KAP model combined with team behavior mutual assistance method in the continuous management of depression outside the hospitalAbstract:Objective To explore the role of the knowledge,attitude and practice(KAP)intervention model combined with team behavior mutual assistance method in outpatient management of depression.Methods A total of 97 depression patients hospitalized from June 2022 to June 2024 were selected as the research subjects.They were randomly divided into the study group and control group using a lottery method.The control group consisted of 49 patients who received routine in-hospital management and continued management,while the study group consisted of 48 patients who received the KAP model combined with team behavior mutual assistance method on this basis.The medication adherence,Beck Depression Inventory(BDI),Psychological Resilience Scale,Self-Aversion Scale,Insomnia Severity Index(ISI),Pittsburgh Sleep Quality Index(PSQI),and quality of life between two groups were compared.Results The medication adherence rate of the study group was significantly higher than that of the control group(P<0.01).There was no significant difference in depression symptoms and self-aversion between the two groups before interventions(P>0.05).After interventions,both groups showed decreased scores,and the study group had lower scores than the control group(P<0.01).There were no significant differences in the scores of various dimensions of psychological resilience between the two groups before interventions(P>0.05).After interventions,the scores of both groups increased,and scores in the study group were significantly higher than the control group(P<0.01).There were no significant differences in the ISI and PSQI scores between the two groups before interventions(P>0.05).After interventions,both groups showed decreased scores,and scores in the study group were significantly lower than the control group(P<0.01).Before interventions,there were no significant differences in the scores of various dimensions of quality of life between the two groups(P>0.05).After interventions,the quality of life scores of both groups increased,and scores were significantly higher in the study group than the control group(P<0.01).Conclusion The combination of KAP model and team behavior mutual assistance method can improve the depressive symptoms and medication compliance rate of patients with depression,and also enhance their psychological resilience,sleep quality,and quality of life,and alleviate their self-aversion.
Impact of sanguinarine in protecting neuronal damage in rats with ischemic stroke by regulating the RIPK1/RIPK3/MLKL axisAbstract:Objective To investigate the impact of sanguinarine(SAG)on neuronal damage in rats with ischemic stroke(IS)by regulating the receptor interacting protein kinase 1/3(RIPK1/RIPK3)/mixed lineage kinase domain-like protein(MLKL)axis.Methods A total of 72 specific pathogen free(SPF)Sprague-Dawley(SD)rats were randomly divided into Sham group,Model group,low-dose,medium-dose and high-dose SAG groups,and positive control group(nimodipine).The modified middle cerebral artery occlusion(MCAO)was performed in rats to prepare the IS model.Loeffler neurological score was graded to evaluate the nerve function and damage degree of rats.The area of cerebral infarction was measured by 2,3,5-triphenyltetrazolium chloride(TTC)staining.The water content of rat brain tissue was measured by weighing.Hematoxylin and eosin(H&E)staining staining was applied to observe the pathological damage of rat hippocampus.Terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL)staining was applied to observe the apoptosis of neurons in rats.The levels of interleukin(IL)-6,tumor necrosis factor-α(TNF-α)and IL-β in rat hippocampus were detected by Enzyme-linked immunosorbent assay(ELISA).Lactate dehydrogenase(LDH),catalase(CAT),malondialdehyde(MDA)and reactive oxygen species(ROS)in rat hippocampus were measured by commercial kits.Western blot was applied to detect the protein expressions of caspase-8,RIPK1,RIPK3 and MLKL in rat hippocampus.Results Compared with sham group,apoptosis rate of nerve cells,water content of brain tissue,percentage of cerebral infarction area,levels of IL-6,IL-β,and TNF-α,LDH release rate,contents of MDA,and ROS,and protein levels of RIPK1,RIPK3,and p-MLKL/MLKL in the model group were significantly higher,while the neurological score,CAT activity and protein level of caspase-8 were significantly lower(P<0.05).Compared with the model group,the neurological score,CAT activity and protein level of caspase-8 in the low-dose,medium-dose and high-dose SAG groups and the positive control group were significantly higher,while the percentage of cerebral infarction area,water content of brain tissue,apoptosis rate of nerve cells,levels of IL-6,IL-β,and TNF-α,LDH release rate,contents of MDA and ROS,and the protein levels of RIPK1,RIPK3,and p-MLKL/MLKL were significantly lower,and the damage of hippocampus was significantly reduced(P<0.05).Conclusion SAG can inhibit oxidative stress and inflammatory reaction,reduce the apoptosis of nerve cells,and play a neuroprotective role on IS rats by inhibiting the RIPK1/RIPK3/MLKL axis.
Effects of polycystic ovary syndrome on pregnancy and neonatal outcomes of single frozen-thawed blastocyst transferAbstract:Objective To evaluate the effects of polycystic ovary syndrome(PCOS)on the clinical outcome of patients with single frozen-thawed blastocyst transfer.Methods Clinical data of 303 cycles of patients who underwent conventional in vitro fertilization(IVF)assisted with single frozen-thawed blastocyst transfer at the Reproductive Medicine Department of Handan Central Hospital from November 2016 to August 2022 were retrospectively analyzed.Patients were divided into PCOS group(n=139)and control group(n=164)based on presence or absence of PCOS.Patients in the control group were non-PCOS,and had simple tubal infertility.Individuals with comorbidities were excluded.General information,embryonic development,pregnancy outcomes,and neonatal outcomes were compared between the two groups.Multivariate linear/Logistic regression analysis with confounder adjustment were performed to explore the correlation of PCOS with pregnancy and neonatal outcomes during thawing and transplantation cycles.Results The female age,basal follicle stimulating hormone(FSH),the total amount of gonadotropin(Gn)and the available embryo rate in the PCOS group were significantly lower than those of the control group(P<0.01).The infertility years,basic luteinizing hormone(LH),basic testosterone(T),anti-Mullerian hormone(AMH),primary infertility ratio and average number of eggs obtained were significantly higher than those of the control group(P<0.01).There were significant differences in the endometrial preparation regimens between the two groups(P<0.01).No significant differences in body mass index(BMI),basal estradiol(E2),Gn ovulation induction time,normal fertilization rate,endometrial thickness and proportion of transplanted blastocyst stage were found between the two groups(P>0.05).There was no significant difference in the pregnancy outcomes between the two groups(P>0.05).In terms of delivery and neonatal conditions,the weight of newborns in the PCOS group was significantly higher than that in the control group(P=0.025).Multivariate linear/Logistic regression analysis showed that after adjusting for confounding factors,there were no significant differences in clinical pregnancy outcomes,mode of delivery,and neonatal outcomes between the two groups of patients(P>0.05).Conclusion There is no significant correlation of PCOS with adverse pregnancy outcome and neonatal outcome when single blastocyst transplantation is performed in PCOS patients assisted by IVF during the thawing cycle,and PCOS patients can safely choose single frozen-thawed blastocyst transfer.
