Effects of polycystic ovary syndrome on pregnancy and neonatal outcomes of single frozen-thawed blastocyst transfer

Abstract:Objective To evaluate the effects of polycystic ovary syndrome(PCOS)on the clinical outcome of patients with single frozen-thawed blastocyst transfer.Methods Clinical data of 303 cycles of patients who underwent conventional in vitro fertilization(IVF)assisted with single frozen-thawed blastocyst transfer at the Reproductive Medicine Department of Handan Central Hospital from November 2016 to August 2022 were retrospectively analyzed.Patients were divided into PCOS group(n=139)and control group(n=164)based on presence or absence of PCOS.Patients in the control group were non-PCOS,and had simple tubal infertility.Individuals with comorbidities were excluded.General information,embryonic development,pregnancy outcomes,and neonatal outcomes were compared between the two groups.Multivariate linear/Logistic regression analysis with confounder adjustment were performed to explore the correlation of PCOS with pregnancy and neonatal outcomes during thawing and transplantation cycles.Results The female age,basal follicle stimulating hormone(FSH),the total amount of gonadotropin(Gn)and the available embryo rate in the PCOS group were significantly lower than those of the control group(P<0.01).The infertility years,basic luteinizing hormone(LH),basic testosterone(T),anti-Mullerian hormone(AMH),primary infertility ratio and average number of eggs obtained were significantly higher than those of the control group(P<0.01).There were significant differences in the endometrial preparation regimens between the two groups(P<0.01).No significant differences in body mass index(BMI),basal estradiol(E2),Gn ovulation induction time,normal fertilization rate,endometrial thickness and proportion of transplanted blastocyst stage were found between the two groups(P>0.05).There was no significant difference in the pregnancy outcomes between the two groups(P>0.05).In terms of delivery and neonatal conditions,the weight of newborns in the PCOS group was significantly higher than that in the control group(P=0.025).Multivariate linear/Logistic regression analysis showed that after adjusting for confounding factors,there were no significant differences in clinical pregnancy outcomes,mode of delivery,and neonatal outcomes between the two groups of patients(P>0.05).Conclusion There is no significant correlation of PCOS with adverse pregnancy outcome and neonatal outcome when single blastocyst transplantation is performed in PCOS patients assisted by IVF during the thawing cycle,and PCOS patients can safely choose single frozen-thawed blastocyst transfer.
