Analysis of the Role of Ulinastatin Combined with Anti-infective Drugs in the Regulation of Inflammatory Factors in Patients with Biliary Severe Acute PancreatitisAbstract:Objective To investigate the effects of ulinastatin combined with anti-infective drugs on inflamma-tory factors, immune function, disease severity, and clinical efficacy in patients with biliary severe acute pancreatitis(SAP).Methods A total of 120 biliary SAP patients admitted to our hospital from January 2021 to December 2024 were selected as the study subjects and randomly divided into a control group and an observation group using a random number table, with 60 cases in each group. The control group received conventional treatment(including fasting, fluid replacement, acid suppression, etc.)combined with anti-infective drugs(ceftriaxone sodium+metronidazole);the observation group re-ceived ulinastatin in addition to the control group's treatment. The treatment course for both groups was 7~10 days.Chang-es in inflammatory factors(TNF-α,IL-6,IL-10),immunoglobulins(IgA,IgM,IgG),and pancreatic enzyme indicators(serum amylase, lipase)were compared between the two groups before and after treatment. The APACHE II score was used to assess disease severity before treatment and on days 1,3,and 7 after treatment. The total clinical effective rates were com-pared between the two groups. Results After treatment, the levels of TNF-α,IL-6,serum amylase, and lipase in the ob-servation group were significantly lower than those in the control group, while the levels of IL-10,IgA,IgM,and IgG were significantly higher(all P<0.001).The APACHE II scores of the observation group on days 1,3,and 7 post-treatment were significantly lower than those of the control group(day 1:P=0.032;day 3:P=0.001;day 7:P<0.001).The overall clinical effective rate was significantly higher in the observation group compared to the control group(95.00%vs.80.00%,P=0.013).Conclusion In the treatment of patients with biliary severe acute pancreatitis, ulinastatin com-bined with anti-infective drugs can effectively control inflammatory factor levels, enhance immune function, effectively reduce serum amylase and lipase levels, and achieve significant clinical efficacy.
Experimental Study on the Radiation Shielding Effectiveness of Different Metal Substrate and Coating CombinationsAbstract:Objective This study evaluates the radiation shielding performance of different combinations of metal substrates and ceramic coatings, focusing on the application potential of lead-free and low-lead ceramic coatings in radiation protection. The goal is to provide scientific evidence for the development of environmentally friendly and efficient radiation shielding materials. Methods Four metal substrates of aluminum,copper,stain-less steel and titanium were selected with lead-free,lead-free,composite and nano-ceramic coatings, and the sam-ples were prepared by spraying method. The transmission dose rate(I)and photon attenuation rate(μ)of different materials were measured under 80 kV,120 kV,160 kV X-ray and 137Cs y-ray conditions, respectively.Lead equiv-alent was calculated to evaluate the shielding effect, and Pearson correlation analysis was performed to determine the linear relationship between the data. Results At 80 kV X-ray conditions, there is a positive correlation be-tween I and μ in most samples, and a strong positive correlation between lead equivalent and μ.Under the condition of 120 kV X-ray,the I values of B2 and C2 of lead-free ceramic coating samples are both low,520 μSv/h and 510μSv/h,respectively,while the μ values of both samples are high,0.70 mm 1 and 0.72 mm-1,respectively,indi-cating that the shielding properties of lead-free ceramic coating samples are strong. However, the I of low-lead ceramic coating sample D1 is the highest, reaching 710 μSv/h,μ is 0.35 mm-1,and lead equivalent is 0.23 mmPb,indicating poor attenuation effect.Under the condition of 160 kV X-ray,I of all samples is generally higher, and the correlation coefficient is concentrated between 0.60 and 0.79,indicating that all samples show higher I and μ,and the increase of μ directly affects the increase of I.In the 137Cs y-ray condition, the I of y-rays is generally lower and the μ is generally higher than that of X-rays.Under 137Cs γ-ray conditions, the I,μ and Pb equivalent of B2 are 500 μSv/h,0.76 mm 1 and 0.50 mmPb respectively, indicating that B2 has strong attenuation to y-rays.In con-trast,the low-lead ceramic coating sample D1 has an I of 690 μSv/h,a μ of 0.40 mm-1,and a lead equivalent of 0.26 mmPb,indicating better y-ray transmission and weak attenuation effect. Conclusion Lead-free ceramic coatings show superior radiation protection ability under experimental conditions, especially the shielding effect of high-en-ergy X-rays and y-ray is better than that of traditional low-lead coatings.

Clinical Efficacy Study of High-Flow Nasal Cannula Oxygen Therapy in Treating Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Type Ⅱ Respiratory FailureAbstract:Objective To investigate the clinical efficacy of high-flow nasal cannula oxygen therapy(HFNC)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated by type Ⅱ respiratory failure. Methods A total of 150 patients with AECOPD and type Ⅱ respiratory failure admitted to the Third People's Hospital of Bengbu City,Anhui Province, from August 2023 to January 2025 were selected as the study subjects.Using a random number table method, all patients were divided into an observation group(HFNC)and a control group(non-invasive positive pressure ventilation),with 75 patients in each group.Blood gas parameters, pulmonary function, and adverse reac-tions were compared between the two groups. Results Before treatment, there were no significant differences in arterial partial pressure of oxygen(PaO₂),arterial partial pressure of carbon dioxide(PaCO₂),pH value, forced expiratory volume in one second(FEV₁),forced vital capacity(FVC),or the FEV₁/FVC ratio between the two groups(P>0.05).After 7 days of treatment, both groups showed significant increases in PaO₂,pH,FEV₁,FVC,and FEV₁/FVC,and a significant decrease in PaCO₂(all P<0.05).Moreover,the observation group had significantly higher PaO₂,pH,FEV₁,FVC,and FEV₁/FVC,and significantly lower PaCO₂ than the control group(P<0.001).The relief times for dyspnea, cough, and expectoration in the observation group were significantly shorter than those in the control group(P<0.001).The incidence of adverse reactions in the observation group was sig-nificantly lower than that in the control group(10.67%vs.26.67%,P<0.05).Conclusion In the treatment of patients with AECOPD and type Ⅱ respiratory failure,HFNC can effectively improve blood gas parameters and respiratory function, promote the relief of clinical symptoms, and exhibits high safety.
Application Demand Analysis of Artificial Intelligence Robots in Medical Service SupportAbstract:Modern warfare is profoundly evolving towards intellectualization,multi-domainization,and hybridiza-tion,leading to complex casualty conditions,pan-domain distribution, and stringent requirements for treatment timeliness. This has plunged traditional medical service systems into disruptive challenges such as resource shortages, delayed evac-uation,and functional failure in extreme environments. The deep integration of artificial intelligence robot technology is a key and essential path to break this predicament.Through the analysis of the evolution of warfare modes and the pain points in traditional treatment, this paper introduces the advantages of artificial intelligence robots in various stages such as battlefield search and rescue, evacuation, and treatment, and deeply analyzes their application demands in medical service support, providing a theoretical basis and important reference for constructing a modern and intelligent medical service support system.
Arthroscopic-Assisted versus Traditional Open Reduction and Internal Fixation in the Treatment of Ankle Fractures:A Comparison of EfficacyAbstract:Objective To investigate the clinical value of arthroscopic-assisted reduction and fixation in the treatment of ankle fractures. Methods Sixty patients with ankle fractures admitted to Lianjiang County General Hospital from March 2022 to March 2025 were selected as the study subjects.According to the different methods of open reduction and internal fixation, they were divided into a control group(traditional open reduction and internal fixation,30 cases)and an observation group(arthroscopic-assisted reduction and fixation,30 cases).Postoperative pain levels, ankle joint function and range of motion recovery, and the incidence of complications were compared between the two groups. Results After treatment, the VAS scores in both groups showed a decreasing trend.Compared with the control group, the VAS scores in the observation group were significantly lower on postoperative day 1(P=0.023),day 3(P=0.001),and day 5(P<0.001).After treatment, the AOFAS scores, plantar flexion range of motion, and dorsiflexion range of motion in both groups showed an increasing trend.Compared with the control group, the AOFAS scores, plantar flexion range of motion, and dorsiflexion range of motion in the observation group were significantly higher at 1 month,3 months,6 months, and 12 months postoperatively(P<0.05).The total incidence of complications in the observation group was significantly lower than that in the control group(10.00%vs.33.33%,P=0.028).Conclusion In the treatment of ankle fracture patients, arthroscopically assisted reduction and fixation is superior to traditional open reduction and internal fixation in terms of alleviating postoperative pain, promoting ankle func-tion recovery, and reducing the risk of postoperative complications.
