Analysis of the Role of Ulinastatin Combined with Anti-infective Drugs in the Regulation of Inflammatory Factors in Patients with Biliary Severe Acute Pancreatitis

Abstract:Objective To investigate the effects of ulinastatin combined with anti-infective drugs on inflamma-tory factors,immune function,disease severity,and clinical efficacy in patients with biliary severe acute pancreatitis(SAP).Methods A total of 120 biliary SAP patients admitted to our hospital from January 2021 to December 2024 were selected as the study subjects and randomly divided into a control group and an observation group using a random number table,with 60 cases in each group.The control group received conventional treatment(including fasting,fluid replacement,acid suppression,etc.)combined with anti-infective drugs(ceftriaxone sodium+metronidazole);the observation group re-ceived ulinastatin in addition to the control group's treatment.The treatment course for both groups was 7~10 days.Chang-es in inflammatory factors(TNF-α,IL-6,IL-10),immunoglobulins(IgA,IgM,IgG),and pancreatic enzyme indicators(serum amylase,lipase)were compared between the two groups before and after treatment.The APACHE II score was used to assess disease severity before treatment and on days 1,3,and 7 after treatment.The total clinical effective rates were com-pared between the two groups.Results After treatment,the levels of TNF-α,IL-6,serum amylase,and lipase in the ob-servation group were significantly lower than those in the control group,while the levels of IL-10,IgA,IgM,and IgG were significantly higher(all P<0.001).The APACHE II scores of the observation group on days 1,3,and 7 post-treatment were significantly lower than those of the control group(day 1:P=0.032;day 3:P=0.001;day 7:P<0.001).The overall clinical effective rate was significantly higher in the observation group compared to the control group(95.00%vs.80.00%,P=0.013).Conclusion In the treatment of patients with biliary severe acute pancreatitis,ulinastatin com-bined with anti-infective drugs can effectively control inflammatory factor levels,enhance immune function,effectively reduce serum amylase and lipase levels,and achieve significant clinical efficacy.
