Abstract:Objective To explore the high-risk factors and key points in the clinical diagnosis and treatment of ovarian corpus luteum rupture. Methods A total of 84 patients with gynecological acute abdomen admitted to Longyan People's Hospital from April 2022 to April 2024 were selected as the study subjects, including 49 patients with ovarian corpus luteum rupture and 35 patients with ruptured ectopic pregnancy. The general data, abdominal pain characteristics, vaginal bleeding and shock status, ultrasonographic findings, hematological indicators, treatment, and prognosis were com-pared between the two groups. Results Patients in both groups primarily presented with unilateral lower abdominal pain(P>0.05).However,the corpus luteum rupture group had significantly lower VAS scores for abdominal pain,a higher pro-portion of dull pain, and a shorter duration of pain(P<0.001).The incidence of vaginal bleeding and shock in the corpus luteum rupture group was significantly lower than that in the ruptured ectopic pregnancy group(P<0.05).Ultrasonography showed that the mass in the corpus luteum rupture group was smaller(P<0.001)and more frequently exhibited annular or semi-annular blood flow signals(P=0.001).Regarding hematological indicators, the serum β-HCG was negative in the cor-pus luteum rupture group, the decrease in hemoglobin was smaller(P<0.001),and the increase in white blood cell count was lower(P<0.001).In terms of treatment, the conservative treatment rate was higher in the corpus luteum rupture group(P<0.05).There was no significant difference in the total effective rate or complication rate between the two groups(P>0.05).Conclusion Differentiating ovarian corpus luteum rupture from ruptured ectopic pregnancy requires comprehen-sive consideration.Corpus luteum rupture is charac-terized by negative serum β-HCG,relatively mild and transient abdominal pain, lower incidence of vaginal bleeding and shock, smaller adnexal mass on ultrasonography often with annular blood flow signals, and a lesser degree of hemoglobin decrease.Clinical diagnosis and treatment should inte-grate the above characteristics to individualize the choice between conservative and surgical management.
