Analysis of Factors Influencing Short-Term Prognosis in Patients with Spontaneously Ruptured Hepatocellular Carcinoma Undergoing Emergency Transcatheter Arterial Chemoembolization

Abstract:Objective To analyze the independent risk factors influencing the short-term prognosis of patients with spontaneously ruptured hepatocellular carcinoma(HCC)undergoing emergency transcatheter arterial chemoembolization(TACE).Methods The clinical data of 60 patients with spontaneously ruptured HCC who underwent emergency TACE at our hospital from March 2022 to March 2025 were retrospectively analyzed.Based on their survival status at 3 months postoperative-ly,they were divided into a favorable prognosis group(43 cases)and a poor prognosis group(17 cases).Baseline characteristics were compared between the two groups,and univariate and multivariate Logistic regression analyses were performed to identify independent risk factors affecting short-term prognosis.Results Univariate analysis showed that tumor diameter,Child-Pugh grade,TNM stage,alpha-fetoprotein(AFP),white blood cell count(WBC),hemoglobin(Hb),and prothrombin time(PT)were significantly different between the two groups(P<0.05).Multivariate Logistic regression analysis confirmed that increased tu-mor diameter(OR=1.610,95%CI:1.225~2.107),elevated Child-Pugh grade(OR=1.624,95%CI:1.263~2.091),and decreased Hb level(OR=0.658,95%CI:0.487~0.832)were independent risk factors for poor prognosis(P<0.001).Conclusion Large tumor diameter,high Child-Pugh grade,and low admission Hb level are independent risk factors for poor short-term prognosis following emergency TACE in patients with ruptured HCC.Clinical attention should focus on high-risk patients with these char-acteristics,and comprehensive intervention measures should be implemented to improve their prognosis.
