A Comparative Study on the Efficacy of Expandable Intramedullary Nails versus Interlocking Intramedullary Nails in the Treatment of Adult Traumatic Femoral Shaft Fractures

Abstract:Objective To compare the clinical efficacy of expandable intramedullary nails and traditional interlock-ing intramedullary nails in the treatment of adult traumatic femoral shaft fractures.Methods A total of 80 patients with traumatic femoral shaft fractures admitted to our hospital from January 2022 to December 2024 were selected and random-ly divided into an observation group(40 cases,treated with expandable intramedullary nail fixation)and a control group(40 cases,treated with interlocking intramedullary nail fixation).The operative time,hospitalization time,fracture healing time,joint function recovery time,postoperative visual analog scale(VAS)pain scores,Lower Extremity Functional Scale(LEFS)scores,modified Barthel index,and complication rates were compared between the two groups.Results The operative time,hospitalization time,fracture healing time,and joint function recovery time in the observation group were significant-ly shorter than those in the control group(all P<0.001).At 1 day,3 days,7 days,and 1 month postoperatively,the VAS scores in the observation group were significantly lower than those in the control group(all P<0.05).At 3 months and 6 months postoperatively,the LEFS scores and modified Barthel index in the observation group were significantly higher than those in the control group(all P<0.05).There was no statistically significant difference in the overall complication rate between the two groups(observation group:2.50%vs.control group:10.00%,P>0.05).Conclusion Compared with traditional interlocking intramedullary nails,the use of expandable intramedullary nails in the treatment of adult trau-matic femoral shaft fractures significantly shortens the surgical and rehabilitation process,effectively reduces early postoperative pain,promotes the recovery of lower limb function and daily living activities,and does not increase the risk of complications.It is a safe and effective minimally invasive internal fixation option.
