Comparison of Mid-Term Efficacy between Early Minimally Invasive Surgery and Standardized Conservative Treatment for Osteoporotic Vertebral Compression Fractures in the Elderly

Abstract:Objective To compare the mid-term efficacy of early minimally invasive surgery[percutaneous ver-tebroplasty(PVP)/percutaneous kyphoplasty(PKP)]versus standardized conservative treatment in elderly patients with osteoporotic vertebral compression fractures(OVCF).Methods The clinical data of 60 elderly OVCF patients admitted to Zhao'an County Hospital,Fujian Province,from January 2017 to December 2024 were retrospectively analyzed.Based on the treatment regimen,the patients were divided into an observation group(early minimally invasive surgery group,30 cases)and a control group(standardized conservative treatment group,30 cases).The time to ambulation,length of hospi-tal stay,visual analog scale(VAS)score for pain,lumbar range of motion(ROM),Oswestry Disability Index(ODI)score,36-Item Short Form Health Survey(SF-36)score,complication rates,and incidence of long-term adverse events were com-pared between the two groups.Results The time to ambulation and length of hospital stay in the early minimally inva-sive surgery group were significantly shorter than those in the standardized conservative treatment group(P<0.001).At 1 month and 3 months after treatment,the VAS scores in the early minimally invasive surgery group were significantly lower than those in the conservative treatment group(P<0.001).At 3 months after treatment,the lumbar flexion angle,extension angle,and SF-36 scores in the early minimally invasive surgery group were significantly higher than those in the conserva-tive treatment group(P<0.001),while the ODI score was significantly lower than that in the conservative treatment group(P<0.001).The total incidence of peritreatment com-plications(6.67%vs.26.67%,P=0.038)and the total incidence of long-term adverse events within the 12-month follow-up period(10.00%vs.26.67%,P=0.044)were signifi-cantly lower in the early minimally invasive surgery group than in the standardized conservative treatment group.Con-clusion For elderly OVCF patients without neurological impairment,compared with standardized conservative treatment,early minimally invasive surgery can relieve pain more rapidly,promote functional recovery,shorten hospital stay,and effectively reduce the risks of peritreatment complications and mid-term adverse events.
