Clinical Efficacy Study of High-Flow Nasal Cannula Oxygen Therapy in Treating Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Type Ⅱ Respiratory Failure

Abstract:Objective To investigate the clinical efficacy of high-flow nasal cannula oxygen therapy(HFNC)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated by type Ⅱ respiratory failure.Methods A total of 150 patients with AECOPD and type Ⅱ respiratory failure admitted to the Third People's Hospital of Bengbu City,Anhui Province,from August 2023 to January 2025 were selected as the study subjects.Using a random number table method,all patients were divided into an observation group(HFNC)and a control group(non-invasive positive pressure ventilation),with 75 patients in each group.Blood gas parameters,pulmonary function,and adverse reac-tions were compared between the two groups.Results Before treatment,there were no significant differences in arterial partial pressure of oxygen(PaO₂),arterial partial pressure of carbon dioxide(PaCO₂),pH value,forced expiratory volume in one second(FEV₁),forced vital capacity(FVC),or the FEV₁/FVC ratio between the two groups(P>0.05).After 7 days of treatment,both groups showed significant increases in PaO₂,pH,FEV₁,FVC,and FEV₁/FVC,and a significant decrease in PaCO₂(all P<0.05).Moreover,the observation group had significantly higher PaO₂,pH,FEV₁,FVC,and FEV₁/FVC,and significantly lower PaCO₂ than the control group(P<0.001).The relief times for dyspnea,cough,and expectoration in the observation group were significantly shorter than those in the control group(P<0.001).The incidence of adverse reactions in the observation group was sig-nificantly lower than that in the control group(10.67%vs.26.67%,P<0.05).Conclusion In the treatment of patients with AECOPD and type Ⅱ respiratory failure,HFNC can effectively improve blood gas parameters and respiratory function,promote the relief of clinical symptoms,and exhibits high safety.
