Qualitative study on dietary management of maintenance hemodialysis patients for over 5 yearsAbstract:Objective To observe daily dietary management of long-term maintenance hemodialysis(MHD),aiming to develop targeted interventions for such patients.Methods Fourteen MHD patients for over 5 years were selected using the purposive sampling and given face-to-face,semi-structured in-depth interviews.The data were collected,analyzed,summarized and extracted themes using the Nvivo 15.0 software.Results Two main themes and nine sub-themes were identified.Theme one was of the personal level,including the six sub-themes of gradual neglect of dietary management,the impact of dietary restrictions on life quality,difficulties in keeping a personal food log,negative emotions and dietary management during dialysis,the impact of impaired self-esteem on dietary management and the need for personalized dietary management.The second theme was of the social and family level,covering three sub-themes:the impact of medical cost burdens on dietary management,the importance of family support for dietary management and the impact of the effectiveness of information on dietary management.Conclusion MHD patients over 5 years face multiple challenges in dietary management,such as dietary restrictions,emotional fluctuations,economic pressure,and difficulties in obtaining information.The patients' compliance,support from their families and guidance from healthcare providers are needed to solve such problems and achieve better dietary management.
Application of Learn-Think-Express combined with I-E-O teaching model in clinical training of emergency department intern nursing studentsAbstract:Objective To explore the application effect of Learn-Think-Express combined with the I-E-Oteaching model in clinical teaching of emergency nursing students.Methods A total of 76 nursing students who practiced in the emergency department of Wuhan central hospital from October 2023 to March 2024 were selected and divided into an experimental group and a control group,each of 38,according to the parity of their rotation months.The control group was given the traditional clinical teaching,while the experimental group underwent Learn-Think-Express combined with the I-E-Oteaching for 4 weeks.Then,the theoretical knowledge scores,practical skill scores,and core job competency were compared between the two groups.Results After training,the theoretical knowledge and practical skill scores of the experimental group were significantly higher than those of the control group(t=-5.483,P<0.001;t=-5.103,P<0.001).Moreover,the scores of all dimensions and the total score of core competency were also significantly better than those of the control group(all P<0.001).Conclusion The new teaching method can significantly improve the theoretical knowledge,practical skills and core job competency of emergency nursing students,and it is worth clinical promotion.

Construction of a prediction model for sarcopenia risk in patients with coronary heart disease and its validationAbstract:Objective To explore the influencing factors of sarcopenia in discharged coronary heart disease patients,so as to construct a predictive model.Methods A total of 472 patients with coronary heart disease who received treatment in the cardiovascular medicine departments of three general hospitals in Shandong province between June 2023 and July 2023 were selected as the study subjects by convenience sampling.Logistic regression was used to screen for influencing factors,while R 4.1.2 software was employed to construct a risk prediction model and draw a nomogram to visualize the model.Then the predictive performance of the model was validated using the receiver operating characteristics(ROC)and Hosmer-Lemeshow test.Results Among the 330 cases in the modeling group,73 cases developed sarcopenia,with an incidence of 22.12%.Logistic regression analysis showed that age(OR=1.054),Charlson comorbidity index(OR=1.413),body mass index(OR=0.545),albumin(OR=0.485),diabetes(OR=2.144),nutritional status(OR=2.003),NYHA cardiac function grade(OR=1.685)were the influencing factors for sarcopenia in patients with coronary heart disease after discharge.The area under the receiver operating characteristic curve was 0.820 in internal validation and 0.837 in external validation.The Hosmer-Lemeshow test results showed:x2=14.281(P=0.075)for internal validation and x2=7.734(P=0.460)for external validation.Conclusion The risk prediction model for sarcopenia in coronary heart disease patients constructed in this study has a good predictive effect,which can help clinical nursing staff identify high-risk patients early and provide guidance for timely intervention.
Research hotspots and trends of continuous care among stroke survivors in China based on CiteSpaceAbstract:Objective To explore the research hotspots and trends of continuous care for stroke survivors in China using CiteSpace visualization analysis,so as to provide references for its better implementation in the future.Methods Articles on continuous care for stroke patients were retrieved from CNKI,VIP,Wanfang,and China Biomedical Literature Database,with the time from inception to May 1,2024.Then,CiteSpace 6.3.R1 was used to analyze the authors,publishing institutions and keywords,and draw the maps.Results A total of 1 882 articles were included.The annual publication volume showed an upward trend initially and then declined from 2009 to 2024.There were 17 core authors,but no core author group was formed.The research institution with the most publications was the fifth affiliated hospital of Xinjiang medical university,and the connections among institutions were not close.Moreover,the research hotspots mainly focused on intervention effect,psychological state and compliance of patients,with the research frontiers being swallowing function and home-based rehabilitation.Conclusion The research on continuous care for stroke patients in China is in a period of rapid development,but the cooperation among authors and institutions is not close enough.Therefore,in the future,it is necessary to strengthen cross-regional and cross-institutional cooperation and exchanges to promote its implementation and development.
The application effect of the early warning supervision mechanism combined with contradictory intention training in patients with severe pneumonia complicated with respiratory failure undergoing mechanical ventilationAbstract:Objective To explore the application effect of the early warning supervision mechanism combined with contradictory intention training in patients with severe pneumonia complicated with respiratory failure undergoing mechanical ventilation.Methods A total of 90 severe pneumonia patients with respiratory failure undergoing mechanical ventilation and admitted to the first people's hospital of Jiujiang city,Jiangxi province between October 2022 and September 2024 were selected as the research objects.They were randomly divided into a control group and an experimental group,each of 45,according to a random number table.Both groups were given conventional mechanical ventilation care,while the experimental group was additionally offered early warning supervision mechanism care combined with contradictory intention training.The hope level,treatment compliance,occurrence of complications and rehabilitation process of the two groups were compared.Results After the intervention,the hope level and treatment compliance of the experimental group were significantly higher than those of the control group(t=-35.410,P<0.001;Z=-4.534,P<0.001),while the incidence of complications was significantly lower(x2=4.865,P=0.027),with the duration of mechanical ventilation and the total hospital stay significantly shorter than that of the control group(t=7.833,P<0.001;t=6.268,P<0.001).Conclusion The early warning supervision mechanism combined with contradictory intention training can improve the patients' hope levels and treatment compliance,and reduce the incidence of complications,promoting their rehabilitation.
The application of family-participatory care and risk warning management in elderly patients after PCI for acute myocardial InfarctionAbstract:Objective To explore the application effect of family-participatory care and risk warning management in elderly patients after percutaneous coronary intervention(PCI)for acute myocardial infarction(AMI).Methods By using the convenience sampling method,80 elderly AMI patients admitted to the department of cardiology of Wuxi people's hospital affiliated to Nanjing medical university between March and December 2023 were selected as the research objects.They were randomly divided into a control group and an observation group,each of 40,according to a random number table.Both groups were given routine rehabilitation care after PCI,while the observation group was additionally provided with family-participatory care and risk warning management.The cardiac function,exercise endurance,life quality and the incidence of adverse cardiovascular events were compared between the two groups.Results After the intervention,the left ventricular ejection fraction,left ventricular end systolic diameters,left ventricular end-diastolic diameter and cardiac output of the observation group were better than those of the control group(t=-6.652,P<0.001;t=7.324,P<0.001;t=6.756,P<0.001;t=-7.231,P<0.001),6-minute walking distance was longer than the latter(t=-3.537,P<0.001),and each dimension of life quality of the observation group were all significantly better than those of the control group(P<0.001 for all),while the incidence of adverse cardiovascular events was significantly lower than the latter(x2=5.000,P=0.025).Conclusion The adoption of family-participatory care and risk warning management for elderly AMI patients after PCI is beneficial for improving their cardiac function and exercise endurance,enhancing their quality of life,and reducing the incidence of adverse cardiovascular events.It is worthy of clinical promotion and application.
Summary of the best evidence for the management of swallowing disorders in patients with amyotrophic lateral sclerosisAbstract:Objective To summarize the relevant evidence on the management of dysphagia in patients with amyotrophic lateral sclerosis(ALS),so as to provide a basis for formulating targeted management strategies in the clinical practice.Methods Articles on the management of dysphagia in ALS patients were searched systematically from relevant domestic and foreign guideline websites,professional association websites,and Chinese and English databases,with the time setting from inception till October 31,2024.After the quality evaluation,the articles were subjected to evidence extraction,induction and summary.Results Ultimately,14 articles were found,including 8 guidelines,3 clinical decisions,1 expert consensus,and 2 systematic reviews.Finally,a total of 24 best pieces of evidence were formed in six aspects:multidisciplinary team collaboration,screening and evaluation,swallowing rehabilitation training,nutrition management,oral care and health education.Conclusion The best evidence on the management of swallowing disorders in patients with ALS can provide evidence-based reference for clinical practice.It is recommended that medical staff should assess the clinical context and combine professional judgement in the process of translating the evidence into practice,thus to provide scientific management of dysphnagia for patients.
The application of childbirth virtual simulation software based on ARCS model in standardized trainning for obstetric nurses with higher vocational educationAbstract:Objective To explore the effect of the childbirth virtual simulation training program based on the ARCS motivation model in the standardized clinical training for obstetric nurses with higher vocational education.Methods Sixty-four obstetric nurses to undergo standardized clinical training at Hebi people's hospital in Henan province were selected as the study subjects.Thirty-two nurses trained in the first 3 month of 2023 were chosen into the control group,while the rest 32 taught in the following 3 months were selected into the observation group.The control group was trained in the traditional method,while the observation group was provided with virtual simulation training based on the ARCS motivation model.After training,the theoretical and practical assessment scores of both groups were compared,along with their training satisfaction.Results The observation group achieved significantly better scores in theoretical and practical skill assessments than those of the control group(t=-4.139,P<0.001;t=-6.594,P<0.001),with significantly higher satisfaction than the latter(t=-18.810,P<0.001).Conclusion The virtual simulation training based on the ARCS model can enhance trainees' mastery of theoretical knowledge and practical skills,thereby improving their training satisfaction.
Health promotion behavior management of patients after percutaneous coronary intervention based on integrated theory of health behavior change:a qualitative studyAbstract:Objective Based on the integrated theory of health behavior change,this paper explores the current status of health promotion behavior management in patients after percutaneous coronary intervention(PCI),so as to provide references for promotion.Methods Using phenomenological research methods,15 patients who underwent PCI in department of cardiology the second affiliated hospital between March and April 2024 were selected for semi-structured interviews,and the data collected were analyzed using the Colaizzi 7-step method.Results Four main themes were extracted,including insufficient awareness of diseases,self-efficacy guiding management belief,self-regulation ability needing improvement,and multiple factors influencing health promotion behaviors.Conclusion Healthcare professionals should implement personalized education,to enhance patients' self-efficacy,guide them to establish positive beliefs and cultivate their self-regulation skills,so as to improve their health promotion behaviors.

The effect of ERAS-based intensive muscle training on postoperative motor function and life quality of patients with lower limb venous malformationsAbstract:Objective To explore the effect of enhanced recovery after surgery(ERAS)-based intensive muscle training on postoperative motor function and life quality of patients with lower limb venous malformation.Methods Totally 152 patients with lower limb venous malformation and admitted to our hospital between June 2023 and June 2024 were selected and randomly divided into an observation group and a control group,each of 76.Both groups underwent conventional rehabilitation training,while the observation group was additionally provided with ERAS-based intensive muscle training.Both groups were evaluated the lower limb motor function using the Fugl-Meyer motor score and the American Orthopaedic Foot Society ankle-foot joint function score(AOFAS),the life quality using the arthritis impact measurement scale 2-short form(AIMS2-SF),the pain using the visual analogue scale(VAS).Moreover the incidence of postoperative complications were recorded.Results At 4 weeks postoperatively,the Fugl-Meyer motor score,AOFAS score of the observation group were significantly higher than that of the control group(t=-2.873,P=0.005;t=-3.318,P<0.001).At 2 weeks of discharge,the AIMS2-SF score was higher than that of the control group(t=-9.520,P<0.001),while the VAS score was significantly lower than the latter(Fintergroup=9.187,P=0.003).However,during the intervention,no significant differences were found between the two groups in the occurrence of postoperative complications(x2=0.599,P=0.439).Conclusion ERAS-based intensive muscle training is effective for the postoperative recovery of patients with lower limb venous malformations,which improves the patients' motor function and life quality,and relieves their postoperative pain.
Frailty status of stroke survivors and its influencing factors:a Meta-analysisAbstract:Objective To evaluate systematically the influencing factors of frailty in stroke survivors.Methods Articles on the influencing factors of frailty in stroke survivors were searched in Chinese and English databases,with the time setting from the inception till June 1,2024.Literature screening,quality evaluation and data extraction were conducted independently by two researchers,and Meta-analysis was performed using RevMan 5.3 and Stata 17.0 software.Results A total of 14 articles were included.The results of Meta-analysis showed that age increasing[OR=1.07,95%CI(1.06,1.09)],old age[OR=3.83,95%CI(2.80,5.24)],disorder daily activites[OR=2.59,95%CI(1.34,5.00)],diabetes[OR=3.99,95%CI(2.23,7.15)],malnutrition[OR=6.13,95%CI(2.93,12.82)],high national institutes of health stroke scale score[OR=3.52,95%CI(2.18,5.68)],sleep disorder[OR=3.22,95%CI(1.66,6.25)],depression[OR=4.79,95%CI(1.96,11.72)],swallowing disorder[OR=2.31,95%CI(1.36,3.91)],stroke frequency[OR=4.39,95%CI(2.69,7.17)]and physical exercise[OR=0.33,95%CI(0.21,0.50)]were influencing factors of frailty in stroke survivors.Conclusion There are many factors affecting stroke patients' asthenia.Therefore,medical staff should identify the high-risk groups as early as possible and implement targeted interventions,avoiding the adverse outcomes and improving their life quality.

Influencing factors of mental and behavior symptoms in patients with Alzheimer's diseaseAbstract:Objective To investigate the occurrence of mental and behavioral symptomsin patients with Alzheimer's disease(AD)and analyze the influencing factors,so as to provide references for formulating targeted intervention.Methods A total of 175 AD patients admitted to the geriatrics department of the third hospital of Longyan city between August 2023 and August 2024 were selected,and surveyed using a general information questionnaire,the disease data questionnoire,the Pittsburgh sleep quality index questionnaire and the neuropsychiatric inventory questionnaire(NPI-Q).Results The incidence of mental and behavioral symptoms in patients with AD is 64.57%.According to the logistic regression analysis,sleep status,duration of physical exercise,living conditions,disease duration,severity of the disease,and white matter hypersignal classification were the influencing factors of their mental and behavioral disorders.Conclusion The incidence of mental and behavioral symptoms is relatively high among AD patients,and influenced by multiple factors.Therefore,clinical medical staff should formulate targeted intervention measures to prevent their occurrence.
The current situation of long-term care needs of the disabled elderly in the community and its influencing factorsAbstract:Objective To explore the status quo of long-term care needs of the disabled elderly in the community,and its influencing factors.Methods Between September 2024 and February 2025,a total of 223 disabled elderly people were selected from 3 communities in Qiqihar city as research objects by convenience sampling method.They were surveyed using a general data questionnaire,Barthel index,the social support rating scale,self-perceived burden scale and long-term care needs questionnaire.Results The score of needs for long-term care of the disabled elderly in community was(94.48±8.49)points.The multiple linear regression results showed that age,number of chronic diseases,Barthel index,self-perceived burden and social support were independent influencing factors for long-term care demand of the research objects(R2=0.661,adjusted R2=0.636,F=26.882,P<0.001).Conclusion The demand for long-term care of the disabled elderly people in the community is of a medium level.Community workers should attach special importance to the disabled elderly people with multiple chronic diseases,or in a state of moderate to severe dependence,or of a heavy self-perceived burden or a low level of social support.
Current situation of exercise phobia in elderly patients with acute coronary syndrome after PCI and its influencing factorsAbstract:Objective To explore the current situation of exercise phobia in elderly patients with acute coronary syndrome(ACS)after percutaneous coronary intervention(PCI),and to analyze its influencing factors.Methods By using the convenience sampling method,180 elderly ACS patients after PCI admitted to the cardiovascular ward of the second affiliated hospital of Shandong first medical university in the second half of 2024 were selected,and surveyed using the Tampa scale for kinesiophobia heart(TSK Heart),the exercise self-efficacy scale,the perceived social support scale and general anxiety disorder scale.Results The TSK-SV Heart score was(42.96±5.98)points.The results of multiple linear regression analysis showed that place of residence,resident manner,number of comorbidities,social support,exercise self-efficacy and anxiety were independent influencing factors of exercise fear among the elderly ACS patients after PCI(R2=0.369,adjusted R2=0.340;F=12.511,P<0.001).Conclusion Exercise phobia is common among elderly ACS patients after PCI.Therefore,the medical staff can formulate targeted intervention strategies,taking into account their living environment,comorbidities,negative emotions,exercise self-efficacy and social support,to lower their exercise phobia level and promote their health behaviors.
The effectiveness of reminiscence therapy as an intervention for Alzheimer's disease patients:a Meta-analysisAbstract:Objective To systematically evaluate the intervention effect of reminiscence therapy on cognitive function,life quality and memory of patients with Alzheimer's disease(AD),so as to provide theoretical basis for future intervention.Methods Randomized controlled trials on the effectiveness of reminiscence therapy interventions for AD patients were searched in China National Knowledge Infrastructure,Wanfang database,VIP database and the Chinese Biological Medicine Literature database,Embase,PubMed,Web of Science,Cochrane Library with time setting from the inception to May 31,2024.After literature screening,data extraction and literature quality evaluation by two researchers,Meta-analysis was performed using the RevMan 5.4 software.Results A total of 7 articles were included,covering 488 AD patients.According to Meta-analysis results,the cognitive function[MD=5.04,95%CI(4.16,5.92),P<0.001],quality of life[MD=4.87,95%CI(0.73,9.02),P=0.020],and memory[MD=2.79,95%CI(1.94,3.63),P<0.001]of the experimental group were significantly better than those of the control group.Conclusion Nostalgia therapy can improve the cognitive function,life quality and memory of AD patients to some extent.
The application of active respiratory cycle exercise under the nurse-patient consensus model in pulmonary rehabilitation for chronic obstructive pulmonary disease patientsAbstract:Objective To explore the application of active respiratory cycle exercise under the nurse-patient consensus mode for patients with chronic obstructive pulmonary disease(COPD).Methods Ninety-six COPD patients treated in Chongqing ninth people's hospital between October 2022 and October 2023 were selected,and randomly divided into an observation group and a control group,each of 48,according to a random number table.The control group conducted routine pulmonary function exercise,while the observation group underwent the active respiratory cycle exercise based on the nurse-patient consensus mode.The exercise compliance,respiratory function and life quality were compared between the two groups.Results After intervention,the exercise compliance,peak respiratory flow rate,forced expiratory volume in one second(FEV1),forced vital capacity(FVC)and FEV1/FVC of the observation group were significantly higher than those of the control group(t=-13.525,P<0.001;t=-10.044,P<0.001;t=-4.021,P<0.001;t=-7.157,P<0.001;t=-3.780,P<0.001),while the life quality of the observation group was significantly better than that of the control group(t=6.225,P<0.001).Conclusion Active respiratory and circulatory exercise under the nurse-patient consensus model can effectively improve functional exercise compliance in COPD patients,promote their respiratory function and enhance their life quality.
A qualitative study on the demand and willingness of dual-income families for minor children's medical escort servicesAbstract:Objective To explore the demand and willingness of dual-income families for medical escort services for their minor children,so as to identify key influencing factors and provide a scientific basis for perfecting the pediatric healthcare service system.Methods A qualitative descriptive research approach was employed.Semi-structured interviews were conducted with 13 guardians of minor children from dual-income families,focusing on family medical experiences,and perceptions and demands regarding medical escort services.Results Three core themes were identified,including the potential value and actual demand for medical escort services(guided medical treatment,medical agency services,information transmission,psychological support,and professional guidance),barriers to choosing medical escort services(flexible working hours,advanced internet,security concerns,no unified price standard,and unclear rights and responsibilities)and urgent need for perfecting the medical escort service system(establishing a safe and reliable medical escort service platform,setting industry standards for medical accompaniment services,and strengthening legal safeguards).Conclusion Dual-income families have a clear demand for medical escort services for their minor children,but their unwillingness to use it is mainly constrained by trust issues and economic costs.It is recommended to build and improve the medical escort service system from aspects such as creating a safe escort environment,enhancing publicity and professional identity and improving service quality,so as to better meet the needs.
