Effects of behavioral interventions based on the concept of information-motivation-behavioral skills model on quality of life and complications in youth patients with maintenance hemodialysisAbstract:Objective To investigate the effects of behavioral interventions based on the concept of information-motivation-behavioral skills(IMB)model on quality of life and complications in youth patients with maintenance hemodialysis.Methods A total of 86 young patients underwent maintenance hemodialysis in Xuzhou Central Hospital from April 2023 to April 2024 were selected.According to the numerical random table method,they were randomly divided into 2 groups,with 43 cases in the control group received routine nursing,and 43 cases in the observation group received behavioral interventions based on the concept of IMB model.The coping style,self-management behavior,complications and quality of life of the two groups were compared.Results Compared with before the nursing care,the scores of the self-management scale for hemodialysis in both groups increased after the nursing care,and the score of the observation group was higher than that of the control group(P<0.05).After the nursing care,the urea clearance rate(KT/V)of the observation group was(1.34±0.28),the urea clearance rate(URR)was(59.82±4.41)%,and the β2-microglobulin(β2-MG)clearance rate was(66.79±5.23)%,which were all higher than those of the control group(1.12±0.32),(54.65±4.35)%,and(66.79±5.23)%respectively(P<0.05).The complication rate of the observation group was 4.65%,which was lower than that of the control group(20.93%)(P<0.05).Compared with before the nursing care,the scores of the kidney disease-related quality of life(KDTA)and the health-related quality of life(SF-36)in both groups increased after the nursing care,and the scores of the observation group were higher than those of the control group(P<0.05).Conclusion In young patients with maintenance hemodialysis,behavioral interventions based on the concept of IMB model can help to further improve their health behaviors,effectively improve the effect of dialysis,reduce the occurrence of complications,and improve the quality of life.
Narrative interventions based on stress system theory improve stigma,self-esteem and coping style of patients with pelvic floor dysfunction and stress urinary incontinenceAbstract:Objective To study the clinical effect of narrative intervention based on stress system theory on improving stigma,self-esteem and coping style of patients with pelvic floor dysfunction(PFD)and stress urinary incontinence(SUI).Methods A total of 68 female patients with postpartum PFD and SUI who were treated in the Obstetrics and Gynecology Department of Beijing Chao-yang Hospital,Capital Medical University from May 2022 to September 2024 were included as the study subjects.They were divided into two groups in chronological order:34 patients from May 2022 to February 2024 were included in the control group,and 34 patients from March 2024 to September 2024 were included in the study group.Conventional management program was applied to all patients,and narrative interventions based on the stress system theory were additionally performed in the study group.The differences of stigma,self-esteem,psychological status,coping style and pelvic floor function rehabilitation were compared between the two groups under different intervention schemes.Results There was no significant difference in urinary leakage symptoms between the two groups before interventions(P>0.05).After interventions,the amount of urinary leakage and the frequency of 24-hour urinary incontinence decreased in both groups,and the score of quality of life for urinary incontinence significantly increased.These changes were more pronounced in the study group than the control group(P<0.05).There were no significant differences in the muscle strength and fatigue of type I and type Ⅱ muscle fibers between the two groups before interventions(P>0.05).After interventions,the muscle strength of typeⅠ and type Ⅱ muscle fibers significantly increased in both groups,while the fatigue of type Ⅰ and type Ⅱ muscle fibers significantly decreased(P<0.05).There were no significant differences in the scores of stigma,social avoidance,anxiety and depression scales between the two groups before interventions(P>0.05),and the scores of all scales significantly decreased after interventions,especially in the study group than the control group(P<0.05).Before interventions,there was no significant difference between the two groups in each dimension of coping styles(P>0.05).After interventions,the scores of avoidance dimension and yield dimension significantly decreased in both groups,while the scores of face dimension significantly increased in both groups,which were more pronounced in the study group than the control group(P<0.05).Conclusion Narration interventions based on stress system theory can improve the stigma,social avoidance,anxiety and depression of PFD patients combined with SUI,promote the patients to actively face the disease and recovery,and help promote the recovery of the disease and improve the quality of life.
Study on the correlation between neonatal weight and the length of central venous catheter inserted through the great saphenous veinAbstract:Objective To analyze the correlation between newborn weight and the length of central venous catheter inserted through the great saphenous vein.Methods A total of 98 newborns requiring peripherally inserted central catheter(PICC)line catheterization from October 2023 to June 2024 were selected as the research subjects.They were divided into≤0.9 Kg(n=20),0.9-1.2 Kg(n=41)and 1.2-1.5 Kg(n=37)groups.Each group was further randomly divided into two subgroups for body surface measurement positioning and newborn weight combined with body surface positioning.The length of central venous catheter inserted through the great saphenous vein in newborns of different weights was recorded.The correlation between newborn weight and the length of central venous catheter inserted through the great saphenous vein was illustrated.Puncture situation,incidence of complications,catheter tip position,and catheter maintenance time under different body weights and positioning methods were compared.Results Compared with newborns weighing≤0.9Kg,those weighing0.9-1.2 Kg and 1.2-1.5 Kg had significantly longer central venous catheter length(P<0.05).Compared with newborns weighing0.9-1.2 Kg,those weighing 1.2-1.5 Kg had a significantly longer central venous catheter length(P<0.05).The weight of newborns was positively correlated with the length of central venous catheters inserted through the great saphenous vein(P<0.05).Compared with the body surface measurement and positioning of newborns with body weight≤0.9 Kg,those weighing 0.9-1.2 Kg and 1.2-1.5 Kg had significantly higher success rate of a single puncture combined with body surface positioning and the optimal position of the catheter tip,and shorter duration of successful puncture and catheter maintenance(P<0.05).The incidence of complications between the two positioning methods was comparable of newborns among the three body weight groups(P>0.05).Conclusion The weight of newborns is positively correlated with the length of central venous catheters inserted through the great saphenous vein.The use of weight combined with surface positioning method for catheter placement in newborns results in ideal puncture results,shorter catheter maintenance time,and a higher proportion of optimal catheter tip positions.
Correlations of ultrafast dynamic contrast-enhance and conventional dynamic contrast-enhance MRI parameters with Ki-67 level in breast cancerAbstract:Objective To investigate the correlation of parameters derived from ultrafast dynamic contrast-enhanced magnetic resonance imaging(MRI)(UF-DCE-MRI)using the Breast model and those from conventional DCE-MRI(Tofts model)in predicting Ki-67 expression status in breast cancer.Methods This study involved 56 female patients with mass-type breast cancer nodules of ductal carcinoma of the breast that was pathologically confirmed in the Fourth Hospital of Hebei Medical University from 1st March 2024 to 30th September 2024,comprising 21 patients with low Ki-67 expression(Ki-67 proliferation index below 14%)and 35 patients with high Ki-67 expression(Ki-67 proliferation index above 14%).The golden-angle radial sparse parallel(GRASP)on MRI was scanned.Clinical and imaging data were collected.Chi-square test,t-test or rank sum test were used to conduct statistical analysis on counting data and measurement data.The independent predictors related to Ki-67 expression status in breast cancer were screened out by univariate and multivariate logistic regression analyses.Receiver operating characteristic(ROC)curves of the independent risk factors and the combination in predicting Ki-67 status in breast cancer were plotted,and the area under the curve(AUC)and indicators associated with the performance were calculated.Results The ultrafast DCE parameters time-to-enhancement(TTE),time-to-peak(TTP)and time-to-maximum slope(TTMS)in the Ki-67 high expression group were significantly lower than the lower expression group(P<0.05).However,no significant differences were observed in the conventional DCE-MRI Tofts model parameters Ktrans(volume transfer constant),Kep(rate constat),and Ve(volume fraction of extravascular extracellular space)between the two groups,although the Ve values in the high Ki-67 expression group were slightly lower than the low-expression group.The combined prediction model based on TTE,TIC curve type and tumor size demonstrated optimal diagnostic performance,achieving an AUC of 0.829(95%CI:0.712-0.946),which was significantly superior to that of individual predictor models.Conclusion Compared with conventional Tofts model parameters,the ultra-rapid breast-specific quantitative model parameters demonstrated stronger correlation with Ki-67 expression levels.These findings provide more clinically relevant insights into the prediction and prognosis of mass-type invasive breast cancer,with potential to improve diagnostic accuracy and optimize treatment planning.
Application of pelvic floor peritoneal reconstruction combined with transanal ileal tube placement in the prevention of anastomosis leakage after laparoscopic precancerous rectal resectionAbstract:Objective To investigate the clinical effect of pelvic floor peritoneal reconstruction combined with transanal ileal tube placement during laparoscopic anterior resection of low rectal cancer.Methods A total of 84 patients with middle to low rectal cancer who underwent surgical treatment in our hospital from January 2022 to May 2024 were selected as the observation subjects.All of them were clinically diagnosed through colonoscopy and pathological examination.They were randomly divided into the control group and observation group,with 42 cases per group.Conventional surgery was performed in all patients,and those in the observation group were additionally treated with pelvic floor peritoneal reconstruction combined with transanal ileal tube placement for drainage.Results All enrolled patients successfully completed laparoscopic resection,without cases of conversion to open surgery or deaths during the operation.After postoperative pathological examination,rectal cancer was confirmed in all cases.Negative margin of the cutting edges on bilateral sides was confirmed by pathology.The incidence of postoperative anastomotic leakage in the observation group was significantly lower than the control group(P<0.05).All patients with postoperative anastomotic leakage were managed by comprehensive conservative treatments,such as parenteral nutrition,pelvic irrigation via drainage tube,and anti-infection,and all of them were cured after active treatments.Postoperative follow-up showed no occurrence of anastomotic stenosis,tumor recurrence,distant metastasis,or other conditions.The postoperative hospitalization time of the observation group was significantly shorter than the control group(P<0.05).No significant differences were found in the anal high-pressure zone,anal resting pressure,anal maximum contraction pressure between groups(P>0.05),which were all significantly reduced after 2 months of treatment,and significantly pronounced in the observation group than the control group(P<0.05).Conclusion During laparoscopic low rectal precancerous resection,the closure of the pelvic floor peritoneum and the placement of the anorectal obstruction catheter can reduce the possibility of contacting intestinal contents,achieve the goal of full decompression of the anastomosis,prevent postoperative anastomosis leakage,promote postoperative recovery,and further improve the anorectal function.
Intervention effect of negative pressure suction toothbrush combined with different concentrations of chlorhexidine on stroke-related pneumoniaAbstract:Objective To explore the intervention effect of negative pressure suction toothbrush combined with different concentrations of chlorhexidine on stroke-related pneumonia(SAP),thus providing a basis for optimal concentration of chlorhexidine for oral care of SAP patients.Methods A total of 162 SAP patients treated in Leshan Hospital of Traditional Chinese Medicine from August 2022 to August 2024 were selected by convenient sampling in this randomized controlled trial.They were randomly divided into six groups according to the randomized numerical method,with 27 cases per group.Negative pressure suction toothbrush combined with saline for oral care was given to patients of the control group.Negative pressure suction toothbrush combined with 0.02%,0.06%,0.12%,0.2%and 2%chlorhexidine were given to those of the 0.02%chlorhexidine group,0.06%chlorhexidine group,0.12%chlorhexidine group,0.2%chlorhexidine group and 2%chlorhexidine group,respectively.Before and after 1 d,3 d,and 5 d of intervention,the Oral Assessment Guideline Scores and Clinical Pulmonary Infection Scores were compared among the six groups.Results After the intervention of the 6 groups of patients with the control group and different concentrations of chlorhexidine,the scores of the oral assessment guide scale all showed a downward trend.However,at 3 days and 5 days of intervention,the scores of the 0.2%chlorhexidine group(10.70±2.57 vs 10.19±2.62)and the 2%chlorhexidine group(10.96±2.41 vs 9.56±2.41)decreased more significantly.The differences between the groups,at different time points,and the interaction between the groups and different time points were all statistically significant(P<0.05).After the intervention of the control group and different concentrations of chlorhexidine,the scores of pulmonary infection all decreased,but the chlorhexidine intervention groups decreased more significantly.The differences between the groups,at different time points,and the interaction between the groups and different time points were all statistically significant(P<0.05).Conclusion Negative pressure suction toothbrush combined with 0.2%chlorhexidine can improve oral hygiene and alleviate pulmonary inflammation of SAP patients.
Regulation of circular RNA Cdr1as in mediating myocardial fibrosis and ventricular remodeling of spontaneous hypertension rats by the TGF-β1/Smad signaling pathwayAbstract:Objective To explore the mechanisms of circular RNA Cdr1as(circRNA Cdr1as)in mediating myocardial fibrosis and ventricular remodeling in spontaneous hypertension rats(SHR)by regulating the transforming growth factor(TGF)-β1/Smad signaling pathway.Methods A total of 30 SHR rats and 15 Wistar rats(control group),male or female,with an average weight of(275.45±10.96)g,were selected.SHR rats were randomly divided into the model group,and circRNA Cdr1as over-expression group,with 15 rats in each group.The systolic blood pressure(SBP)was measured on day 1,and on days 21 and 42.Enzyme-linked immunosorbent assay(ELISA)was used to detect plasma creatine kinase isoenzymes(CK-MB),cardiac troponin(cTnI),type B brain natriuretic peptide(BNP),and angiotensin Ⅱ(Ang Ⅱ).Left ventricular posterior wall thickness at end-diastole and end-systole(LVPWd and LVPWs)were measured by echocardiography.The thickness of apical myocardial tissue was measured and left ventricular mass index(LVMI)was calculated.Masson staining was used to calculate myocardial collagen volume fraction(CVF).The quantitative reverse-transcription polymerase chain reaction(qRT-PCR)was used to detect type I and Ⅲ collagen(Col I and Col-Ⅲ),and circRNA Cdr1as in myocardial tissues.Western blot was used to detect protein levels of TGF-β1 and p-Smad 3 in myocardial tissues.Results The systolic blood pressure,serum AngⅡ,LVPWd,LVPWs,thickness of myocardial tissue at the apex,LVMI,CVF,Col-ⅠmRNA,Col-ⅢmRNA,myocardial CircRNA Cdr1as mRNA,TGF-β1 and p-Smad 3 protein levels in the overexpression group,model group and control group on the 1st,21st and 42nd days all showed a gradually increasing trend,with the overexpression group showing the most significant increase(all P<0.05).There were statistically significant differences in systolic blood pressure,serum AngⅡ,LVPWd,LVPWs,thickness of myocardial tissue at the apex,LVMI,CVF,Col-ⅠmRNA,Col-ⅢmRNA,myocardial CircRNA Cdr1as mRNA,TGF-β1 and p-Smad 3 protein levels among the three groups,at different time points and in the interaction between groups and time points(all P<0.05).Conclusion Overexpression of circRNA Cdr1as can increase blood pressure,and induce myocardial fibrosis,and ventricular remodeling in SHR rats by regulating the TGF-β1/Smad signaling pathway.
Influence of the modified Huangqi Biejia Powder in promoting the apoptosis of CT26 cells by the Bcl-2/Bax/Caspase-3/Caspase-8 signaling pathwayAbstract:Objective To explore the influence of the Astragalus and Turtle shell Powder(modified Huangqi Biejia Powder)on the proliferation and apoptosis of CT26 cells by regulating the B cell lymphoma-2(Bcl-2)/Bcl-2-associated X protein(Bax)/Caspase-3/Caspase-8 signaling pathway.Methods A total of 24 male Sprague-Dawley(SD)rats were randomly divided into the control group,and low-dose,medium-dose and high-dose Huangqi Biejia Powder groups(modified Huangqi Biejia Powder at 6,12 and 24g·kg-1·d-1,respectively),with 6 rats per group.CT26 cells were induced with drug-containing serum.Cell survival and colony formation ability were tested by cell counting kit-8(CCK-8)assay and colony formation assay.Cell cycle progression and apoptosis were detected by flow cytometry.Protein levels of Bcl-2,Bax,Caspase-3 and Caspase-8 were detected by Western blot.Results Compared with the control group,significantly decreased colonies were observed in CT26 cells induced with low-dose,medium-dose and high-dose Huangqi Biejia Powder;significantly decreased survival was observed in CT26 cells induced with medium-dose and high-dose Huangqi Biejia Powder;significantly increased apoptotic rate was observed in CT26 cells induced with high-dose Huangqi Biejia Powder(all P<0.05).Low-dose,medium-dose and high-dose Huangqi Biejia Powder treatment significantly upregulated apoptosis-related proteins,including Bax,Caspase-3 and Caspase-8,but downregulated the oncogene Bcl-2(all P<0.05).The proportion of cells in the G1 phase was significantly reduced in CT26 cells induced with high-dose Huangqi Biejia Powder(P<0.05).Significantly decreased proportion of cells in the G2 phase,and increased proportion of cells in the S phase were observed in CT26 cells induced with low-dose,medium-dose and high-dose Huangqi Biejia Powder(all P<0.05).Conclusion The drug-containing serum of modified Huangqi Biejia Powder significantly inhibits proliferation,induces apoptosis,and causes cell cycle arrest in the S phase of CT26 cells.
Application of enhanced rehabilitation coordination mode combined with stepwise analgesia mode in patients after cholecystolithiasis surgeryAbstract:Objective To evaluate the intervention value of enhanced rehabilitation coordination mode combined with stepwise analgesia mode in patients after cholecystolithiasis surgery.Methods A total of 150 patients who underwent cholecystolithiasis surgery in our hospital from April 2022 to October 2023 were selected and randomly divided into control group and observation group,with 75 patients in each group.The control group received stepwise analgesia mode,and the observation group received enhanced rehabilitation coordination mode combined with stepwise analgesia mode.The two groups were compared for pain degree,Generic Quality of Life Inventory-74(GQOLI-74),Herth Hope Index(HHI),and complications.Results After the surgery,the observation group had significantly lower numeric rating scale(NRS)score,significantly higher GQOLI-74 scores and HHI scores,and significantly lower overall incidence of complications than the control group(all P<0.05).Conclusion Enhanced rehabilitation coordination mode combined with stepwise analgesia mode effectively relieves pain,improves the quality of life and hope level,avoids triggering resistance or negative emotions that could undermine rehabilitation outcomes.
Influence of integrated nursing management based on evidence-based medicine on the prognosis of pediatric pneumonia complicated with heart failureAbstract:Objective To explore the effect of integrated nursing management based on evidence-based medicine on the prognosis of pediatric pneumonia complicated with heart failure.Methods A total of 128 children with pneumonia complicated with heart failure treated in the pediatric Intensive Care Unit of the First Affiliated Hospital of Air Force Medical University from June 2022 to June 2024 were selected.They were randomly divided into two groups according to the numerical random table method,with 64 cases in the control group receiving routine nursing,and 64 cases in the observation group receiving comprehensive nursing management based on evidence-based medicine.The psychological state,treatment cooperation,disease control effect and prognosis of the two groups were compared.Results After the intervention,the scores of the Child Medical Fear Survey Scale(CMFS)and the Screening for Child Anxiety Related Emotional Disorders(SCARED)in both groups were lower than those before the intervention,and the score of the observation group was lower than that of the control group(P<0.05);the treatment cooperation rate of the observation group was 71.88%,higher than 31.25%of the control group,while the partial cooperation rate was 25.00%and the non-cooperation rate was 3.13%,both lower than 59.38%and 9.38%of the control group(P<0.05);after the intervention,the tidal volume(VT)of both groups increased compared with that before the intervention,and the observation group was higher than the control group(P<0.05),while the respiratory rate(RR),N-terminal pro-brain natriuretic peptide(NT-proBNP),and creatine kinase isoenzyme(CK-MB)decreased compared with that before the intervention,and the observation group was lower than the control group(P<0.05);after the intervention,the pediatric critical illness score(PCIS)of both groups increased compared with that before the intervention,and the observation group was higher than the control group(P<0.05).Conclusion The application of integrated nursing management based on evidence-based medicine in children with pneumonia complicated with heart failure can effectively improve their psychological state and treatment cooperation,and enhance the disease control effect,as well as further improve their prognosis.
Progress in the pathogenesis of systemic lupus erythematosus with thrombocytopeniaAbstract:Systemic lupus erythematosus(SLE)is an autoimmune disease involving multiple organ systems,characterized by multiple organ involvement,especially often leading to blood system damage.Platelet reduction is one of the common hematological initial symptoms of the disease.The pathogenesis of SLE with thrombocytopenia is complex,and clarifying the pathogenesis and timely effective intervention measures can effectively improve clinical efficacy and prognosis outcome.This article discusses the possible pathogenesis of SLE with thrombocytopenia based on immune disorders,megakaryocyte interference,genetic influences,secondary reduction due to pooled other diseases,platelet distribution factors,and other possible factors,with a view to providing a reference for further clarifying the relevant mechanisms of SLE with thrombocytopenia.
The mechanism of lncRNA SNHG16 in mediating the proliferation and metastasis of clear cell renal cell carcinoma cells via regulating miR-506-3pAbstract:Objective To investigate the effects of long non-coding RNA(lncRNA)SNHG16 by targeting miR-506-3p on cell proliferation and metastasis of clear cell renal cell carcinoma(ccRCC)and its mechanism.Methods The ENCORI/starBase database was used to predict the binding sites of lncRNA SNHG16 and PTBP1 to miR-506-3p.The interaction of lncRNA SNHG16 and PTBP1 with miR-506-3p was determined by dual-luciferase reporter assay and RNA immunoprecipitation assay.The expression levels of lncRNA SNHG16 in ccRCC cell lines(Caki-1,A498,ACHN,786-O and OSRC-2)versus human normal renal cortical proximal tubular epithelial cells(HK-2)were measured by quantitative reverse-transcription polymerase chain reaction(qRT-PCR).Meanwhile,miR-506-3p level and PTBP1 mRNA expression in A498 cells were measured by qRT-PCR as well.A498 cells were transfected with short hairpin RNA targeting lncRNA SNHG16(sh-lncRNA SNHG16)and its negative control(shNC),miR-506-3p inhibitor and its negative control(NC inhibitor),miR-506-3p mimic(miR-506-3p)and its negative control(miR-NC),PTBP1-overexpressing plasmid(pcDNA3.1-PTBP1),and the empty vector pcDNA3.1.Subcellular localization of lncRNA SNHG16 was determined by cytoplasmic RNA isolation.The proliferative,migratory and invasive abilities of A498 cells were assessed by cell counting kit-8(CCK-8)assay,EdU and Transwell assays,respectively.Western blot was conducted to measure the protein expression of PTBP1.Results qRT-PCR results showed that compared with normal cells,lncRNA SNHG16 was significantly upregulated in ccRCC cells(P<0.05),which was localized in the cytoplasm(P<0.05).Compared with shNC group,the viability,proliferation,migration and invasion were significantly reduced in A498 cells transfected with sh-lncRNA SNHG16(P<0.05).Overexpression of miR-506-3p significantly decreased the luciferase activity of SNHG16-WT in A498 cells(P<0.05),and Ago2 antibody was significantly enriched in both lncRNA SNHG16 and miR-506-3p(P<0.05).Overexpression of miR-506-3p downregulated lncRNA SNHG16(P<0.05).Knockdown of miR-506-3p significantly weakened the inhibitory effects of silenced lncRNA SNHG16 on the proliferation,migration and invasion of ccRCC cells(P<0.05).Restoration of miR-506-3p inhibited luciferase activity of PTBP1-WT and mRNA and protein levels of PTBP1 in A498 cells(P<0.05).PTBP1 overexpression attenuated the suppressive effects of miR-506-3p on the proliferation,migration and invasion of ccRCC cells(P<0.05).Conclusion LncRNA SNHG16 is upregulated in ccRCC,and knockdown of lncRNA SNHG16 inhibits the proliferation and metastasis of ccRCC cells by targeting the miR-506-3p/PTBP1 axis.
Notch1 regulates angiogenesis in a cardiac hypertrophy model by mediating VEGF and PDGF-BBAbstract:Objective To investigate the role and mechanism of Notch1 in regulating angiogenesis in a cardiac hypertrophy model.Methods The transverse aortic constriction(TAC)was performed to establish a mouse model of cardiac hypertrophy,and angiotensin Ⅱ(Ang Ⅱ)induction was performed to create a human coronary artery endothelial cell(HCAEC)injury model.Cardiac function was assessed by echocardiography,and the cardiac hypertrophy model was validated by morphological observation of the heart.The effect of Ang Ⅱ on endothelial cell viability was detected by MTT assay.The protein expressions of Notch1,vascular endothelial growth factor(VEGF),and platelet-derived growth factor-BB(PDGF-BB)were detected by Western blot.Endothelial cell migration was evaluated by the scratch assay.Results Echocardiography and cardiac morphology results revealed that compared to the sham-operated rats,the left ventricular ejection fraction(LVEF)and fractional shortening(LVFS)significantly decreased in cardiac hypertrophy rats,while the heart weight/body weight ratio and heart weight/tibia length ratio significantly increased,indicating a successful construction of the cardiac hypertrophy model(P<0.05).MTT assay showed a significant decrease in the viability of Ang Ⅱ-induced HCAECs,suggesting that Ang Ⅱhad a damaging effect on HCAECs.Western blot demonstrated that in both the cardiac hypertrophy and endothelial injury models,Notch1,and angiogenesis markers VEGF and PDGF-BB were significantly downregulated(P<0.05).In vitro,Notch1,VEGF and PDGF-BB were significantly upregulated in Ang Ⅱ-induced HCAECs overexpressing Notch1 than those of Ang Ⅱ-induced HCAECs(P<0.05).Overexpression of Notch1 in Ang Ⅱ-induced HCAECs significantly increased the migration(P<0.05).Conclusion Activation of Notch1 may promote angiogenesis by affecting the protein expressions of VEGF and PDGF-BB,thereby alleviating cardiac hypertrophy.
Effects of highfrequency electrocautery under painless digestive endoscopy combined with dietary intervention on postoperative complications and recovery in patients with gastrointestinal polypsAbstract:Objective To evaluate the effects of highfrequency electrocautery under painless digestive endoscopy combined with dietary intervention on postoperative complications and recovery in patients with gastrointestinal polyps.Methods A total of 98 patients with gastrointestinal polyps who were admitted to our hospital from September 2022 to October 2023 were selected and randomly divided into the control group and the observation group,with 49 cases in each group.The control group underwent conventional endoscopic resection,while the observation group received painless endoscopic resection using high-frequency electrocautery plus a structured dietary intervention.We compared markers of oxidative stress,gastrointestinal hormones,and nutritional status before and after treatment,recorded postoperative complications,and evaluated recovery outcomes.Results After treatment,Both groups showed significantly decreased malondialdehyde(MDA),and increased superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px),the reductions were significantly greater in the observation group(P<0.05).Serum gastrin(GAS),motilin(MOT)and vasoactive intestinal peptide(VIP)levels were significantly decreased in both groups posttreatment,with more pronounced changes in the observation group(P<0.05).Nutritional indicators improved in both groups after treatment,with significantly greater improvement in the observation group(P<0.05).The observation group experienced fewer postoperative complications and faster postoperative recovery compared with the control group(P<0.05).Conclusion For patients with gastrointestinal polyps,high-frequency electrocautery under painless digestive endoscopy combined with dietary intervention can reduce postoperative inflammation,enhance immune function,improve nutritional status,and promote postoperative recovery.
Burnout Rate and influencing factors among nurses in a regional comprehensive hospitalAbstract:Objective To explore the current status of burnout and its influencing factors among nurses in regional comprehensive hospitals,thus providing a reference for improving mental health and career development in nurses.Methods A convenient sampling method was used to select 379 nursing staff from a regional hospital in Shanghai as the research subjects.Through the Wenjuanxing platform,a survey was conducted covering demographic characteristics,the Maslach Burnout Inventory-Human Services Survey(MBI-HSS),the Self-Rating Anxiety Scale(SAS),and the Insomnia Severity Index(ISI).Chi-square test and multivariate Logistic regression analysis were applied to explore the burnout rate and its influencing factors among nurses in regional hospitals.Results Among the 379 nursing staff,there were 356 females(93.93%)and23 males(6.07%).The proportion of nurses with high emotional exhaustion,high depersonalization,low personal accomplishment,high burnout risk,and burnout syndrome was 27.97%,26.65%,67.55%,35.09%,and 17.68%,respectively.The results of multivariate Logistic regression analysis showed that:caring for 11-15 patients per day,caring for 16-20 patients per day,and anxiety level were common influencing factors for high burnout risk and burnout syndrome(P<0.05).Only-child,caring for≤5 patients per day,and insomnia severity were influencing factors for high burnout risk(P<0.05).Insufficient allocation of various support resources and few promotion opportunities were influencing factors for burnout syndrome in nurses(P<0.05).Conclusion The rate of high burnout among nurses in regional comprehensive hospitals is at a moderate level.Medical institutions should attach importance to this issue and take corresponding measures to improve nurses'work and living conditions,so as to enhance their physical and mental health and job satisfaction.
Effect of repeated transcranial magnetic stimulation combined with mirror neuron rehabilitation training on improving post-stroke aphasiaAbstract:Objective To evaluate the effect of repeated transcranial magnetic stimulation combined with mirror neuron rehabilitation training on post-stroke aphasia.Methods A total of 150 patients with post-stroke aphasia admitted from July 2022 to May 2024 were selected and randomly divided into the control and observation groups,with 75 patients per group.Routine language training intervention was applied to patients of both groups,and repeated transcranial magnetic stimulation combined with mirror neuron rehabilitation training was additionally given to patients of the observation group.The language function,cognitive function,rehabilitation efficacy,sleep quality,and nursing satisfaction of the two groups were compared.Results The Western Aphasia Battery(WAB)scores and aphasia index were significantly higher after interventions in both groups than pre-intervention values,which were significantly higher in the observation group than the control group(P<0.05).NLCA scores were significantly higher after interventions in both groups than pre-intervention values,which were significantly higher in the observation group than the control group(P<0.05).The overall efficacy rate was significantly higher in the observation group than the control group(94.67%vs 80.00%,P<0.05).The Pittsburgh Sleep Quality Index(PSQI)was significantly reduced after interventions in both groups than pre-intervention values,which was significantly lower in the observation group than the control group(P<0.05).The satisfaction score was significantly higher in the observation group than the control group(P<0.05).Conclusion Repetitive transcranial magnetic stimulation combined with mirror neuron rehabilitation training can effectively improve the language function,cognitive function,and sleep quality of patients with post-stroke aphasia,and promote the improvement of rehabilitation efficacy and nursing satisfaction.
The impact of sleep quality improvement on quality of life in Parkinson's disease patientsAbstract:Objective To investigate the impact of sleep quality on quality of life in patients with Parkinson's disease(PD),and to examine the potential mediating roles of fatigue and social isolation,thus providing a theoretical basis for integrated interventions of non-motor symptoms.Methods A prospective controlled trial was conducted,involving 120 patients diagnosed with PD in the outpatient clinic or hospitalization in the Jiangsu Province Hospital from June 2023 to June 2024.They were randomly assigned to the intervention group and the control group(n=60 each).Patients in the intervention group received an 8-week multidimensional intervention,while those in the control group received routine health education.Sleep quality,fatigue,social support,and quality of life were assessed before and after the intervention using the Parkinson's Disease Sleep Scale-2(PDSS-2),Fatigue Severity Scale(FSS),Lubben Social Network Scale(LSNS),and Parkinson's Disease Questionnaire-8(PDQ-8)scales.Group comparisons,correlation analysis,and mediation modeling were performed.Results The intervention group showed significant improvements in sleep quality,fatigue,social support,and quality of life compared to the control group(P<0.05).Sleep quality was positively correlated with fatigue and quality of life,and negatively correlated with social support.Mediation analysis revealed that sleep quality had a direct positive effect on quality of life,which also exerted partial indirect effects through fatigue and social isolation,accounting for 16.04%and 36.92%of the total effect,respectively.Conclusion Sleep disturbance is a key factor affecting quality of life in PD patients.Fatigue and social isolation partially mediate this relationship.Multidimensional interventions can effectively improve non-motor symptoms and enhance overall quality of life.Individualized management and strengthened social support are recommended in clinical practice.
Clinical research progress on liver cirrhosis complicated with spontaneous bacterial peritonitisAbstract:Liver cirrhosis is a common clinical disease.For a long time,the incidence and recurrence rate of liver cirrhosis complicated with spontaneous bacterial peritonitis(SBP)remains high,and an early diagnosis of SBP has become a difficult point of treatment.In recent years,with the continuous deepening of the research on liver cirrhosis combined with SBP,the early diagnosis and treatment have been largely improved.SBP has become a complication that can be intervened and treated in patients with liver cirrhosis,largely reducing the recurrence rate,morbidity and mortality rate of liver cirrhosis complicated with SBP.The article reviewed the pathogenesis,risk factors,diagnosis,treatment,prognosis and prevention of SBP complicated with liver cirrhosis via reviewing the latest literatures,hoping to provide some new ideas for the clinical diagnosis and treatment of liver cirrhosis complicated with SBP,so as to improve the quality of life and prognosis,and reduce the burden of family and society.
Quantitative assessment and grading intervention combined with respiratory training on the rehabilitation of elderly patients with chronic heart failureAbstract:Objective To analyze the application effect of quantitative assessment and grading intervention combined with respiratory training in elderly patients with chronic heart failure(CHF).Methods From June 2021 to December 2023,122 elderly CHF patients were randomly divided into the control group(n=61,routine cardiology nursing)and test group(n=61,quantitative assessment and grading intervention combined with respiratory training on the basis of routine cardiology nursing)using a random number table method.Cardiopulmonary function,respiratory muscle strength,quality of life,6-minute walking distance(6MWD),and incidence of major adverse cardiac events(MACEs)were compared.Results After interventions,the peak oxygen uptake(VO2peak),anaerobic threshold,left ventricular ejection fraction(LVEF),forced expiratory volume at 1 second(FEV1),forced vital capacity(FVC),maximum inspiratory pressure(MIP),peak inspiratory flow rate(PIF),and 6MWD of both groups were significantly elevated than pre-intervention values(P<0.05),and the carbon dioxide equivalent slope(VE/VCO2slop)and the Minnesota Living with Heart Failure Questionnaire(MLHFQ)scores of both groups were significantly reduced(P<0.05).The above changes were significantly pronounced in the test group than the control group(P<0.05).The overall incidence of MACEs in the test group(3.28%,2/61)was significantly lower than that of the control group(14.75%,9/61)(P<0.05).Conclusion Quantitative assessment and grading intervention combined with respiratory training can improve cardiopulmonary function,respiratory muscle strength,and quality of life,prolong 6MWD,and reduce MACEs in elderly CHF patients.
Construction of a nomogram to predict moderate-to-severe acute radiation dermatitis in breast cancer based on R-shiny and its applicationAbstract:Objective To analyze the factors influencing moderate-to-severe acute radiation dermatitis in breast cancer,and to construct a nomogram to predict it and validate the performance.Methods Retrospectively,245 patients with acute radiation dermatitis of breast cancer in Affiliated Cancer Hospital of Xinjiang Medical University from May 2022 to June 2023 were collected as a modeling group.They were classified into mild(n=188)and moderate-to-severe groups(n=57),and the influencing factors of moderate-to-severe acute radiation dermatitis of breast cancer were analyzed by logistic regression.A nomogram was created to predict moderate-to-severe acute radiation dermatitis in breast cancer.A total of 105 patients with acute radiation dermatitis of breast cancer collected from July to November 2023 were selected for external validation.Results Logistic regression showed that diabetes mellitus,body mass index(BMI)>24kg/m2,positive regional lymph node metastasis,and high expression of value-added index were the influencing factors for moderate-to-severe acute radiation dermatitis in breast cancer.The area under the curve(AUC)of the nomogram for internal validation and external validation was 0.737 and 0.899,respectively.Conclusion The nomogram has a good efficacy and helps healthcare professionals to identify patients at high risk of moderate-to-severe acute radiation dermatitis in breast cancer early and accurately.
