Correlations of ultrafast dynamic contrast-enhance and conventional dynamic contrast-enhance MRI parameters with Ki-67 level in breast cancer

Abstract:Objective To investigate the correlation of parameters derived from ultrafast dynamic contrast-enhanced magnetic resonance imaging(MRI)(UF-DCE-MRI)using the Breast model and those from conventional DCE-MRI(Tofts model)in predicting Ki-67 expression status in breast cancer.Methods This study involved 56 female patients with mass-type breast cancer nodules of ductal carcinoma of the breast that was pathologically confirmed in the Fourth Hospital of Hebei Medical University from 1st March 2024 to 30th September 2024,comprising 21 patients with low Ki-67 expression(Ki-67 proliferation index below 14%)and 35 patients with high Ki-67 expression(Ki-67 proliferation index above 14%).The golden-angle radial sparse parallel(GRASP)on MRI was scanned.Clinical and imaging data were collected.Chi-square test,t-test or rank sum test were used to conduct statistical analysis on counting data and measurement data.The independent predictors related to Ki-67 expression status in breast cancer were screened out by univariate and multivariate logistic regression analyses.Receiver operating characteristic(ROC)curves of the independent risk factors and the combination in predicting Ki-67 status in breast cancer were plotted,and the area under the curve(AUC)and indicators associated with the performance were calculated.Results The ultrafast DCE parameters time-to-enhancement(TTE),time-to-peak(TTP)and time-to-maximum slope(TTMS)in the Ki-67 high expression group were significantly lower than the lower expression group(P<0.05).However,no significant differences were observed in the conventional DCE-MRI Tofts model parameters Ktrans(volume transfer constant),Kep(rate constat),and Ve(volume fraction of extravascular extracellular space)between the two groups,although the Ve values in the high Ki-67 expression group were slightly lower than the low-expression group.The combined prediction model based on TTE,TIC curve type and tumor size demonstrated optimal diagnostic performance,achieving an AUC of 0.829(95%CI:0.712-0.946),which was significantly superior to that of individual predictor models.Conclusion Compared with conventional Tofts model parameters,the ultra-rapid breast-specific quantitative model parameters demonstrated stronger correlation with Ki-67 expression levels.These findings provide more clinically relevant insights into the prediction and prognosis of mass-type invasive breast cancer,with potential to improve diagnostic accuracy and optimize treatment planning.
