Intervention effect of negative pressure suction toothbrush combined with different concentrations of chlorhexidine on stroke-related pneumonia

Abstract:Objective To explore the intervention effect of negative pressure suction toothbrush combined with different concentrations of chlorhexidine on stroke-related pneumonia(SAP),thus providing a basis for optimal concentration of chlorhexidine for oral care of SAP patients.Methods A total of 162 SAP patients treated in Leshan Hospital of Traditional Chinese Medicine from August 2022 to August 2024 were selected by convenient sampling in this randomized controlled trial.They were randomly divided into six groups according to the randomized numerical method,with 27 cases per group.Negative pressure suction toothbrush combined with saline for oral care was given to patients of the control group.Negative pressure suction toothbrush combined with 0.02%,0.06%,0.12%,0.2%and 2%chlorhexidine were given to those of the 0.02%chlorhexidine group,0.06%chlorhexidine group,0.12%chlorhexidine group,0.2%chlorhexidine group and 2%chlorhexidine group,respectively.Before and after 1 d,3 d,and 5 d of intervention,the Oral Assessment Guideline Scores and Clinical Pulmonary Infection Scores were compared among the six groups.Results After the intervention of the 6 groups of patients with the control group and different concentrations of chlorhexidine,the scores of the oral assessment guide scale all showed a downward trend.However,at 3 days and 5 days of intervention,the scores of the 0.2%chlorhexidine group(10.70±2.57 vs 10.19±2.62)and the 2%chlorhexidine group(10.96±2.41 vs 9.56±2.41)decreased more significantly.The differences between the groups,at different time points,and the interaction between the groups and different time points were all statistically significant(P<0.05).After the intervention of the control group and different concentrations of chlorhexidine,the scores of pulmonary infection all decreased,but the chlorhexidine intervention groups decreased more significantly.The differences between the groups,at different time points,and the interaction between the groups and different time points were all statistically significant(P<0.05).Conclusion Negative pressure suction toothbrush combined with 0.2%chlorhexidine can improve oral hygiene and alleviate pulmonary inflammation of SAP patients.
