Narrative interventions based on stress system theory improve stigma,self-esteem and coping style of patients with pelvic floor dysfunction and stress urinary incontinence

Abstract:Objective To study the clinical effect of narrative intervention based on stress system theory on improving stigma,self-esteem and coping style of patients with pelvic floor dysfunction(PFD)and stress urinary incontinence(SUI).Methods A total of 68 female patients with postpartum PFD and SUI who were treated in the Obstetrics and Gynecology Department of Beijing Chao-yang Hospital,Capital Medical University from May 2022 to September 2024 were included as the study subjects.They were divided into two groups in chronological order:34 patients from May 2022 to February 2024 were included in the control group,and 34 patients from March 2024 to September 2024 were included in the study group.Conventional management program was applied to all patients,and narrative interventions based on the stress system theory were additionally performed in the study group.The differences of stigma,self-esteem,psychological status,coping style and pelvic floor function rehabilitation were compared between the two groups under different intervention schemes.Results There was no significant difference in urinary leakage symptoms between the two groups before interventions(P>0.05).After interventions,the amount of urinary leakage and the frequency of 24-hour urinary incontinence decreased in both groups,and the score of quality of life for urinary incontinence significantly increased.These changes were more pronounced in the study group than the control group(P<0.05).There were no significant differences in the muscle strength and fatigue of type I and type Ⅱ muscle fibers between the two groups before interventions(P>0.05).After interventions,the muscle strength of typeⅠ and type Ⅱ muscle fibers significantly increased in both groups,while the fatigue of type Ⅰ and type Ⅱ muscle fibers significantly decreased(P<0.05).There were no significant differences in the scores of stigma,social avoidance,anxiety and depression scales between the two groups before interventions(P>0.05),and the scores of all scales significantly decreased after interventions,especially in the study group than the control group(P<0.05).Before interventions,there was no significant difference between the two groups in each dimension of coping styles(P>0.05).After interventions,the scores of avoidance dimension and yield dimension significantly decreased in both groups,while the scores of face dimension significantly increased in both groups,which were more pronounced in the study group than the control group(P<0.05).Conclusion Narration interventions based on stress system theory can improve the stigma,social avoidance,anxiety and depression of PFD patients combined with SUI,promote the patients to actively face the disease and recovery,and help promote the recovery of the disease and improve the quality of life.
