Effect of enhanced recovery after surgery combined with anal lifting exercise on urination function in patients with prostatic hyperplasiaAbstract:Objective To analyze the effect of enhanced recovery after surgery(ERAS)combined with anal lifting exercise on postoperative complications and rehabilitation of patients with prostatic hyperplasia.Methods A total of 92 surgically treated patients with prostate hyperplasia in the urology department from April 2022 to October 2023 were selected as subjects.They were randomly divided into two groups based on the computer random number table method,with 46 cases per group.Routine nursing was applied to patients in the control group,and ERAS combined with anal lifting exercise was given to those of the observation group.The occurrence of complications,recovery process,urination function and erectile function were compared between the two groups.Results The incidence of postoperative complications in the observation group(2.17%)was lower than that in the control group(13.04%)(P<0.05).The time for bladder irrigation,first postoperative exhaust,indwelling catheter,first getting out of bed,first urination and hospital stay in the observation group were shorter than those in the control group(P<0.05).Compared with before nursing,the residual urine volume,urine volume,maximum urine flow rate and average urine flow rate of both groups improved at 3 months after surgery,and the residual urine volume in the observation group was less than that in the control group,the urine volume was more than that in the control group,and the maximum urine flow rate and average urine flow rate were also higher than those in the control group(P<0.05).Compared with before nursing,the erectile hardness score(EHS)and international index of erectile function(IIEF-5)of both groups decreased at 3 months after surgery,and the observation group was higher than the control group(P<0.05).Conclusion ERAS combined with anal lifting exercise in patients with prostate hyperplasia surgery can achieve remarkable results,which can not only reduce complications,shorten the postoperative rehabilitation process of patients,but also promote the recovery of urination function and erectile function.
Evaluation of the effectiveness of early rehabilitation strategies in improving the long-term prognosis of sepsis patientsAbstract:Objective To evaluate the effectiveness of early rehabilitation strategy development in the long-term prognosis of sepsis.Methods A total of 80 sepsis patients admitted to the Intensive Care Unit(ICU)of Cangzhou People's Hospital,from August 2022 to August 2024 were selected.They were randomly divided into two groups,with 40 cases in each group.Patients in the control group received routine intervention,and those in the observation group implemented early rehabilitation strategies for intervention.The nutritional status,gastrointestinal function,immune function,long-term efficacy,and rehabilitation self-efficacy levels of the two groups were compared.Results After the intervention,the levels of serum prealbumin,serum transferrin and serum albumin in both groups were higher than those before the intervention,and the levels in the observation group were higher than those in the control group(P<0.05).After the intervention,the indicators related to gastrointestinal function in both groups were lower than those before the intervention,and the levels in the observation group were lower than those in the control group(P<0.05).After the intervention,the levels of CD3+and CD4+in both groups were higher than those before the intervention,and the level of CD8+was lower than that before the intervention.Moreover,the levels of CD3+and CD4+in the observation group were higher than those in the control group,and the level of CD8+was lower than that in the control group(P<0.05).After the intervention,the APACHE Ⅱ score and SOFA score in both groups were lower than those before the intervention,and the COOLI-74 score was higher than that before the intervention.Moreover,the APACHE Ⅱ score and SOFA score in the observation group were lower than those in the control group,and the COOLI-74 score was higher than that in the control group(P<0.05).After the intervention,the SER score in both groups was higher than that before the intervention,and the score in the observation group was higher than that in the control group(P<0.05).Conclusion The development of early rehabilitation strategies for sepsis patients can effectively improve their gastrointestinal function,nutritional status,immune function,long-term efficacy,and rehabilitation self-efficacy,with significant effects.
Construction and application of a health education program for patients with chronic gastritis complicated with gastric ulcer based on the protection motivation theoryAbstract:Objective To establish a health education program for patients with chronic gastritis complicated with gastric ulcer based on the protection motivation theory(PMT),and to explore its application effect.Methods A total of 96 patients with chronic gastritis complicated with gastric ulcer admitted from May 2023 to May 2024 were selected.They were assigned into the conventional group(n=42)and PMT group(n=54)based on different interventions.Through literature review,group discussion and Delphi expert inquiry,a PMT-based health education program for patients with chronic gastritis complicated with gastric ulcer was constructed.Patients in both groups received the conventional intervention method,and those in the PMT group were additionally intervened with the PMT-based health education program.Health knowledge,negative emotions,compliance behavior,health behavior,disease cognition,self-care ability,quality of life and intervention satisfaction were compared between the two groups.Results During the construction of the PMT-based health education program,the authority of experts,importance of expert coordination degree,and feasibility in the first round were 0.838,0.505,and 0.412,respectively.In the second round,they were 0.836,0.568 and 0.429,respectively.After the intervention,the scores for health knowledge acquisition,compliance behavior,healthy behavior,disease cognition,self-care ability,and quality of life in both groups increased,and the scores in the PMT group were higher than those in the conventional group(P<0.05).The score for negative emotions decreased,and the PMT group was lower than the conventional group(P<0.05).Conclusion PMT-based health education program for patients with chronic gastritis complicated with gastric ulcer is reliable and scientific.It can strengthen patients'compliance behavior and self-care ability,and improve health behaviors,emotional states,and quality of life.
Effects of Tongxinluo Capsules combined with enhanced external counterpulsation on dual-heart treatment and cognitive function in patients with ischemic cardiomyopathy heart failureAbstract:Objective To investigate the effects of Tongxinluo Capsules combined with enhanced external counterpulsation(EECP)on dual-heart treatment outcomes and cognitive function in patients with ischemic cardiomyopathy(ICM)with heart failure.Methods Select 236 patients diagnosed with ICM heart failure who were hospitalized from February 2022 to June 2024.Randomly divide them into the control group and the experimental group,with 118 cases in each group.All patients received conventional heart failure pharmacotherapy.Patients in the control group additionally underwent EECP,while those in the experimental group received both EECP and Tongxinluo Capsules(4 capsules,3 times daily).EECP was administered 5 times weekly(60 minutes/session).Before and after 35 weeks of treatment,the 6-minute walking distance(6MWD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),anxiety(Self-Rating Anxiety Scale[SAS]),depression(Self-Rating Depression Scale[SDS]),sleep quality(the Pittsburgh Sleep Quality Index[PSQI]),and cognition(Mini-Mental State Examination[MMSE],Montreal Cognitive Assessment[MoCA])were compared.Fasting venous blood samples were analyzed for triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),aspartate aminotransferase(AST),alanine aminotransferase(ALT),creatinine(Cr),and brain natriuretic peptide(BNP)via immunoassay.Results Baseline 6 MWD,LVEF,LVEDD,BNP,MoCA,MMSE,SAS,SDS,PSQI,TC,TG,HDL-C,LDL-C,ALT,AST,and Cr showed no significant differences between groups(P>0.05).After 35 weeks,both groups exhibited significantly increased 6MWD,LVEF,MoCA,and MMSE scores(P<0.05),and reduced BNP,LVEDD,SAS,SDS,and PSQI scores(P<0.05).The experimental group demonstrated superior improvements in the 6MWD,LVEF,MoCA,and MMSE(P<0.05),with significantly lower BNP,LVEDD,SAS,SDS,and PSQI scores compared to the control group(P<0.05).Conclusion Tongxinluo Capsules combined with EECP improves dual-heart treatment outcomes and cognitive function in ICM with heart failure patients,suggesting a novel therapeutic approach for this population.
Research on the establishment of the combined intensive care and general ward cardiac rehabilitation management plan for patients with type A aortic dissectionAbstract:Objective To explore the application effect of the combined intensive care and general ward cardiac rehabilitation management program in postoperative patients with type A aortic dissection.Methods A total of 120 patients with type A aortic dissection hospitalized for surgical treatment from January 2023 to December 2023 were selected.They were divided into the control group and intervention group,with 60 cases per group.Patients in the intervention group were managed by the combined intensive care and general ward cardiac rehabilitation management program.The 6-min walking distance(6MWT),left ventricular ejection fraction(LVEF),self-management ability(psychological state,social ability,cognitive function,lifestyle and treatment compliance),durations for staying in the intensive care unit(ICU),off-bed activities and postoperative hospitalization,and nursing satisfaction at discharge were compared.Results Compared with the control group,patients in the intervention group had significantly better 6MWT,LVEF,psychological state,cognitive function,lifestyle and treatment compliance,durations for staying in the ICU,off-bed activities and postoperative hospitalization,and nursing satisfaction at discharge(P<0.05).Conclusion The combined intensive care and general ward cardiac rehabilitation management program can promote the improvement of self-management ability,shorten rehabilitation time,improve heart function and increase nursing satisfaction in patients with type A aortic dissection after surgery,which is worthy of further clinical promotion and application.
Preoperative risk factors and preventive control measures for cerebral hypoperfusion syndrome after carotid artery stenting in patients with carotid stenosisAbstract:Objective To analyze the main preoperative risk factors for cerebral hypoperfusion syndrome(CHS)after carotid artery stenting(CAS)in patients with carotid stenosis via a retrospective analysis,thus providing references for formulating prevention and treatment measures of CHS.Methods A total of 210 patients with carotid artery stenosis who underwent CAS treatment in our hospital from October 2022 to October 2023 were selected.They were divided into the CHS group and the non-CHS group based on whether postoperative CHS occurred.General information,past disease history,collateral circulation compensation,preoperative vascular stenosis rate,preoperative cerebral blood flow asymmetry index(AI),preoperative mean blood flow velocity in the middle cerebral artery(MCAVmean)and surgical time window(onset to treatment time)were collected.Baseline characteristics were analyzed via univariate and multivariate logistic regression and compared between groups.The receiver operating characteristic(ROC)curve was plotted to analyze the predictive efficacy of independent risk factors for postoperative CHS in patients with carotid stenosis.Results Among 210 patients with carotid stenosis treated with CAS,12 cases experienced postoperative CHS(CHS group)and 198 patients did not experience CHS(non CHS group),showing an incidence of postoperative CHS of 5.71%(12/210).Significant differences in history of diabetes,new cerebral infarction within 1 month,collateral circulation compensation,preoperative vascular stenosis rate,AI,preoperative MCAVmean and operation time window were found between groups(P<0.05).Logistic regression analysis showed that diabetes,new cerebral infarction within 1 month,poor collateral circulation compensation,low AI and slow preoperative MCAVmean were independent risk factors for postoperative CHS in patients with carotid stenosis after CAS treatment(P<0.05).The ROC curve showed that the predicted AUC of diabetes,new cerebral infarction within 1 month,collateral circulation compensation,AI and preoperative MCAVmean for postoperative CHS in patients with carotid stenosis after CAS treatment was 0.677,0.638,0.756,0.734 and 0.743,respectively,with a sensitivity of 60.00%,57.50%,78.50%,70.00%and 72.50%,and a specificity of 50.00%,62.50%,71.50%,71.50%,71.50%and 82.50%,respectively.Conclusion Previous complications of diabetes,new cerebral infarction within 1 month,poor collateral circulation compensation,low AI and slow MCAVmean are the main preoperative risk factors for postoperative CHS in patients with carotid stenosis treated with CAS,all of which have certain predictive effect.Clinical attention should be focused on whether patients contain the above risk factors,and preventive control measures should be taken as early as possible to reduce the incidence of CHS.
Effects of aerobic exercise on glucose and lipid metabolism,islet β cells and the NF-κB/IRS2 signaling pathway in obese rats with type 2 diabetes mellitusAbstract:Objective To investigate the effects of aerobic exercise on glucose and lipid metabolism,islet β cells and the nuclear factor-kappaB(NF-κB)/insulin receptor substrate 2(IRS2)signaling pathway in obese rats with type 2 diabetes mellitus(T2DM).Methods A total of 40 Sprague-Dawley(SD)male rats in the specific pathogen free(SPF)level were randomly divided into normal(NO)group,model(MO)group,metformin(ME)group and metformin+aerobic exercise(MA)group,with 10 rats in each group.The obese T2DM rat model was established in the MO,ME and MA groups by streptozocin+high-fat and high-sugar feeding,while the model was not established in the NO group.After the successful modeling,oral gavage of 250 mg/kg metformin was given to rats of the ME group,oral gavage of 250 mg/kg metformin plus aerobic exercise was given to rats of the MA group.The same volume of normal saline was given to rats of the NO and MO groups.Changes in the rat body weight were observed.After the experiment,glucose and lipid metabolism of rats was detected,and the apoptosis of islet β cells was detected by terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL)method.Islet expressions of proteins involved in the NF-κB/IRS2 signaling pathway were detected by Western blotting.Results Compared with NO group,rats in the MO group had significantly higher body weight,fasting blood glucose,fasting insulin,insulin resistance index,total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),apoptosis of islet β cells and protein expression of NF-κB in islet tissue,but lower high-density lipoprotein cholesterol(HDL-C)and expression level of IRS2(all P<0.05).Body weight,fasting blood glucose,fasting insulin,insulin resistance index,TC,TG,LDL-C,apoptosis of islet β cells and protein expression of NF-κB in islet tissues of ME and MA groups were significantly lower,and HDL-C and expression level of IRS2 were significantly higher than those of MO group(all P<0.05),which were more pronounced in the MA group than the ME group(all P<0.05).Conclusion Aerobic exercise has a significant effect on obese rats with T2DM,which can significantly improve the glucose and lipid metabolism,and inhibit the apoptosis of pancreatic β cells by regulating the NF-κB/IRS2 signaling pathway.
The influence of finger exercises based on physical medical integration model on the attack frequency and pain degree of Raynaud's phenomenon associated with connective tissue diseaseAbstract:Objective To analyze the influence of finger exercises based on physical medical integration model on the attack frequency and pain degree of Raynaud's phenomenon associated with connective tissue disease.Methods From October 2021 to September 2024,176 patients with connective tissue disease accompanied by Raynaud's phenomenon treated in our hospital were randomly divided into the control group and observation group using a random number table method,with 88 cases per group.Patients in both groups were given routine interventions,while those in the observation group were additionally given finger exercise based on physical medical integration model.Patients in both groups were treated for 3 months.Hand ulcer score,Raynaud's score,Raynaud's phenomenon onset time,numbness,pain,finger cold time,cold frequency,anxiety and satisfaction were compared between groups.Results After 3 months of intervention,compared with the control group,patients in the observation group had significantly lower hand ulcer score,Raynaud's score,attack duration,attack frequency,numbness time,numbness score,pain time,pain score,finger cold time,cold frequency and Hamilton Anxiety Rating Scale(HAMA),and higher satisfaction(P<0.05).Conclusion Finger exercise based on physical medicine fusion mode has a definite effect on the treatment of connective tissue disease related Raynaud's phenomenon,which can significantly reduce the degree of pain and reduce the frequency of attacks.
Evidence-based practice of nursing intervention for patients with orthostatic hypotensionAbstract:Objective To develop an evidence-based nursing protocol for patients with orthostatic hypotension and apply it in clinical practice,with the aim of scientifically assessing its application effects.Methods Through a series of literature search and screening,comprehensive grading evaluations were conducted based on factors such as research methods,sample size,and result reliability,leading to the formation of best evidence entries.Based on the derived best evidence,and combined with clinical practice experience and patient needs,an evidence-based nursing protocol for patients with orthostatic hypotension was constructed by the evidence-based team.Patients with orthostatic hypotension treated between May 2022 and November 2022 were selected as the study subjects.Through simple random grouping within the ward,patients were divided into the experimental group and control group,receiving the evidence-based nursing protocol and routine nursing protocol,respectively.Comparisons were made regarding the changes in the incidence and severity of intolerance symptoms in patients with orthostatic hypotension before and after the application between groups.Results After the application of the evidence-based nursing,the incidence of intolerance symptoms in the experimental group significantly decreased,including the dizziness(90.79%vs 54.61%,χ2=50.134,P<0.001),blurred vision(40.79%vs 23.03%,χ2=11.037,P=0.001),instability on standing(25%vs 14.47%,χ2=5.316,P=0.021),palpitations(9.21%vs 1.97%,χ2=7.539,P=0.006),and nausea(5.26%vs 0.66%,χ2=4.122,P=0.042).The severity of intolerant symptoms also significantly decreased(Z=-10.084,P<0.001),showing a significant improvement in the experimental group than the control group(P<0.05).Conclusion Developing and applying an evidence-based nursing protocol for patients with orthostatic hypotension effectively reduce the incidence and severity of intolerant symptoms.
Diagnostic value of IL-17,CCL2 and HBP in bronchoalveolar lavage fluid for pulmonary infection in patients with chronic obstructive pulmonary diseaseAbstract:Objective To investigate the diagnostic value of chemokine ligand 2(CCL2),interleukin-17(IL-17)and heparin-binding protein(HBP)levels in bronchoalveolar lavage fluid(BALF)for pulmonary infection in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 109 COPD patients admitted to the respiratory department of our hospital from December 2021 to December 2023 were enrolled,and they were divided into infected group(n=35)and non-infected group(n=74)according to presence or absence of pulmonary infection.Clinical data,conventional infection markers including high sensitive C-reactive protein(hs-CRP)level,procalcitonin(PCT),serum amyloid A(SAA),and IL-17,CCL2 and HBP levels in BALF were compared between groups,and the correlation between IL-17,CCL2 and HBP in BALF and conventional infection markers was analyzed.Receiver operating characteristic(ROC)curve was plotted to evaluate the diagnostic value of IL-17,CCL2,HBP and conventional infection markers in BALF for pulmonary infection in COPD patients.In addition,net reclassification improvement(NRI),and integrated discrimination improvement(IDI)were performed to verify the effect of the combined diagnosis of IL-17,CCL2 and HBP in BALF versus single diagnosis or conventional infection markers.Results The levels of hs-CRP,PCT,SAA,IL-17,CCL2 and HBP in BALF in infected group were significantly higher than those in non-infected group(P<0.05).Pearson correlation analysis showed that IL-17,CCL2 and HBP in BALF of COPD patients were significantly positively correlated with hs-CRP,PCT and SAA(P<0.05).The area under the ROC curve(AUC)of the combination of IL-17,CCL2 and HBP was significantly higher than that of single indicator(0.931 vs 0.821 vs 0.833 vs 0.837),with the optimum sensitivity and specificity of 89.78%and 93.57%,respectively.The AUC of the combination of hs-CRP,PCT and SAA was significantly higher than that of single indicator(0.915 vs 0.813 vs 0.779 vs 0.827),with the optimum sensitivity and specificity of 80.00%and 89.19%,respectively.Compared with a single index,IL-17+CCL2+HBP and hs-CRP+PCT+SAA significantly improved the diagnostic ability of pulmonary infection in COPD patients,especially IL-17+CCL2+HBP.NRI and IDI were 0.736(0.581-0.927)and 0.075(0.052-0.131),respectively.Conclusion The levels of IL-17,CCL2 and HBP in BALF of COPD patients with pulmonary infection are significantly increased,and the combined detection of the three can improve the diagnostic efficiency.Therefore,the levels of IL-17,CCL2 and HBP should be monitored clinically in order to take timely treatment measures and improve the prognosis of patients.
MiR-543 regulates biological functions of ectopic endometrial stromal cells in endometriosis by targeting TWIST1Abstract:Objective To explore the effect of miR-543 in mediating the biological functions of ectopic endometrial stromal cells(EESCs)by targeting TWIST1.Methods The reverse transcription-quantitative polymerase chain reaction(RT-qPCR)was used to determine the expression level of miR-543.The cell counting kit-8(CCK-8)assay was utilized to evaluate the proliferation ability of EESCs.Transwell assay was conducted to assess the migration and invasion capabilities of EESCs.Flow cytometry was used to detect the apoptotic rate of EESCs.Additionally,the targeting relationship between miR-543 and TWIST1 was confirmed through a dual-luciferase reporter assay.Results Compared with the normal group,the expression of miR-543 in endometrial tissues of the EM group was decreased,while the expression of TWIST1 mRNA was increased(P<0.05).Compared with the NC mimics group,the transfection rate of miR-543 mimics group was increased(P<0.05).Compared with the NC mimics group,the miR-543 mimics group showed a lower OD450 value(P<0.05).Compared with the NC mimics group,the number of migrations and invasions in the miR-543 mimics group was decreased(P<0.05).Flow cytometry analysis indicated that the apoptosis rate in the miR-543 mimics group was increased compared with the NC mimics group(P<0.05).Compared with the NC mimics group,the luciferase activity of TWIST1wt in the miR-543 mimics group was decreased(P<0.05).Compared with the NC mimics group,the expression of TWIST1 protein in the miR-543 mimics group was decreased(P<0.05).Compared with the NC mimics group,the miR-543 mimics+Vector group showed a lower OD450 value(P<0.05).Compared with the miR-543 mimics+Vector group,the miR-543 mimics+pcDNA-TWIST1 group showed a higher OD450 value(P<0.05).Compared with the NC mimics group,the number of migrations and invasions in the miR-543 mimics+Vector group was decreased(P<0.05),and the apoptosis rate was increased(P<0.05).Compared with the miR-543 mimics+Vector group,the number of migrations and invasions in the miR-543 mimics+pcDNA-TWIST1 group was increased(P<0.05),and the apoptosis rate was decreased(P<0.05).Conclusion Overexpression of miR-543 inhibits the proliferation,migration,and invasion abilities of EESCs by regulating TWIST1,suggesting that miR-543 may be a potential therapeutic target for endometriosis.
Expression of lncRNA MELTF-AS1 in bladder cancer tissues and its effect on malignant biological behaviors of bladder cancer cellsAbstract:Objective To detect the expression of long non-coding RNA(lncRNA)MELTF-AS1 in bladder cancer(BC)tissues and cells,the relationship between its expression and tumor stage(T stage)of BC patients,and its impact on biological behaviors of BC(e.g.,cell proliferation,migration and invasion).Methods Cancer tissues and corresponding normal bladder urinary tract epithelial tissue adjacent to cancer tissues were surgically collected from 42 BC patients who underwent surgical treatment and confirmed by pathology in the Fourth Hospital of Hebei Medical University from September 2021 to October 2022.The expression of MELTF-AS1 in BC tissues,BC cell lines(5637,T24,SW780)and normal bladder epithelial cell line(SV-HUC-1)was detected by real-time quantitative PCR.Additionally,the relationship between MELTF-AS1 level and clinical parameters in BC patients was analyzed.The overexpression vector pcDNA3.1-MELTF-AS1 and knockdown vector si-MELTF-AS1 were constructed and transfected into 5637,T24,and SW780 cells.The effects of overexpression or knockdown of MELTF-AS1 on the proliferation,migration and invasion of BC cells were detected by MTS assay,colony formation assay,wound healing assay,and Transwell invasion assay.Results The relative expression of MELTF-AS1 in BC tissue was significantly higher than that in its corresponding normal bladder urothelium adjacent to cancer tissue.Compared with non-muscle invasive BC patients,the expression level of MELTF-AS1 in muscle invasive tissues of BC patients was significant higher.Its expression was significantly related to the depth of tumor invasion(T stage).MELTF-AS1 was up-regulated in all three bladder cancer cell lines compared with SV-HUC-1 cells,with the highest expression in 5637 cells.Knockdown of MELTF-AS1 significantly inhibited the proliferation,migration,and invasion abilities of 5637 cells,while overexpression of it significantly promoted the proliferation,migration,and invasion abilities of T24 cells(P<0.05).Conclusion MELTF-AS1 is overexpressed in BC tissues and cells,which can promote the biological behaviors of proliferation,migration and invasion in BC cells.
Clinical utility of serum matrix metalloproteinase 7 measurement in primary biliary cholangitisAbstract:Objective To evaluate the clinical value of serum matrix metalloproteinase7(MMP7)measurement in patients with primary biliary cholangitis(PBC).Methods From December 2022 to June 2024,113 PBC patients admitted to the Affiliated Hospital of Qingdao University were enrolled and subdivided into noncirrhotic PBC(n=80)and PBC with liver cirrhosis(n=33).A disease control group included 65 patients with chronic hepatitis B(CHB),comprising 44 noncirrhotic and 21 cirrhotic CHB patients.A healthy control(HC)group included 63 individuals undergoing routine physical examinations.Serum MMP-7 was measured by chemiluminescence immunoassay.Routine liver function tests(AST,ALT,GGT,ALP,Alb,TBil,DBil),fibrosis indicators(APRI,FIB4),platelet count,and other relevant parameters were also collected.Betweengroup comparisons and pre/posttreatment changes in followup cases were analyzed.Spearman correlation was used to assess associations.Diagnostic performance of MMP-7 for PBC was evaluated by receiver operating characteristic(ROC)analysis and area under the curve(AUC).Results Median serum MMP-7 in the PBC group(4.13[2.70-4.64]ng/mL)was significantly higher than in the CHB group(2.69[1.88-2.85]ng/mL]and the HC group(1.98±0.47 ng/mL)(P<0.01).MMP-7 was significantly higher in PBC patients with cirrhosis(5.94[3.25-8.56]ng/mL]than in noncirrhotic PBC patients(3.38[2.57-4.05]ng/mL](P<0.01).The AUC for discriminating PBC from healthy controls was 0.884;at a cutoff of 2.47 ng/mL,sensitivity was 80.5%,specificity 88.9%,positive predictive value 92.8%,and negative predictive value 71.8%.Combining MMP-7 with APRI and FIB4 yielded an AUC of 0.913,a modest,nonsignificant improvement over MMP-7 alone(P=0.051).The AUC for discriminating PBC from CHB was 0.757(95%CI 0.683-0.830).The AUC for differentiating cirrhotic from noncirrhotic PBC was 0.713(95%CI 0.595-0.832).Spearman analysis showed positive correlations between MMP-7 and APRI(r=0.491,P<0.01)and between MMP-7 and FIB4(r=0.459,P<0.01).In patients followed after 3-4 months of ursodeoxycholic acid(UDCA)therapy,MMP-7 levels declined significantly(P<0.05).Conclusion Serum MMP-7 is elevated in PBC patients,particularly those with cirrhosis.MMP-7 may serve as a potential marker for assessing disease progression in PBC.
Related factors and prediction model construction of re-tear in osteoporosis patients with rotator cuff injury after arthroscopic repair surgeryAbstract:Objective To explore the risk factors for re-tear in osteoporosis patients with rotator cuff injuries after arthroscopic repair surgery,and to construct a nomogram to predict its risk.Methods A total of 123 osteoporosis patients with rotator cuff injuries who underwent arthroscopic repair surgery from March 2020 to April 2023 were included.According to the 1-year occurrence of re-tear after surgery,they were divided into the occurrence group(36 cases)and non-occurrence group(87 cases).The risk factors for re-tear occurrence in patients were analyzed,and a nomogram was constructed.Results After preliminary comparison of the general data between the occurrence group and the non-occurrence group,a regression model was constructed.The results showed that the history of diabetes,smoking,fat infiltration,tear size,rotator cuff retraction,preoperative low levels of serum 25(OH)D3,and surgical single row repair were the risk factors for postoperative re-tear in osteoporosis patients with rotator cuff injury(OR>1,P<0.05).A nomogram was constructed for parallel internal validation,and the internal calibration curve of the model fits well with the ideal curve,with an area under the curve(AUC)of 0.895(95%CI:0.835-0.955),indicating good prediction accuracy.Conclusion History of diabetes,smoking,fat infiltration,tear size,rotator cuff retraction,low levels of serum 25(OH)D3 before surgery,and single row repair surgery are all risk factors for postoperative rotator cuff tear in osteoporosis patients with rotator cuff injury.The nomogram can be used to predict the risk of re-tear.
Application of patient participation in decision-making to long-term management of exercise rehabilitation for lung cancer patients with surgical treatmentAbstract:Objective To investigate the application effect of patient participation in decision-making schemes in the long-term management of exercise rehabilitation for lung cancer patients with surgical treatment.Methods A total of 120 lung cancer patients admitted from June 2022 to December 2023 were included as the study subjects.They were randomly divided into the study group and control group by drawing lots.The control group of 60 cases received a routine management plan,while the study group of 60 cases implemented a long-term management plan for exercise rehabilitation with patient participation in decision-making.Lung function indicators(forced vital capacity[FVC],forced expiratory volume in one second[FEV1],peak expiratory flow[PEF]),fatigue symptoms(Eastern Cooperative Oncology Group Performance Status[ECOG-PS],Cancer Fatigue Scale[CFS),6-minute walk test[6MWT]),self-efficacy(exercise compliance rate,General Self-Efficacy Scale[GSES]),and quality of life indicators between two groups of patients under different management plans.Results There were no significant differences in FVC,FEV1,and PEF indicators between the two groups before interventions(P>0.05).After interventions,the levels of various lung function indicators in both groups significantly increased compared to pre-intervention values,which were significantly better in the study group than the control group(P<0.05).Before interventions,there were no significant differences in ECOG-PS,CFS,and 6MWT between the two groups(P>0.05).After interventions,the ECOG-PS and CFS scores of both groups significantly decreased compared to pre-intervention values,which were significantly lower in the study group than the control group.The 6MWT significantly increased compared to pre-intervention values,which was significantly longer in the study group than the control group(P<0.05).There were no significant differences in exercise compliance rate and GSES score between the two groups before interventions(P>0.05).After interventions,they significantly increased compared to pre-intervention values,which were significantly better in the study group than the control group(P<0.05).There were no significant differences in the scores of various dimensions of quality of life between the two groups before interventions(P>0.05).After interventions,the scores of both groups significantly increased compared to pre-intervention values,which were significantly higher in the study group than the control group(P<0.05).Conclusion Long-term exercise rehabilitation management with patient participation in decision-making can improve exercise rehabilitation compliance and self-efficacy in lung cancer patients,enhance lung function and exercise endurance,and improve fatigue symptoms and quality of life.
Exploration of the value of comforting interventions during gastrointestinal endoscopy with situational simulation educationAbstract:Objective To investigate the application value of comforting interventions under situational simulation education during gastrointestinal endoscopy.Methods From May 2022 to May 2024,128 patients examined with gastrointestinal endoscopy were selected and randomly divided into the control group and the study group,with 64 cases per group.Conventional interventions were given to all patients,and comforting interventions under situational simulation education were additionally performed in the study group.Vital signs,the Mini-Mental State Examination(MMSE),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Global Congenital Quotient(GCQ),and Visual Analog Scale(VAS)scores,physiological stress levels,quality of interventions and satisfaction rate were compared between groups.Results After the intervention,patients in the study group had significantly better vital signs,lower SAS,SDS and VAS scores,physiological stress levels and incidence of adverse events,but higher MMSE and Glasgow Coma Scale(GCS)scores,health knowledge mastery level,compliance score,and overall satisfaction than the control group(P<0.05).Conclusion Comforting interventions under situational simulation education during gastrointestinal endoscopy effectively alleviate negative emotion,maintain vital signs,reduce adverse events and improve the satisfaction.
Effects of dopamine hydrochloride injection combined with hyperbaric oxygen therapy on the emergency treatment outcome,cardiac function and blood glucose metabolism in rats with acute heart failureAbstract:Objective To investigate the effects of dopamine hydrochloride injection combined with hyperbaric oxygen therapy(HBOT)on the emergency treatment outcome,cardiac function and blood glucose metabolism in rats with acute heart failure.Methods A total of 75 Sprague-Dawley(SD)male rats in the specific pathogen free(SPF)level were randomly divided into sham operation group,model group,dopamine injection group,HBOT group and combination group,with 15 rats per group.An acute heart failure rat model was established in all groups except for the sham operation group.After successful modeling,rats in the dopamine injection group were intraperitoneally injected with 2.0 mg/kg dopamine hydrochloride injection,and HBOT was given to those in the HBOT group.Both dopamine hydrochloride injection and HBOT were given to those in the combination group.The number of deaths,heart rate(HR)and mean arterial pressure(MAP)were recorded.The D700 LAB color ultrasound diagnostic instrument was used to detect cardiac function.Glucose metabolism index was detected by the commercial kit.The pathological morphology of myocardium was observed by hematoxylin-eosin(H&E)staining.The protein levels of p38 mitogen activated protein kinase(p38MAPK),extracellular regulated kinases 1(ERK1)and protein kinase C(PKC)were detected by Western blotting.Results Compared with sham operation group,rats in the model group had significantly higher number of deaths,mean MAP,left ventricular internal dimension at end-diastole(LVIDd),Left ventricular internal dimension at end-systole(LVIDs),glucose(GLU),glycated serum protein(GSP),fasting insulin(FINS),plasma glucose(PG),and protein levels of p38MAPK,ERK1 and PKC,but lower HR,left ventricular ejection fraction(LVEF)and left ventricular fraction shortening(LVFS)(all P<0.05).Compared with model group,rats in the dopamine injection and HBOT groups had significantly lower number of deaths,mean MAP,LVIDd,LVIDs,GLU,GSP,FINS,PG,and protein levels of p38MAPK,ERK1 and PKC,but higher HR,LVEF and LVFS(all P<0.05),but comparable between the latter two groups(P>0.05).Compared with HBOT group,rats in the combination group had significantly lower number of deaths,mean MAP,LVIDd,LVIDs,GLU,GSP,FINS,PG,and protein levels of p38MAPK,ERK1 and PKC,but higher HR,LVEF and LVFS(all P<0.05).Conclusion Dopamine hydrochloride injection combined with HBOT can improve the emergency treatment outcome of acute heart failure rats,improve cardiac function,reduce blood sugar and play a role in cardiac protection.
Advances of Src protein kinase in liver cancerAbstract:Primary liver cancer(PLC)is one of the most prevalent and lethal malignancies worldwide,characterized by complex pathogenesis and poor prognosis.Src is the first identified transformation product of oncogenic viruses and the prototype of SRC family kinase(SFK),which plays a critical role in cell growth,division,migration and survival signaling pathways.It is involved in several signaling pathways such as mitogen-activated protein kinases(MAPK),phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT1),signal transducers and activators of transcription(STATs),and closely related to the development of various tumors such as hepatocellular carcinoma.Aberrant activation of Src in PLC promotes metastasis,inhibits apoptosis,and enhances energy metabolism,thereby accelerating tumor progression and correlating significantly with unfavorable clinical outcomes.Furthermore,Src contributes to cancer-induced bone pain by modulating osteoclast activity and the tumor microenvironment,facilitating bone metastasis.This review systematically summarized the structural features and biological functions of Src,as well as its associated signaling pathways,with a focus on elucidating the molecular regulatory mechanisms of Src in the pathogenesis of PLC.The findings may provide novel theoretical insights for the development of targeted therapeutic strategies against PLC.
Advances in multiple mechanisms of thrombospondin-1 in regulating tumour angiogenesisAbstract:Thrombospondin-1(TSP-1)is a multifunctional extracellular matrix protein that plays a complex dual role in tumor angiogenesis.Recent research has made significant progress in elucidating the mechanisms by which TSP-1 both inhibits and promotes angiogenesis.TSP-1 exerts its regulatory effects for tumor angiogenesis primarily via interactions with cell membrane receptors like CD36,CD47,and integrins.The CD36-mediated signaling pathway inhibits tumor angiogenesis and induces endothelial cell apoptosis,whereas TSP-1 binding to integrins can potentially stimulate neovascularization.Additionally,TSP-1 influences the tumor microenvironment via activating the transforming growth factor-beta(TGF-β)signaling pathway and modulating the activity of matrix metalloproteinases(MMPs),further affecting the dynamic balance of tumor angiogenesis.Due to its multifaceted functions,TSP-1 exhibits distinct biological effects in different tumor types and disease stages.A deeper understanding of its mechanisms is critical for deciphering the key regulatory networks of tumor angiogenesis and can provide novel therapeutic strategies targeting TSP-1-mediated pathways.Future studies should further explore the tissue-specific roles of TSP-1 and integrate molecular targeting approaches to enhance the precision and efficacy of cancer therapies.
Influencing factors for ventilator alarm fatigue among medical staff in the intensive care unitAbstract:Objective To explore the influencing factors for ventilator alarm fatigue among medical staff in the intensive care unit(ICU).Methods A total of 100 ICU medical staff in five medical institutions were involved in the use of ventilators and met the inclusion criteria were selected.They were surveyed about ventilator alarm fatigue using a questionnaire survey,and the survey was completed on May 30,2024.The condition of ventilator alarm fatigue was counted.After screening of univariate factors for ventilator alarm fatigue among ICU medical staff,logistic regression was performed to identify influencing factors.Results In this survey,the total score of ventilator alarm fatigue of ICU medical staff was 24.45±3.25 points,showing a severe state of ventilator alarm fatigue.There was a significant difference in the alarm fatigue degree among ICU medical staff with different positions,working years,monthly night shift number,ventilator alarm habit,job satisfaction and humanistic care quality(P<0.05).Logistic regression analysis showed that years of work,number of night shifts per month,habit of setting ventilator alarm,job satisfaction and quality of humanistic care were the influencing factors for ventilator alarm fatigue in ICU medical staff.Conclusion The ventilator alarm fatigue behavior of ICU medical staff is affected by many factors,and corresponding measures should be actively formulated and implemented to improve their ventilator alarm fatigue behavior.
