Diagnostic value of IL-17,CCL2 and HBP in bronchoalveolar lavage fluid for pulmonary infection in patients with chronic obstructive pulmonary disease

Abstract:Objective To investigate the diagnostic value of chemokine ligand 2(CCL2),interleukin-17(IL-17)and heparin-binding protein(HBP)levels in bronchoalveolar lavage fluid(BALF)for pulmonary infection in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 109 COPD patients admitted to the respiratory department of our hospital from December 2021 to December 2023 were enrolled,and they were divided into infected group(n=35)and non-infected group(n=74)according to presence or absence of pulmonary infection.Clinical data,conventional infection markers including high sensitive C-reactive protein(hs-CRP)level,procalcitonin(PCT),serum amyloid A(SAA),and IL-17,CCL2 and HBP levels in BALF were compared between groups,and the correlation between IL-17,CCL2 and HBP in BALF and conventional infection markers was analyzed.Receiver operating characteristic(ROC)curve was plotted to evaluate the diagnostic value of IL-17,CCL2,HBP and conventional infection markers in BALF for pulmonary infection in COPD patients.In addition,net reclassification improvement(NRI),and integrated discrimination improvement(IDI)were performed to verify the effect of the combined diagnosis of IL-17,CCL2 and HBP in BALF versus single diagnosis or conventional infection markers.Results The levels of hs-CRP,PCT,SAA,IL-17,CCL2 and HBP in BALF in infected group were significantly higher than those in non-infected group(P<0.05).Pearson correlation analysis showed that IL-17,CCL2 and HBP in BALF of COPD patients were significantly positively correlated with hs-CRP,PCT and SAA(P<0.05).The area under the ROC curve(AUC)of the combination of IL-17,CCL2 and HBP was significantly higher than that of single indicator(0.931 vs 0.821 vs 0.833 vs 0.837),with the optimum sensitivity and specificity of 89.78%and 93.57%,respectively.The AUC of the combination of hs-CRP,PCT and SAA was significantly higher than that of single indicator(0.915 vs 0.813 vs 0.779 vs 0.827),with the optimum sensitivity and specificity of 80.00%and 89.19%,respectively.Compared with a single index,IL-17+CCL2+HBP and hs-CRP+PCT+SAA significantly improved the diagnostic ability of pulmonary infection in COPD patients,especially IL-17+CCL2+HBP.NRI and IDI were 0.736(0.581-0.927)and 0.075(0.052-0.131),respectively.Conclusion The levels of IL-17,CCL2 and HBP in BALF of COPD patients with pulmonary infection are significantly increased,and the combined detection of the three can improve the diagnostic efficiency.Therefore,the levels of IL-17,CCL2 and HBP should be monitored clinically in order to take timely treatment measures and improve the prognosis of patients.
