[Journal Article]HUO Xiaoran, FENG Yingpu, ZHANG Jingshuang et al.-Chinese General Practice Nursing2026, No.01

Abstract:Objective:To explore the core symptoms and nursing strategies in the symptom network in first stroke patients,so as to provide a basis for precise symptom intervention and nursing. Methods:A total of 255 patients with first stroke who were hospitalized in the neurology department of a tertiary grade A general hospital in Zhengzhou from October 2023 to June 2024 were conveniently selected as the investigation subjects. The Stroke Symptom Experience Scale was used for the investigation, and R software was applied for symptom network analysis, including network relationship analysis, core symptoms analysis, and accuracy and stability analysis of the network structure. Results:Symptom network analysis showed that the symptom"unable to do what one wants due to illness or hospitalization"has the highest incidence, and"decreased self-care ability"is the most severe symptom.A total of three symptom clusters, namely physical dysfunction, cognitive and speech dysfunction, and psychological dysfunction were extracted.Symptoms"limited limb movement A1"(rs=1.440),"decreased attention B2"(rs=1.175),and"lack of confidence in recovery C3"(rs=1.185)were the more intense symptoms among the three symptom clusters. After inspection, the accuracy and stability of the network were good, and the network model was relatively reliable. Conclusions:This study utilized the symptom network analysis method to explore the symptom network of patients with first stroke. For the symptoms with the strongest relationship and high predictability in the network,it suggested that nursing staff should prevtively cut off the strong connections among these symptoms during the intervention process to improve the intervention efficiency."Limited limb movement"is the most core symptom in the network, suggesting that in future research, this symptom should be targeted for intervention, and scientific intervention measures should be formulated to promote the improvement of patients'health conditions.

Efficacy of six machine learning models in predicting the risk of peritoneal dialysis-related peritonitis
[Journal Article]YANG Fang, ZHAO Jianqiu, QIE Shuwen et al.-Chinese General Practice Nursing2025, No.24

Abstract:Objective:To construct risk prediction models for peritoneal dialysis-associated peritonitis(PDAP)based on different machine learning(ML)algorithms, providing a reference for identifying high-risk patients. Methods:Retrospectively collect patients who underwent peritoneal dialysis in Guizhou Provincial People's Hospital from December 2009 to May 2024.Randomly divide them into a training set and a validation set at a ratio of 7∶3.In the training set, independent variables were screened through Lasso regression.Risk prediction models for PDAP were constructed based on six machine-learning algorithms, namely Logistic regression(LR),decision tree, support vector machines, random forest(RF),extreme gradient boosting, and artificial neural network. The performance of the models was evaluated based on the area under the receiver operating characteristic curve(AUC),accuracy, precision, recall, and F1-score,and the optimal model was selected. Results:A total of 982 peritoneal dialysis patients were included, among whom 221 patients developed PDAP,with an incidence rate of 22.51%.After five independent variables were screened out by LASSO regression based on ten-fold cross-validation,six ML models were constructed. In the training set,LR(AUC=0.800)performed the best compared with other models. In the validation set,RF(AUC=0.772)had the best performance. The LR model had a relatively high AUC value in the training set, which might indicate over-fitting.Further,based on the RF model, the feature variables were ranked in terms of importance,in the order of dialysis vintage, white blood cell count,contact-line contamination, catheter exit-site infection and/or tunnel infection, and constipation or diarrhea. Conclusion:The PDAP risk prediction model constructed based on the RF algorithm has the optimal performance, which can assist clinical medical staff in early assessment and prevention of PDAP.

[Journal Article]TAN Wanlan, ZHAI Juan, YOU Yiqing et al.-Chinese General Practice Nursing2025, No.24

Abstract:Objective:To establish a risk prediction model for hypostatic pneumonia in patients with severe craniocerebral injury undergoing tracheotomy, and to verify its effect. Methods:Convenient sampling was used to select 188 and 55 patients with severe craniocerebral injury who underwent tracheotomy at Friendliness Hospital Yangzhou in Jiangsu Province from January 2022 to October 2024 and November 2024 to January 2025,respectively.A modeling group and a verification group were established respectively. The patient information was collected by general data questionnaire and the number of patients with hypostatic pneumonia was counted. The risk factors of hypostatic pneumonia in patients with severe craniocerebral injury were screened by univariate and multivariate logistic regression analysis, and the risk prediction model was constructed based on the results of regression analysis, and its effect was verified. Results:The statistics showed that 22 patients had hypostatic pneumonia, the incidence was 11.70%.Univariate and multivariate logistic regression analysis showed that age≥60 years old, diabetes mellitus,Glasgow Coma Scale(GCS)score<7,hypoproteinemia,use of more than 2 types of antibiotics, and regular sputum aspiration were the risk factors for hypostatic pneumonia in patients with severe craniocerebral injury undergoing tracheotomy(P<0.05).The area under the receiver operating characteristic(ROC)curve of the subjects in the modeling group and the verification group were 0.821[95%CI(0.772,0.869)]and 0.817[95%CI(0.814,0.821)],respectively,and the Youden index were 0.568 and 0.600,respectively.The Hosmer-Lemeshow test of the modeling group and the verification group showed that P was>0.05,and the difference was not statistically significant. The calibration curve showed that the calibration curve was close to the reference line, and the prediction probability was in good consistency with the actual occurrence probability. This suggests that the model had good calibration degree and clinical practicability. Conclusion:Patients with severe craniocerebral injury have the risk of hypostatic pneumonia, and its risk factors are complex. The risk prediction model of hypostatic pneumonia in patients with severe craniocerebral injury undergoing tracheotomy established in this study has a good calibration degree and differentiation degree, which can effectively predict the risk of hypostatic pneumonia, provide a reliable basis for clinical nursing, and help strengthen the prevention effect, reduce the incidence of hypostatic pneumonia.

To construct a risk prediction model and nursing strategies for hypotension during arginine challenge test in children with short stature based on decision tree algorithm
[Journal Article]CHEN Lulu, YAO Haixia, YE Juan-Chinese General Practice Nursing2025, No.23

Abstract:Objective:To analyze risk factors for combined hypotension during the arginine stimulation test(AST)in children with short stature, build a decision tree model, and implement targeted nursing strategies. Methods:A total of 898 children with short stature admitted to the hospital from November 2020 to December 2024 were included in the study. All children underwent AST and were divided into a hypotension group(n=155)and a non-hypotension group(n=743)based on whether hypotension occurred during the test.Clinical data from both groups were collected.Univariate and multivariate logistic regression analysis were performed to identify risk factors for hypotension.A decision tree risk prediction model was created using SPSS Modeler software, and its predictive performance was evaluated. Results:Multivariate logistic regression revealed that malnutrition, insufficient rest, inadequate pre-test education, and body position changes during the first full meal were risk factors for hypotension during AST in children with short stature, while high-quality nursing was a protective factor(P<0.05).The decision tree model included five variables, with six levels and 15 nodes, with the first meal being the most influential factor. The area under the curve(AUC)for the decision tree was 0.681,compared to 0.650 for the logistic regression model. The Delong test showed Z=2.179,P=0.029 3.Conclusion:Malnutrition,inadequate rest the day before the test, insufficient pre-test education, body position changes during the procedure, and having a full meal after the first meal are risk factors for hypotension during AST in children with short stature.High-quality nursing serves as a protective factor. The decision tree model based on these risk factors demonstrates strong predictive power and can provide a theoretical basis for reducing hypotension during AST in children with short stature and guiding targeted nursing interventions.
