Prediction of Histological Grade in Invasive Ductal Carcinoma of the Breast Using Multi-parameter MRI Radiomic FeaturesAbstract:Objective To evaluate the predictive efficacy of multiparametric dynamic contrast-enhanced MRI(DCE-MRI)radiomics for histological grading in invasive ductal carcinoma(IDC)of the breast.Methods A total of 183 cases of IDC confirmed by pathology the First Affiliated Hospital of Jinzhou Medical University were included(139 cases of grade Ⅰ-Ⅱ and 44 cases of grade Ⅲ),and they were divided into training and test sets at a ratio of 7:3.Preprocessing,3D segmentation and feature extraction were conducted on T2WI,DWI,ADC and enhanced S1-S5 phases;features were selected through t-test,LASSO and variance inflation factor,and random forest(RF),support vector machine(SVM),logistic regression(LR),naive Bayes(NB)and multi-model fusion(RF+SVM+LR)were constructed.Receiver operating characteristic(ROC)curves were used to calculate the area under the curve(AUC),with DeLong's test comparing model performance.Decision curve analysis(DCA)and calibration curves assessed clinical applicability.Results The S3 phase(third post-contrast phase)demonstrated optimal performance among single-sequence models;in the testing set,RF,SVM,LR,and NB models trained on S3-phase features achieved AUCs of approximately 0.81,0.84,0.85,and 0.75,respectively;the multi-model fusion(S3 phase)achieved an AUC of 0.88.In the training set,RF and SVM outperformed LR,while LR surpassed NB(most comparisons P<0.05);RF marginally outperformed SVM(P>0.05).In the testing set,only the fusion model showed significantly higher performance than NB(P<0.05).DCA revealed that RF and LR multiparametric models yielded the highest net clinical benefit,with excellent calibration.Conclusion Multiparametric DCE-MRI radiomics can effectively differentiate high-and low-grade IDC.The S3-phase radiomic features and multi-model fusion strategy demonstrated superior predictive performance,highlighting their potential for clinical application.
The Mechanism of Ginkgo biloba Against Tongue Cancer Based on Network Pharmacology and Cellular ExperimentsAbstract:Objective To explore the mechanism of Ginkgo biloba against tongue cancer based on network pharmacology and cellular experiments.Methods The active components and their targets of Ginkgo biloba were retrieved from databases such as TCMSP,Swiss Target Prediction,PubChem,and GeneCards.Tongue cancer-related genes were obtained from GeneCards,and a PPI network was constructed.GO and KEGG enrichment analyses were performed,and molecular docking was used for validation.In vitro,the viability of tongue cancer cells(Tca-8113)was detected by CCK-8 assay,and apoptosis-related proteins were detected by Western Blot.Results A total of 234 targets of Ginkgo biloba,924 tongue cancer genes,and 81 common targets between the disease and the drug were identified.The core targets included BCL2,PTGS2,MMP9,AKT1,etc.The biological process(BP)enrichment results showed that the targets were related to the Ephrin receptor signaling pathway,one-carbon metabolism,and epidermal growth factor receptor signaling pathway.The cellular component(CC)enrichment results indicated strong associations with receptor complexes,membrane rafts,and plasma membranes.The molecular function(MF)enrichment results revealed associations with carbonic anhydrase activity,histone H3Y41 kinase activity,histone H2AXY142 kinase activity,and transmembrane receptor protein tyrosine kinase activity.The KEGG pathway enrichment results showed that the targets of Ginkgo biloba against tongue cancer were mainly involved in nitrogen metabolism,the PI3K-Akt signaling pathway,proteoglycans,endocrine resistance,and epidermal growth factor receptor tyrosine kinase inhibitors.The in vitro experimental results showed that the viability of tongue cancer cells decreased with increasing concentrations of Ginkgo biloba extract,and the extract inhibited the expression of Bcl-2 protein and the activation of the p-AKT/AKT signaling pathway.Conclusion Network pharmacology has revealed that Ginkgo biloba exerts its anti-tongue cancer effects through multiple targets and pathways.Cellular experiments have demonstrated that Ginkgo biloba may inhibit the proliferation of tongue cancer cells by downregulating the expression of anti-apoptotic factor Bcl-2 and inhibiting the activation of the PI3K-Akt signaling pathway.
Comparison of Oliceridine and Sufentanil for Postoperative Analgesia After Laparoscopic HysterectomyAbstract:Objective To investigate the efficacy and safety of oliceridine versus sufentanil for postoperative analgesia following laparoscopic hysterectomy.Methods The clinical data of 60 patients who underwent laparoscopic hysterectomy at Dongguan Songshan Lake Central Hospital Affiliated to Guangdong Medical University from February 2024 to December 2024 were included.The patients were randomly divided into the experimental group(O group)and the control group(C group)using a random drawing method.The O group received postoperative analgesia with oxilezide,while the C group received traditional sufentanil postoperative analgesia.The analgesic conditions 24 hours after the operation were studied.The analgesic effects,the dosage of analgesic drugs,and adverse reactions of the two groups of patients were compared.Results Postoperative analgesic efficacy was comparable between the two groups,with no statistically significant difference(P>0.05).No statistically significant difference was found in the mean number of PCA presses between the two patient groups(P>0.05).The incidence of nausea and vomiting was 10.00%in the O group,significantly lower than the 36.67%in the C group(P<0.05).There were no statistically significant differences in the occurrence of other adverse reactions between the two groups(P>0.05).Conclusion Oliceridine provides comparable analgesic efficacy to sufentanil for postoperative analgesia following gynecological laparoscopic hysterectomy,with a lower incidence of nausea and vomiting.
Study on Preparation and Anxiolytic Effect of Proteolytic Hydrolysates from Semen Ziziphi SpinosaeAbstract:Objective To improve the extraction process of Semen Ziziphi Spinosae protein hydrolysates(SZPHs)using Semen Ziziphi Spinosae as raw material and study their biological activities.Methods Defatted Semen Ziziphi Spinosae powder was used to obtain Semen Ziziphi Spinosae protein by alkali extraction and acid precipitation.Alkaline protease and neutral protease were used to hydrolyze Semen Ziziphi Spinosae protein at different ratios.The 1,1-diphenyl-2-picrylhydrazyl(DPPH)free radical scavenging capacity of SZPHs was compared to screen the optimal protease for Semen Ziziphi Spinosae.The anti-anxiety effect of SZPHs was verified by mouse elevated plus maze test(EPM),light-dark box test,and serum glutamate(Glu)level.Results The DPPH scavenging capacity of SZPHs was significantly higher than that of Semen Ziziphi Spinosae protein.Among them,the hydrolysate of alkaline protease showed the strongest activity,with a DPPH free radical scavenging rate of up to 87.40%,indicating that SZPHs have antioxidant activity.SZPHs could increase the total arm entries,open time percentage(OT%),open arm entries percentage(OE%)in EPM,and total shuttle times in the light-dark box test(P<0.01),while reducing the serum Glu content(P<0.01).Conclusion SZPHs have antioxidant and anti-anxiety activities,showing broad prospects in the food industry.
The Study on the Effect and Mechanism of miR-363-3p on the Biological Behavior of Laryngeal Squamous Cell CarcinomaAbstract:Objective To investigate the effects of miR-363-3p on the biological behavior of laryngeal cancer TU686 cells,and to analyze the target genes and mechanism of miR-363-3p.Methods A laryngeal cancer model overexpressing miR-363-3p was constructed through in vitro transfection;the effects of miR-363-3p on the functional status of laryngeal cancer TU686 cells were detected by CCK-8,Transwell experiments,and flow cytometry;the downstream target gene HIPK3 of miR-363-3p was predicted using the Targetscan bioinformatics database,and the double luciferase experiment was adopted for verification;RT-qPCR and Western Blot were used to detect the expression of HIPK3 in each group of cells after miR-363-3p overexpression.Results After overexpression of miR-363-3p,the proliferation rate of laryngeal cancer cells increased(P<0.05),the invasion rate increased(P<0.001),and the number of apoptotic cells decreased significantly(P<0.001).The double luciferase experiment confirmed the targeting relationship between miR-363-3p and HIPK3,and the results of RT-qPCR and Western Blot experiments showed that the expression level of HIPK3 in the overexpression group decreased(P<0.05).Conclusion miR-363-3p can promote the proliferation and invasion ability of laryngeal cancer TU686 cells,inhibit the apoptosis of laryngeal cancer TU686 cells,and there is a targeted regulatory relationship between miR-363-3p and HIPK3,promoting the occurrence and development of laryngeal cancer TU686 cells.
Observation on the Effect of Donepezil Combined with Rosuvastatin in the Treatment of Ischemic Stroke Complicated with Vascular DementiaAbstract:Objective To explore the clinical efficacy of the combined application of donepezil and rosuvastatin in the treatment of patients with ischemic stroke complicated with vascular dementia.Methods A total of 124 patients with ischemic stroke complicated with vascular dementia who were diagnosed and treated in Pharmacy Department of Kaifeng People's Hospital from March 2021 to May 2024 were selected as the research subjects and randomly divided into the observation group(n=62)and the control group(n=62)according to the odd-even number method.The control group was treated with atorvastatin combined with donepezil,and the observation group was treated with rosuvastatin combined with donepezil.The dementia conditions before and after treatment were compared between the two groups and evaluated using the Hasegawa Dementia Scale(HDS)and the Mini-Mental State Examination(MMSE);the changes in blood lipid levels,including high-density lipoprotein cholesterol(HDL-C),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and triglyceride(TG)levels;the changes in inflammatory factors,including C-reactive protein(CRP)and interleukin-1β(IL-1β)levels;the vascular endothelial function before and after treatment in the two groups was shown by the changes in endothelin-1(ET-1)and nitric oxide(NO)levels;and the adverse reactions occurred during the treatment process in the two groups were compared.Results After treatment,there was no significant difference in treatment efficacy between the observation group and the control group(P>0.05).The HDS,MMSE scores,and NO levels of the two groups were significantly higher than those before treatment,and the scores of the observation group were higher than those of the control group(P<0.05).The levels of CRP,IL-1β,and ET-1 in the two groups were lower than those before treatment,and the levels of CRP,IL-1β,and ET-1 in the observation group were lower than those in the control group(P<0.05).The levels of TG,TC,and LDL-C in the observation group were lower than those before treatment,and the level of HDL-C was higher than that before treatment(P<0.05).There was no significant change in the blood lipid levels of the control group compared with those before treatment(P>0.05).The levels of TG,TC,and LDL-C in the observation group were significantly lower than those in the control group,and the level of HDL-C was significantly higher(P<0.05).There was no significant difference in the total incidence of adverse reactions such as dizziness,diarrhea,sleep disorders,and joint pain between the two groups(P>0.05).Conclusion Donepezil combined with rosuvastatin has a prominent therapeutic effect in the treatment of ischemic stroke complicated with vascular dementia,which can reduce the inflammatory level of patients,reduce the damage of lipotoxicity to vascular endothelium,and improve cognitive ability.This regimen has good safety and is worthy of clinical promotion and use.
Analysis of Key Influencing Factors Affecting the Quality of HE-stained Sections Based on Multifactor Logistic Regression ModelAbstract:Objective To identify key adverse and beneficial factors by systematically investigating the relationship between various factors in the entire tissue preparation workflow and the quality of hematoxylin and eosin(H&E)-stained sections,providing a quantitative basis for quality control in the pathology department.Methods All sections from 2019 to 2023 were first defined as the sampling population.Subsequently,random sampling was performed using computer-generated random numbers,ultimately including 1,064 sections for analysis.All sections were graded for quality by pathologists.Binary Logistic regression was used to analyze the association between 15 technical parameters and the dichotomized section quality(optimal/non-optimal).A nomogram was constructed based on the results.Results Section quality was significantly associated with formalin fixation time,microtome service years,section thickness,tissue hardness,tissue homogeneity,presence of intra-tissue foreign bodies,and coverslipping procedure(P<0.05).The nomogram,constructed based on the multivariable logistic regression model,can be used to identify key factors associated with the occurrence of suboptimal quality and to quantify the strength of their associations.Conclusion This multivariable analysis identified the main factors associated with the occurrence of non-optimal sections.Optimizing the preparation process should focus on the aforementioned steps to achieve standardization and continuous improvement in pathological section quality.
Preparation of Metaplexis japonic Total Flavonoids-loaded Liposomes and Analysis of Their in Vitro Anti-tumor ActivityAbstract:Objective To prepare liposomes of total flavonoids from the Metaplexis japonic and analyze its anti-tumor activity in vitro.Methods The total flavonoid liposomes of Metaplexis japonic were prepared using the thin film dispersion method.The encapsulation efficiency was used as the evaluation index,and the lipid drug ratio,phosphate bile ratio,phosphate buffer solution dosage,and phospholipid mass concentration(PBS,pH=6.5)were the influencing factors.The Box-behnken response surface methodology was used to optimize the preparation process of the total flavonoid liposomes of Metaplexis japonic;the MTT assay was used to evaluate the inhibitory effect of the obtained liposomes on human liver cancer cell line HepG2.Results The optimal preparation process for total flavonoids liposomes from Metaplexis japonic was a lipid drug ratio of 8:1,phosphate bile ratio of 9:1,PBS dosage of 10 mL,and phospholipid mass concentration of 22 mg/mL,the encapsulation efficiency of 87.20%,and the cumulative release rate within 24 hours was 70.20%.MTT experiments showed that the liposomes had a more significant inhibitory effect on HepG2 cells.Conclusion The optimized preparation process of total flavonoids liposomes from Metaplexis japonica is feasible,and the liposomes have certain sustained release and anti-tumor activity in vitro.
Pulmonary Enteric Adenocarcinoma:A Report of 2 CasesAbstract:Pulmonary enteric adenocarcinoma(PEAC)is a rare subtype of non-small cell lung cancer(NSCLC).The etiology of this disease remains unclear,and its clinical manifestations and radiological features lack specificity.This article retrospectively analyzed two cases of PEAC confirmed by pathology and other comprehensive diagnostic methods,aiming to explore the clinicopathological characteristics and therapeutic strategies of PEAC.Pathological examination of PEAC showed that the intestinal differentiation component accounts for more than 50.00%,and the immunohistochemical markers CK20,CDX2,and CK7 were highly specific.The treatment of PEAC mainly followed the therapeutic principles of non-small cell lung cancer,with surgical resection being the primary approach in the early stages.For advanced patients,chemotherapy,targeted therapy,and immunotherapy were available strategies.The cases reported in this study represent the third domestic case of PEAC confirmed by genetic testing to harbor the KRAS G12V mutation,and the patients were treated with a combination of immunotherapy and chemotherapy.This article aims to provide reference experience for the clinical treatment of PEAC and to guide future research,with the hope of developing more effective therapeutic methods and strategies in the future.
Construction and Validation of a Predictive Model for Left Ventricular Hypertrophy in Chronic Kidney Disease Patients Based on Machine LearningAbstract:Objective To assess the risk factors for left ventricular hypertrophy(LVH)in chronic kidney disease(CKD)patients using machine learning algorithms and to construct a relevant predictive model.Methods A total of 674 patients with mild to moderate CKD,who visited the Nephrology Department of the Fifth Hospital of Xiamen between June 2019 and June 2024,were enrolled in the study.Left ventricular mass index(LVMI)was calculated,and the patients were divided into the LVH group(238)and the normal group(436).Chi-square test and independent sample t-test were used for group comparison.The"randomForest","gbm","treebag","pls","nnet",and"rjags"packages were used for random forest,gradient boosting,treebag model,partial least squares,neural networks,and Bayesian algorithms to explore the best predictive model for CKD patients'LVH risk.A nomogram was created based on the optimal algorithm's significant risk factors.The"p ROC"package was used to plot the ROC curve for model performance.The"rms"and"ResourceSelection"packages were used for calibration curve plotting,calibration C-index analysis,and Hosmer-Lemeshow test.Results The predictive performance of the algorithms for LVH in CKD patients was ranked from highest to lowest as follows:random forest,treebag model,gradient boosting,Bayesian,partial least squares,and neural networks.The most important risk factors for LVH in the random forest model were gender,serum creatinine,glomerular filtration rate,age,BMI,CKD stage,and hemoglobin levels.The nomogram based on these factors demonstrated a high predictive performance.ROC curve analysis of the internal validation showed AUCs of 0.721 for the training set and 0.747 for the validation set.The calibration curves demonstrated that the predicted probabilities closely matched the actual observed results,with both training and validation sets'calibration curves closely aligning with the ideal diagonal,indicating the model's reliability and accurate prediction of risk.Conclusion This study constructed a predictive model for LVH occurrence in CKD patients using machine learning algorithms,with random forest showing the best performance.The nomogram model showed good predictive power,providing an effective tool for the early prediction of LVH.
Preparation Process of Total Alkaloids Liposomes from Suaeda salsa and Analysis of Their Antibacterial ActivityAbstract:Objective To optimize the preparation process of total alkaloid liposomes from Suaeda salsa and study their antibacterial activity.Methods The total alkaloid liposomes of Suaeda salsa were prepared using the thin film dispersion method.The encapsulation efficiency of the liposomes was used as an indicator.Based on a single factor experiment,the Box Behnken experimental design principle was used to investigate the effects of phospholipid to cholesterol molar ratio,phospholipid to drug mass ratio,hydration time and phospholipid concentration on encapsulation efficiency.Results The optimal prescription for the total alkaloid liposomes of Suaeda salsa was a phospholipid to cholesterol molar ratio of 6∶1,a phospholipid to drug mass ratio of 11∶1,a hydration time of 2 hours and a phospholipid mass concentration of 19 mg/mL.At this time,the encapsulation efficiency of the liposomes was 81.52%.In vitro release experiments showed a cumulative release rate of 63.10%within 24 hours.The total alkaloid liposomes of Suaeda salsa had antibacterial activity against Staphylococcus aureus and Bacillus subtilis,with minimum inhibitory concentrations of 500 and 700 μg/mL,respectively.Conclusion The optimized prescription process of this experiment is stable and feasible,and the total alkaloid liposomes of Suaeda salsa can prolong the drug's action time and have good antibacterial activity.

Screening of Serum miRNAs Combination Biomarkers for Colorectal Cancer and Analysis on Multi-tumor Spectrum ExpansionAbstract:Objective To systematically screen differentially expressed microRNAs(miRNAs)in serum of colorectal cancer patients based on high-throughput sequencing technology,construct a multi-miRNA combination diagnostic biomarker,and provide novel molecular markers for early non-invasive diagnosis of colorectal cancer.Methods A case-control study design was adopted,including 30 patients with colorectal cancer and 20 healthy controls.High-throughput small RNA sequencing was performed using the Illumina HiSeq 2500 platform to screen for differentially expressed miRNAs(log2 Fold Change,|log2FC|≥1,P<0.05).Target genes were predicted using miRanda and RNAhybrid algorithms,and Gene Ontology(GO)functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses were conducted using the DAVID platform.Receiver operating characteristic(ROC)curve analysis and logistic regression methods were employed to construct the optimal miRNA combination diagnostic model.Results High-throughput sequencing identified a total of 205 differentially expressed miRNAs,including 98 upregulated and 107 downregulated.Functional enrichment analysis revealed that differential miRNAs were mainly enriched in tumor-related biological processes such as Notch signaling pathway(P<0.01),autophagy-related pathways(P<0.05),and transmembrane receptor protein tyrosine kinase signaling pathway(P<0.001).A 5-miRNA combination diagnostic biomarker consisting of miR-21-5p,miR-92a-3p,miR-155-5p,miR-143-3p,and miR-145-5p was successfully constructed,with an area under the ROC curve of 0.924(95%CI:0.867-0.981),diagnostic sensitivity of 90.00%,and specificity of 85.00%,significantly superior to traditional serum tumor markers CEA(AUC=0.663)and CA19-9(AUC=0.598).This biomarker also demonstrated excellent diagnostic efficacy for early-stage colorectal cancer(stage Ⅰ-Ⅱ)(AUC=0.895).Conclusion The 5-miRNA serum combination biomarker screened based on high-throughput sequencing has excellent diagnostic efficacy for colorectal cancer,especially suitable for early non-invasive screening,providing a new molecular tool for the precise diagnosis and clinical decision-making of colorectal cancer.
Synergistic Effect of Tooth Extraction Socket Characteristics on Implant Selection and Implant TorqueAbstract:Objective To explore the correlations between the key characteristics of the tooth extraction socket(shape,healing time,root tip depth)and the selection of implant shape as well as the implant torque,in order to provide evidence-based medical evidence for personalized implant treatment plans.Methods A retrospective study design was adopted,and 132 cases of single tooth extraction followed by implant restoration were included.The shape and root apex depth of the extraction socket were precisely measured using CBCT,and the shape of the implant and the actual implant torque data were extracted from the surgical records.Variance analysis,chi-square test,and multiple linear regression models were used to systematically analyze the association strength and statistical significance between the characteristics of the extraction socket and the selection of implant shape as well as the implant torque.Results The proportion of choosing conical implants for irregular-shaped extraction sockets(68.20%)was significantly higher than that for circular extraction sockets(23.50%);the implant torque in the delayed implant group(3 months or more after extraction)was(21.3±3.5)(N·cm),which was significantly higher than that in the immediate implant group(12.6±2.8)(N·cm);multivariate linear regression analysis showed that for every 1 mm increase in root apex depth,the average implant torque increased by 1.2(N·cm).Conclusion The shape of the extraction socket,the healing time,and the root tip depth are the core key factors influencing the selection of implant shape and the implant torque.Among them,the irregular-shaped extraction socket is more suitable for conical implants.Extending the healing time and increasing the root tip depth can significantly enhance the implant torque.
Application Effect Analysis of King's Interaction Goal Attainment Theory Combined with Multidisciplinary Team in Patients with Pulmonary EmbolismAbstract:Objective To explore and evaluate the rehabilitation effect of multidisciplinary team(MDT)intervention based on King's Theory of Goal Attainment in patients with pulmonary embolism(PE).Methods A total of 60 patients with acute pulmonary embolism(APE)were consecutively recruited from the Respiratory Medicine Ward of the First Affiliated Hospital of Jinzhou Medical University between August 2025 and November 2025.A computer-generated random number table was used to allocate these patients into a control group and an observation group in a 1∶1 ratio,with 30 cases in each group.The control group received routine nursing care for respiratory diseases,while the observation group was given an additional multidisciplinary team collaborative nursing intervention based on the King's Theory of Goal Attainment on top of the routine care.Both groups underwent synchronous in-hospital nursing and follow-up management for 4 weeks after discharge.After the intervention,the two groups were compared in terms of chest pain intensity,peak lower extremity venous blood flow velocity,coagulation function parameters,psychological status and quality of life scores.Results After the intervention,the chest pain severity in the observation group was significantly lighter than that in the control group(P<0.05).After the intervention,the peak velocity of lower extremity venous blood flow,activated partial thromboplastin time(APTT),prothrombin time(PT),and thrombin time(TT)increased in both groups,while the D-dimer(D-D)and fibrinogen(FIB)decreased.In addition,the improvement degree of each index in the observation group was significantly better than that in the control group(P<0.05).After the intervention,the emotional scores evaluated by the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)decreased in both groups,with a greater decrease in the observation group,and the scores were significantly lower than those in the control group(P<0.05);the scores of all dimensions of quality of life increased in both groups,with a more significant increase in the observation group,and the scores of all dimensions were better than those in the control group(P<0.05).Conclusion Multidisciplinary team-based nursing intervention integrated with King's Theory of Goal Attainment can significantly alleviate chest pain symptoms,improve coagulation function and hemodynamic indexes,relieve anxiety and depression,and remarkably enhance the quality of life in patients with pulmonary embolism.All outcome indicators demonstrate that this intervention yields superior effects compared with routine nursing care,indicating its suitability for clinical promotion.
Clinical Study on the Treatment of Post-Traumatic ARDS with Sivelestat Sodium Combined with DexamethasoneAbstract:Objective To investigate the clinical efficacy of sivelestat sodium combined with aexamethasone in the treatment of post-traumatic acute respiratory distress syndrome(ARDS).Methods A total of 80 patients with ARDS at Kaifeng People's Hospital from October 2022 to October 2024 were randomly selected for the study.Using a random number table,the patients were equally assigned to a control group(n=40)and an observation group(n=40).The control group received dexamethasone sodium phosphate injection,while the observation group received a combination of dexamethasone sodium phosphate injection and sivelestat sodium.The following outcomes were compared between the two groups:Murray Lung Injury Score(MLIS),Sequential Organ Failure Assessment(SOFA)score,serum inflammatory markers including procalcitonin(PCT),interleukin-6(IL-6),and C-reactive protein(CRP)levels,oxygenation index(PaO2/FiO2),blood lactic acid(Lac)concentration,carbonic anhydrase(CA)activity,adverse events,as well as clinical recovery and prognosis.Results Some patients in both groups died after treatment.The mortality rate of the observation group was 45.00%,which showed no statistical significance compared with the control group(P>0.05).The mechanical ventilation time(8.10±2.06)days was significantly lower than that of the control group(P<0.05).However,the ICU hospitalization time(11.83±2.42)days and the mortality rate of 45.00%showed no statistical significance compared with the control group(P>0.05).After treatment,the MLIS(5.10±2.16)and SOFA(7.23±2.86)scores in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the observation group had lower levels of PCT(0.35±0.04)ng/mL,IL-6(121.39±16.37)pg/mL,and CRP(16.26±3.41)mg/L compared to the control group(P<0.05).The PaO2/FiO2(90.82±10.82)mmHg in the observation group was higher than that in the control group,while Lac(3.59±0.53)mmol/L was lower(P<0.05).The total incidence of adverse reactions in the observation group(10.00%)was slightly higher than that in the control group(5.00%),but the difference was not statistically significant(P>0.05).The mechanical ventilation time(8.10±2.06)days in the observation group was significantly shorter than that in the control group(P<0.05),while there was no statistically significant difference in ICU length of stay(11.83±2.42)days or mortality rate(45.00%)between the two groups(P>0.05).Conclusion The combination of sivelestat sodium and dexamethasone in the treatment of post-traumatic ARDS significantly improves lung injury scores,reduces serum inflammatory factor levels,increases the oxygenation index,decreases lactate levels,and shortens mechanical ventilation time,demonstrating favorable efficacy.However,limitations remain in terms of ICU length of stay and mortality reduction,necessitating further research to optimize the treatment regimen.
Effect of bFGF Gel on Epidermal Barrier Function of Facial Acne Scar After Laser TreatmentAbstract:Objective To explore the adjuvant therapeutic effect of bovine basic fibroblast growth factor gel on facial acne scar after dot matrix laser treatment.Methods 80 patients with facial acne scars received in our hospital from June 2022 to June 2024 were selected as the research object.They were divided into two groups by drawing lots,40 cases in each group.All patients were treated with dot matrix laser,and the control group was given collagen dressing,while the observation group was treated with bovine basic fibroblast growth factor gel.The clinical effect,Echelle D'Evaluation Clinique des Cicatrices D'Acne(ECCA)grading scale,scab peeling time(SPT),skin barrier function(SBF),scar symptom score(SSS),collagen Ⅰ(COL Ⅰ),collagen Ⅱ(COL Ⅱ)transforming growth factor-beta 1(TGF-β1),transforming growth factor-β-Activated kinase 1(TAK1)and vascular endothelial growth factor(VEGF)were compared between the two groups.Results The total effective rate in the observation group was 92.50%,which was significantly higher than that in the control group(P<0.05).After treatment,the score of ECCA in the observation group was(11.00±1.97)and the scab peeling time was(6.55±0.90)d,which was significantly lower than that in the control group(P<0.05).After treatment,the oil(56.42±2.94)AU,epidermal water loss(18.51±2.01)g/(h·m2)and scar symptom score(15.38±3.99)in the observation group were significantly lower than those in the control group.The skin moisture content(49.68±4.28)AU in this group was significantly higher than that in the control group(P<0.05).Type Ⅰ collagen(57.58±9.34)μg/mg and type Ⅲ collagen(26.32±5.21)μg/mg in the observation group were significantly lower than those in the control group(P<0.05).TGF(31.58±5.86)μg/mg,TAK(22.36±3.64)μg/mg and VEGF(77.81±10.35)μg/mg in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Bovine basic fibroblast growth factor gel can significantly improve the therapeutic effect,improve the epidermal barrier function of patients,alleviate scar symptoms and adjust the level of related growth factors as an auxiliary treatment after dot matrix laser treatment of facial acne scars.
Research Progress on Causes and Mechanisms of Brain Insulin ResistanceAbstract:Brain insulin resistance is characterized by the weakened effect of insulin in the brain,and is regarded as a common pathological feature of metabolic disorders and neurodegenerative diseases.To explore the causes and pathological mechanisms of brain insulin resistance,we collected and analyzed the literature on brain insulin resistance in the PubMed database from 1946 to 2024.The result shows that apart from a small amount produced by some brain tissues,most of the insulin in the brain enters through the blood-brain barrier under the regulation of receptors;the lack of insulin in the brain due to insulin receptor resistance and reduced permeability of the blood-brain barrier,as well as the reduced insulin sensitivity of brain tissues themselves,are the two main manifestations of brain insulin resistance;the molecular mechanisms regulating the formation of brain insulin resistance involve multiple aspects such as inflammatory factors,oxidative stress,microtubule-associated protein Tau,glycogen synthase kinase 3β,sugar transporters 4,and neurotoxic substances;from a macroscopic perspective,behavioral factors such as high-sugar and high-fat diets,as well as tissue lesions such as fatty liver,aging,and brain trauma,can also lead to the situation of brain insulin resistance.This study systematically explored the formation mechanism,molecular mechanism and main regulatory factors of brain insulin resistance,aiming to provide a theoretical basis for future targeted intervention of brain insulin resistance to address metabolic disorders or related brain function diseases.

The Study on the Improvement of Spleen and Stomach Function After Surgery for Elderly Patients with Hip Fracture by MDT Mode Combined with Meridian Flow Timing MoxibustionAbstract:Objective To explore the effect of multidisciplinary team,(MDT)combined with meridian-based timed moxibustion on improving gastrointestinal function in elderly patients after hip fracture surgery.Methods 80 elderly patients with hip fracture who underwent surgical treatment in the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from May 2022 to May 2024 were selected as the research object.They were randomly divided into observation group and control group,with 40 cases in each group.The patients in the control group received routine management under MDT mode,while the patients in the observation group received additional intervention measures of moxibustion at the right time based on MDT management mode.The intervention period of the two groups was 4 weeks.The indexes of intestinal function recovery,gastrointestinal hormone level,spleen and stomach weakness syndrome score,constipation severity and incidence of adverse reactions were compared on the day after operation and after the whole intervention period.Results After the whole intervention cycle,the recovery indexes of gastrointestinal function,such as the recovery of bowel sounds,the first exhaust and the first defecation time,were significantly better in the observation group than in the control group(P<0.05).The incidence of constipation in the observation group was lower than that in the control group(P<0.05).At the same time,the level of gastrointestinal hormones in the observation group was more significant than that in the control group.In terms of the improvement of the total score of spleen and stomach weakness syndrome,the clinical symptom score of constipation(CCS)and the Bristol Stool Form Scale(BSFS),the observation group was better than the control group(P<0.05).In terms of safety,the incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The intervention plan of MDT mode combined with meridian flow injection timing moxibustion can significantly promote the recovery of gastrointestinal function in elderly patients with hip fractures after surgery,and effectively improve spleen and stomach weakness and postoperative constipation problems,with good safety.This plan has high promotion and application value.
