Clinical Efficacy of Self-Blood Acupoint Injection in Treating Stable Chronic Obstructive Pulmonary Disease

Abstract:Objective:To evaluate the clinical efficacy of self-blood acupoint injection in treating stable chronic obstructive pulmonary disease(COPD).Methods:A total of 120 COPD patients were selected and divided into control group and observation group using a random number table,with 60 patients in each group.During the observation period,3 and 2 patients respectively dropped out from the control and obser-vation groups,respectively.The control group(57 cases)was treated with budesonide,while the observation group(58 cases)was treated with self-blood acupoint injection in addition to the treatment provided to the control group.The clinical efficacy of the two groups was evaluated by assessing the incidence of adverse reactions and comparing the traditional Chinese medicine(TCM)syndrome scores,pulmonary function indexes,and serum inflammatory indexes[tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)levels]and the COPD Assessment Test(CAT)scores before and after 8 weeks of treatment.Results:The total effective rate of the observation group was 93.10%,significantly higher than that in the control group(73.68%)(P<0.05).After 8 weeks of treatment,TCM syndrome scores,serum TNF-α,IL-8 levels and CAT scores in both groups were significantly lower than before treatment(P<0.05),with the observation group showing significantly better results(P<0.001).The forced vital capacity(FVC)and forced expiratory volume in first second(FEV1)in both groups were significantly higher than before treatment(P<0.05),with the observation group showing significantly better results than the control group(P<0.05,P<0.001).There was no significant difference in the incidence of adverse reactions between the two groups:observation group(8.62%)and the control group(5.26%)(P>0.05).Conclusions:Self-blood acupoint injection therapy for patients with stable COPD.It can significantly reduce inflammation,improve symptoms,enhance lung function,and has a low incidence of adverse reactions.

Abstract:Chronic subjective tinnitus with anxiety has become a significant burden on patients,affecting both their work and daily life.As the exact pathogenesis has not been fully clarified,the effectiveness of western medicine in treating this condition has been limited.Guided by the thoughts of holism and syndrome differentiation and treatment,traditional Chinese medicine(TCM)employs Chinese medicinals,TCM formulas,acupuncture,moxibustion,tuina,and various treatment methods internally and externally to treat this disease,which have demonstrated notable advantages in managing this disorder.In this paper,viewpoints from the literature was summarized,and it was held that the occurrence chronic subjective tinnitus with anxiety state is closely related to heart,liver,spleen,kidney and other zang-fu organs,and the fundamental disease mechanism is mixed deficiency and excess.The excessive syndromes usually involve liver fire up stirring and liver qi stagnation,while the deficient syndromes involve deficiency of heart and spleen,heart and kidney failing to communicate,and deficiency of kidney essence.The literature was reviewed from Chinese medicinals treatment,integrated treatment by TCM and western medicine,and external treatment,totally 3 aspects,so as to provide a reference for the clinical treat and medicinal research of this disease,and to improve the clinical efficacy.A total of 41 research papers are cited in the references.

Abstract:HUANG Yuanyu founded the theoretical doctrine of"qi circulation,the four dimensions of the earth pivot".Based on the theory of qi transformation and ascending and descending,and combined with the theory of five evolutive phases and six climatic factors,a physiological model of six qi uniting the twelve meridians is established,which categorizes the activities of the meridians and internal organs as changes of qi,and a unique theoretical system is formed by qi transformation model of"the four dimentions of the earth pivot"combined with the detailed elaboration of the mechanism of qi ascending and descending.Based on the qi transformation and ascending and descending theoretical model of HUANG Yuanyu,this study explains the characteristics of the theoretical model,and chooses stranguria as the main line to compare the differences between HUANG Yuanyu's diagnosis and treatment of stranguria and the traditional understanding and discusses the characte-ristics of paying attention to the middle jiao qi and restoring qi transformation and ascending and descending as the first priority in his medication and prescription.

Abstract:Papillary thyroid carcinoma is one of the most common malignant tumors in the endocrine system.There is no unified standard of traditional Chinese medicine(TCM)syndrome differentiation and treatment.From the perspective of sweat pore,this paper discusses the theoretical connotation of facial sweat pore and its connection with the reprogramming of glucose metabolism for the first time,further clarifies that the occurrence and development of papillary thyroid carcinoma may be related to the reprogramming of glucose metabolism induced by the glucose sweat pore stagnation.Therefore,it is proposed that the method of relieving facial sweat pore stagnation can be used clinically to inhibit the reprogramming state of tumor glucose metabolism,thereby delaying the progression of the disease,which provides a new idea for TCM diagnosis and treatment of papillary thyroid carcinoma.

Abstract:Chronic heart failure combined with intestinal dysfunction is one of the end-stage complications of patients with heart failure.The interaction between chronic heart failure and intestinal dysfunction in the occurrence and development process has attracted increasing attention from medical practitioners,but the current treatment of this disease is still limited.Based on the theory of"The heart is exteriorly and interiorly related to the small intestine",and according to the clinical manifestation and pathogenesis of the disease,the paper proposes the staged treatment strategy of invigorating qi and strengthening spleen and nourishing heart,dispelling dampness,activating collaterals and protecting the intestine in the early stage of the disease;warming yang,invigorating spleen and nourishing heart,dissipating phlegm,clearing collaterals and stabilizing the intestine in the middle stage of the disease;and tonifying kidney,invigorating spleen and nourishing heart,regulating water,clearing collaterals and streng-thening intestine in the late stage of the disease.The goal of this study is to explore the feasibility and advantages of traditional Chinese medicine in the differentiation and treatment of chronic heart failure complicated with intestinal dysfunction by combining classical theory with modern researches,so as to provide new ideas and methods for the clinical diagnosis and treatment of this disease.

Clinical Study on Poge Jiuxin Decoction(破格救心汤)in Treatment of Acute Heart Failure After Myocardial Infarction

Abstract:Objective:To explore the clinical efficacy of Poge Jiuxin Decoction(破格救心汤)in the treatment of acute heart failure after myocardial infarction.Method:Ninety-one patients with acute heart failure after myocardial infarction were selected and divided by using random number table method,including 42 patients in the observation group and 49 patients in the control group.The control group was treated with conventional western medicine,and the observa-tion group was treated with Poge Jiuxin Decoction orally on the basis of the treatment of the control group.The changes of resting heart rate and systolic blood pressure before and after treatment were observed.N-terminal B-type natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),left ventricular end-diastolic inner diameter(LVESD),6 min walking test(6MWT)and Minnesota Living with Heart Failure Questionnaire(MLHFQ)score were compared before and after treatment.The number of hospitalizations due to cardiovascular adverse events within half a year and the occurrence of cardiovascular adverse events during the study period in the two groups were analyzed.Results:There were no significant differences in resting heart rate,systolic blood pressure,LVEF and LVESD between the two groups before and after treatment(P>0.05).Before treatment,there was no significant difference in NT-proBNP,6MWT and MLHFQ score between the two groups(P>0.05).After treatment,NT-proBNP and MLHFQ score in both groups were lower than those before treatment,and 6MWT was higher than that before treatment,the difference was statistically significant(P<0.05).After treatment,NT-proBNP in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse cardiovascular events between the two groups and the number of hospitalizations due to cardio vascular adverse events within half a year(P>0.05).Conclusion:Poge Jiuxin Decoction can improve short-term cardiac function of patients with acute heart failure after myocardial infarction,but it can not improve the long-term cardiac function.

Abstract:Objective:To explore the effect of superficial needling therapy combined with comprehensive reha-bilitation training in the treatment of aphasia after stroke of qi deficiency and blood stasis type and its effect on the neurological function of patients.Methods:A total of 80 patients with aphasia after stroke of qi deficiency and blood stasis type were selected as the study subjects,and were divided into the observation group and the control group with random number table method,with 40 cases in each group.The control group received comprehensive rehabilitation training treatment,while the observation group was supplemented with superficial needling therapy.Traditional Chinese medicine(TCM)syndrome score,Chinese Functional Communication Profile(CFCP)score,level of neural-functional defect score,clinical efficacy and incidence of adverse reactions of the two groups of patients were compared before and after treatment.Results:After treatment,the scores of speech retardation,hemiplegia and hypoesthesia in both groups were lower than those before treatment,and the scores of the observation group were lower than those of the control group,the difference was statistically significant(P<0.05).Treatment for two months,three months(end of treatment),five months after treatment,CFCP score increased and neural-functional defect score decreased in both groups compared with those before treatment,the degree of change in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The clinical efficacy of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).Conclusions:Superficial needling therapy combined with comprehensive rehabilitation training can significantly improve the therapeutic effect of patients with aphasia after stroke,relieve the clinical symptoms,promote the verbal communication ability and neurological function,and its safety is guaranteed to some extent.

Abstract:Objective:To evaluate the clinical efficacy and safety of Zizhu Ointment(紫朱软膏)in non-ischemic diabetic foot ulcer(DFU)treatment and its effect on the expression of ferroptosis-related biomarkers in patients with wound.Methods:According to the random number table method,all patients were allocated into the observation group and control group,with 53 cases in each group.On the premise of routine treatment combined with syndrome differentiation and internal treatment by Chinese medicine,Zizhu Ointment gauze dressings were applied in the observation group,while rb-bFGF combined with petroleum gauze dressings were applied in the control group.Dressings were changed once a day for 8 weeks in both group.The clinical efficacy,wound area,DMIST scale score,GSH and MDA content in wound tissue of the two groups were observed before and after treatment.Immunohistochemical changes of GPX4,SLC7A11 and TFRC in wound tissue were also compared between before and after treatment in the observation group.Adverse events were recorded during the treatment,and safety evaluation was conducted after the treatment.Results:On the clinical efficacy,wound area,DMIST scale score,GSH and MDA content in wound tissue,the observation group was superior to the control group(P<0.05).Positive expressions of GPX4 and SLC7A11 in wound tissue were increased in the observation group while TFRC expression was decreased(P<0.05).There was no adverse event in both groups.Conclusions:Zizhu Ointment is applicable for non-ischemic DFU in all courses.It is with good clinical efficacy and high safety,for it can reduce the wound area,improve DMIST scale score of the wound,enhance the antioxidant capacity of wound tissue,reduce the level of lipid peroxidation,and play a certain role in regulating the biomarkers related to ferroptosis in the process of wound healing.

Abstract:With the aging process of Chinese society,the prevention and treatment of perimenopausal osteoporosis have increasingly gained attention.Based on the interconnection among the heart,kidney and bone,this paper innovatively proposes the concept of the"heart-kidney-bone"axis,and the pathogenesis of perimenopausal osteoporosis is discussed from the perspective of traditional Chinese and western medicine on this basis:From the perspective of western medicine,it can be explained by neuroendocrine comorbidity;The pathogenesis of traditional Chinese medicine can be summarized as disharmony between the heart and kidney rooted in kidney deficiency.Therefore,the treatment should focus on tonifying the kidney and regulating the heart,with therapeutic principles of tonifying kidney and replenishing marrow,communi-cating the heart and the kidney.

Abstract:Objective:To investigate the effect of Bazhen Decoction(八珍汤)combined with highly active antiretroviral therapy(HAART)on the symptoms and quality of life of patients with human immunodeficiency virus infection/acquired immunodeficiency syndrome(HIV/AIDS)with deficiency of qi and blood.Methods:80 HIV/AIDS patients with deficiency of qi and blood syndrome were divided into the control group and the observation group by using random number table method,with 40 cases in each group,the observation group was treated with Bazhen Decoction combined with HAART,and the control group was treated with HAART.Symptom and sign scores,quality of life series scores,body mass index(BMI)and CD4+T lymphocyte count were compared between the two groups before and after treatment,and the safety of the treatment regimen was evaluated.Results:After treatment,the scores of symptoms and signs in both groups were lower than those before treatment(P<0.05),and the observation group was lower than the control group(P<0.05).After treatment,the total health perception degree,psychological field,social relation field and total score of quality of life in the observation group were increased compared with those before treatment(P<0.05),and the social relation field and total score of quality of life in the control group were increased compared with those before treatment(P<0.05).The total score of quality of life and the scores of overall quality of life,physiology,psychology and environment in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,BMI of the observation group was lower than that of the control group(P<0.05).After treatment,CD4+T lymphocyte count increased in both groups(P<0.05),and the observation group was higher than the control group(P<0.05).Conclusion:Bazhen Decoction combined with HAART can effectively relieve the symptoms and signs of HIV/AIDS patients with qi and blood deficiency syndrome,improve the quality of life and it is of good safety.

Application of Bitter Medicinals with Effect of Promoting Diuresis in Shennong's Classic of Materia Medica in Classical Prescriptions

Abstract:The article first screened out 27 kinds of bitter medicinals with the effect of promoting diuresis from Shennong's Classic of Materia Medica,analyzed their efficacy characteristics,then combined the original text of Treatise on Cold Pathogenic and Miscellaneous Diseases to analyze the compatibility and application regularities of 15 medicinals used in classical prescriptions,and finally drew the following conclusions.①In the medicinals with the effect of promoting diuresis recorded in Shennong's Classic of Materia Medica,the bitter medicinals accounted for the largest proportion.Most of them were cold in nature and have the efficacy characteristics of dissipating masses,eliminating food stagnation,removing blood stasis,and clearing heat.②The bitter medicinals with the effect of promoting diuresis in Shennong's Classic of Materia Medica were mostly used in classical prescriptions to treat various syndromes of qi counterflow caused by stagnation.When combined with pungent medicinals and bitter medicinals with other effects,they were respectively applicable to the stagnation symptoms caused by the accumulation of qi and water in the chest,abdomen and epigastrium,and water accumulation diseases gathering in the blood or accompanied by a heat syndrome.③The primary characteristic of bitter medicinals with the effect of promoting diuresis lies in their downward-directing and breaking/dispersing properties.④The herb pair of Chishao(Paeoniae Radix Rubra)-Fuling(Poria)has the effect of nourishing yin and promoting diuresis.Its application in classical prescriptions suggests that water accumulation can also cause exterior heat.⑤Zhimu(Anemarrhenae Rhizoma)in Guizhi Shaoyao Zhimu Decoction(桂知母汤)serves dual functions:"draining water"and clearing heat.

Abstract:Objective:To observe the clinical efficacy of Longsha opening-closing six-qi acupuncture(here-after referred to as six-qi acupuncture)combined with Duhuo Jisheng Decoction(独活寄生汤)in treating knee osteoarthritis of the liver and kidney deficiency type.Methods:A total of 60 patients with knee osteoarthritis of the liver and kidney deficiency type were selected and divided into a control group and an observation group according to the random number table method,with 30 cases in each group.Patients in the control group were treated with oral glucosamine hydrochloride capsules,while patients in the observation group were treated with six-qi acupuncture combined with Duhuo Jisheng Decoction.Both groups underwent continuous treatment for 4 weeks.The visual analogue scale(VAS)score for pain,the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,the Lequesne index score,adverse reactions,and clinical efficacy were compared between the two groups.Results:After treatment,the scores of VAS,WOMAC,and Lequesne index in both groups were lower than those before treatment(P<0.05),and the scores of the above items in the observation group were lower than those in the control group(P<0.05).During the treatment process,1 patient in the control group presented with mild nausea and vomiting symptoms,while no obvious adverse reactions were observed in the other patients.After treatment,the total effective rate of the observation group(93.3%)was higher than that of the control group(63.3%),and the difference was statistically significant(P<0.05).Conclusions:The combination of six-qi acupuncture and Duhuo Jisheng Decoction has a definite therapeutic effect on knee osteoarthritis of the liver and kidney deficiency type.It can significantly relieve the knee joint pain and inflammation in patients,and improve the knee joint mobility of patients with high safety.

Abstract:Parkinson's disease is a common neurodegenerative disorder of the nervous system,which is characterized primarily by the degeneration of dopaminergic neurons in the substantia nigra,leading to a reduction in dopamine neurotransmitters in the striatum and subsequent motor dysfunction.In recent years,Yinyanghuo(Epimedii Folium)has garnered widespread attention in the treatment of Parkinson's disease.This article reviews the mechanism of action of Yinyanghuo in the treatment of Parkinson's disease and elaborates on the specific mechanisms by which the main active components of icaritin and icariin,exert neuroprotective effects through antioxidation,improvement of mitochondrial dysfunction,inhibition of neuroinflammation,inhibition of neuronal cell apoptosis,and regulation of autophagy pathway,with a prospect of providing references and ideas for the clinical treatment of Parkinson's disease in the later stage.There are 46 references.inson's disease and summarizes its mechanisms of action,aiming to provide references for clinical treatment.

Abstract:The Pulse Classic by WANG Shuhe of the Western Jin Dynasty is the first to systematically categorize pulse characteristics into 24 types.This categorization has been the foundation and core of subsequent pulse inheritance and studies in traditional Chinese medicine.Among these 24 pulses,the drumskin pulse(革脉)and the soft pulse(软脉)have undergone changes during their transmission.Later transmitters often fail to understand the original reasons of the changes,leading to varying degrees of misinformation and misinterpretation:the soft pulse is replaced by the"soggy pulse"(濡脉),and the drumskin pulse becomes entangled with the"firm pulse"(牢脉).In-depth research reveals that the"drumskin pulse"and the"firm pulse"are essentially the same pulse,which should originally be the"strong pulse"(坚脉).Moreover,the soft pulse and the strong pulse are a pair of"relative pulses",differing in the force responded to the finger during palpation;the former is soft and yielding,while the latter is firm and strong.

Abstract:Evidence-based medicine is the main paradigm of contemporary medicine."Syndrome differentiation and treatment"as a distinctive feature of traditional Chinese medicine has long been regarded as the convergence point for the evidence-based transformation of traditional Chinese medicine.However,the"syndrome"in"syndrome differentiation and treatment"is not the"evidence"emphasized by evidence-based medicine.Therefore,traditional Chinese medicine should re-examine its own characteristics and explore the possibility of compatibility with evidence-based medicine under the identity of"disease-oriented".The idea of"disease-oriented"requires the traditional Chinese medicine community to undergo self-transformation in two aspects to adapt to the requirements of evidence-based medicine:one is to reconstruct the classification of traditional Chinese medicine diseases to lay the foundation for evidence-based research in traditional Chinese medicine;the other is to combine the theory of traditional Chinese medicine pathogenesis with the methodology of linguistics under the guidance of structuralist linguistics.

Abstract:"First aid stabbing of Shuigou acupoint"is a critical therapy in traditional Chinese medicine,and the mechanisms and clinical applications of this therapy in the treatment of acute encephalopathy have received significant attention.This review summarises the clinical efficacy and underlying mechanisms of the Shuigou acupoint in the treatment of acute cerebral diseases(e.g.traumatic brain injury,stroke and hepatic encephalopathy).Based on classical theories and modern research,stimulation of the Shuigou acupoint exerts therapeutic effects via multiple pathways and targets by stimulating the trigeminal nerve and other neuroanatomical structures.This includes regulating cerebral haemodynamics,inhibiting neuronal apoptosis,modulating inflammatory responses and influencing key signalling pathways.There are 43 references.

Abstract:Acupuncture is an effective method for the treatment of peripheral facial paralysis,but intractable facial paralysis is a major difficulty in acupuncture treatment.Based on the conventional acupoint selection and applying Guangming(GB37)and Hegu(LI4)source-connecting acupoints combination,the author's team can significantly relieve symptoms and enhance clinical efficacy with acupuncture in the treatment of intractable facial paralysis.Guangming acupoint can connect the liver and gallbladder,relieve depression and clear heat.Ancient literature records that it has a special therapeutic effect on eye diseases.Modern research also shows that needling the Guangming acupoint can improve blood circulation in the eyes and the neuronal activity of visual unblocking.Hegu acupoint is good at treating various diseases of the head and face.It can clear heat and improve eyesight.When combined with the Guangming acupoint,it can enhance the therapeutic effect on eye diseases.The application of Guangming and Hegu can not only effectively relieve the ocular symptoms after facial paralysis,but also promote the recovery of intractable facial paralysis by regulating the balance of yin and yang and qi and blood.
