The investigation of relationship between intestinal microflora and inflammatory bowel disease

Abstract:Objective To investigate the relationship between intestinal microflora and inflammatory bowel disease (IBD). Methods 106 cases IBD patients (observation group) were collected in our hospital from June 2014 to June 2015. All patients were divided into ulcerative colitis group (42 cases) and Crohn's disease group(64 cases) according to pathological type. 106 cases IBD patients were divided into symptom re-lief group(22 cases), mild activity group(34 cases) and moderate-severe activity group(50 cases) according to clinical severity scales. At the same time, 100 cases healthy people (control group) were collected from physical examination in our hospital. The infection rates of Staphylococcus aureus, Clostridium difficile and Salmonella in all people were analyzed. Results The infection rates of Staphylococcus aureus, Clostridium difficile and Salmonella in observation group were all higher than that of control group, and the differences all had statistical significance (Pall<0.05). In observation group, the infecion rates of Staphylococcus aureus, Clostridium difficile and Salmonella in Crohn's disease patients were all higher than that of ulcerative colitis patients, but the differences had no statistical significance(Pall>0.05). The infection rates of three bacteria in moderate-severe activity group were all higher than that of mild activity group and symptom relief group , and the differences all had statistical significance (Pall<0.05). Conclusion Intestinal microflora may be an im-portant factor to inducing IBD. The infection of Staphylococcus aureus, Clostridium difficile and Salmonella are closely related to occurrence and development of IBD.

Application of pro-brain natriuretic peptide and procalcitonin levels detection in the ventilation offline

Abstract:Objective To investigate the pro-brain natriuretic peptide (pro-BNP) and procalcitonin (PCT) levels detected in the application effect of mechanical ventilation in the offline analysis. Methods 80 patients in our hospital from January 2014 to December 2015 admitted with respiratory failure treated with me-chanical ventilation therapy were collected as research subjects. All subjects were given standard tests to achieve spontaneous breathing off machine , and which were divided into success group and failure group ac-cording offline research object. The levels of plasma pro-BNP, PCT, oxygenation index, average heart rate and mean arterial pressure and other indicators of all the patients were detected , and independent risk factors for the success of mechanical ventilation offline were analyzed. Results The levels of pro-BNP and PCT in success group were all lower than that of failure group , but oxygenation index was higher than that of failure group, and the differences all had statistical significance(Pall<0.05). The differences of average heart rate and mean arterial pressure of the two groups had no statistical significance (Pall>0.05). Pro-BNP and PCT were all independent risk factors for mechanical ventilation offline success. Conclusion The plasma pro-BNP and PCT levels can respond effectively offline risk mechanically ventilated patients , administration of a sponta-neous breathing trial linking and inspection of pro-BNP and PCT levels can be significantly improved mechan-ically ventilated patients offline success rate.

Clinical application of real-time fluorescence quantitative PCR detection in Mycobacterium tuberculosis pathological diagnosis

Abstract:Objective To study clinical application of real-time fluorescence quantitative PCR detec-tion in Mycobacterium tuberculosis (MTB) pathological diagnosis. Methods 144 patients with tuberculosis (TB) were collected in our hospital from May 2013 to February 2015. Paraffin-embedded human tissues of TB patients were detected by histopathological examination , acid-fast staining and FQ-PCR method. And positive rates of all methods were analyzed statistically. Results In 144 patients with the clinical diagnosis of TB, 68 cases (47.22%) were positive for MTB by FQ-PCR, 22 cases (15.28%) were positive for acid-fast staining and of which 74 cases (51.39%) for histopathological examination. The differences had statistical significance (P<0.01). The positive rates of MTB by FQ-PCR method in lung biopsy samples, lobectomy specimens and pleural resection specimens were all higher than those of histopathological examination and acid-fast staining. The positive rates of MTB by FQ-PCR method in lymph node biopsy and other tissue samples were higher than that of acid-fast staining, and lower than that of histopathological examination. Conclusion FQ-PCR assay can be effective in the classification of TB, and it is more sensitive than other special staining methods (acid-fast), which provides liable basis for the pathological diagnosis.

The clinical value of NSE and S100βjoint detection in the diagnosis of children with hand-foot-and-mouth disease combine brain injuries

Abstract:Objective To study the clinical application value of S100βserum protein and serum neu-ron-specific enolase (NSE) joint detection in the diagnosis of children with hand-foot-and-mouth disease (HFMD) combine brain injuries. Methods 318 cases of children with HFMD from October 2014 to February 2015 in our hospital were collected, and they were divided into mild group (157 cases), severe group (104 cases) and critical illness group (57 cases) according to the clinical manifestations. 100 cases of healthy chil-dren were selected as the control group. The S100βprotein and NSE levels of all the children were detected , and the results were analyzed statistically. Results There were statistical significance in the differences of S100βprotein and NSE levels among HFMD each groups and control group (F=7.91, F=6.37, Pall=0.010). The S100βprotein and NSE levels in HFMD each groups were all higher than that of control group , and the differences all had statistical significance(Pall<0.05). The S100βprotein and NSE levels increased along with the severity of patient's condition, and the differences all had statistical significance between each two HFMD groups (Pall<0.05). The S100βprotein and NSE levels of severe group in post treatment were all lower than that of before treatment, and the differences all had statistical significance (t=12.35, t=9.71, Pall<0.05). There were no statistical significance in the differences of S100βprotein and NSE levels of critical illness group between before and post treatment(t=0.23, t=0.21, Pall>0.05). Conclusion The detection of serum S100βprotein and NSE can be used for the early diagnosis, condition judgment, and the evaluation of treatment effect and prognosis of HFMD combine brain injuries.

The correlation of vitamin D, HbA1c and HOMA-IR in patients with type 2 diabetes mellitus

Abstract:Objective To discuss the relation of vitamin D with hemoglobin A1c (HbA1c) and HOMA-IR in patients with type 2 diabetes mellitus (T2DM). Methods 75 cases T2DM patients (research group) and 50 cases healthy people (control group) were collected in our hospital from March 2014 to August 2014. The fasting plasma glucose(FPG), fasting insulin(FIN), HbA1c, 25-hydroxy vitamin D3[25-(OH)D3] levels of all subjects were detected and HOMA-IR was calculation , all data were analyzed statistically. Results The levels of FPG, FIN, HbA1c and HOMA-IR in T2DM group were all higher than that of control group, and the differences all had statistical significance(Pall<0.05). The level of 25-(OH)D3 in T2DM group was lower than that of control group, and the difference had statistical significance (P<0.05). The level of 25-(OH)D3 had negative correlation with HbA1c and HOMA-IR(r=-0.548, r=-0.454, Pall<0.05). Conclusion The level of serum VitD has a remarkably decrease in T2DM patients , and has negative correlation with HbA1c level and HOMA-IR.

Molecular epidemiology survey of abnormal hemoglobin in Shaoguan region of Guangdong Province

Abstract:Objective To investigate the molecular epidemiological characteristics of abnormal hemoglobin (Hb) in Shaoguan, Guangdong Province. Methods A total of 9731 cases local people from November 2011 to May 2012 were subjected to blood cell analysis and Hb electrophoresis screening. Elec-trophoresis abnormal specimens were further gene sequencing. Results 45 cases of abnormal Hb were found. The incidence was 0.46%,including 7 case E zone, 9 cases G/D zone, 7 cases J zone,6 cases K zone,16 cas-es Q zone. Gene sequencing results showed that there were 25 cases α likeglobin mutation and 20 cases βlikeglobin mutation,and the gene frequency were 1.285×10-3 and 1.028×10-3, respectively. 9 kinds of abnor-mal Hb were found including 16 cases of Hb Q-Thailand, 7 cases of HbE, 6 cases of Hb G-Chinese, 6 cases of Hb New York, 3 cases of Hb G-Coushatta, 3 cases of Hb J-Bangkok, 2 cases of Hb J-Broussais, 1 case of Hb Ottawa, 1 case of Hb G-Taipei. Conclusion The incidence of abnormal Hb in Shaoguan is higher than the average of China. The gene type and distribution show a particular Hakka genetic characteristic of gene flow confluence of the south and north of China.

The early predictive value of serum procalcitonin combined blood culture in the early stage of pathogenic bacteria infection

Abstract:Objective To investigate the early predictive value of serum procalcitonin (PCT) com-bined blood culture in intensive care unit(ICU) patients with blood infection. Methods Retrospective analy-sis the clinical data of ICU patients whose blood culture were positive from January 2012 to June 2015 in our hospital. The serum PCT levels of all the patients were detected , and the results were analyzed statistically. Results In 107 cases patients, 62 cases [57.94%(62/107)] patients were gram negative bacterial infection, 40 cases[37.38%(40/107)] patients were gram positive bacterial infection, and 5 cases[4.68%(5/107)] pa-tients were fungi infection. There were statistical significance in the difference of PCT levels among gram nega-tive bacterial infection patients, gram positive bacterial infection patients and fungi infection patients (H=9.378,P=0.000). The PCT level of gram negative bacterial infection patients was higher than that of gram posi-tive bacterial infection patients and fungi infection patients , and PCT level of fungi infection patients was high-er than that of gram positive bacterial infection patients , and the differences all had statistical significance (Pall<0.05). The positive rate of 107 cases of infection patients was 78.50%(84/107). There were statistical significance in the difference of PCT positive rate among gram negative bacterial infection patients , gram posi-tive bacterial infection patients and fungi infection patients (χ2=12.635,P=0.020). The PCT positive rate of gram negative bacterial infection patients was higher than that of gram positive bacterial infection patients and fungi infection patients, and PCT positive rate of gram positive bacterial infection patients was higher than that of fungi infection patients, and the differences all had statistical significance (Pall<0.05). When the cutoff value was 3.15 ng/mL, the area under receiver operating characteristic curve was 0.895, sensitivity was 84.0%, specificity was 92.0%, positive predictive value was 91.0%, negative predictive value was 85.0%,positive likelihood ratio was 10.50, negative likelihood ratio was 0.17, Youden index number was 0.76 of PCT level for diagnosing gram negative bacterial infection. Conclusion Serum PCT has high value in the early predict of pathogenic bacteria in ICU patients with blood infection.
