Clinical Study on Modified Shenling Baizhu Powder(参苓白术散)Combined with Abdominal Acupuncture Catgut-Embedding in the Treatment of Simple Obesity of Spleen Deficiency and Dampness Excess Type

Abstract:Objective:To explore the application effect of Modified Shenling Baizhu Powder(参苓白术散)combined with abdominal acupuncture catgut-embedding on patients with simple obesity of spleen deficiency and dampness excess type.Methods:A total of 105 patients with simple obesity of spleen deficiency and dampness excess type were selected as the research subjects and randomly divided into the conventional group,the catgut-embedding group and the combination group by using the random number table method,with 35 cases in each group.The conventional group was given life guidance.On this basis,the catgut-embedding group was treated with abdominal acupuncture catgut-embedding,and the combination group was treated with abdominal acupuncture catgut-embedding combined with modified Shenling Baizhu Powder.The curative effects,traditional Chinese medicine(TCM)syndrome scores,changes in obesity signs[waist-hip ratio,body mass index(BMI)],intestinal flora(DNA counts of bifidobacterium,Bacteroides,Enterococcus),blood lipid levels[total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG)]and safety of the three groups were compared.Results:The therapeutic effect classification of the combination group was superior to that of the conventional group and the acupuncture embedding group(P<0.05).The total effective rates of the combination group and the acupuncture embedding group were 94.29%and 82.86%,respectively,both of which were higher than that of the conventional group(60.00%)(P<0.05).After the treatment,except for the number of intestinal flora DNA in the conventional group,the other indicators of the three groups were all improved compared with those before the treatment(P<0.05).After treatment,the comparisons of waist-hip ratio,BMI,as well as the main symptoms,secondary symptoms and total score of TCM syndromes among the three groups showed that the combination group<the catgut-embedding group<the conventional group(P<0.05).After treatment,the comparison of TC,TG and enterococcus DNA numbers showed that the combination group<the catgut-embedding group<the conventional group,and the comparison of HDL-C and Bacteroides,Bifidobacterium DNA numbers showed that the combination group>the catgut-embedding group>the conventional group(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion:Modified Shenling Baizhu Powder combined with abdominal acupuncture catgut-embedding has a reliable effective therapeutic effect on patients with simple obesity of spleen deficiency and dampness excess type.It can regulate the intestinal flora of patients,improve lipid metabolism,alleviate clinical signs of patients,and has high safety.

Research Progress on Neutrophil Extracellular Traps in Lung Cancer Based on the Theory of "Blood Stasis and Toxin"

Abstract:Neutrophil extracellular traps(NETs)are DNA-protein complex reticular structures released by activated neutrophils.NETs promote the pathological progression of lung cancer by mediating the release of inflammatory factors,promoting tumor-associated thrombosis and disrupting the homeostasis of the microen-vironment.Traditional Chinese medicine(TCM)believes that blood stasis accumulates internally and leads to toxin generation on the basis of a deficiency of healthy qi,which subsequently induces pathological changes,and ultimately results in the interweaving of stasis and toxins,which is one of the core pathogenic mechanisms of lung cancer.The hypercoagulable state of blood and hemodynamic changes mediated by NETs can be analogized to the TCM pathogenesis of"obstruction of blood vessels due to blood stasis",while the NETs-induced release of inflammatory factors leading to malignant imbalance of the microenvironment corresponds to the"accumulation of pathogenic toxins"arising from"deficiency of healthy qi".These two processes are causally linked and mutually reinforcing,jointly forming the pathological state of"intermingled stasis and toxin".Focusing on lung cancer,this article integrates traditional Chinese theory with modern medical research,elaborates on the correlation between NETs and the TCM concepts of stasis and toxin,and summarizes the mechanisms by which Chinese medicine exerts anti-lung cancer effects through the inhibition of NETs formation,with a prospect of providing scientific support for the therapeutic rationale of Chinese medicine in lung cancer treatment.

Real-World Efficacy Evaluation of Traditional Chinese Medicine Combined with Sancai Pricking Method in the Treatment of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome(ME/CFS)

Abstract:Objective:To evaluate the clinical efficacy of Chaihu Jia Longgu Muli Decoction(柴胡加龙骨牡蛎汤)combined with Sancai pricking method in the treatment of myalgic encephalomyelitis/chronic fatigue syndrome(ME/CFS)based on real-world evaluation.Methods:The diagnosis and treatment data of 400 patients with ME/CFS were collected and divided into the acupuncture group and the combination of acupuncture and medication group(combination group)according to the treatment methods.Patients in the acupuncture group were treated with Sancai pricking method,while those in the combination group were treated with oral administration of modified Chaihu Jia Longgu Muli Decoction on the basis of the Sancai pricking method.After 1∶1 propensity score matching(PSM),the baselines of the two groups were balanced and comparable.The fatigue scale-14(FS-14)score,the symptom checklist 90(SCL-90)score,fatigue syndrome mini-symptom score scale(SG)score,the traditional Chinese medicine(TCM)syndrome score,the improvement of clinical symptoms and safety of the two groups of patients before and after treatment were compared.Results:After correction by PSM,91 patients with baseline equilibrium in terms of age,gender,body mass index,disease duration,educational level,basic medical history,history of depression,smoking and drinking history,FS-14 score,SCL-90 score,SG score and TCM syndrome score were successfully matched in each group(P>0.05).Compared with those before treatment,the FS-14 score,SG score and TCM syndrome score of both groups decreased(P<0.05 or P<0.01),and the SCL-90 score of the combination group decreased(P<0.05)after treatment.In the intergroup comparison,after treatment,the FS-14 score,SCL-90 score and TCM syndrome score of the combination group were lower than those of the acupuncture group(P<0.05).In terms of symptom improvement,after treatment,the incidence epigastric or hypochondriac pain,abdominal distension,general heaviness,fatigue and weakness,numbness or pain in limbs in the combination group was lower than that before treatment(P<0.05),the incedence of abdominal distension,general heaviness,fatigue and weakness,numbness or pain in limbs in the combination group was lower than that in the acupuncture group(P<0.05).After the treatment,no adverse reactions occurred in either group.Conclusion:Chaihu Jia Longgu Muli Decoction combined with Sancai pricking method has a good clinical efficacy in the treatment of ME/CFS.It can effectively reduce the FS-14 score,SCL-90 score and TCM syndrome score,relieve clinical symptoms and has good safety.

Abstract:This paper introduces Professor SHI Bin's experience in the treatment of post-stroke dysarticulation and dysphagia with"Kaiyin Tongyan Acupuncture Method".Professor SHI Bin believes that post-stroke dysarticulation and dysphagia are manifested in the dysfunction of the tongue and pharynx.The location of the disease is in the brain,and the root cause lies in the lack of nourishment of the brain orifices,which leads to the loss of nourishment of the tongue and pharynx.In treatment,"Kaiyin Tongyan Acupuncture Method"is used to regulate the viscera,communicate the qi movement,awaken the brain and open the orifices,and achieve the effect of enriching the brain marrow,regulating the brain spirit and opening the brain orifices for post-stroke dysarticulation and dysphagia.This paper elaborates in details on the theoretical basis and operation of "Kaiyin Tongyan Acupuncture Method",and two test cases are attched for verification.

Abstract:Premenstrual dysphoric disorder(PMDD)is a severe subtype of Premenstrual Syndrome(PMS)in which patients experience a series of negative emotions such as irritability,anxiety or depression before menstruation.The sensitivity of the γ-aminobutyric acid(GABA)energy system to neurosteroid fluctuations during the menstrual cycle is reduced,leading to the emergence of negative emotions in PMDD patients.From the perspective of traditional Chinese medicine constitution theory,this paper expounds the manifestation of constitution susceptibility in the GABA energy system of PMDD patients,and explores the similarity between the formation process of PMDD susceptibility constitution types and the key factors affecting the imbalance of the GABA energy system based on the special physiological,psychological and emotional factors leading to liver catharsis disorder in women of childbearing age who are prone to PMDD.

Abstract:Systematic sphygmology for pattern differentiation is of great significance in the clinical diagnosis and treatment of traditional Chinese medicine(TCM).As the core method of TCM diagnosis,pulse diagnosis can reveal the etiology,pathogenesis and prognosis of diseases by analyzing the characteristics of pulses,and provide a basis for formulating precise therapeutic principles,treatment methods and prescriptions.There is an intrinsic compatibility between pulse diagnosis and traditional Chinese medicine.In clinical practice,the goal of"the syndrome emerges from the pulse and the medication is based on the syndrome"can be achieved.Through the objective analysis and reasoning of systematic sphygmology for pattern differentiation,we can not only select the appropriate prescription and drugs,adjust the drug flavors,dosage and com-patibility relationship,but also establish a new model of"pulse-herbs correspondence",so as to promote the precision and personalization of clinical diagnosis and treatment of TCM.Therefore,after the discussion and review of relevant experts from multiple fields,the clinical application specification of"pulse-herbs correspondence"of systematic sphygmology for pattern differentiation is finally constructed,which is conducive to the standardization and standardized application of the concept of"pulse-herbs correspondence"in clinical practice.

Clinical Study on Ultrasound-Guided Needle Knife Combined with Platelet-Rich Plasma Injection in the Treatment of Lateral Epicondylitis of the Humerus

Abstract:Objective:To explore the clinical efficacy of ultrasound-guided needle knife combined with platelet rich plasma(PRP)injection in the treatment of lateral epicondylitis of the humerus.Methods:Eighty patients with lateral epicondylitis of the humerus were selected and divided into an observa-tion group and a control group by using the random number table method,with 40 cases in each group.The control group was treated with ultrasound-guided needle knife and local block therapy while the observation group was treated with ultrasound-guided needle knife combined with PRP injection.The visual analogue scale(VAS)score,modified Hospital for Special Surgery(HSS)score,Mayo elbow performance score(MEPS),the levels of inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin(IL)-1β,IL-6]and the shear wave velocity(maximum,minimum,and average)of the common tendon of the inferior extensor muscle under ultrasound of the two groups were compared.The clinical efficacy of the two groups was evaluated.Results:After treatment,the VAS scores of both groups were lower than those before treatment,and the scores of the observation group were lower than those of the control group(P<0.05).The modified HSS scores and MEPS were higher than those before treatment,and the scores of the observation group were higher than those of the control group(P<0.05).After treatment,the levels of TNF-α,IL-1β and IL-6 in both groups decreased,and those in the observation group were lower than those in the control group(P<0.05).After treatment,the shear wave velocity of the total extensor tendon in both groups increased,and that in the observation group was higher than that in the control group(P<0.05).After treatment,the total effective rate of the observation group was 90%,which was higher than 65%of the control group(P<0.05).Conclusion:Ultrasound-guided needle knife combined with PRP injection can improve the clinical efficacy of patients with lateral epicondylitis of the humerus,relieve elbow joint pain,and facilitate the functional recovery of the elbow joint.

Abstract:This paper summarizes the clinical experience of Chief Physician WANG Jun in treating migraines with acupuncture.Migraine is a disease name in Western medicine.According to Western medical diagnosis,Master WANG classifies all symptoms into two categories:the main symptoms and the concurrent symptoms.He also conducts a traditional Chinese medicine(TCM)location analysis based on the different characteristics of the main symptoms and the concurrent symptoms,and believes that the TCM location of migraine mainly lies in the head,involving the three yang meridians of the head,the eyes,ears,tongue and other official orifices,as well as the liver,heart,spleen,stomach and kidneys and other internal organs.Based on the TCM locations and symptom characteristics of migraine,the following acupoint selection principles were established for acupuncture treatment:(1)For the main symptoms,selection is primarily based on meridian differentiation,which is combined with proximal acupoints selection;(2)For concurrent symptoms,the selection is primarily based on the differentiation of zang-fu organs.(3)For combined treat-ment of main and concurrent symptoms,selection is primarily based on root and node differentiation.Besides conventional acupuncture techniques,Master WANG emphasizes supplementary acupuncture maneuvers and patient education and care to ensure the maximum benefit from the treatment.

Effect of Yangxin Decoction(养心汤)Combined with Metoprolol on NT-proBNP,Heart Rate Variability and Efficacy in Patients with Arrhythmia

Abstract:Objective:To explore the effect of Yangxin Decoction(养心汤)combined with metoprolol on N-terminal B-type natriuretic peptide(NT-proBNP),heart rate variability and efficacy in patients with ventricular arrhythmia.Methods:Eighty patients with ventricular arrhythmia were randomly divided into control group and research group,40 patients in each group.The control group was treated with oral administration of metoprolol,and the research group was further treated with oral administration of Yangxin Decoction.Both groups were treated for 3 months.The total clinical effective rate and adverse reactions of the two groups after treatment were recorded.The traditional Chinese medicine(TCM)syndrome scores,the levels of serum high-sensitivity C-reactive protein(hs-CRP),NT-proBNP and adiponectin(APN)were detected before and after treatment.The standard deviation of all sinus beats(SDNN),standard deviation of mean sinus beats(SDANN),and the percentage of successive normal cardiac interbeat intervals greater than 50 msec(pNN50)were recorded by 24-hr continuous dynamic electrocardiogram recorder.Results:The total effective rate of the research group(75.00%)was higher than that of the control group(52.50%)(P<0.05).After the treatment,in both groups the TCM syndrome scores,hs-CRP and NT-proBNP levels decreased,APN,SDNN,SDANN and pNN50 increased(P<0.05).There was no statistically significance in adverse reactions between the two groups of patients.Conclusion:Yangxin Decoction combined with metoprolol can effectively reduce NT-ProBNP in serum of patients with ventricular arrhythmia with improved efficacy.

Effects of Shujin Huoxue Qutong Ointment(舒筋活血祛痛膏)on Pain Degree and Rehabilitation of Lumbodorsal Function in Patients with Thoracolumbar Vertebral Compression Fractures after Percutaneous Vertebroplasty(PVP)

Abstract:Objective:To analyze the effects of Shujin Huoxue Qutong Ointment(舒筋活血祛痛膏)on pain degree and rehabilitation of lumbodorsal function in patients with thoracolumbar vertebral compression fractures after percutaneous vertebroplasty(PVP).Methods:A total of 168 patients with thoracolumbar vertebral compression fractures undergoing PVP in The Third Department of Spinal Surgery,Luoyang Orthopedic-Traumatological Hospital of Henan Province were enrolled from August 2022 to August 2024.Random number table method was applied.Patients were divided into observation group and control group,84 patients in each group.The control group was treated with routine western medicines(calcium carbonate D3 tablets+alendronate sodium tablets),while treatment group was treated with Shujin Huoxue Qutong Ointment besides treatment in the control group.Both groups were treated for 3 courses.The clinical efficacy after treatment,scores of visual analogue scale(VAS),Oswestry Disability Index(ODI)and Japanese Orthopaedic Association(JOA),levels of serum interleukin(IL)-6 and tumor necrosis factor-α(TNF-α)1 day,1 month and 3 months after surgery were compared between the two groups.The occurrence of complications in the two groups was observed.Results:The total effectiveness rate of treatment group was significantly higher than that of control group(P<0.05).One month and 3 months after surgery,VAS scores in the observation group were significantly lower than those in control group(all P<0.05).One month and 3 months after surgery,ODI scores in the observation group were lower than those in control group,and JOA scores were higher than those in control group(all P<0.05).One month and 3 months after surgery,levels of IL-6 and TNF-α in the observation group were lower than those in control group(all P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Shujin Huoxue Qutong Ointment is good on the effectiveness rate in thoracolumbar vertebral compression fractures after PVP.It can significantly alleviate postoperative pain,promote the recovery of thoracolumbar function and relieve inflammatory response,with certain safety.

Abstract:The etiology of polymyositis/dermatomyositis combined with interstitial lung disease has not been clarified.Combined with the relevant traditional Chinese medicine(TCM)classics,it is believed that its clinical manifestations can be related to the lung and spleen in the five viscera of TCM.The theory of"origin-analogue"plays an important role in TCM.This paper expounds the"origin-analogue"relationship between lung and spleen from the aspects of zang-fu function,meridians and five elements.Based on the theory of"origin-analogue",the etiology and pathogenesis of the disease are explored from the two aspects of exogenous pathogen invasion and lung and spleen injury,and corresponding treatment methods are proposed in different stages(acute attack stage and remission stage),providing theoretical support for the TCM treatment of polymyositis/dermatomyositis combined with interstitial lung disease,and providing new ideas for the clinical treatment of the disease.

Observation on Efficacy of Tongue Acupuncture Combined with Ultrasound Therapy in Patients with Swallowing Disorders after Radiotherapy for Nasopharyngeal Carcinoma

Abstract:Objective:To study the effect of tongue acupuncture therapy combined with ultrasound on swallowing function of patients with dysphagia after radiotherapy for nasopharyngeal carcinoma.Methods:Sixty patients with dysphagia after radiotherapy for nasopharyngeal carcinoma who visited Cangzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2022 to May 2023 were selected and randomly divided into the control group(conventional rehabilitation and ultrasound treatment)and the observation group(conventional rehabilitation,ultrasound therapy,and tongue acupuncture therapy)according to the order of treatment,with 30 cases in each group.The duration of the swallowing period before and after treatment in the two groups of patients was determined by television fluorescence swallowing study(VFSS).The water swallowing test(WST)grades of the two groups of patients before and after treatment were evaluated by using the WST test.The clinical efficacy was valuated based on the changes in WST grades.The swallowing function of the two groups of patients before and after treatment was evaluated by using the Chinese version of the Anderson Dysphagia Scale(MDADI).Results:After treatment,the durations of the oral phase,swallowing reflex,pharyngeal phase,laryngeal closure phase in both groups decreased.Moreover,the observation group were greater than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the WST grades of the two groups of patients were better than those before treatment,and the observation group after treatment were better than those of the control group.The clinical efficacy of the observation group after treatment was better than the control group.MDADI score in both groups decreased,the observation group was greater than those in the control group.Conclusions:Tongue acupuncture therapy combined with ultrasound treatment can reduce the time required for swallowing food ball,liquids,and coughing during swallowing in patients with dysphagia after radiotherapy for nasopharyngeal carcinoma,as well as improve the swallowing function and enhance the quality of life.

Abstract:This article introduces Professor WANG Wenjian's"three-in-one"academic thought on paste prescriptions,which revolves around the three modules of"disease,symptom,and constitution",and achieves precise identification,tailored treatment,and flexible medication through the combination of disease diagnosis,symptom diagnosis,and constitution diagnosis.The guiding principle of the"three-in-one"is"conditioning in winter and curing diseases as the first priority",aiming to emphasize that the original intention,nature,and purpose of traditional Chinese medicine paste prescriptions are for treating diseases and promoting health,rather than just for nourishing the body.The first layer of the"three-in-one"is based on disease differentiation,and the same disease is treated with similar way;the second layer involves modifying based on symptom differentiation and balancing the body and spirit;the third layer is adjusting treatment based on the body constitution differentiation,making appropriate treatment individualized to patient,season and locality.One test case is attached.

Abstract:Ulcerative colitis is a chronic non-specific intestinal disease characterized by inflammatory changes in the mucosa of the colon and rectum.Toxicity and turbidity run through the entire course of the disease.The theory of"turbid-toxin lurking in the collaterals"proposed by renowned traditional Chinese medicine Master LI Diangui is of great significance in explaining the dynamic evolution process of this disease.The intestinal collaterals are intricately interwoven,capable of circulating qi and blood,and nourishing and irrigating the intestines.The latent toxins and pathogenic factors have characteristics such as lurking in the abyss,being sticky and difficult to dissolve,remaining dormant and recurring,and gradually accumulating in the collaterals.Therefore,during the active stage of ulcerative colitis,it is triggered by external factors,with latent toxins moving internally,toxicity lurking in the collaterals,and damage to the collaterals and tissues;in the remission stage,the pathogenesis is characterized by long-term illness causing damage to the collaterals,latent toxins remaining internally,turbid-toxin hidden in the collaterals,and intestinal weakness with blood stasis in the collaterals.In clinical treatment,the"eliminating the stagnated pathogen"method is used for differentiation and treatment based on the syndrome.During the active stage,pungent herbs are used to unblock the collaterals,eliminate turbidity and disperse toxins,remove stagnant matter and expel pathogenic factors;in the remission stage,sweet herbs are used to nourish the meridians,harmonize turbidity and alleviate toxins,and strengthen the collaterals and eliminate stagnated pathogen.

Abstract:Chronic atrophic gastritis with intestinal metaplasia is a chronic and intractable digestive disease,which is clinically characterised by weakness of the spleen and stomach,deficient earth being subjugated by wood,and emotional anxiety.The pathogenesis is characterized by dysfunction of the vital energy mechanism,the liver's failure of governing conveyance and dispersion,and weakness of the spleen and stomach.Therefore,based on the Shan's method of soothing the liver and regulating spirit,a new acupuncture treatment method of"strengthening the spleen,soothing the liver and regulating the spirit"is created,which combines acupuncture and moxi-bustion.This method follows the principle of tonifying deficiency and eliminating excess,and harmonizing yin and yang.It selects acupoints such as Baihui(DU20),Yintang(EX-HN3),Shenting(DU24),Neiguan(PC6),Taichong(LR3),Zhongwan(RN12),Liangmen(ST21),Shangwan(RN 13),Xiawan(RN 10),Gongsun(SP4),Taibai(SP3),Zusanli(ST36),Jiexi(ST41),etc.,and makes adjustments according to the symptoms.It advocates a comfortable sensation from the needles,and properly tonifying and purging,aiming at regulating the spirit as a guide,reducing the stagnation of the liver qi,and tonifying the spleen and stomach.One test case is attached.

Abstract:Narcolepsy is a chronic sleep disorder disease,with the main clinical symptoms of daytime sleepiness,cataplexy,nighttime hallucinations,and sleep paralysis.Professor GUO Yi believes that the etiology and pathogenesis of narcolepsy are mainly based on spleen-kidney yang deficiency,with phlegm turbidity and blood stasis as manifestations.In terms of treatment,attention should be paid to warming and tonifying yang qi,resolving phlegm and removing blood stasis,and attaching importance to regulating emotions.The application of the"four dredging methods"—drugs to dredge the spleen and kidney by warming and tonifying the spleen and kidney,regulating qi and blood;acupuncture to dredge the heart and kidney by stimulating the heart-kidney connection and regulat-ing qi and strengthening the spleen,bloodletting to dredge the brain and refresh the mind by selecting the hand twelve Jing acupoints for puncture and bloodletting,and moxibustion to dredge the yang qi of the governor vessel and the whole body to warm the body's yang qi and achieve the effects of warming the governor vessel and awakening the spirit.

Abstract:Systematic sphygmology for pattern differentiation is a brand-new pulse science system that follows the fundamental principles and laws of systems theory,integrates clinical experience,and has unique insights.It encompasses multiple disciplines and covers multiple levels.It focuses on the development and differentiation of finger sensory functions,the establishment of a"scenario memory system"for various pulse element sensations,and the diagnosis and examination of objective"evidence chains"during the disease process,demonstrating characteristics such as systematicness,scientificity,standardization,practicality and ease of operation.However,the absence of operational specifications has hindered the clinical promotion and application as well as further innovative development of the pulse diagnosis technique of systematic sphygmology for pattern differentiation.This article,based on existing research findings,related literature on systematic sphygmology for pattern differentiation,and expert consensus,standardizes the techniques of systematic syndrome differentiation and pulse diagnosis from aspects such as pulse palpation,search and identification pulse information,analysis and generalization pulse information,aiming to promote the popularization and innovative develop-ment of the pulse diagnosis technique of systematic sphygmology for pattern differentiation.
