miR-25-3p Promotes Motor Function Recovery After Spinal Cord Injury in Rats by Targeting PTEN Through the AKT/mTOR PathwayAbstract:Objective To investigate the effects of miR-25-3p on neuronal apoptosis and motor function in rats after spinal cord injury(SCI).Methods 103 male SD rats were selected and randomly divided into 4 groups:the sham operation group(Sham group),the spinal cord injury group(SCI group),the miR-25-3 p mimic injection group(agomiR group),and the miRNA negative control injection group(agoNC).The expression changes of miR-25-3p and PTEN before and after SCI were detected by real-time fluorescent quantitative PCR and protein immunoblotting;the expression changes of PTEN and AKT/mTOR pathway as well as apoptosis-related proteins Bcl-2,Bax and C-caspase3 before and after miR-25-3p mimics injection were also examined;the binding between miR-25-3p and PTEN was verified by dual luciferase reporter gene assay;the changes in motor function and the number of spinal motor neurons in each group of rats were evaluated by BBB score and Nissl staining.Results The apoptosis level of spinal cord tissue after SCI significantly increased,while the expression of miR-25-3p decreased and the expression of PTEN increased.At the same time,the expressions of p-AKT,p-mTOR and p-S6 significantly decreased.miR-25-3p can target and inhibit the expression of PTEN.Local injection of miR-25-3p mimics can significantly inhibit the apoptosis level of spinal cord tissue,increase the number of surviving motor neurons,and promote the recovery of motor function in rats.Conclusion After miR-25-3p targets PTEN,it inhibits neuronal apoptosis through the AKT/mTOR pathway,thereby promoting the recovery of motor function in rats.
The Intervention Effect of Predictive Comprehensive Insulation Nursing on Patients Undergoing Open Abdominal SurgeryAbstract:Objective To analyze the effect of comprehensive insulation care intervention in advance in patients undergoing open abdominal surgery in the operating room and its impact on the complication rate.Methods 88 patients undergoing open abdominal surgery in the operating room at the First Affiliated Hospital of Jinzhou Medical University were randomly divided into a control group(n=44)and a study group(n=44)by lottery.The control group received routine surgical care,while the study group received comprehensive insulation care intervention in advance in addition.The body temperature,coagulation function,blood pressure,respiration,heart rate,stress indicators and complication rates of the patients in each group at different times were compared.The research period was from May 2020 to May 2021.Results The body temperature of the study group at T1,T2,T3,and T4 was significantly higher than that of the control group(P<0.05);the APTT and TT levels of the study group after surgery were significantly higher than those of the control group,and the PT and FIB levels were significantly lower than those of the control group(P<0.05);the SBP,DBP,and HR levels of the study group after surgery were significantly higher than those of the control group(P<0.05);the extubation time and postoperative recovery time of the study group were significantly earlier than those of the control group,and the hospital stay was shorter than that of the control group(P<0.05);the postoperative complication rate of the study group was only 4.55%,significantly lower than that of the control group(18.18%)(P<0.05).Conclusion Comprehensive insulation care intervention in advance for patients undergoing open abdominal surgery in the operating room has a significant effect,can better maintain the patient's body temperature,stabilize the patient's vital signs and coagulation function,and also reduce the risk of postoperative complications,which can promote the improvement of the safety of operating room nursing and is worthy of clinical promotion.
Analysis of the Imaging Characteristics and Risk Factors for Brucellar SpondylitisAbstract:Objective To explore the imaging characteristics and potential risk factors of Brucellar spondylitis.Methods Inpatients with brucellosis in Liaoyang Infectious Disease Hospital from January 2020 to May 2023 were selected and divided into spondylitis group(study group)and non-spondylitis group(control group).The imaging characteristics of brucellosis spondylitis were analyzed.Univariate analysis was used to compare the differences in baseline indicators,clinical characteristics and laboratory test results between the two groups.The corresponding indicators were screened out and the potential risk factors of brucella spondylitis were analyzed by multivariate logistic regression analysis.Results(1)The main CT imaging changes of Brucella spondylitis are vertebral bone destruction,involved intervertebral disc stenosis,bone periosteal hyperplasia,sclerosis and paravertebral abscess at the edge of the vertebral body;the main imaging findings of MRI included diffuse hyperemia and edema of the vertebral body,worm erosion of the adjacent vertebral body,bone hyperplasia at the edge of the vertebral body,and narrowing of the intervertebral space.The lumbar spine is the most common site of involvement.L3-L4,L4-L5 and L5-S1 intervertebral discs are the most common areas of intervertebral disc involvement in patients with Brucella spondylitis.(2)Compared with the control group,there were significant differences in age,smoking,diabetes,lymphadenopathy,arthritis,pneumonia,pleurisy,PDW,CRP,ALP,ALB and LDL-C between the study group and the non-spondylitis group(P<0.05).(3)Multivariate logistic regression analysis revealed that smoking(OR=2.168,P=O.004),arthritis(OR=2.678,P=0.000),pleurisy(OR=5.582,P=0.015),and elevated levels of low-density lipoprotein cholesterol(LDL-C)(OR=1.503,P=0.025)were significantly associated with the occurrence of the disease and are independent risk factors for the development of brucellar spondylitis.Conclusion(1)The imaging features of Brucella spondylitis include vertebral bone destruction,intervertebral disc stenosis,perivertebral osteoperiosteal hyperplasia,sclerosis and paravertebral abscess.(2)Smoking,arthritis,pleurisy and LDL-C are independent risk factors for Brucella spondylitis.
Correlation Analysis Between Progressive Stroke and Serum Free Fatty AcidsAbstract:Objective To discuss the correlation between serum free fatty acids and acute progressive ischemic stroke,and explore the significance of changes in characteristic serum fatty acid profiles and serum free fatty acid levels in patients with acute progressive ischemic stroke,so as to provide reference for targeted clinical intervention.Methods A total of 130 patients with acute ischemic stroke hospitalized in the First Affiliated Hospital of Jinzhou Medical University from January 2023 to October 2023 were selected,among which 65 patients with acute progressive ischemic stroke were taken as the study group and 65 patients with non-progressive acute ischemic patients were taken as the control group.The free fatty acids of the patients were detected by liquid chromatography tandem mass spectrometry.It includes palmitic acid,linolenic acid,linoleic acid,oleic acid,eicosapentaenoic acid(C20∶5),docosahexaenoic acid(C22∶6),total saturated fatty acids,total monounsaturated fatty acids,total ω3 and total ω6.SPSS soft was used for statistical analysis.Results Compared with the control group,the levels of palmitic acid and linoleic acid in the serum of the study group were higher,with statistically significant differences(P<0.05).The levels of eicosapentaenoic acid(C20∶5),docosahexaenoic acid(C22∶6),and total ω3 in the study group were lower than those in the control group,with statistically significant differences(P<0.05).However,there were no statistically significant differences in the levels of the other fatty acids(all P>0.05).Multivariate regression analysis further confirmed that the above five serum free fatty acid metabolites were correlated with progressive stroke.The correlation analysis of serum free fatty acids and the increase in NIHSS score in the study group indicated that palmitic acid was positively correlated with the increase in NIHSS score;total ω3,eicosapentaenoic acid(C20∶5),and docosahexaenoic acid(C22∶6)were negatively correlated with the increase in NIHSS score;while linoleic acid was not correlated with the increase in NIHSS score.Conclusion Serum free fatty acid levels are correlated with progressive ischemic stroke.Changes in palmitic acid,linoleic acid,total 3,eicosapentaenoic acid(C20∶5),and docosahexaenoic acid(C22∶6)can serve as potential markers for patients with acute progressive ischemic stroke.
Prediction of Neoadjuvant Chemotherapy for Advanced Colon Cancer by NLR,NLR/PA,and Tumor MarkersAbstract:Objective To explore the evaluation value of the neutrophil-to-lymphocyte ratio(NLR),the ratio of NLR to prealbumin(NLR/PA),and combined with tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125)]in the efficacy of neoadjuvant chemotherapy for patients with advanced colon cancer.Methods 82 patients with advanced colon cancer who received neoadjuvant chemotherapy at the First Affiliated Hospital of Henan University of Science and Technology from January 2021 to January 2023 were selected as the research subjects.After 3 cycles of chemotherapy,they were followed up for 1 month.According to the effect of neoadjuvant chemotherapy,they were divided into the effective group and the ineffective group.The differences in NLR,NLR/PA,CEA,and CA125 levels between the two groups of patients were compared,and the ROC curve was drawn to analyze the evaluation value of NLR,NLR/PA,CEA,and CA125 levels before neoadjuvant chemotherapy for the new adjuvant chemotherapy effect of patients with advanced colon cancer;the clinical data of the two groups of patients were collected and Logistic regression analysis was conducted to identify the factors influencing the new adjuvant chemotherapy effect.Results One month after the completion of neoadjuvant chemotherapy,among the 82 patients,14 achieved complete remission,31 had partial remission,27 remained stable,and 10 experienced progression.The chemotherapy effective rate was 53.66%(44/82).The levels of NLR,NLR/PA,CEA,and CA125 in the neoadjuvant chemotherapy effective group were all lower than those in the ineffective group(P<0.05).NLR,NLR/PA,CEA,and CA125 were all factors influencing the neoadjuvant chemotherapy effect in patients with advanced colon cancer(P<0.05).When the cutoff values of NLR,NLR/PA,CEA,and CA125 were set at 2.85,0.05,4.79 ng/mL,and 42.21 U/mL respectively,the areas under the curve(AUC)of these indicators for single diagnosis of the neoadjuvant chemotherapy effect in patients with advanced colon cancer were 0.747,0.754,0.862,and 0.879 respectively,and the AUC of the combined diagnosis was 0.997(P<0.05).Conclusion An increase in NLR,NLR/PA,and tumor markers CEA and CA125 may lead to the ineffectiveness of neoadjuvant chemotherapy in patients with advanced colon cancer.The combination of these four indicators can be used as an auxiliary indicator for clinical assessment of the effectiveness of neoadjuvant chemotherapy in patients.
Clinical Efficacy Analysis of Yishenhuashi Granule Combined with ARB Drug in the Treatment of Diabetic NephropathyAbstract:Objective To explore the efficacy and safety of Yishenhuashi Granule in the treatment of diabetic nephropathy.Methods The clinical data of 124 patients with diabetic nephropathy from July 2021 to December 2021 were included.The patients were randomly divided into the observation group and the control group.The observation group was treated with Yishenhuashi Granule combined with ARB drug and the control group was treated with placebo and ARB drug.The study lasted for 6 months.The relevant clinical indicators,treatment efficacy and adverse reactions between the two groups of patients were compared.Results There was no statistically significant difference in serum albumin,creatinine,urea,glycated hemoglobin,24-hour urine protein quantification and urine microalbumin between the two groups before treatment(P>0.05).After 6 months of treatment,the total effective rate was 68.55%.The effective rate of the observation group was 77.42%,which was better than that of the control group(59.68%)(P<0.05).The observation group showed a more significant increase in serum albumin(P<0.05),and a more significant decrease in 24-hour urine protein quantification and urine microalbumin levels(P<0.05).There were no serious adverse reactions in both groups.Conclusion Yishenhuashi Granule combined with ARB drugs can effectively reduce the urinary protein of patients with diabetic nephropathy and improve the level of serum albumin with high safety and little impact on renal function.It is a preferable choice for patients with diabetic nephropathy.
Therapeutic Effect of Glycyrrhizic Acid on Rats with OsteoarthritisAbstract:Objective To investigate the therapeutic effect of glycyrrhizic acid in regulating silence information regulator 1(SIRT1)/forkhead transcription factor O1(FOXO1)signaling pathway in rat osteoarthritis model.Methods OA models were prepared,and rats were randomly divided into sham operation group,OA group,and glycyrrhizic acid group(80 mg/kg),with 9 rats in each group.The mechanical pain threshold and thermal pain threshold of rats in each group were measured;the enzyme-linked immunosorbent assay(ELISA)technique was used to detect interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),and prostaglandin E2(PGE2)in serum and joint fluid;the kit was used to detect the activity of superoxide dismutase(SOD),the content of malondialdehyde(MDA),and the content of glutathione peroxidase(GSH-Px);immunoblotting was employed to examine the expression levels of MMP-3,MMP-13,SIRT1,and acetylated FOXO1 proteins.Results Compared with the sham operation group,the expression of IL-1β,TNF-α,IL-6,MDA,MMP3,MMP13,PGE2,and acetylated FOXO1 increased in the OA group(P<0.05),while the expression of IL-10,SOD,GSH-Px,and SIRT1 decreased(P<0.05);compared with the OA group,the glycyrrhizic acid treatment group showed decreased expressions of IL-1β,TNF-α,IL-6,MDA,MMP3,MMP13,PGE2,and acetylated FOXO1(P<0.05),while increased expressions of IL-10,SOD,GSH-Px,and SIRT1(P<0.05).Conclusion Glycyrrhizic acid may alleviate the analgesic response in OA rats by regulating the SIRT1/FOXO1 signaling pathway.
Mechanism of Paeoniae Rubra Radix in Ischemic Stroke Based on Network Pharmacology Analysis and Experimental InvestigationAbstract:Objective To explore the molecular mechanism of Paeoniae Rubra Radix(PRR)in the treatment of ischemic stroke(IS).Methods After obtaining the chemical components of PRR from the TCMSP database,potential active ingredients were screened.The potential targets of the chemical components of PRR were predicted using the PharmMapper and SwissTargetPrediction databases.The relevant targets of ischemic stroke were obtained through the Drgbank,TTD and Genecards databases.The chemical components of PRR and the common targets of ischemic stroke were obtained by using the online platform Venny 2.1.0.The protein-protein interactions(PPI)network was established through the STRING database.The PPI network was visualized by Cytoscape 3.8.2.The GO function and KEGG pathway enrichment analysis were completed through the DAVID database."Component-Target-Pathway"was constructed by Cytoscape 3.8.2 software.After the rat model was successfully established(thread plug method),PRR intervention was given,and the neurological deficits of rats in each group were evaluated by Longa score.The brain tissues of rats in each group were taken for enzyme-linked immunosorbent assay(ELISA)kit detection.qRT-PCR was used to verify the anti-apoptotic effect of PRR.The injury of cerebral cortical neurons in each group of rats was investigated by Hematoxylin and Eosin Staining Kit(HE)and Nissl staining technique.Results Through network pharmacological analysis and screening,20 potential active components of PRR were obtained,among which 13 were potential therapeutic targets for ischemic stroke.Five core targets,namely TNF,SERPINE1,ESR1,FGF2,andF3,were screened out based on the degree value.KEGG pathway enrichment analysis showed that PRR exerts therapeutic effects on ischemic stroke through signal pathways such as complement and coagulation cascade reactions.The Longa score and brain water content results indicated that the neurological function of rats in the PRR group recovered significantly,and tissue edema was significantly reduced.The ELISA results indicated that PRR significantly inhibited the inflammatory response in the model rats.The results of HE and Nissl staining tests showed that compared with the model group,the number of Nissl body positive cells in the PRR administration group was significantly increased,and tissue damage was significantly alleviated.Conclusion PRR may target and regulate factors such as TNF and SERPINE1 through its active ingredients(such as paeoniflorin),affecting the complement and coagulation cascade pathway,thereby alleviating ischemic stroke injury.
The Influence of Modified Mirror Therapy on the Structure of White Matter Fiber Tracts in Patients with Cerebral Infarction and HemiplegiaAbstract:Objective To investigate the modified effect of modified mirror therapy on clinical neurological deficits in patients with cerebral infarction and hemiplegia,and to observe the changes in white matter fiber bundles through DTI combined with DTT imaging.Methods 80 patients with acute cerebral infarction who visited the Department of Neurology of the First Affiliated Hospital of Jinzhou Medical University from January to December 2023 were randomly divided into a control group(conventional rehabilitation treatment)and an observation group(conventional rehabilitation treatment+modified mirror therapy),with 40 cases in each group.After three months of treatment for both groups of patients,the FMA-UE and ADL scores were used to compare the improvement of neurological deficits before and after rehabilitation in the two groups.At the same time,the differences in the fractional anisotropy(FA)of white matter fiber tracts in the brain between the two groups were compared using magnetic resonance diffusion tensor imaging(DTI)combined with diffusion tensor tractography(DTT)sequences.Results At the beginning of the group enrollment,a comparison of the baseline data between the observation group and the control group showed no statistically significant difference(P>0.05);both the FMA-UE and ADL scores of the two groups after treatment were higher than those before treatment,and the observation group was significantly higher than the control group,with a statistically significant difference(P<0.05);the FA values of the corticospinal tract in both groups after treatment were higher than those before treatment(P<0.05),and the FA value of the observation group was(0.561±0.038),which was significantly higher than that of the control group(0.521±0.037),with a statistically significant difference(P<0.05).The FA value of the corticospinal tract in the observation group after treatment was positively correlated with the FMA-UE and ADL scores(r=0.391,P<0.001;r=0.395,P<0.001).Conclusion The modified mirror therapy may promote the recovery of clinical neurological deficits in patients with cerebral infarction and hemiplegia by improving the structure of the white matter fiber tracts in their brains.
The Role and Mechanism of Anisodamine in Alleviating Neuronal Damage by Regulating Oxidative StressAbstract:Objective To explore the protective mechanism of macelignan(MLN)against oxidative stress-induced damage in PC12 neuronal cells induced by hydrogen peroxide(H2O2).Methods The oxidative stress model of PC12 cells was induced by H2O2.The cells were divided into the Control group,the H2O2 group(150 μmol/L),and the H2O2+MLN group(10 μmol/L).Cell Counting Kit-8(CCK8)was used to determine the H2O2 induction concentration and the optimal administration concentration,and to detect the toxicity of MLN.The expression levels of malondialdehyde(MDA),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px),and catalase(CAT)were detected by the kit.The DCFH-DA kit was used to detect the reactive oxygen species(ROS)levels in each group.The mitochondrial membrane potential kit JC-1 was used to detect the mitochondrial membrane potential in each group.Immunofluorescence was used to detect the apoptotic factor Cleaved-capase3.Western Blot was used to detect the expression levels of Cleaved-capase3,B lymphoma-2(Bcl-2),and Bcl-2 related x protein(Bax)in each group.Results The CCK8 results showed that the optimal administration concentration of MLN was 10 μmol/L at an H2O2 induction concentration of 150μmol/L.This concentration had no toxic effect on the cells.Compared with the Control group,the level of oxidative stress factor MDA in the H2O2 group was higher than that in the Control group,while the levels of SOD,GSH-Px,and CAT decreased.Compared with the H2O2 group,the MDA level in the MLN treatment group decreased,and the levels of SOD,GSH-Px,and CAT increased.The results of the ROS kit assay indicated that there was an excessive accumulation of ROS within the cells in the H2O2 group.After MLN treatment,the level of ROS in the cells significantly decreased.The JC-1 results showed that the mitochondrial membrane potential in the H2O2 group was lower than that in the Control group.After MLN intervention treatment,there was a tendency for the mitochondrial membrane potential to increase.The immunofluorescence results showed that the fluorescence intensity of Cleaved-capase3 in the H2O2 group was significantly higher than that in the MLN treatment group.The WB results indicated that the protein levels of Cleaved-capase3 and Bax increased in the H2O2 treatment group,while the protein level of Bcl-2 decreased.After MLN treatment,the protein levels of Cleaved-capase3 and Bax decreased,while the protein level of Bcl-2 increased,and all of these showed statistical significance(P<0.05).Conclusion MLN regulates cellular oxidative stress to inhibit apoptosis and alleviate H2O2-induced neuronal damage in PC12 cells.
O-GlcNAc Glycosylation Regulates YAP1 and Participates in the Proliferation and Invasion of Colorectal CancerAbstract:Objective The glycosylation of O-linked beta-N-acetylglucosamine(O-GlcNAc)and Yes-associated protein 1(YAP1),as well as their expression patterns and the mechanisms of their functions in colorectal cancer,have not yet been fully elucidated.This study aims to explore the roles of O-GlcNAc glycosylation and YAP1 glycosylation in colorectal cancer by using the TCGA database,tissue specimens,and cell lines.Methods The expression levels of O-GlcNAc and OGT proteins in colorectal tissues and adjacent tissues were detected by immunohistochemistry;the expression levels of OGT mRNA in colorectal tissues and adjacent tissues were detected by qPCR;the expression status of OGT and its impact on prognosis were understood through online analysis of the TCGA database;the inhibitory effect of OSMI-1 on O-GlcNAc glycosylation was detected by Western Blot;CCK-8 and Transwell assays were used to observe the changes in the proliferation,invasion and migration abilities of colorectal cancer cells after inhibition of O-glycosylation;the expression status of YAP1 and its O-glycosylation sites were analyzed through the database,and YAP1 was determined to have undergone O-glycosylation modification by mass spectrometry analysis;the binding relationship between YAP1 and OGT was verified by Co-IP and immunofluorescence experiments;the changes in the expression location of YAP1 and the expression of downstream effect genes after inhibition of O-GlcNAc glycosylation were clarified by qPCR and immunofluorescence.Results The study found that the expression levels of OGT protein and mRNA were significantly increased in colorectal tissues,but they had no significant impact on the prognosis of patients;OSMI-1 can inhibit the O-GlcNAc glycosylation of intestinal cancer cells and also inhibit cell proliferation,invasion and migration abilities;YAP1 can bind to OGT and undergo O-GlcNAc glycosylation.When O-glycosylation is inhibited,the number of YAP1 entering the nucleus decreases and its transcriptional activity reduces.Conclusion In colorectal cancer,the level of O-GlcNAc glycosylation shows an increasing trend.OGT and YAP1 bind together,causing O-glycosylation modification of YAP1 and enhancing its transcriptional activity.
The Influence of Regional Medical Consortium Model Combined with Continuous Intelligent Nursing Service on Colorectal Cancer PatientsAbstract:Objective To explore and analyze the intervention effects of a follow-up management model that combines regional medical consortium(RMC)+continuous smart nursing services for colorectal cancer patients during their treatment.Methods The study selected colorectal cancer patients admitted to the hospital from July 2017 to September 2019 as the subjects.Within this time frame,110 patients were selected as the research sample and divided into a control group(55 patients who received routine care)and a treatment group(55 patients who received follow-up management through a regional medical consortium model combined with continuous smart nursing services).The study compared the self-management ability,self-care ability,quality of life,psychological state,self-perceived burden,hope level,social support,and nutritional status between the two groups.Results Compared with the control group,the study group had higher scores in self-management ability,ESCA,quality of life,HHI and SSRS,and lower scores in HAMA,HAMD and SPBS(P<0.05).In hemoglobin and serum albumin,the study group was higher than the control group(P<0.05).Conclusion The follow-up management of regional medical consortium model+continuous intelligent nursing service in the care of colorectal cancer patients can help enhance self-management ability and self-care ability,improve quality of life,improve psychological state and nutritional state,reduce self-perceived burden,improve hope level and social support,and has high clinical application value.
Effect of Compound Scutellaria Gargle on Periodontal Clinical Index in Patients with Chronic PeriodontitisAbstract:Objective To study the effect of compound Scutellaria gargle on serum leptin(LEP)level and periodontal clinical index in patients with chronic periodontitis,providing reference for clinical treatment.Methods Patients with moderate and severe chronic periodontitis were randomly selected 6 weeks after periodontal basic treatment.Group A was gargarized with compound Scutellaria gargle and group B was gargarized with normal saline,with 30 cases in each group.Probing depth(PD),loss of attachment(AL),plaque index(PLI)and bleeding on probe(BOP)were recorded at baseline and after 1 and 3 months of treatment.Serum LEP levels were determined by ELISA.Results There was no significant difference in periodontal clinical index and LEP level between group A and group B at baseline(P>0.05).Compared with baseline,the levels of PD,PLI,BOP and LEP in group A were significantly decreased at 1 and 3 months after treatment(P<0.05),while AL had no significant change at 1 month after treatment(P>0.05),and was significantly decreased only at 3 months(P<0.05);however,the periodontal clinical treatment indexes of group B had no significant changes at 1 and 3 months after treatment(P>0.05).Compared with group B,the periodontal clinical indexes and LEP levels in group A were significantly decreased at 1 and 3 days after treatment(P<0.05).Conclusion In chronic periodontitis patients,gargling with compound scutellaria gargle after 1 month of periodontal basic treatment can effectively reduce periodontal clinical indexes and serum leptin levels.
Prediction of Lymph Node Metastasis by Intratumoral and Peritumoral CT Radiomics Combined with Clinical Features in Rectal CancerAbstract:Objective This study aims to establish a machine learning model based on conventional CT radiomics features to predict the preoperative lymph node metastasis(LNM)status of rectal cancer and evaluate the gain value of peritumoral quantitative features.Methods A retrospective study included 371 patients with pathologically confirmed rectal cancer who underwent CT examinations at Pingyi County Traditional Chinese Medicine Hospital from January 2019 to December 2023.The patients were randomly divided into the training set and the test set in a ratio of 7∶3.6,264 radiomics features were extracted from the intratumoral and peritumoral(3,5,7 mm)areas of CT.After feature screening,intratumoral and peritumoral radiomics machine learning prediction models were constructed respectively.In addition,clinical models and combined models were established in combination with clinical factors.The performance of each model in predicting LNM was analyzed and compared using the receiver operating characteristic(ROC)curve,and indicators such as the area under the curve(AUC),sensitivity,and specificity were calculated.And further construct the nomogram of the best model.Results Among the enrolled patients,144 cases(38.81%)had lymph node metastasis.In the test set,the model based on the radiomics characteristics of 5 mm around the tumor had the best predictive efficacy(AUC=0.810),and was significantly better than the intratumoral model(AUC=0.709,P=0.046).The comprehensive model constructed by combining the best intratumoral and peritumoral models with clinical factors achieved AUC values of 0.871 and 0.890 respectively in the training set and test set,which was significantly better than the single clinical model(AUC=0.736 and 0.710,P<0.05),and had the highest clinical benefit.Conclusion The combined model that integrates the intratumoral and peritumoral radiomics features of CT and combines clinical features has shown excellent efficacy in predicting the preoperative lymph node metastasis status of rectal cancer.The quantitative characteristics around the tumor may reflect the unique biological process of metastasis and are of great value for improving the non-invasive prediction of lymph node metastasis status of rectal cancer.The nomogram constructed based on the joint model is expected to provide strong support for clinical individualized diagnosis and treatment decisions.
