A study on the application of Prunus Speciosa Composition:Efficacy for sensitive scalp care and anti-hair lossAbstract:Objective To verify the effects of a cost-effective,low-concentration,and sensi-tive scalp-friendly anti-hair loss cherry blossom composition(PSC),aimed at overcoming the limi-tations of existing strategies which focus solely on stopping hair loss while neglecting the scalp care,and the reliance on a single cherry blossom extract at high concentration.Methods The composition of PSC was prepared,and the DPPH antioxidant assay was used to evaluate its free radical scavenging ability.The inhibition of PSC on TNF-α and IL-1 β was assessed to evaluate its soothing effect.The effects of PSC on oil control and prevention of hair loss were measured by as-sessing the inhibition of 5α-reductase activity in both the liver and epididymis.A total of 30 sub-jects with sensitive scalps were enrolled in this study.A 12-week clinical trial was conducted to e-valuate hair loss and overall hair density,and a questionnaire was used to assess the scalp condi-tion,hair density,and prevention of hair loss.Results The DPPH radical scavenging rate of PSC was 8.03%.PSC inhibited TNF-α and IL-1β by 30.70%and 44.40%,respectively,while in-hibiting 5 α-reductase by 42.59%(epididymis-derived)and 37.50%(liver-derived).Treatment of the scalp with a scalp essence for 12 weeks decreased hair loss by 14.79%(P<0.001),while increasing hair density by as much as 33.33%,with an average increase of 5.32%(P<0.05).Questionnaires showed that over 80% of participants reported decreased scalp sebum secretion,re-duced sensitivity or discomfort,and noticeable reduction in hair loss.Conclusion PSC with a low concentration of PS can achieve the dual efficacy in scalp care and anti-hair loss,providing a high-ly effective and cost-efficient solution for individuals with sensitive scalps.
Analysis of clinical and pathological characteristics of 14 cases of granulomatous rosaceaAbstract:Objective To explore the clinical manifestations,histopathological features,diag-nosis,and therapeutic characteristics of granulomatous rosacea(GR),thereby enhancing the clin-ical physicians' understanding of this condition.Methods This study conducted a retrospective a-nalysis of clinical and pathological features of 14 cases of GR at Guangzhou Dermatology Hospital from June 2019 to May 2021.Results Among 14 patients,11 were female,and 3 were male.The most frequently involved area was the cheek,and none of the patients showed extrafacial le-sions.Histologically,the mixed type was the most common(64.29%,9/14),followed by the perifollicular type(28.57%,4/14)and nodular type(7.14%,1/14).None of the biopsy spec-imens showed diffuse granulomatous infiltration.Among the confirmed cases,71.43%(10/14)of patients had previously received oral treatments such as antihistamines,hydroxychloroquine,or tetracycline antibiotics,but the therapeutic effects were unsatisfactory.After diagnosis,the pa-tients adopted the treatment plan of oral isotretinoin combined with low-dose prednisone,resulting in gradual subsidence of the skin lesions.Conclusions GR is more common in middle-aged women.The typical skin lesions are symmetrical papules and erythema on both cheeks with mild subjective symptoms.The histopathological feature is primarily a mixed type of non-caseous epi-thelioid cell granuloma.The clinical misdiagnosis rate of GR is high,and a clear diagnosis should be made by combining the characteristic clinical manifestations with histopathological examination.Oral isotretinoin combined with low-dose glucocorticoid therapy has proven effective.
Research progress on the diagnosis and treatment of vibrio vulnificus infectionAbstract:Vibrio vulnificus is a Gram-negative bacterium that inhabits warm seawater,and the wound infections it causes are characterized by critical severity and rapid progression,with an ex-tremely high mortality rate.The clinical manifestations are diverse and can rapidly progress from localized wound infections to necrotizing fasciitis or even fatal sepsis.Current standard treatment emphasizes multidisciplinary collaboration and early intervention,with diagnosis requiring a com-bination of epidemiological history,typical clinical manifestations,and laboratory and imaging ex-aminations.The key to treatment lies in early and adequate empirical antibiotic therapy,which in-cludes the use of third-generation cephalosporins combined with tetracyclines,along with prompt and thorough surgical debridement.In the face of challenges such as antibiotic resistance,emer-ging therapies,including phage therapy,monoclonal antibodies targeting RtxA1 toxins,topically applied nano-silver dressings,and antimicrobial peptides,show great potential.This article sys-tematically reviews the latest progress in the diagnosis and treatment of Vibrio vulnificus infection,covering its pathogenic characteristics,clinical manifestations,and the latest diagnostic and thera-peutic methods.Future research directions include the development of rapid bedside diagnostic technologies,optimization of individualized dosing regimens,and the promotion of clinical transla-tion of novel antimicrobial strategies.Establishing an integrated management system that combines rapid diagnosis,precise debridement,rational drug use,and cutting-edge adjuvant therapies will be central to improve patient outcomes.
Classic Kaposi's sarcoma:A report of three casesAbstract:Three cases of classic Kaposi's sarcoma are reported and the relevant literature is reviewed.Case 1 was a 69-year-old man.Purplish-red papules and nodules on both feet lasted for 3 years and worsened for half a year.Dermatological examination revealed multiple dark purplish-red papules and nodules of varying sizes on both dorsal feet,soles,dorsal hands,and palms.Some were hemispherical in shape,while others fused into plaques.Black scabs were noticed on some surfaces,accompanied by a moderate texture and mild tenderness.Both lower limbs exhibi-ted non-pitting edema.Case 2 was a 70-year-old man with purplish-red papules and nodules on both lower legs and feet for 4 years.Dermatological examination showed dark purplish-red papules and nodules on both lower legs,dorsal feet,and the right ankle.Some of these were hemispher-ical,with a rough and thick surface,without tenderness.Both lower limbs had non-pitting edema.Case 3 involved a 73-year-old man who experienced purplish-red papules and nodules on the left lower leg for 2 years,with worsening over the past month.Dermatological examination revealed multiple dark purplish-red nodules measuring 0.5-1 cm in diameter on the left lower leg,appea-ring hemispherical with slight tenderness.The HIV antibody test for all three patients was nega-tive.Histopathological examination of all three patients showed characteristic features of spindle cells and vascular proliferation,with positive immunohistochemical staining for CD34,CD31,HHV8,and Fil-1.Kaposi's sarcoma was diagnosed based on clinical manifestations,skin his-topathological examination,and immunohistochemistry.Follow up:Patient 1 discontinued treat-ment due to personal financial reasons.Patients 2 and 3 received regular radiation therapy at the Cancer Radiotherapy Department of our hospital.After six months of treatment,a telephone fol-low-up was conducted.Patient 2 reported that most of the skin lesions had subsided,and treat-ment had been discontinued for one month,with occasional recurrence.After 7 months of treat-ment,a follow-up call was made to patient 3,who reported partial subsidence with occasional re-currence.The patient is currently still undergoing treatment.
Mechanisms of Guomin Decoction in the treatment of atopic dermatitis:Based on network pharmacology and molecular dockingAbstract:Objective To investigate the potential mechanisms of Guomin Decoction(GMD)in the treatment of atopic dermatitis(AD)using network pharmacology,and to verify the regulato-ry effects of its core components in cell cultures.Methods The active components and corre-sponding targets of GMD were retrieved from TCMSP and relevant literature.Targets related to AD were obtained from OMIM and Genecards databases.Common targets were identified and imported into the String database for protein-protein interaction(PPI)analysis to screen core targets,which were then visualized using Cytoscape software.A cell model of AD was established by treating HaCaT cells with histamine.Intervention groups were treated with quercetin and kaempferol at dif-ferent concentrations(1 μmol/L,5 μmol/L).The levels of TNF-α and IL-1 β in cell superna-tants were detected by ELISA,and the mRNA expression levels of STAT3 and MAPK1 were meas-ured by RT-PCR to verify the effects of the core components.Results A total of 36 active compo-nents of GMD were identified,corresponding to 1 161 drug targets.A total of 1 780 AD-related disease targets were obtained,and 383 drug-disease common targets were finally determined.PPI network analysis combined with the Cytohubba plugin screened out core targets,including STAT3,MAPK1,RELA,JUN,MAPK8,and TNF-α.GO enrichment analysis showed that the core tar-gets were mainly involved in biological processes such as"inflammatory response"and"regulation of MAPK cascade reaction".KEGG enrichment analysis confirmed that these targets were signifi-cantly enriched in 10 key pathways,including the TNF signaling pathway,JAK-STAT signaling pathway,MAPK signaling pathway,and IL-17 signaling pathway.The results of cell cultures showed that,compared with the AD model group,the quercetin and kaempferol(at 5 μmol/L)significantly reduced the levels of TNF-α and IL-1 β in the supernatants(P<0.01)and downreg-ulated the mRNA expression levels of the core targets' STAT3 and MAPK1(P<0.05),effective-ly inhibiting the inflammatory response in the cell model of AD.Conclusions GMD acts on 383 common disease targets through 36 active components to regulate 10 key pathways,such as those involving TNF and JAK-STAT.Its core components,quercetin and kaempferol,can significantly reduce the levels of TNF-α and IL-1β and downregulate the mRNA expression of STAT3 and MAPK1 in the cell model of AD,thereby effectively inhibiting the inflammatory response.
The efficacy of negative pressure wound therapy combined with nano-silver antibacterial dressings in the treatment of burn wounds in childrenAbstract:Objective To explore the efficacy of negative pressure wound therapy combined with nano-silver antibacterial dressings in the treatment of burn wounds in children.Methods A total of 120 burned children admitted to our hospital from November 2023 to May 2025 were ran-domly divided into a control group(60 cases)and an observation group(60 cases).The control group was treated with negative pressure wound therapy,while the observation group was treated with negative pressure wound therapy combined with nano-silver medical antibacterial dressings.After 10 days of treatment,the vancouver scar scale(VSS)was used to assess the degree of scar hyperplasia,while the degree of pain was evaluated using the visual analogue scale(VAS)and the levels of inflammatory factors,including CRP,IL-6,TNF-α,and IL-8,were measured before treatment and 10 days after treatment using ELISA.Additionally,we compared the wound healing time,hospital stay,and dressing change frequency between the two groups.Results The wound healing time,hospital stay,dressing change frequency,and VSS score in the observation group were all significantly lower than those in the control group(P<0.05).After 10 days of treat-ment,the VAS scores and the levels of CRP,IL-6,TNF-α,IL-8 in the serum of patients in both groups were lower than those before treatment,with the observation group showing lower levels than the control group(all P<0.05).Conclusion The application of negative pressure wound therapy combined with nano-silver medical antibacterial dressings in the treatment of burned chil-dren can significantly improve wound healing,alleviate inflammation and pain,and accelerate the recovery process.
Research progress on skin barrier damage and repair mechanismsAbstract:In recent years,the mechanisms of damage and repair of the skin barrier have e-volved from traditional factors(physical,chemical,biological,hereditary,etc.)to the levels of molecules,genes,and signaling pathways.Meanwhile,attempts have been made to elaborate on their association with skin diseases.Correspondingly,emerging repair strategies have been contin-uously developed,such as molecular targeted therapy,gene therapy,modulation of signaling path-way,the application of new materials,microecological modulation,optoelectronic technology,and traditional Chinese medicine.These strategies provide new solutions and ideas for the treatment of skin barrier-related diseases.However,many key issues remain unresolved.This article elabo-rates on the research progress in the mechanisms of skin barrier damage and repair,and provides perspectives on future research directions.
Advances in the innate immune evasion by Chlamydia trachomatisAbstract:Chlamydia trachomatis(Ct)is the pathogen responsible for the most common bac-terial sexually transmitted disease.Its unique intracellular survival mode and complex immune evasion mechanisms enable it to establish long-term infections within the host,triggering severe chronic pathological reactions.This review provides insights into Ct's biological characteristics and various innate immune evasion strategies.The key roles of the cGAS-STING signaling pathway and crucial innate immune cells in host anti-infection responses are emphasized.The molecular mecha-nisms by which Ct achieves immune evasion,such as interfering with the γ-interferon response,manipulating host cell death,and inhibiting antigen presentation,are also elaborated.
Clinical observation of botox-A combined with triamcinolone acetonide injection and pulsed dye laser in the treatment of pathological scarsAbstract:Objective To evaluate the clinical efficacy and safety of combined treatment with botulinum toxin type A(BTX-A)injections,triamcinolone acetonide(TAC)injections,and pulsed dye laser(PDL)therapy for pathological scars.Methods A retrospective analysis was conducted on patients with hypertrophic scars or keloids diagnosed at Guangzhou Dermatology Hos-pital from January 2022 to September 2023.The treatment protocol for one cycle(3 months)com-prised a single perilesional injection of BTX-A in the first month,followed by intralesional injec-tion of TAC solution combined with PDL therapy in both the second and third months.Patients re-ceived 2 to 4 such cycles.Evaluations were performed before treatment,at 6 months(mid-treat-ment),and at the 6-month follow-up after treatment completion.Assessment tools included the vancouver scar scale(VSS)and a physician's global assessment,and adverse events were recor-ded.Results Twelve patients(4 males,8 females),aged 23-65 years(mean:38.4 years),with 15 scars were included.The VSS scores were 10.60±2.64 at pre-treatment,6.33±2.90 at mid-treatment,and 4.53±2.39 at the 6-month follow-up.The differences in VSS scores across these time points were statistically significant(F=101.71,P<0.001).The VSS score at mid-treatment was significantly lower than the pre-treatment score,with a further significant reduction at the 6-month follow-up compared to the pre-treatment(both P<0.001).Clinical assessment in-dicated significant improvement in 10 scars and improvement in 5 scars,resulting in a significant improvement rate of 66.67% and an overall effective rate of 100%.Mild adverse events were ob-served in four cases,including telangiectasia(2 cases),folliculitis(1 case),and local epidermal necrosis(1 case).Conclusion The combination therapy of BTX-A,TAC injections,and PDL significantly improves pathological scars with minimal adverse effects.
A case of malignant hidroacanthoma simplex on the hipAbstract:We report a case of malignant hidroacanthoma simplex on the hip.An 85-year-old female presented with a 6-month history of a papule on her left hip.Dermatological examination showed a brown-colored hyperkeratotic plaque measuring 3 cm × 3 cm with irregular margins and visible surface crusts.Histopathological examination revealed marked hyperkeratosis,acanthosis,papillomatosis,and well-circumscribed intraepidermal tumor nests.Atypical cells with large,hy-perchromatic nuclei and mitotic activity were observed within the tumor nests.Immunohistochemi-cal staining was positive for EMA,CK14,CK20(a few cells),and Cam5.2(a few cells)in the lesional skin.The diagnosis was malignant hidroacanthoma simplex.Complete excision was per-formed,and no recurrence was observed during the 12-month follow-up period.
Syringocystadenocarcinoma papilliferum of the scalp:A case report and literature reviewAbstract:This article reports a case of SCACP in a 27-year-old female with a congenital scalp mass for 27 years,which exhibited surface ulceration and crusting in the recent 2 years.Pathologi-cal examination revealed infiltrative growth of the tumor with basaloid and squamous differentia-tion.Immunohistochemistry showed BerEP4(+),EMA(+),P40(+),GCDFP-15(+),CK7(+),and Ki-67(10%+).This article also systematically reviews the epidemiological characteristics,clinicopathological features,immunohistochemical diagnostic criteria and differen-tial diagnosis of SCACP.In addition,it discusses the principles of staging,surgical treatment strategies(including indications for lymph node dissection),the clinical significance of radiothera-py and chemotherapy,and the application value of NGS testing in targeted therapy selection.Fur-thermore,it analyzes the key factors affecting prognosis and strategies for improving survival rate,aiming to provide reference for clinical diagnosis and treatment.
Analysis of influencing factors on patient satisfaction in the dermatology outpatient depart-ment of a tertiary public hospitalAbstract:Objective To analyze outpatient experience satisfaction and its influencing factors among dermatology patients at a tertiary public hospital,and to improve dermatologists' under-standing of patients' needs while enhancing the overall quality of medical services in dermatology.Methods From June to September 2024,an outpatient satisfaction questionnaire was distributed via SMS to patients on the day of their visit to the dermatology clinic of Shenzhen University Affili-ated South China Hospital.The patients' satisfaction with medical services and its determinants were analyzed.Results A total of 762 valid questionnaires were collected.The overall satisfaction rate was 87.53%.Regarding overall impression of the hospital,patients with infectious diseases reported the highest satisfaction rate(88.72%),while patients with disfigurement-related disea-ses reported the lowest(82.08%).In terms of medical care and treatment attitude,the satisfac-tion rate of patients with inflammatory diseases and acute conditions was lower than that of patients with infectious diseases and chronic conditions.Additionally,the satisfaction rate of pediatric pa-tients was lower than that of elderly patients,and the satisfaction rate of young patients was lower than that of middle-aged patients(P<0.05).Regarding process and queuing,the satisfaction rate for patients with surgical and disfigurement-related diseases was lower than that for patients with infectious diseases(P<0.05),and young patients reported lower satisfaction compared to middle-aged patients(P<0.05).In terms of medical insurance fees,the satisfaction rate among patients with surgical conditions was lower than that of patients with infectious diseases(P<0.05).Regarding hospital infrastructure and services,patients with disfigurement-related disea-ses,chronic conditions,and elderly patients had lower satisfaction rates compared to those with infectious diseases,acute conditions,and young patients(P<0.05).Conclusions The satisfac-tion of dermatology outpatients across various dimensions was influenced by age,clinical duration,and disease type.Further enhancements are needed in the domains of medical care/attitude,process/queuing,medical insurance/charges,and hospital infrastructure/services.
Research on the current status and influencing factors of sexual attitudes among Chinese adults based on CGSS 2023Abstract:Objective To analyze the current status and influencing factors of Chinese adults'attitudes towards premarital,extramarital,and homosexual sexual behaviors based on data from the Chinese General Social Survey 2023(CGSS 2023),providing a scientific basis for the prevention and control of sexually transmitted diseases(STDs).Methods Utilizing nationally representative data from CGSS 2023,a total of 5 252 valid adult samples were selected.Descriptive analysis,one-way ANOVA,and multiple linear regression models were employed to analyze the impact of socio-demographic characteristics,psychological factors,and behavioral factors on the three types of sexual attitudes(premarital sexual behavior,homosexual behavior,extramarital sexual behav-ior).Results Significant differences were found in the acceptance of the three types of sexual behaviors among Chinese adults(P<0.001).Acceptance was highest for premarital sexual be-havior(2.15±1.28),followed by homosexual behavior(1.60±1.03),and lowest for extramar-ital sexual behavior(1.25±0.67).Ordinal logistic regression analysis indicated that males showed higher acceptance of premarital and extramarital sexual behaviors(B=0.314,P<0.001;B=0.394,P<0.001),while females showed higher acceptance of homosexual behavior(B=-0.353,P<0.001).Younger and highly educated groups held more open attitudes towards pre-marital and homosexual behaviors(P<0.05).Individuals with a higher self-rated frequency of depressive symptoms showed greater acceptance of homosexual behavior(B=0.559,P=0.002).Conclusions The diverse sexual attitudes among Chinese adults,shaped by multiple factors,ne-cessitate targeted risk-reduction education and behavioral interventions for populations with more permissive attitudes to prevent unsafe sex and lower STI rates.
Clinical and pathological characteristics of skin malignant cancers:An analysis of 648 casesAbstract:Objective To investigate the clinical and pathological characteristics of cutaneous malignancies in the population of South China.Methods A total of 648 patients with histologi-cally confirmed cutaneous malignancies who visited the Dermatology Hospital of Southern Medical University between January 2020 and July 2023 were retrospectively analyzed.Clinical features and treatment patterns were summarized.Results Among the 648 patients(male-to-female ratio:1.25∶1),squamous cell carcinoma(31.94%)and basal cell carcinoma(31.17%)were the most common malignancies,followed by cutaneous lymphomas(13.89%),dendritic cell and his-tiocytic tumors(13.27%),and melanoma(9.72%).The distribution of tumor types differed significantly between sexes(P=0.014).Patients aged≥60 years accounted for the largest pro-portion(53.55%).Basal cell carcinoma and squamous cell carcinoma predominantly involved the head and face(91.10%and 48.79%,respectively).Melanoma mainly presented as the ac-ral subtype(79.37%),while cutaneous lymphomas most often affected the trunk and extremities(62.22%).Nodular-type basal cell carcinoma was the most common histologic subtype(74.75%),with 78.22%classified as low-risk.In squamous cell carcinoma,invasive disease accounted for 91.79%.Female predominance was observed in melanoma(63.49%),with Clark level IV and Breslow thickness≤1 mm being the most frequent.Cutaneous lymphomas were mainly of T/NK-cell origin(93.33%),with mycosis fungoides being the most common subtype(59.52%).Among dendritic cell and histiocytic tumors,xanthogranuloma predominated(82.56%).Surgical excision was the primary treatment modality for basal cell carcinoma and squamous cell carcinoma(95.54%and 95.17%,respectively).Conclusions In South China,squamous cell carcinoma and basal cell carcinoma are the most prevalent cutaneous malignancies,with elderly individuals and sun-exposed head and face regions representing high-risk groups.Mel-anoma is predominantly acral and more common in women,whereas cutaneous lymphomas occur relatively more frequently in younger patients and are mainly of T/NK-cell origin.These regional and age-related patterns provide important implications for precision diagnosis,risk assessment,and tailored management strategies for cutaneous malignancies.
A preliminary evaluation of the clinical efficacy of a sebum-regulating and balancing acid complex in the treatment of scalp seborrheic dermatitis:A single-arm,open-label studyAbstract:Objective To evaluate the clinical efficacy,safety,and mechanisms of a sebum-regulating and balancing acid complex in treating scalp seborrheic dermatitis(SSD).Methods Thirty eligible SSD patients were enrolled in this single-arm,open-label,self-controlled study.The intervention consisted of four weekly scalp acid peeling sessions.Subjective symptoms(oili-ness,itching,pain)were self-assessed by patients,while clinical signs(erythema,papules,scales)were evaluated by investigators via dermoscopy before each treatment.Fungal fluorescence staining of scalp scales was performed at baseline and post-treatment.Patients were followed up weekly for four weeks,starting two weeks after the last session.Results After four treatments,the subjective symptom scores(greasy sensation,itching,pain)and clinical sign scores(erythe-ma,scaling,papules)of the subjects significantly decreased(P<0.01).The overall effective rate was 96.7%.Among the 27 patients who tested positive for fungi at baseline,12(44.44%)achieved complete fungal clearance after treatment.During the treatment period,two patients ex-perienced transient mild itching of the scalp and fever,which resolved spontaneously after 30 mi-nutes of observation,with no serious adverse reactions reported.During the 4-week follow-up,symptom and sign scores showed no significant changes(P>0.05),except for a further improve-ment in erythema.Conclusions The compound acid treatment significantly ameliorates symptoms such as greasiness,itching,and pain in patients with SSD,as well as signs like erythema,pap-ules,and scales,while demonstrating good safety.
Research progress in photodynamic therapy for actinic keratosisAbstract:Photodynamic therapy(PDT)has been widely used in the clinical treatment of ac-tinic keratosis(AK),and has demonstrated good clinical efficacy.This article reviews the effica-cy of PDT using different photosensitizers,various light-regimens,and combined with different pretreatment methods,as well as the safety and patient satisfaction during PDT,to provide a basis for selecting clinical treatment regimens for AK.
The role of B cells in local immune defense against five traditional sexually transmitted dis-eases:Mechanisms and translational perspectivesAbstract:As the core effectors of humoral immunity,B cells play a complex and dynamic bi-directional regulatory role in local immune defense against the five traditional sexually transmitted diseases,including syphilis,gonorrhea,chancre,sexually transmitted lymphogranuloma,and in-guinal granuloma.This article systematically reviews the response mechanisms of B cells within the infectious microenvironment and their clinical translational value.In syphilis,B cells participate in neural tissue damage and host defense through ectopic germinal center formation and specific antibody production.In gonorrhea,the dysfunction of B cells is due to pathogen-induced apoptosis and immunosuppression.The Haemophilus ducreyi in chancroid directly inhibits B cell prolifera-tion through cytolethal distending toxin.In Lymphogranuloma Venereum,the B cells limit the spread of Chlamydia trachomatis through a dual mechanism of antibody neutralization and antigen presentation.In Granuloma Inguinale,B cells clear intracellular pathogens through antibody-mac-rophage synergy.These mechanisms provide a theoretical basis for the development of novel diag-nostic biomarkers(such as CXCL13 and B cells),vaccine design(e.g.,multi epitope vaccines and mucosal delivery systems),and targeted therapies(e.g.,immunomodulators and toxin inhib-itors).In the future,it is necessary to combine single-cell sequencing and spatial transcriptomics technology to analyze the heterogeneity of B cell subpopulations and their dynamic interactions with the local microenvironment,in order to promote the precise development of immune prevention and treatment of sexually transmitted diseases.
Progress in the application of dupilumab for non-atopic dermatitisAbstract:Dupilumab inhibits Th2-mediated inflammatory responses by specifically blocking the signaling pathways of interleukin-4 and interleukin-13.Recently,in addition to its approved indication for the treatment of moderate-to-severe atopic dermatitis,dupilumab has also shown effi-cacy in treating other inflammatory skin diseases,such as prurigo nodularis,bullous pemphigoid,chronic spontaneous urticaria,allergic contact dermatitis,nummular eczema,chronic hand ecze-ma,chronic actinic dermatitis,lichen amyloidosis,chronic pruritus,cutaneous immune-related adverse events,eosinophilic pustular folliculitis,alopecia areata,keloids,and some genetic skin diseases.This article reviews the progress of dupilumab in the treatment of non-atopic dermatitis skin diseases to provide more theoretical basis and treatment options for the clinicians.
Treatment of pemphigus vulgaris with protein A immunoadsorptionAbstract:A 25-year-old woman presented with a four-year history of widespread erythema,blisters,and erosions,accompanied by pain that had worsened over the past week.Four years ago,she was diagnosed with pemphigus vulgaris(PV)based on multiple oral ulcers,widespread erythema,and thin-walled blisters.Despite treatment with corticosteroids,immunosuppressants,and biologics such as rituximab,her condition remained poorly controlled with recurrent flares.A general examination revealed cushingoid features,a buffalo hump,and a Body Mass Index of 28.08.The dermatological examination showed extensive erythema,flaccid bullae,erosions,and crusting on the scalp,trunk,and extremities,with profuse serous exudate.Erythematous bullous,verrucous plaques were also noted in the axillary and genital areas.The Pemphigus Disease Area Index score was 49.Laboratory tests revealed anti-Dsg1 antibodies at 237.8 U/mL and anti-Dsg3 antibodies at 174.5 U/mL.Skin secretion cultures grew multiple pathogenic organisms.The final diagnoses were pemphigus vulgaris,cutaneous infection,and drug-induced Cushing's syndrome,and others.Following multidisciplinary consultation and informed consent,the patient received corticosteroids,methotrexate,and protein A immunoadsorption(PA-IA)therapy for a total of 10 sessions.After discharge,her autoantibody titers decreased,and skin lesions improved significant-ly.During a 6-month follow-up period,the patient remained lesion-free,with a sustained decline in pemphigus-related autoantibody titers.
Comparison of Ureaplasma urealyticum detection results by fluorescence quantitative PCR and liquid culture methods,and drug susceptibility analysis of positive specimensAbstract:Objective To compare the positive rates of fluorescence quantitative PCR and liq-uid culture methods in detecting Ureaplasma urealyticum(Uu)and to conduct drug susceptibility analysis on specimens with isolated Uu positivity,thereby providing evidence for the clinical diag-nosis and treatment of Uu infections.Methods A retrospective analysis was performed on the re-sults of Uu detection by PCR and liquid culture methods in 368 cases at Chongqing Traditional Chinese Medicine Hospital from January 2022 to December 2024.The positive rates of Uu detec-tion by the two methods were compared.The data from samples positive for Uu by both methods were analyzed,and drug susceptibility analysis was conducted on specimens with isolated Uu posi-tivity.Results There was a significant difference in the positive rates of Uu detection between PCR and liquid culture methods(P=0.004).A total of 71 patients were positive for Uu by both methods,yielding a positive rate of 19.29%,including 32 females(35.56%)and 39 males(14.03%).The proportion of females was significantly higher than that of males(x2=20.23,P<0.001).Additionally,98 cases were positive only by liquid culture method,representing a positive rate of 26.63%,while 80 cases were positive only by PCR method,with a positive rate of 21.74%.The consistency of detection results between the two methods was moderate(Kappa=0.734).The majority of patients who tested positive for Uu by both methods were concentrated in the 20~39 age group.Among 58 cases with isolated Uu positivity,the susceptibility rates to mi-nocycline and josamycin were both 100%,and the susceptibility rates to doxycycline and clar-ithromycin were all above 90%.Conclusions There are differences in the positive rates of Uu de-tection between fluorescence quantitative PCR and liquid culture methods.Combined detection u-sing both methods should be considered when necessary to enhance diagnostic accuracy.In clini-cal practice,minocycline,josamycin,doxycycline,and clarithromycin,which demonstrate high susceptibility,can be recommended as first-line treatments for Uu-positive patients.
