Latent profile analysis of postoperative care dependency in elderly patients with coronary heart disease undergoing percutaneous coronary intervention

Abstract:Objective:To classify and investigate the postoperative care dependency of elderly patients with coronary heart disease(CHD)undergoing percutaneous coronary intervention(PCI)through latent profile analysis(LPA).Methods:A total of 442 elderly patients with CHD undergoing PCI were selected as research subjects through convenience sampling in hospital from January 2022 to January 2024.The basic situation questionnaire,Hamilton Depression Scale,Hamilton Anxiety Scale,Chinese People's Social Support Scale,Scale of the Quality of Life of the Coronary Heart Disease and Care Dependency Scale were selected to conduct the investigation.Results:The care dependency for elderly patients with CHD after PCI could be divided into three latent profiles,high care dependency type(20.36%),medium care dependency type(52.04%),and low care dependency type(27.60%).Logistic regression analysis showed that age 60~<65 years old,absence of concomitant chronic heart failure,absence of other chronic diseases,social support and quality of life were significant influencing factors of PCI postoperative care dependence in elderly patients with CHD(P<0.05).Conclusions:The PCI postoperative care dependence of elderly patients with CHD can be divided into three latent profiles,which are low care dependence type,medium care dependence type and high care dependence type.Medical staff should provide corresponding health education,psychological intervention,and continuity of care for elderly patients with CHD with different types of nursing dependence to meet their care dependence.

Development and reliability and validity test of Exercise Rehabilitation Behavior Questionnaire for middle-aged and elderly patients with coronary heart disease after PCI

Abstract:Objective:To develop a test of Exercise Rehabilitation Behavior Questionnaire for middle-aged and elderly patients with coronary heart disease after percutaneous coronary intervention(PCI)and to test its reliability and validity.Methods:Based on the health belief model and the theory of planned behavior integrated model,the initial draft of the questionnaire was formed through literature review,qualitative interviews,Delphi expert consultation,and a small sample pre-survey.A convenience sampling method was used to conduct a survey on 190 middle-aged and elderly patients with coronary heart disease after PCI.The questionnaire was evaluated using item analysis,content validity,structural validity,internal consistency reliability,split-half reliability,and test-retest reliability.Results:The Exercise Rehabilitation Behavior Questionnaire for middle-aged and elderly patients with coronary heart disease after PCI included 6 dimensions and 30 items.The item-level content validity index(I-CVI)of the questionnaire ranged from 0.93 to 1.00,and the scale-level content validity index(S-CVI)was 0.99.Exploratory factor analysis extracted 6 common factors,with a cumulative variance contribution rate of 68.168%.The Cronbach's α coefficient of the total questionnaire was 0.884,the split-half reliability was 0.934,and the test-retest reliability was 0.776.The Cronbach's α coefficients of each dimension ranged from 0.807 to 0.956,and the test-retest reliability ranged from 0.743 to 0.948.Conclusion:The questionnaire has good reliability and validity and can be used as a tool to evaluate the exercise rehabilitation behavior of middle-aged and elderly patients with coronary heart disease after PCI.

Vertical integration construction and practice of midwifery professional practice courses based on position requirements

Abstract:Objective:To evaluate the application effect of vertically integrated practical courses based on job requirements in midwifery teaching.Methods:In the 2021 cohort of the midwifery major,a longitudinal integrated social practice course titled"Case Management of Pregnancy and Postpartum Period"was launched.Students completed the observation and guidance of the entire reproductive process of pregnant women from early,middle and late pregnancy,delivery to the puerperium through case tracking.After the course,the professional performance,empathy ability and communication skills of the 2020 and 2021 cohorts were compared to evaluate the teaching effect.Results:The empathy and communication skills of the 2021 cohort of midwifery students were superior to those of the 2020 cohort,with statistically significant differences(P<0.05).The theoretical examination scores of the 2020 cohort in Obstetrics and Midwifery were(71.37±9.85)points,and the comprehensive operation scores were(91.25±3.62)points.The theoretical examination scores of the 2021 cohort were(76.23±8.20)points,and the comprehensive operation scores were(92.48±3.37)points.The differences were statistically significant(P<0.05).Conclusion:Based on position requirements,the longitudinal integrated social practice course is offered to effectively improve students'empathy and communication skills,and enhance their professional performance.

Correlation analysis of traditional Chinese medicine biased constitution in patients infected with helicobacter pylori in the border regions of Guangxi,Yunnan and Guizhou

Abstract:Objective:To investigate the correlation between helicobacter pylori(Hp)infection and traditional Chinese medicine biased constitution in the border regions of Guangxi,Yunnan and Guizhou.Methods:A total of 469 patients with Hp infection were evaluated by general data questionnaire and Traditional Chinese Medicine Constitution Scale,and the distribution of traditional Chinese medicine biased constitution in Hp infection patients were analyzed by cross-sectional investigation,and the factors that might affect physical fitness were analyzed by multi-factor regression.Results:The distribution of traditional Chinese medicine biased constitution in Hp patients was yang-deficient constitution(25.26%)>damp-heat constitution(19.05%)>qi-deficient constitution(16.98%)>phlegm-dampness constitution(14.28%)>stagnant qi constitution(9.32%)>yin-deficient constitution(8.70%)>stagnant blood constitution(4.55%)>inherited special constitution(1.86%).Yang-deficient constitution,qi-deficient constitution,phlegm-dampness constitution,stagnant qi constitution,stagnant blood constitution and inherited special constitution were higher in women than in men,and damp-heat constitution and yin-deficient constitution were higher in men than in women.There were statistical differences in the distribution of traditional Chinese medicine biased constitution in Hp infected patients of different genders(P<0.05).Yang-deficient constitution was the main traditional Chinese medicine biased constitution of Hp infection,followed by damp-heat constitution,in which gender,drinking,peace,well water,mineral water were independent factors of yang-deficient constitution,staying up all night,drinking,mineral water,meat and fish were independent factors of Damp-heat Constitution.Conclusion:Yang-deficient constitution and damp-heat constitution are the common types of traditional Chinese medicine biased constitution in Hp infection patients in the border regions of Guangxi,Yunnan and Guizhou.It can help to reduce Hp infection rate and provide reference for disease prevention by conditioning from biased constitution and taking targeted prevention and intervention.

Construction and application of"Internet+chronic wound care"management model based on the compact urban medical consortium

Abstract:Objective:Based on the construction of the compact urban medical consortium,this study innovatively established a hospital-community-family ternary linkage"Internet+chronic wound care"management system,and systematically evaluated its clinical application effect.Methods:A hospital-community-family three-level linkage team was established to clarify responsibilities and formulate systems,connect with the"Internet hospital"platform,train community medical staff and family caregivers in wound management skills,and form a management model based on"Internet+chronic wound care".A total of 110 patients with chronic wounds in Huzhou urban medical consortium from January 2020 to June 2023 were selected as the observation objects by using the convenient sampling method.The wound Pressure Ulcer Scale for Healing(PUSH)score,quality of life score and caregiver burden score were evaluated in the baseline period,the first week and the second week of intervention,respectively.The nursing satisfaction of patients were evaluated at baseline and the second week of intervention.At the same time,the professional theoretical knowledge and operational skills of 50 community nurses were longitudinally compared and analyzed.Results:There were statistically significant differences in the PUSH score,overall quality of life score,functional dimension score,symptom dimension score and caregiver burden score of patients at different time points(P<0.001).The overall quality of life score and the score of the functional dimension showed an upward trend over time,while the PUSH score,the score of the symptom dimension and the score of the caregiver burden showed a downward trend over time.Patient satisfaction increased from 61.82%to 92.73%(P<0.001),and the pass rate of community nurses'assessment rose from 28.00%to 72.00%.Both patient satisfaction and the pass rate of community nurses'assessment showed significant improvements(P<0.001).Conclusions:The"Internet+nursing"ternary linkage chronic wound management system constructed in this study improves the chronic wound nursing service system,realizes hierarchical management,and significantly improves the quality of life of patients.Through standardized management methods,this model not only improves the ability of community medical staff to manage chronic wounds,but also reduces the physical and mental burden of patients and their families,and greatly improves patient satisfaction.This innovative management model provides new ideas and practical paths for the standardized management of chronic wounds.

Identification of symptom clusters and core symptoms in early postoperative rehabilitation of oral cancer patients

Abstract:Objective:To investigate the occurrence of symptoms in patients in the early stage of postoperative rehabilitation for oral cancer,determine the symptom clusters and construct the symptom network,and identify the core symptoms in the symptom network.Methods:By using the convenience sampling method,patients with oral cancer who were re-examined in the head and neck surgery departments of two tertiary grade A hospitals in Hunan province from June 2024 to February 2025 and one month after the operation were selected as the research subjects.The general information questionnaire and M.D.Anderson Symptom Inventory-Head&Neck(MDASI-H&N)were used for the investigation.Exploratory factor analysis was adopted to extract symptom clusters.The R software was used to construct the symptom network and calculate the centrality index.Results:The top three common and severe symptoms in the early stage of postoperative rehabilitation for oral cancer patients are dry mouth,difficulty in swallowing or chewing,and abnormal taste.A total of four symptom clusters were extracted,namely the oral and throat symptom cluster,the emotional and sleep symptom cluster,the gastrointestinal symptom cluster,and the neurosensory related symptom cluster.Among them,the oral and throat symptom cluster was the most severe.In the symptom network,nausea and vomiting,pain and soreness in the mouth or throat,and dry mouth were strongly correlated with mucus in the mouth or throat.Dry mouth(rs=1.26,rE=1.26)and mucus in the mouth or throat(rs=1.13,rE=1.13)were symptoms with higher intensity and expected impact.Conclusions:Patients in the early stage of postoperative rehabilitation for oral cancer have multiple symptoms and symptom clusters.Dry mouth and oral or pharyngeal mucus are the core symptoms of patients in the early stage of postoperative rehabilitation for oral cancer.Medical staff can formulate precise and efficient intervention strategies based on the core symptoms to reduce their transmission effect in the symptom network and improve the quality of life of patients.

Abstract:Cardiovascular and cerebrovascular diseases(CVDs)have a high incidence and disability rate worldwide and have become a major public health problem.Unhealthy lifestyles can accelerate the development of this disease.Dietary management in lifestyle has become an economical and efficient way of prevention and treatment.However,the incidence of CVDs remains high,indicating that the current dietary pattern has little effect in prevention and treatment.The main pathogenesis of CVDs include inflammation,oxidative stress and metabolic disorders,etc.The Family Nurse Diet Therapy pointed out that a diet rich in non-nutrients can prevent and treat chronic diseases through anti-inflammation,antioxidative stress and metabolic disorder improvement pathways.Based on this,under the guidance of the Family Nurse Diet Therapy,our team formed a personalized intervention dietary patterns-the Family Nurse Diet Model(FNDM).Previous studies have found that it has certain effects in the prevention and treatment of CVDs.This study mainly reported the research effects of different indicators in FNDM cardiovascular and cerebrovascular diseases and their related risk factors,with the aim of providing new ideas and tools for medical staff in the prevention and treatment of chronic diseases.

Abstract:Under the background of global population aging and lifestyle transformation,type 2 diabetes mellitus(T2DM)has become a major chronic metabolic disease that seriously endangers human health.At present,research on"different treatments of the same disease"mainly focused on traditional theoretical explanations,while systematic explorations of its modern pathophysiological mechanisms are relatively rare.At present,the research on"different treatments for the same disease"mainly focuses on the interpretation of traditional theories,while the systematic exploration of its modern pathophysiological mechanism is relatively scarce.Therefore,this article reviewed the different dietary care and personalized intervention regimens adopted of the T2DM.These differential regimens can also effectively control blood sugar.Through network pharmacological analysis,it was found that the core components playing a role in these dietary therapy regimens are non-nutrient active ingredients such as quercetin,kaempferol,and proanthocyanidins.The key mechanisms of their prevention and treatment of T2DM can be summarized as:functioning through three common core pathways:anti-inflammation,anti-oxidation,and improvement of metabolic disorders.This review aimed to explain the core concept of non-nutrient-assisted treatment of chronic diseases in the Family Nurse Diet Therapy(FNDT),and attempted to explore the scientific connotation of the principle of"different treatments for the same disease"in traditional Chinese medicine from the perspective of modern pathophysiology.

Quality of life and its influencing factors in patients with end-stage renal disease-associated chronic itch

Abstract:Objective:To investigate the status quo of quality of life in patients with end-stage renal disease-associated chronic itch(ESRDCI)and analyze its influencing factors.Methods:From January to September 2022,ESRDCI patients who underwent maintenance hemodialysis in 9 tertiary grade A hospitals in Fuzhou urban area were selected as the research subjects by convenience sampling method.The investigation adopted the general information questionnaire,the Four-Item Itch Scale(FIIQ),the Numerical Rating Scale(NRS),the Chinese version of Itch Catastrophizing Scale and the Pruritus-Specific Quality of Life Instrument(ItchyQoL)for pruritus patients.Results:A total of 360 valid questionnaires were retrieved,and the quality of life score of ESRDCI patients was(47.02±12.41).The results of multiple linear regression analysis showed that the severity of pruritus,the intensity of scratching,and the catastrophization of pruritus were the influencing factors of their quality of life(P<0.05).Conclusions:The quality of life of patients with ESRDCI is relatively low and is influenced by multiple factors.Clinical medical staff should focus on and alleviate the severity of pruritus symptoms and the intensity of scratching in patients,take effective psychological intervention measures to help patients actively cope with pruritus,reduce the perception of pruritus as a disaster,and thereby improve their quality of life.

Analysis of international research hotspots for Mendelian randomization in the field of depression

Abstract:Objective:To explore the status quo and research hotspots of Mendelian randomization(MR)studies in the field of depression.Methods:Taking the Web of Science Core Collection database as the data source,relevant literature on the application of MR in the study of depression from 2014 to 2024 were retrieved and visualized for analysis.Results:A total of 606 documents were retrieved,and the related research entered a stage of rapid development in 2022.Internationally,a relatively stable network of cooperative relationships among countries and institutions has been formed.The research hotspots of MR in the field of depression included exploring the causal relationship between depression and other diseases(cardiovascular diseases,cancer,inflammation),as well as the genetic mechanism and biomarker(IL-6,CRP)research of depression.Conclusions:Bibliometric analysis has demonstrated the research trend of MR in the field of depression.Foreign countries started early in this research field.Although China has published the largest number of papers in recent years,international exchanges and cooperation are scarce,and the cooperation between institutions and authors needs to be further strengthened.In the future,MR methods will provide a deeper understanding of the complexity of depression and offer more effective guidance for clinical practice.

Research progress on social participation of elderly people in China from the perspective of active aging:a bibliometric analysis based on CiteSpace

Abstract:Objective:To comprehensively review the research hotspots,frontiers,and trends of elderly social participation in China,providing a scientific basis for policy-making and subsequent research.Methods:Search Chinese and English literatures from multiple databases such as CNKI,Wanfang database,and Web of Science from January 1,2000,to June 30,2024.After screening,CiteSpace software was used for keyword co-occurrence,clustering,and burst analysis to construct and interpret research visual maps.Results:A total of 570 Chinese and 2 137 English literatures were included.The number of publications showed a trend of"steady start-slow growth-rapid growth",and both Chinese and English publications remained at a relatively high level after 2015.Keyword analysis showed that research hotspots focused on influencing factors,aging,etc.,and changed dynamically over time.Conclusions:The research on elderly social participation in China is evolving from"basic security"to"quality improvement",expanding from"urban-centered"to"urban-rural coordination",and deepening from"group inclusiveness"to"differential empowerment".In the future,it is necessary to establish a trinity support system of"policy-community-technology"to achieve the goal of active aging.

Resistance exercise in patients with maintenance hemodialysis:a scoping review

Abstract:Objective:To review the relevant studies regarding resistance exercise in maintenance hemodialysis patients,with the aim of providing reference for the development and implementation of standardized resistance exercise plans for them.Methods:The Chinese and English databases such as China National Knowledge Infrastructure(CNKI),CBM,PubMed,CINAHL,Embase,and the Cochrane Library were retrieved by computer.The retrieval period was from the establishment of the databases to April 1,2025.The included literatures were screened and summarized,and a scope review was conducted.Results:A total of 27 literatures were included,among which 23 were randomized controlled trials and 4 were quasi-experimental studies.Resistance exercise were conducted three times weekly,each with a duration of approximately 30 minutes,and the overall exercise program spanned 12 weeks.Resistance exercise had varying degrees of improvement in muscle strength,muscle mass,biochemical indicators,physical function,psychological state,and quality of life in maintenance hemodialysis patients,while also having certain safety and effectiveness.Conclusions:The application of resistance exercise in maintenance hemodialysis patients has integrity,effectiveness,and safety.Future medical personnel will further optimize resistance exercise intervention measures based on evidence and guided by scientific theories,develop safe,effective,and scientific individualized intervention plans,and further conduct large-scale,multi center,high-quality research to verify the impact of resistance training on ongoing hemodialysis patients.

Prevalence of pressure injuries among inpatients in Suqian city:a cross-sectional survey and analysis

Abstract:Objectives:To investigate the status quo,occurrence characteristics,and prevalence trends of pressure injuries(PI)among inpatients in Suqian city and to provide a basis for implementing appropriate preventive measures.Methods:A skin survey was conducted on inpatients in secondary and tertiary hospitals in Suqian city at 8:00 AM on November 16,2023.Two nurses simultaneously examined the patients'skin to detect the presence of PI.Results:A total of 8,532 inpatients were surveyed,and 8,231 valid questionnaires were collected.The prevalence rate of PI was 1.04%,and the prevalence rate of hospital-acquired pressure injury(HAPI)was 0.22%.The prevalence rate of PI in adults was 1.04%,and the prevalence rate of PI in minors was 0%.The prevalence rate of PI in secondary hospitals was 0.61%,and the prevalence rate of HAPI was 0.07%.The prevalence rate of PI in tertiary hospitals was 0.44%,and the prevalence rate of HAPI was 0.13%.The prevalence rate of PI in general hospitals was 0.94%,and the prevalence rate of HAPI was 0.17%.The prevalence rate of PI in specialized hospitals was 0.11%,and the prevalence rate of HAPI was 0.04%.The sites of PI were the sacrum(65.12%),the iliac crest(11.63%),and the heel(3.5%).PI occurred in a single site(73.26%),and in two or more sites(26.74%).In terms of injury stage,the prevalence rate was stage 1(18.6%),stage 2(39.5%),stage 3(9.3%),stage 4(10.5%),and unstageable(10.5%).Deep tissue injury was 2.3%,and multi-stage injury coexisted in 9.3%.Logistic regression analysis showed that a Braden score≤16,age,and hypoproteinemia were independent risk factors for PI.Conclusion:An information-based hospital-community collaborative model could be developed for PI prevention in Suqian city.Longitudinal research analyzing could be used for the influencing factors and outcomes of PI,in order to develop the collaborative prevention and treatment strategies within and beyond the hospital setting.Furthermore,further investigation could be conducted to examine the impact of different caregivers on PI.It would be more meaningful to implement PI prevention measures according to knowledge and skill needs of different caregiver groups.

Identification of symptom clusters,core symptoms,and bridge symptoms in patients with chronic heart failure

Abstract:Objective:To identify the symptom clusters of patients with chronic heart failure(CHF)and construct a symptom network to explore the core symptoms and bridge symptoms.Methods:A total of 342 CHF patients who visited a tertiary grade A level Chinese medicine hospital from September 2022 to December 2023 were selected as the research subjects by using a convenience sampling method.The general information questionnaire and the Chinese version of the Memorial Symptom Assessment Scale-Heart Failure(MSAS-HF)were used to investigate the patients.Exploratory factor analysis was used to extract the symptom clusters.The concurrent symptom network was constructed using R software,and the centrality index was calculated.Results:Five symptom clusters were extracted by exploratory factor analysis:the sickness symptom cluster,the sleep-digestion symptom cluster,the dyspnea symptom cluster,the mental and psychological symptom cluster,and the peripheral congestion symptom cluster.The top three symptoms of node strength centrality are sweating(rs=1.918),waking up from sleep at night(rs=1.167),and difficulty concentrating(rs=0.099).The top three symptoms of bridge strength centrality are waking up from sleep at night(rbs=4.418),abdominal distension(rbs=4.183),and easy sleepiness(rbs=4.093).Conclusions:The core symptom of CHF patients is sweating,and waking up from sleep at night,abdominal distension and easy sleepiness play a major bridging role between symptom clusters.Medical staff should develop precise intervention plans based on relevant target symptoms to improve the efficiency of symptom management.

Development of an early multisensory stimulation program for hospitalized premature infants based on evidence-based and Delphi methodology

Abstract:Objective:To develop an early multisensory stimulation program for hospitalized premature infants.Methods:The relevant literature on multisensory stimulation intervention for premature infants was retrieved from various Chinese and English databases by computer.The quality of the included literature was evaluated and the content was extracted.After discussion by the research group,the initial draft of the early multisensory stimulation plan for hospitalized premature infants was formed,and the Delphi method was used to verify and revise it.Results:A total of 12 articles were included,including 11 randomized controlled trials(RCTs)and 1 quasi-experimental study.The response rate for both rounds of expert inquiries was 100%.The basis for experts'judgment on the questionnaire was 0.89,the familiarity level was 0.84,and the expert authority coefficient was 0.865.In the second round of expert consultation,the coefficient of variation for item importance scores ranged from 0 to 0.22,and the Kendall concordance coefficient was 0.370(χ²=281.007,P<0.001).The coefficient of variation for feasibility scores ranged from 0.06 to 0.24,and the Kendall concordance coefficient was 0.361(χ²=274.088,P<0.001).The final early multisensory stimulation protocol for hospitalized premature infants included 2 first-level items,9 second-level items,and 22 third-level items.Conclusion:This study developed an early multisensory stimulation protocol for hospitalized premature infants with rigorous and scientifically sound procedures,which could provide reference for clinical staff in implementing early intervention for hospitalized premature infants.

Using the latent growth model to reveal the progression trajectory and key variables of fear of disease in chronic heart failure patients

Abstract:Objective:To analyze the progression trajectory and key influencing factors of fear of disease progression(FoP)in patients with chronic heart failure(CHF)using the latent growth model(LGM).Methods:A total of 154 patients with CHF admitted to Suzhou Municipal Hospital,Jiangsu province between January 2021 and January 2024 were selected in the study.A general information questionnaire,the Hospital Comprehensive Anxiety and Depression Scale(HADS),the Social Support Rating Scale(SSRS),and the General Self-Efficacy Scale(GSES)were used to investigate their status quo.The Fear of Progression Questionnaire-Short Form(FoP-Q-SF)was used to assess the FoP on the day of discharge,3 months,and 6 months after discharge.Latent categories of the trajectory of FoP were identified based on the LGM model,and logistic regression analysis was used to identify influencing factors.Results:The FoP score was(37.19±6.72)at T1,(33.63±6.48)at T2,and(28.88±5.75)at T3.The total score and each dimension of FoP showed a gradual downward trend at different stages after surgery.The FoP trajectory of patients could be divided into group C1(high fear group-slow decrease group),group C2(moderate fear group-rapid decrease group),and group C3(low fear group).Compared with group C3,influencing factors for patients in group C1 included NYHA class,number of hospitalizations,anxiety and depression,self-efficacy,and social support Influencing factors for patients in group C2 included age,number of hospitalizations,anxiety and depression,self-efficacy,and social support(P<0.05).Conclusion:CHF patients exhibit different trajectories and latent categories of FoP,and the influencing factors varied within each category.This suggests that clinicians should provide personalized health guidance based on patients'physiological characteristics,psychological status,and overall social background.

Analysis of factors influencing postoperative moderate to severe pain in elderly patients with prostatic hyperplasia

Abstract:Objective:To investigate the factors influencing postoperative moderate to severe pain in elderly patients with prostatic hyperplasia(BPH)and to provide scientific basis for the development of preventive nursing care.Methods:A retrospective analysis was conducted on the clinical data of 214 elderly patients after benign prostatic hyperplasia surgery in the Department of Urology of a tertiary grade A hospital in Guilin City from October 2022 to September 2024,including general information,postoperative pain assessment and nursing records.The influencing factors were analyzed based on whether moderate to severe pain occurred after the operation.Results:Among the 214 patients,the incidence of moderate to severe postoperative pain was 29.4%,mainly moderate pain.The results of multivariate Logistic regression analysis showed that age 70~79 years old,transurethral laser resection of the prostate,and combined with other chronic diseases were risk factors for moderate to severe pain after surgery(P<0.05).Conclusion:The risk of moderate to severe postoperative pain in elderly patients with benign prostatic hyperplasia is closely related to factors such as age,surgical methods,and the presence of other chronic diseases.Targeted intervention measures can help reduce the incidence of moderate to severe postoperative pain,promote rapid recovery of patients and improve their quality of life.

Construction of a risk prediction model for pelvic lymphocele in patients with gynecological malignancies postoperatively based on Logistic regression and decision trees

Abstract:Objective:To construct a risk prediction model for postoperative pelvic lymphocele(PL)in patients with gynecological malignancies based on logistic regression and decision trees.Methods:A total of 240 patients with gynecological malignancies admitted to our hospital between March 1,2022 and March 31,2024 were enrolled in the study.The patients were divided into a cyst group(n=73)and a control group(n=167)based on the occurrence of postoperative PL.Clinical data were collected from the patients.A decision tree model and a Logistic regression model were constructed for the prediction of postoperative PL in patients with gynecological malignancies,and their predictive performance was compared.Results:A total of 73 patients with gynecological malignancies developed PL among 240 patients,with an incidence of 30.42%.Multivariate analysis showed that tumor stage Ⅲ-Ⅳ[OR=5.752,95%CI(2.527,13.093)],number of lymph nodes≥20[OR=3.812,95%CI(1.804,8.053)],para-aortic lymph node clearance[OR=5.401,95%CI(2.271,12.848)],postoperative chemoradiotherapy[OR=7.053,95%CI(3.269,15.219)],and postoperative serum albumin<34 g/L[OR=3.510,95%CI(1.632,7.548)]were risk factors for postoperative PL in patients with gynecological malignancies(P<0.05).A decision tree model was constructed based on these risk factors,with 4 in model depth,13 in the number of nodes,and 7 in the number of terminal nodes.Postoperative chemoradiotherapy,tumor stage,para-aortic lymphadenectomy,number of lymph nodes cleared,and postoperative serum albumin were selected as explanatory variables.Postoperative chemoradiotherapy was the most important predictor of postoperative PL in patients with gynecological malignancies.The area under curve(AUC)of the Logistic regression model was 0.884,95%CI[0.837,0.930],and the AUC of the decision tree model was 0.846[95%CI(0.792,0.900)].There was no statistically significant difference between the two models(Z=1.369,P=0.091).Conclusion:Tumor stage Ⅲ-Ⅳ,≥20 lymph node dissections,para-aortic lymphadenectomy,postoperative chemoradiotherapy,and postoperative serum albumin<34 g/L are risk factors for postoperative PL in patients with gynecological malignancies.The Logistic regression and decision tree models constructed in this study have high predictive value.
