Effect of carboplatin dose adjustment on the efficacy of neoadjuvant chemotherapy in HER2-positive breast cancer

Abstract:Objective To investigate the effect of reducing the carboplatin dose due to adverse reactions during the use of trastuzumab+pertuzumab+docetaxel+carboplatin(TCbHP)on the efficacy of neoadjuvant therapy in patients with HER2-positive breast cancer.Methods The clinical data of patients with HER2-positive breast cancer who underwent neoadjuvant therapy with(TCbHP)in Xuzhou Central Hospital from January 2019 to June 2024 were retrospectively collected,and the overall pathologic complete response rate(tpCR rate)of patients in the carboplatin standard dose group,carboplatin reduction group and no carboplatin group were compared.Results A total of 58 patients were included,including 19 in the carboplatin standard dose group,29 in the carboplatin reduction group,and 10 in the no carboplatin group,and the tpCR rates of the three groups were 52.6%,65.5%,and 50.0%,respectively,with no significant difference(P=0.595).There was no significant difference in the severity of adverse reactions such as thrombocytopenia,leukopenia,neutropenia and liver damage between the three groups(P<0.05).Conclusions In clinical practice,when using TCbHP for neoadjuvant therapy in HER2-positive breast cancer patients,adjusting the carboplatin dose due to adverse reactions does not affect the efficacy of neoadjuvant therapy.

The application of DBT combined with DCE-MRI in differential diagnosis of benign and malignant breast lesions

Abstract:Objective To explore the application value of digital breast tomosynthesis(DBT)combined with dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)in the differential diagnosis of benign and malignant breast lesions.Methods One hundred and thirty patients with suspected breast malignant tumor admitted to Handan Central Hospital from January 2022 to January 2024 were studied and given DBT and DCE-MRI examination.The diagnostic efficacy of DBT combined with DCE-MRI was evaluated by using pathological results as the gold standard.The imaging findings(structural distortion,mass,calcification,mixed type)and the measurement results of breast cancer were compared under different examination methods.Results Of the 130 patients,95 were malignant lesions and 35 were benign lesions.DBT,DCE-MRI,DBT+DCE-MRI have a good consistency rate in differentiating benign and malignant from pathological examination;the area under the curve(AUC)of DBT,DCE-MRI and DBT+DCE-MRI were 0.626,0.689 and 0.917(P<0.05),respectively;the detection rate of DBT combined with DCE-MRI in the diagnosis of non-calcified lesions and calcified lesions was higher than that of DBT alone(P<0.05);DBT,DCE-MRI,DBT+DCE-MRI showed significant positive correlation with the mean maximum diameter of lesions measured by pathology(r=0.219,0.392,0.498,P<0.05).Conclusions DBT combined with DCE-MRI can effectively differentiate between benign and malignant breast lesions,improve the accuracy of clinical diagnosis,and improve the diagnosis rate of breast cancer imaging features.

Revolutionary advances of artificial intelligence in the diagnosis and treatment of head and neck tumors:Clinical practice applications and future prospects

Abstract:Head and neck tumors,as one of the common malignant tumors worldwide,pose significant challenges to clinical diagnosis and treatment due to their complex anatomical locations and high heterogeneity.In recent years,artificial intelligence(AI)technology has shown great potential in the medical field,especially in early diagnosis,image analysis,treatment planning,and prognosis evaluation of tumors.This review systematically summarizes the latest application progress of AI in the entire process of head and neck tumor management,including early screening,imaging diagnosis,pathological analysis,treatment decision-making,and prognosis prediction.Based on the current status of domestic clinical practice,this report further explores the key challenges in the in-depth integration of AI technology with the diagnosis and treatment of head and neck tumors,and proposes practical implementation paths and future development directions.Looking ahead,AI is expected to transform the traditional diagnosis and treatment model of head and neck tumors by integrating multi-modal data,establishing a standardized laboratory verification system,and improving ethical frameworks,thereby achieving a paradigm shift from empirical medicine to data-driven medicine and ultimately benefiting a large number of patients.

Expert consensus on the prevention and treatment of chemotherapy-induced peripheral neuropathy by integrated Chinese and Western medicine

Abstract:Chemotherapy-induced peripheral neuropathy(CIPN)is one of the common adverse reactions of chemotherapy.CIPN increases the suffering of tumor patients and reduces the quality of life.Therefore,some patients have to reduce the dose of chemotherapy drugs or even stop chemotherapy,which affects the treatment of tumors.This expert consensus discusses and recommends the definition,clinical features,pathogenesis,risk factors,diagnosis and clinical evaluation,TCM understanding and syndrome differentiation,prevention,treatment,education,and management of CIPN based on the literature review and expert voting.It is hoped that this consensus will improve clinicians'understanding and attention to CIPN,standardize the management of CIPN,and improve patient prognosis.
