Effect of carboplatin dose adjustment on the efficacy of neoadjuvant chemotherapy in HER2-positive breast cancer

Abstract:Objective To investigate the effect of reducing the carboplatin dose due to adverse reactions during the use of trastuzumab+pertuzumab+docetaxel+carboplatin(TCbHP)on the efficacy of neoadjuvant therapy in patients with HER2-positive breast cancer.Methods The clinical data of patients with HER2-positive breast cancer who underwent neoadjuvant therapy with(TCbHP)in Xuzhou Central Hospital from January 2019 to June 2024 were retrospectively collected,and the overall pathologic complete response rate(tpCR rate)of patients in the carboplatin standard dose group,carboplatin reduction group and no carboplatin group were compared.Results A total of 58 patients were included,including 19 in the carboplatin standard dose group,29 in the carboplatin reduction group,and 10 in the no carboplatin group,and the tpCR rates of the three groups were 52.6%,65.5%,and 50.0%,respectively,with no significant difference(P=0.595).There was no significant difference in the severity of adverse reactions such as thrombocytopenia,leukopenia,neutropenia and liver damage between the three groups(P<0.05).Conclusions In clinical practice,when using TCbHP for neoadjuvant therapy in HER2-positive breast cancer patients,adjusting the carboplatin dose due to adverse reactions does not affect the efficacy of neoadjuvant therapy.
