Correlation between serum total bilirubin,albumin levels and cognitive function in first-episode drug-naïve patients with schizophrenia

Abstract:Objective:To investigate the correlation between peripheral non-enzymatic antioxidant levels,such as serum uric acid(UA),total bilirubin(TBIL),and albumin(ALB),and cognitive impairment in first-episode drug-naïve(FEDN)patients with schizophrenia(SZ).Methods:A total of 167 FEDN schizophrenia patients from the department of psychiatry,First Hospital of Shanxi Medical University,were enrolled as the case group from October 2022 to October 2024,along with 103 matched healthy volunteers as the control group.Fasting venous blood was collected to measure serum UA,TBIL,and ALB levels.Cognitive function was assessed using the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS).Results:The case group showed significantly lower serum ALB and TBIL levels compared to the healthy control group(P<0.001).Total scores of RBANS as well as scores across all domains were also lower in the case group(P<0.001).After adjusting for confounding factors,serum TBIL levels were positively correlated with immediate memory(β=0.67,P<0.001),delayed memory(β=1.06,P<0.001),and the RBANS total scores(β=0.47,P<0.001).Serum ALB levels were positively correlated with immediate memory(β=0.60,P<0.001)and delayed memory scores(β=1.39,P<0.001).However,no significant correlations were observed between serum TBIL or ALB levels and the RBANS scores in the healthy control group.Conclusions:In clinical practice,early correction of peripheral non-enzymatic antioxidant levels,such as specifically serum total bilirubin and albumin,in FEDN schizophrenia patients may enhance antioxidant capacity and improve cognitive function,particularly memory,highlighting its clinical importance.

Latent transition analysis of comprehensive ability among older adults and its influencing factors

Abstract:Objective:To explore the potential profiles of comprehensive ability among older adults in China,the transition patterns over time,and the influencing factors,so as to provide a reference for establishing person-centered individualized care and developing targeted intervention plans.Methods:Using data from the China Health and Retirement Longitudinal Study(CHARLS)in 2015 and 2018,latent profile analysis and latent transition analysis were employed to explore the potential categories and transition patterns of comprehensive ability among older adults.Multinomial Logistic regression was used to analyze the influencing factors of transitions in comprehensive ability.Results:The comprehensive ability of older adults could be categorized into five latent classes in 2015 and 2018:the relatively healthy group,the emotional disability group,the mild disability group,the moderate disability group,and the severe disability group.The comprehensive ability of older adults with disabilities showed instability over time,presenting both risks of decline and a degree of reversibility.Logistic regression analysis showed that gender,type of permanent residence,marital status,frequency of visits from children,government or social subsidies,and sleep duration were influencing factors for transitions in comprehensive ability among older adults(P<0.05).Conclusions:The comprehensive ability of older adults in China demonstrates population heterogeneity at different time points and exhibits instability over time,with potential for both decline and reversibility.It is essential to establish a person-centered general practice management model with a focus on mobility and activities of daily living,to enhance the assessment and long-term monitoring of comprehensive ability in older adults,and to implement personalized interventions tailored to the characteristics of different older adult groups.

Application of intelligent robot rounds-based health education in the perioperative period of PCI patients

Abstract:Objective:To explore the application effect of intelligent robot rounds-based health education in the perioperative period of patients undergoing percutaneous coronary intervention(PCI),and to provide a basis for optimizing post-PCI health education models.Methods:A total of 83 PCI patients admitted to the cardiology department of a tertiary hospital in Hangzhou from January to April 2025 were selected.They were divided into control group(37 cases)and intervention group(46 cases)according to the time of admission.The control group received conventional health education,while the intervention group received rounds-based health education delivered by an intelligent robot.The discharge readiness,health education satisfaction,and cost-effectiveness were compared between the two groups,along with the intervention group's feedback on the use of the intelligent robot.Results:The discharge readiness score of the intervention group was higher than that of the control group.Their satisfaction with guidance on activity and rest,and the method of health education was higher than that of the control group,but their satisfaction with psychological guidance was lower,and all differences were statistically significant(P<0.05).The cost of health education(21.60 per person-time)was lower than that of the control group(74.52 per person-time).Patients in the intervention group highly recognized the robot's ease of use and educational value and expressed expectations for its future application.Conclusions:Intelligent robot rounds-based health education can effectively improve the discharge readiness and health education satisfaction of PCI patients during the perioperative period,demonstrating significant effects particularly in guidance on activity and rest,along with lower health education costs.

Quality assessment of clinical guidelines and consensus statements for acute myocardial infarction

Abstract:Objective:To systematically evaluate the methodological and reporting quality of clinical guidelines and consensus statements for acute myocardial infarction.Methods:Databases including PubMed,EMbase,Web of Science,China Biomedical Literature Database(CBM),WanFang Data,and China National Knowledge Infrastructure(CNKI)were systematically searched,supplemented by searches of the Guidelines International Network(GIN),the UK National Institute for Health and Care Excellence(NICE),the US National Guideline Clearinghouse(NGC),and the Medlive website.The search period covered from database inception to April 30,2025.Two researchers independently screened the literature based on predefined inclusion and exclusion criteria and extracted relevant data.The methodological and reporting quality of the included literature was assessed using the Appraisal of Guidelines for Research and Evaluation(AGREE Ⅱ)instrument and the Reporting Items for practice Guidelines in HealThcare(RIGHT)checklist.Results:A total of 11 guidelines and consensus statements on acute myocardial infarction were included.Subgroup analysis showed that guidelines scored higher than consensus statements in two AGREE Ⅱ domains("scope and purpose"and"stakeholder involvement").Guidelines and consensus statements developed based on evidence-based medicine methods scored higher in all six AGREE Ⅱ domains and in the overall RIGHT score compared to those based on expert opinion or reviews.Internationally developed guidelines and consensus statements scored higher in all six AGREE Ⅱ domains and in the overall RIGHT score compared to those developed in China.Conclusions:The currently published clinical guidelines and consensus statements for acute myocardial infarction demonstrate suboptimal methodological and reporting quality.Chinese guidelines and consensus statements still have room for improvement compared to their international counterparts.It is recommended that relevant professional organizations and researchers refer to the AGREE Ⅱ and RIGHT criteria to develop and widely promote high-quality clinical guidelines and consensus statements for the diagnosis and treatment of acute myocardial infarction.

Association between obesity and frailty in community-dwelling middle-aged and elderly people in China

Abstract:Objective:To explore the association between general obesity,central obesity,and frailty among middle-aged and elderly Chinese adults.Methods:Based on data from the China Health and Retirement Longitudinal Study(CHARLS)in 2013,2015,and 2018,2 696 individuals aged 45 and above were included.Information on demographic characteristics,self-reported health-related factors,and current disease status was collected.Multivariate logistic regression and restricted cubic spline analyses were used to examine the association between general obesity,central obesity,and frailty in the middle-aged and elderly population.Results:The mean age of the 2 696 participants was(61.94±8.74)years,with a body mass index(BMI)of(24.34±8.71)kg/m² and a waist circumference of(87.84±9.05)cm.The prevalence of pre-frailty and frailty among the middle-aged and elderly was 41.95%and 1.26%,respectively.Multivariate logistic regression analysis,after adjusting for confounding factors,showed that age 60-69[OR=1.58,95%CI(1.18,2.11)],age≥70[OR=1.60,95%CI(1.15,2.25)],central obesity[OR=1.71,95%CI(1.32,2.22)],general obesity group[OR=1.48,95%CI(1.11,1.98)],urban residence[OR=0.61,95%CI(0.46,0.81)],participation in social activities[OR=0.66,95%CI(0.51,0.85)],and absence of comorbidities[OR=0.02,95%CI(0.02,0.03)]were associated with frailty.Restricted cubic spline fitting of the multiple Logistic regression model revealed that there was a non-linear relationship between waist circumference and the risk of frailty in both men and women(Pnonlinear<0.05),and a non-linear relationship between BMI and the risk of frailty in women(Pnonlinear<0.05).Conclusions:General obesity and central obesity are risk factors for frailty in the middle-aged and elderly population in China,and this association exhibits sex differences.In women,both general obesity and central obesity are associated with an increased risk of frailty,whereas in men,only central obesity is linked to an elevated frailty risk..

Effect of pre-sleep care communication checklist on improving sleep quality in urological surgery patients

Abstract:Objective:To explore the effect of the pre-sleep care communication checklist nursing intervention on sleep quality in urological surgery patients.Methods:Using a random number table,120 patients were divided into the intervention group and the control group,with 60 patients in each group.The control group received routine care,while the intervention group received the pre-sleep care communication checklist intervention in addition to routine care.The checklist covered environmental management,pain management,psychological support,and sleep guidance.The Pittsburgh Sleep Quality Index(PSQI),Self-Rating Anxiety Scale(SAS),and Numerical Rating Scale(NRS)were used to assess patients' sleep quality,anxiety levels,and pain intensity,respectively.Results:On the third postoperative day,the PSQI score in the intervention group was(6.5±1.2),lower than that in the control group(8.2±1.5);the SAS score was(52.1±6.0),lower than that in the control group(60.2±6.8);and the NRS scores at 24 and 48 hours postoperatively were(3.5±0.8)and(2.8±0.7),respectively,better than those in the control group[(4.2±0.9)and(3.6±0.8)].All differences were statistically significant(P<0.01).Conclusions:The multi-dimensionally designed pre-sleep care communication checklist,through interventions such as optimizing the ward environment,standardizing pain management,providing psychological support,and offering scientific sleep guidance,can effectively improve sleep quality,alleviate anxiety,and reduce pain in urological surgery patients.

Latent profile analysis of person-environment fit creativity of clinical healthcare workers and its influencing factors

Abstract:Objective:To explore the latent profiles of person-environment fit creativity among healthcare workers and to analyze the characteristic differences and influencing factors across these profiles.Methods:A total of 492 healthcare workers from four tertiary grade A general hospitals in Hunan province were surveyed from November 2024 to January 2025 using general information questionnaires,the Person-Environment Fit Creativity Scale,the Creative Self-Efficacy Scale,the Innovative Behavior Scale,the Leader Empowering Behavior Scale,and the Work Engagement Scale.Latent profile analysis was used to identify the potential categories of person-environment fit creativity.Logistic regression was applied to analyze the influencing factors of different profiles.Results:The latent profile analysis revealed that healthcare workers' person-environment fit creativity could be categorized into three latent classes:low-level adaptive type(32%),medium-level dynamic type(58%),and high-level synergistic type(10%).Logistic regression results indicated that education level,average monthly income,publication of papers or patents,creative self-efficacy,innovative behavior,leader empowering behavior,and work engagement were influencing factors for the different categories of person-environment fit creativity(P<0.05).Conclusions:Healthcare workers' person-environment fit creativity is at a medium level with significant group heterogeneity.It is recommended that healthcare institutions implement targeted incentive policies based on the characteristics of different categories of person-environment fit creativity,and gradually build an ecosystem that stimulates healthcare workers' creativity.

Abstract:Objective:To clarify the definition and connotation of nurses' professional growth.Methods:The databases of CNKI,Wanfang,VIP,PubMed,Web of Science,CINAHL,ProQuest and PsycINFO were searched and the concept analysis method proposed by Walker and Avant was used to analyze the concept of nurses' professional growth.Results:The defining attributes of nurses' professional growth included professional growth motivation,professional growth opportunities,professional learning and practice,professional identity,and professional improvement.The antecedent factors included personal characteristics,organizational support and social support,and the posterior factors included personal level,organizational level and patient level.Good professional growth of nurses ultimately leads to the promotion of nurses,the formation of a good organizational culture and the provision of high-quality nursing for patients.Conclusions:In future research,researchers can combine the core attributes of professional growth of nurses to develop specific assessment tools for the professional growth of nurses and construct intervention programs to promote the professional growth of nurses,thereby promoting the professional growth of nurses.

Abstract:Objective:To adapt the Seniors Empowerment and Advocacy in Patient Safety(SEAPS)scale into the local context and to validate its among elderly stroke patients,so as to provide a reliable tool for assessing the level of self-advocacy in older patients.Methods:The Brislin translation model was used for translation,back-translation,and cultural adaptation of the SEAPS.A convenience sample of 431 elderly stroke patients from a Grade A tertiary hospital in Wuxi city was surveyed to test the reliability and validity of the scale.Results:Exploratory factor analysis extracted four factors:behavior,self-efficacy,outcome efficacy,and attitude,with a cumulative variance contribution rate of 83.495%.Item analysis showed correlations between each item and the total score.Content validity analysis indicated that the item-level content validity index(I-CVI)ranged from 0.83 to 1.00,and the scale-level content validity index(S-CVI/Ave)was 0.968.Confirmatory factor analysis demonstrated a good model fit for the scale.The scale's Cronbach's α coefficient was 0.966,and the test-retest reliability was 0.916.Conclusions:The Chinese version of the SEAPS has good psychometric properties.It can be effectively used to assess the level of self-advocacy in elderly stroke patients.

Multidimensional influencing factors study on the types and duration of caregivers for disabled older adults in China

Abstract:Objective:To analyze the current status of caregiver types and care duration for disabled older adults in China and their multidimensional influencing factors based on the 2020 China Health and Retirement Longitudinal Study(CHARLS)data,providing empirical evidence for improving the care service and security system for disabled older adults.Methods:Utilizing the older adult sample from the CHARLS 2020 data,Chi-square tests and Softmax regression were employed to analyze the factors influencing caregiver types,while t-tests and multivariate regression were used to analyze the factors influencing care duration.Results:A total of 3,349 disabled older adults were included.The results showed that gender,age,marital status,education level,disability level,depression status,economic support,and annual household income per capita all had significant effects on both caregiver types and care duration(P<0.05).Advanced age,having a spouse,higher education level,severe disability,negative economic support,and higher household income were associated with longer care duration,while depression status was associated with shorter care duration.Conclusions:The care system for disabled older adults in China faces issues of structural imbalance and uneven resource distribution.It is recommended to implement comprehensive measures,including strengthening the community care system,promoting long-term care insurance,providing family caregiver skills training,establishing psychological early warning mechanisms,and encouraging social capital involvement in elderly care services,so as to optimize the allocation of care resources and to meet the diverse care needs of disabled older adults.

Construction and validation of risk prediction model for workplace bullying among nurses based on the Chinese Nurses' Health Cohort Study(TARGET)

Abstract:Objective:To analyze the influencing factors of workplace bullying among nurses and construct risk prediction model to assist hospital administrators in developing targeted preventive measures and reducing the incidence of workplace bullying in this population.Methods:Based on data from the Chinese Nurses' Health Cohort Study(TARGET)covering 46 790 nurses from February 2021 to February 2024,surveys were conducted using the General Information Questionnaire,the Chinese version of the Negative Acts Questionnaire,the Generalized Anxiety Disorder Questionnaire,the Patient Health Questionnaire,the Chinese version of the Ten-Item Big Five Inventory,the Trait Coping Style Questionnaire,and the Chinese version of the Perceived Stress Scale.Participants were randomly divided into a modeling group(32 752 individuals)and a validation group(14 038 individuals)at a ratio of 7∶3.Univariate and logistic regression analyses were used to screen the influencing factors of workplace bullying among nurses and establish a risk prediction model.The Bootstrap method was employed for internal validation of the prediction model,and external validation was performed using the validation group data.The calibration curve was used to evaluate the predictive performance of the model.Results:Among the 46 790 nurses,9 274 experienced workplace bullying,with an incidence rate of 19.82%.Logistic regression analysis showed that age,gender,position,years of work experience,anxiety level,depression level,perceived stress score,positive coping score,negative coping score,Big Five personality traits(extraversion,agreeableness,emotional stability,openness),education level,department,and employment type were influencing factors for workplace bullying among nurses(P<0.05).The areas under the receiver operating characteristic curve for the modeling group and validation group were 0.729[95%CI(0.723,0.736)]and 0.728[95%CI(0.718,0.738)],respectively,with specificity of 0.664 and 0.631 and sensitivity of 0.683 and 0.718.The calibration curve indicated a good fit.Conclusions:Based on the TARGET cohort data,this study screened the relevant influencing factors of workplace bullying among Chinese nurses,constructed and validated risk prediction model for bullying in this population.The model can help identify high-risk individuals early and provide a reference for targeted preventive interventions and the optimization of clinical management practices.

Family communication patterns of depressed adolescents and its influencing factors

Abstract:Objective:To investigate the characteristics of family communication patterns among depressed adolescents,and to analyze their influencing factors,so as to provide reference for formulating personalized auxiliary plans for the prevention and control of depression among adolescents.Methods:By using the convenience sampling method,a total of 563 depressed adolescents were selected as the research subjects who visited the outpatient department of psychological medicine in a tertiary grade A hospital in Zhengzhou city,Henan province from October 2023 to August 2024.The investigation was conducted by using the general information questionnaire,the Center for Epidemiologic Studies Depression Scale(CES-D),the Revised Family Communication Patterns Instrument(RFCP),the Emotion Regulation Questionnaire(ERQ)and the Adolescents Self-harm Scale.Results:The characteristics of family communication pattern among depressed adolescents were mainly protective type in 183 cases(32.5%).The total score of the RFCP was 5.64±0.92.The score of conversation orientation was 2.91±0.87.The score of obedience orientation was 2.71±0.94.The results of linear regression analysis showed that the gender of depressed adolescents,whether they were the only child,whether they had history of being left behind,the educational level of the mother,the place of residence,the average monthly income per capita of the family,the score of Adolescents Self-harm Scale,and the score of ERQ were the influencing factors of family communication patterns(P<0.05).Conclusions:Medical staff should formulate targeted intervention measures based on the influencing factors of family communication patterns,to help depressed adolescents and their parents actively cope with the disease,improve their family communication patterns,and thereby alleviate their negative emotions and psychological disorders.
