Clinical study of radiofrequency ablation for relieving inguinal pain caused by radiotherapy for cervical cancerAbstract:Objective To investigate the clinical effect of radiofrequency ablation on relieving inguinal pain caused by cervical cancer radiotherapy,and to evaluate its impact on patients'quality of life.Methods 80 patients from April 2022 to April 2024 with inguinal pain after radiotherapy of cervical cancer in Changzhou Second People's Hospital were divided into two groups by random number table method.Conventional medication and physical therapy were used in the control group,while the observation group received radiofrequency ablation under ultrasound guidance.Pain levels,quality of life and treatment satisfaction were compared between the two groups.Results After treatment,the pain score of the observation group decreased significantly,which was significantly different from that of the control group(P<0.05).At the same time,the quality of life scores of the observation group were significantly improved in all dimensions(P<0.05).In terms of treatment satisfaction,it was significantly higher in the observation group than in the control group(P<0.05).Conclusion Compared with traditional therapy,radiofrequency ablation is more targeted and effective in the treatment of inguinal pain caused by cervical cancer radiotherapy.It can not only effectively relieve pain and improve the quality of life,but also improve the treatment satisfaction of patients due to its characteristics of minimal invasion,safety and quick recovery.
Chronic mesenteric ischemia:interpretation of the Clinical Practice Guidelines by the Society for Vascular SurgeryAbstract:Chronic mesenteric ischemia(CMI)is typically caused by the atherosclerotic occlusive disease at the origins of mesenteric arteries.With the increasing trend of population aging,the incidence of CMI is rising annually.The Clinical Practice Guidelines for Chronic Mesenteric Ischemia issued by the Society for Vascular Surgery(SVS)aims to provide the best evidence for the diagnosis and treatment of CMI.The guidelines primarily cover the following aspects:diagnosis and management of CMI,endovascular or open surgical revascularization,perioperative patient management,follow-up strategies,salvage therapy for restenosis,and future perspectives.Its publication can facilitate the optimization of diagnosis,treatment and comprehensive management for CMI patients.To promote its clinical application,this article elaborates and interprets the key points of the guidelines in detail.
Laparoscopic sleeve gastrectomy with proximal jejunal bypass and hiatal hernia repairAbstract:The patient was a young male diagnosed with severe obesity,esophageal hiatal hernia,hypertension,hyperlipidemia,hyperuricemia,hyperinsulinemia,obstructive sleep apnea syndrome and reflux esophagitis.He underwent laparoscopic sleeve gastrectomy combined with proximal jejunal bypass(SG+PJB)and hiatal hernia repair(HHR).SG+PJB can reduce the volume of gastric storage by means of a sleeve gastrectomy,with the exclusion of 300 cm of jejunum.This procedure is designed to achieve results comparable to or better than those of gastric bypass but with fewer complications,technical difficulties and potential nutritional deficiencies.Reflux esophagitis is a common complication following sleeve gastrectomy,while intraoperative repair of the loose esophageal hiatus is an effective measure to prevent this condition.
Influence of family participatory nursing based on mindfulness psychological support on medical compliance and psychological status of patients with diabetes foot complicationsAbstract:Objective To explore the influence of family participatory nursing based on mindfulness psychological support on compliance and psychological status of patients with diabetes foot complications.Methods 80 patients with diabetes foot complications admitted to Changzhou Second People's Hospital from January to October 2024 were selected for a prospective group design study.They were randomly divided into two groups with 40 patients in each group.The control group received routine nursing,while the observation group received family participatory nursing based on mindfulness psychological support.The foot self-care ability,compliance with medical treatment,family care index,and psychological status were compared between two groups before and after intervention,and the deterioration of the condition was recorded.Results After intervention,the total score of foot self-care ability and the five compliance scores in both groups increased compared with those before intervention,and the total score of foot self-care ability and the above compliance scores in the observation group were higher than those in the control group(P<0.05).After intervention,the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores of both groups decreased compared with those before intervention,while the family care index increased.The negative psychological scores of the observation group were lower than those of the control group,and the family care index was higher than that of the control group(P<0.05).There were no significant differences in the readmission rate,foot necrosis rate,and amputation rate between the two groups(P>0.05).Conclusion The application of family participatory nursing based on mindfulness psychological support in patients with diabetes foot complications has significant effect,which can effectively improve the patients'family care index and foot self-care ability,improve patients'negative psychology,and promote the formation of medical compliance behavior.
Total laparoscopic expanded radical resection of right colon cancer(D3+CME)Abstract:The patient was a middle-aged woman with cancer of the hepatic flexure of the colon(cT 3-4 NxM 0).Total laparoscopic radical extended right hemicolectomy(D 3+CME)was performed via caudal-dorsal approach.First,the posterior space of the ascending colon and the anterior pancreaticoduodenal space behind the transverse colon were dissected.Then,the left edge of the superior mesenteric artery was used as the boundary of mesocolon of right half colon,and the relevant arteries and veins were ligated,and the lymphatic tissue was dissected from the roots.Finally,No.206 lymph nodes were freed and dissected from the gastroepiploic arch.The transverse colon and ileum was performed followed by side-to-side anastomosis(Overlap).Total laparoscopic extended right hemicolectomy(D3+CME)can completely remove the mesentery within the tumor bearing area under the guidance of membrane anatomy,reduce intraoperative bleeding,and especially completely remove the cancer metastasis between the fat in the mesentery,that is,"the fifth metastasis".
Pathological response to TACE combined with molecular targeted therapy plus immune checkpoint inhibitors versus TACE alone in BCLC-B HCC:A retrospective studyAbstract:Objective To explore the pathological response and its potential related factors in patients with BCLC stage B hepatocellular carcinoma(HCC)who underwent transarterial chemoembolization(TACE)combined with molecular targeted therapy plus immune checkpoint inhibitors or TACE alone.Methods This retrospective study included 200 HCC patients(representing a total of 234 target lesions)who underwent surgical resection after receiving either TACE combined with molecular targeted therapy plus immune checkpoint inhibitors or TACE alone.This study was conducted at the First Affiliated Hospital of Zhejiang University School of Medicine from January 2020 to March 2024.The enrolled HCC target lesions were divided into two groups,the triple therapy group(TACE+molecular targeted therapy+immune checkpoint inhibitors,n=101)and the TACE alone group(n=133).Pathological outcomes were compared between the two groups using three key metrics:pathological necrosis ratio(PNR),pathological complete response(pCR,0%viable tumor),and major pathological response(MPR,≤10%residual viable tumor).Multivariate logistic regression analysis was performed to identify the potential related factors associated with pCR.Results The triple therapy group showed superior median PNR(90.0%vs 80.0%),pCR(30.7%vs.17.3%)and MPR rates(56.4%vs.42.1%)compared with the TACE alone group(all P<0.05).Multivariate analysis identified moderate tumor differentiation(OR=3.34),histological subtypes(OR=32.45)and triple therapy(OR=3.02)as independent predictors for pCR.Conclusion Triple therapy integrating TACE with molecular targeted therapy plus immune checkpoint inhibitors can significantly enhance pathological response rates in BCLC B-stage HCC compared with TACE alone therapy,with tumor biological characteristics,particularly the differentiation status and histological subtype,serving as potential critical predictive factors for pCR.
