Abstract:Chronic non-communicable diseases have become the primary disease burden both globally and in China,which occurrence and progression are largely driven by long-term poor dietary patterns and lifestyles.However,it is difficult for the current health service system to achieve effective intervention before diseases occur.To address this structural challenge,this paper,from the perspective of the big health industry,expounds the theoretical positioning of Family Nurse Diet Therapy and systematically clarifies its value in the prevention of chronic diseases and health management.On this basis,this paper focuses on demonstrating how this theory can serve as a key theoretical hub to strategically drive the deep integration and upgrading of agriculture,food industry,medical and health care,and service industries.This paper aims to provide high-level theoretical guidance and systematic strategic analysis for industrial innovation under"Healthy China"initiative.

Strategic considerations for diet therapy-centered lifestyle medicine included into secondary disciplines of nursing under the Healthy China initiative

Abstract:Against the backdrop of the"Healthy China"initiative,nursing science is undergoing a paradigm shift from a"disease-centered model"to one"oriented toward health maintenance and health promotion."From the perspectives of disciplinary development and knowledge system construction,this paper systematically analyzes the necessity and feasibility of developing diet therapy-centered lifestyle medicine as an emerging secondary discipline within nursing.The current structure of nursing secondary disciplines is primarily organized along the dimensions of"population-problem-care setting,"with limited incorporation of knowledge integration based on health intervention mechanisms,thereby constraining the systematic inclusion of methodologically driven frontier fields such as lifestyle interventions.This study demonstrates that lifestyle medicine,with its focus on behavior change and health promotion,is highly aligned with the core professional attributes of nursing,including continuity of care,contextualized practice,and patient education.However,the disciplinary subjectivity of nursing and the systematic construction of knowledge in this field remain insufficiently developed.Notably,the nursing discipline has already achieved a critical transition in this domain,shifting from exploratory practice to theoretical construction.Existing studies indicate that,grounded in"Family Nurse Diet Therapy"and supported by relevant clinical research evidence,diet therapy-centered lifestyle nursing interventions have established a relatively coherent knowledge system encompassing theoretical frameworks,practice pathways,and outcome evaluation,could independently assess the effectiveness of nursing practice,reflect the uniqueness of nursing practice and the professional value of nurses.These developments provide a solid academic foundation for its evolution into an independent secondary discipline of nursing.Elevating this direction to the level of a secondary discipline characterized by health intervention mechanisms would not only refine the existing logic of disciplinary classification but also offer essential academic support for nursing's transformation from a traditional role in disease care to a leading role in health intervention and guidance within the"Healthy China"initiative.Promoting the disciplinary institutionalization and systematic development of this field represents a critical pathway for expanding the professional boundaries of nursing in health promotion and for constructing a nursing disciplinary system with distinct Chinese characteristics.

Abstract:The global prevalence of chronic diseases is becoming increasingly severe and has become the leading cause of death.The disease spectrum and prevention and treatment models of chronic diseases are undergoing profound changes.Metabolic disorders,as the common pathological basis of various chronic diseases,through core mechanisms such as insulin resistance and chronic low-grade inflammation,have established a metabolic comorbidity network characterized by the progression of a single disease and the coexistence of multiple diseases,promoting the transformation of chronic disease medicine from traditional single-disease diagnosis and treatment to the era of metabolic medicine centered on metabolic regulation.Against this backdrop of the times,metabolic medicine has developed a comprehensive prevention and treatment system with lifestyle management as the foundation and precise dietary intervention as the core.However,in practice,this system still faces bottlenecks such as an imperfect theoretical framework,insufficient personalized intervention,and poor patient adherence.Family Nurse Diet Therapy through an innovative path of non-nutrient holistic prevention and treatment,can effectively break through the above predicament and provide important theoretical support for dietary prevention and treatment in the era of chronic disease metabolic medicine.The promotion and application of this theory is expected to drive a fundamental transformation in the prevention and control of chronic diseases,from passive response to active prevention and control,and from target intervention to the restoration of system homeostasis,providing new ideas and references for global chronic disease prevention and control.

Analysis on status quo and influencing factors of operating room nurses' application ability in nursing process

Abstract:Objective:To investigate the status quo of the application ability of operating room nurses to implement nursing process and its influencing factors,and to provide a basis for formulating corresponding intervention measures.Methods:By using the convenience sampling method,a cross-sectional survey was conducted among 1,218 operating room nurses in Sichuan province from January to April 2025,using a general information questionnaire,a Competency of Nursing Process Questionnaire,the Job Crafting Scale,a Reflective Ability Scale for Clinical Nurses,and the General Self-Efficacy Scale.Results:The total score of the application of nursing process by operating room nurses was(92.76±12.67),the score of reflective ability of clinical nurses was(82.90±11.95),the score of job crafting was(91.47±12.45),and the score of general self-efficacy was(33.66±5.44).The application ability of nursing process of operating room nurses was positively correlated with job craftingg,self-efficacy and reflective ability(r values were 0.714,0.671 and 0.809 respectively,all P<0.01).Multivariate analysis showed that professional title,training experience,job crafting ability,self-efficacy and clinical reflective ability had significant effects on the application ability of nursing process(P<0.05).Conclusions:The application ability of nursing process by nurses in the operating room is good but still has room for improvement.Nursing managers should play a leading role well,provide nurses with sufficient work support and resources,enhance their self-efficacy and reflective ability,so as to ensure the quality of nursing and promote the postoperative recovery of patients.

Abstract:Objective:To systematically clarify the connotation and extension of the emerging concept of unaccompanied care by using Rodgers' evolutionary concept analysis method,and to provide theoretical basis and practical guidance for the standardization of practice of this nursing model,the formulation of relevant policies,and future research.Methods:The system retrieves Chinese and English databases such as China National Knowledge Infrastructure(CNKI),Wanfang Database,PubMed,and Web of Science.The retrieval period was from the establishment of the databases to July 31,2025.To extract and summarize the definition attributes,prerequisites,evolutionary context and related concepts of no-companion care according to Rodgers' evolutionary concept analysis method.Results:The analysis of 24 core articles revealed that the concept of unaccompanied care evolved from spontaneous substitution to systematic management and was now developing towards professional and intelligent integrated services.Its core definitional attributes included professionalism,systematicness,comprehensiveness,and substitutability.The prerequisites for its formation involved patient and family level,healthcare institution level,and societal level.By differentiating it from related concepts such as group nursing,case nursing,the conceptual boundaries of unattended care were further clarified.Conclusion:Clarifying the concept of no-companion care is conducive to the construction of the theoretical system and the exploration of related fields,and provides a clear conceptual framework for it,laying a theoretical foundation for building a no-companion care service system that is adapted to China's national conditions.

Influencing factors of hypothermia management behaviors of medical staff in pediatric cardiac surgery under cardiopulmonary bypass:a qualitative study

Abstract:Objective:To explore the influencing factors of hypothermia management behaviors of medical staff during pediatric cardiac surgery under cardiopulmonary bypass,and to provide a basis for effectively dealing with intraoperative hypothermia in children undergoing cardiac surgery under cardiopulmonary bypass.Methods:By using purposive sampling,face-to-face semi-structured interviews were conducted with 13 medical staff from a tertiary grade A hospital in Zhengzhou city from January to February 2025.The data were analyzed using the traditional content analysis method.Results:The main promoting factors for medical staff to implement unplanned hypothermia management during surgery included attaching importance to the significance of child body temperature management,conscious awareness of role responsibilities,and appropriate resource allocation.Obstacle factors included knowledge and skills(lack of a complete knowledge system,lack of professional operation methods and techniques),teams and resources(focus on key links,lack of multi-disciplinary team communication and collaboration,team member interference),systems and supervision(lack of temperature safety handover systems,lack of quality evaluation and feedback mechanisms).Conclusion:Managers should enhance training to improve the knowledge and skills of operating room medical staff in managing intraoperative hypothermia under cardiopulmonary circulation,formulate standardized temperature protection procedures to promote behavioral implementation,promote teamwork,strengthen the collection of quality evaluation indicators,conduct regular evaluations and feedback,and jointly deal with the occurrence of intraoperative hypothermia to improve its clinical outcomes.

Summary of the best evidence for the prevention and management of pocket hematoma in patients with cardiovascular implantable electronic devices

Abstract:Objective:To summarize the relevant evidence on the prevention and management of pocket hematoma in patients with cardiovascular implantable electronic devices(CIED)at home and abroad,and lay an evidence-based foundation for clinical practice.Methods:PIPOST framework was used to construct evidence-based questions,computer searches were conducted in databases and websites such as National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,BMJ Best Practice,the Joanna Briggs Institute(JB I)Evidence-Based Healthcare Center Database,UpToDate,the Cochrane Library,PubMed,and CNKI to retrieve literature related to the prevention and management of pocket hematoma in patients with CIED.The types of literature included clinical decisions,practice guidelines,evidence summaries,specialist consensus,and systematic reviews.The search period ranged from the establishment of the databases to March 22,2025.Two researchers trained in evidence-based nursing independently assessed the quality of the included researches,extracted data and integrated evidence.Results:A total of 23 articles were ultimately included,comprising 1 practice guideline,1 evidence summary,6 specialist consensus and 15 systematic reviews.The evidence was summarized into 22 recommendations across five aspects,anticoagulation management,pocket wound management,posture management,activity management of the surgical limb,and continuity of care.Conclusion:This study has collated the relevant evidence on the prevention and management of pouch hematoma in CIED patients.It suggests that healthcare providers should combine with the clinical situation and the patient's wishes to formulate scientific and standardized measures for the prevention and management of pouch hematoma in CIED patients,thereby reducing the occurrence of such hematomas.

Qualitative study on nursing researchers'perceptions and application experiences of generative artificial intelligence

Abstract:Objective:To investigate the real perceptions,practical experiences,and ethical considerations of nursing researchers regarding the application of generative artificial intelligence(Gen-AI)in nursing research,thereby providing a theoretical basis for standardizing the use of Gen-AI in nursing research and formulating relevant training strategies.Methods:A descriptive qualitative research method was adopted.Using purposive sampling,semi-structured in-depth interviews were conducted from January to March 2025 with 14 nursing staff engaged in research work in three tertiary grade A hospitals in Tianjin.Data analysis was performed using Colaizzi's 7-step analysis method.Results:The perceptions and practical experiences of nursing researchers regarding Gen-AI were summarized into four themes and fourteen sub-themes,the enhancing effect of Gen-AI on research efficiency and capability(research process optimization,lowering the entry barrier,stimulation of innovative thinking);risks and ethical dilemmas in Gen-AI application(concerns about information reliability,blurring of academic integrity boundaries,risk of data privacy leakage,anxiety about the weakening of researcher capability,anxiety about ambiguous academic contribution attribution);competence and role reconstruction of nursing researchers driven by Gen-AI(enhancement of critical thinking,cultivation of Gen-AI literacy,transition of researcher roles,restructuring of research team composition and division of labor);future pathways for Gen-AI usage norms and professional development(urgent need to establish standardized usage guidelines,expectation for developing specialized nursing vertical models).Conclusions:Nursing researchers hold a cautiously optimistic attitude towards the application of Gen-AI in scientific research,acknowledging its potential for technological empowerment while placing high importance on the accompanying ethical challenges and the need for competence reconstruction.It is recommended that relevant administrative bodies and academic organizations jointly promote the standardized and professional development of Gen-AI usage to achieve high-quality innovation in nursing research through human-machine collaboration.

Current status survey and influencing factors analysis of physical resilience in elderly patients with cancer pain

Abstract:Objective:To explore the current status of physical resilience(PR)in elderly patients with cancer pain and analyze its influencing factors.Methods:Using convenience sampling method,195 hospitalized elderly patients with chronic cancer pain were selected from a tertiary A tumor specialist hospital in Fujian province from April 2023 to April 2024.The participants were investigated using a general information questionnaire,Physical Resilience Instrument for Older Adults(PRIFOR),the Barthel Index(BI),the Social Support Rating Scale(SSRS),and Pain Catastrophizing Scale(PCS).Univariate analysis and multiple linear regression analysis were used to identify influencing factors of PR in elderly patients with cancer pain.Results:The total PRIFOR score of elderly patients with cancer pain was(42.34±11.03),which was at a moderately low level.PRIFOR score was positively correlated with BI(r=0.588,P<0.001)and SSRS score(r=0.369,P<0.001),and negatively correlated with PCS score(r=-0.205,P=0.004).Multiple linear regression analysis showed that fall risk,average pain intensity,activities of daily living(ADL)ability,social support,and pain catastrophizing level were influencing factors of PR in elderly patients with cancer pain(P<0.05).Conclusions:The overall PR level of elderly patients with cancer pain is relatively low,influenced by physiological,psychological,and social factors.In cancer pain nursing,attention should be paid to ADL training,pain catastrophizing intervention,and strengthening of social support to enhance patients'resilience and quality of life.

Longitudinal study on the changes of cancer recurrence fear in patients with primary liver cancer after the first transcatheter arterial chemoembolization

Abstract:Objective:To explore the longitudinal changes and influencing factors of fear of cancer recurrence in patients with primary liver cancer after the first transcatheter arterial chemoembolization(TACE),and to provide the basis for phased psychological intervention.Methods:A total of 183 patients with primary liver cancer who underwent their first TACE in a hospital from February to December 2023 were selected by convenience sampling.Fear of cancer recurrence was assessed using the Fear of Progression Questionnaire-Short Form at 3 days,1 month,6 months,and 12 months after the procedure.Results:The fear of cancer recurrence scores at these four time points were(35.68±8.02),(36.83±7.53),(31.07±6.45),and(29.12±5.74)respectively,showing an overall decreasing trend(F=38.353,P<0.001).The results of multiple linear regression analysis showed that medical burden,depression,and social support predicted fear of cancer recurrence at each time points within one year after surgery;disease perception predicted the fear of recurrence at 1 month,6 months,and 12 months postoperatively;age 18~44 years predicted the fear of recurrence at 1 month and 6 months postoperatively;and employed predicted the fear of recurrence at 12 months postoperatively.Conclusions:Fear of cancer recurrence in primary liver cancer patients gradually alleviates over time after the first TACE,with predictors showing dynamic changes.It is recommended to implement targeted psychological interventions based on the characteristics of different postoperative phases to reduce fear of cancer recurrence.

Longitudinal qualitative study on the journey map of supportive care needs of primary caregivers of moderately to severely disabled elderly

Abstract:Objective:To identify the multidimensional supportive care needs of primary caregivers of moderately to severely disabled elderly based on a journey map,aimed to provide a reference for developing comprehensive supportive care interventions throughout their caregiving journey.Methods:A longitudinal descriptive qualitative study was conducted.Using purposive sampling,12 primary caregivers of home-dwelling moderately to severely disabled elderly in a community in Guiyang city from October 2024 to May 2025 were selected to participate in three semi-structured interviews.Data were analyzed using content analysis,and a supportive care needs journey map for primary caregivers was constructed.Results:Taking the acute stress period,adaptation fatigue period,and long-term care period of moderately to severely disabled elderly people as the time horizontal axis of the journey map,the supportive care needs of the main caregivers were subdivided and summarized into 27 themes such as establishing initial care arrangements and receiving and learning care knowledge from three dimensions,tasks,emotions,and pain points,and the journey map was subsequently developed.Conclusions:The supportive care journey for primary caregivers of moderately to severely disabled elderly is lengthy and complex,with supportive care needs being dynamic and multidimensional.Healthcare professionals and community workers can develop targeted supportive care strategies based on these needs to provide precise support.Future efforts could integrate digital and intelligent intervention technologies to achieve holistic,personalized,and precise care management,thereby enhancing the quality of life for both disabled older adults and their caregivers.

Exploration and practice of nursing performance reform plan in public hospitals under the background of high-quality development

Abstract:Objective:To explore the establishment of a compensation distribution system that integrates job responsibilities with nursing value by introducing key indicators of diagnosis related groups(DRGs)and the objectives and key results(OKR)framework in the context of high-quality development in hospitals,and to stimulate nursing vitality through comprehensive evaluation methods,improve management efficiency,and support the realization of hospital strategic goals.Methods:From January to December 2023,a nursing performance management plan based on work performance points using the resource-based relative value scale(RBRVS)method was implemented.From January to December 2024,a DRGs+RBRVS performance assessment method was adopted to calculate the direct and indirect nursing work efficiency and performance coefficients for different nursing units,forming a tiered monthly assessment mechanism.Based on OKR,with high-quality nursing development as the key objective,five assessment points,nursing safety,nursing service,nursing quality,nursing education and research,and nursing competence,were quantified and weighted.These were incorporated into the monthly assessment in the form of assessment scores,with the surplus used for annual target management comprehensive evaluation rewards.A comparative analysis of nursing work performance,nurse satisfaction,and patient satisfaction before and after the performance reform was conducted.Results:After the implementation of this nursing performance reform plan,the departmental target management responsibility score was 97.8 points,compared to 96.4 points before implementation,showing an improvement rate of 1.45%.The situation of various nursing work indicators had improved significantly.Nurse satisfaction with performance assessment and patient satisfaction significantly improved compared to before implementation(P<0.05).Conclusion:The nursing performance reform plan based on DRGs and OKR,through lean management methods,reinforces"high performance,high reward"based on"more work,more pay."It achieves an organic integration of nursing work and nursing performance,promoting the high-quality development of nursing.

Research on the influencing factors of relapse in late-life depression based on health ecology model

Abstract:Objective:To explore the influencing factors of relapse in late-life depression based on the health ecology model,and to provide a basis for the management,prevention,and treatment of relapse in late-life depression in China.Methods:Elderly patients with depression who were treated in the outpatient and inpatient departments of a mental health specialty hospital in Shandong province from March to October 2024 were selected as the survey subjects.Based on the health ecology model,a questionnaire on the influencing factors of late-life depression relapse among the elderly was designed for on-site investigation.The contents included the personal trait layer,psychological and behavioral characteristics layer,interpersonal network layer,work and life conditions layer,and policy environment layer.Logistic regression analysis was used to analyze the influencing factors of late-life depression relapse.Results:Among 522 elderly patients with depression,371 cases had relapse,with a relapse rate of 71.1%.The results of Logistic regression analysis showed that family history[OR=2.554,95%CI(1.428,4.567)],the number of chronic diseases≥3 types[OR=2.804,95%CI(1.268,6.199)],disease duration of 6~12 months[OR=4.748,95%CI(2.154,10.465)],disease duration of 12~24 months[OR=3.426,95%CI(1.670,7.029)],disease duration>24 months[OR=17.522,95%CI(9.407,32.637)],poor medication compliance[OR=3.111,95%CI(1.883,5.140)],and low social support level[OR=2.241,95%CI(1.015,4.946)]were risk factors for late-life depression relapse.Conclusions:The relapse in late-life depression is the result of multiple factors acting together.Nursing staff can identify high-risk patient groups based on relevant influencing factors and provide targeted interventions to reduce the risk of relapse.

Current status survey and influencing factors analysis of health education competency among intensive care unit nurses

Abstract:Objective:To investigate the current status of health education competency among intensive care unit(ICU)nurses in a specific region of Henan province and analyze its influencing factors,providing a basis for developing targeted training programs for ICU nurses'health education competency.Methods:A questionnaire survey was conducted among 283 ICU nurses from five secondary and tertiary hospitals in a specific region of Henan province using a general information questionnaire,the Chinese version of the Nurse Health Education Competency Scale,and Caring Ability Inventory.Results:The total score of health education competency among 283 ICU nurses was(235.58±32.85),and the humanistic care ability score was(202.07±28.84).Multiple linear regression analysis showed that average daily time spent on health education,whether the healthcare institution provides health education guidelines,tools,or regular health education training,whether they had participated in health education training,academic lectures,or relevant advanced studies,the degree of interest in health education,and nurses'humanistic care ability were factors influencing ICU nurses'health education competency(P<0.005),explaining 67.50%of the total variance(F=65.98,P<0.001).Conclusions:The health education competency of 283 ICU nurses is at a moderately high level and requires further improvement.It is recommended that hospitals enhance ICU nurses'health education competency by providing health education guidelines,offering diverse health education tools,and conducting ICU-specialized health education related training or academic lectures,thereby enabling them to fully leverage their leading role and advantages in patient health management.

Summary of the best evidence for pulmonary rehabilitation management based on wearable devices in patients with chronic obstructive pulmonary disease

Abstract:Objective:To systematically summarize the best evidence on pulmonary rehabilitation management supported by wearable devices for patients with chronic obstructive pulmonary disease,providing an evidence base for clinical practice and implementation.Methods:The PIPOST framework was used to define research questions.Systematic searches were conducted in Chinese and international databases as well as guideline repositories.Eligible studies included randomized controlled trials,observational studies,guidelines,evidence summaries,expert consensus statements,systematic reviews,and Meta-analyses.Quality was appraised using the JBI and AGREEⅡtools.Results:Twelve studies were finally included:three guidelines,one expert consensus,four systematic reviews,two RCTs,and two observational studies.Six evidence themes and twenty-three evidence statements were generated,covering assessment and baseline measurement,training modalities and indicators,wearable and digital interventions,adherence and patient experience,safety and termination criteria,and localization and dissemination.Conclusion:Pulmonary rehabilitation management supported by wearable devices can effectively complement traditional rehabilitation evaluation and intervention models,demonstrating good feasibility,though further standardization of device selection and intervention processes is needed.

Abstract:Objective:To screen and summarize the best evidence for the medical transition preparedness of children and adolescents with tuberculosis to adult healthcare.Methods:Relevant literatures were searched from BMJ Best Practice,UpToDate,CINAHL,Embase,the Cochrane Library,Joanna Briggs Institute Evidence-based Healthcare Center(Australia),CBM,CNKI,Chinese Medical Journal Full-text Database,Medlink,PubMed,Wanfang Database,Web of Science,American Academy of Pediatrics(AAP),European Academy of Paediatrics(EAP),and Danish Medical Association(DMA)according to the"5S"evidence pyramid model.The quality of the retrieved literatures was evaluated according to the evaluation criteria,and the relevant evidence for the medical transition preparedness of children and adolescents with tuberculosis was extracted.Results:A total of 11 articles were selected,including 3 expert consensuses,2 guidelines,2 systematic reviews,2 evidence summaries,and 2 clinical decisions.A total of 18 items of best evidence were summarized from 4 aspects:transition preparedness principles,transition preparedness assessment,transition preparedness measures,and enhancing adaptability.Conclusion:The best evidence for the medical transition preparedness of children and adolescents with tuberculosis can provide an evidence-based basis for medical staff.It helps to promote the standardization and scientificization of the medical transition for this group,and improve the long-term health and quality of life of patients.

Abstract:The newly released Dietary Guidelines for Americans(2025-2030)marks a significant shift in the nation's nutrition policy,pivoting from a"healthy eating pattern"to a core emphasis on"eating real food",while introducing transformative recommendations regarding protein,fat,sugar,and processed foods.Professor Han Shifan's team first proposed the"Family Nurse Diet Therapy"in 2023 as an innovative dietary approach for chronic disease management.Having completed the"0 to 1"conceptual validation domestically,it is now advancing toward the"1 to N"stage of scaled application.This article aims to systematically interpret the scientific essence and practical guidance of the Dietary Guidelines for Americans(2025-2030)based on the"Family Nurse Diet Therapy",offering important insights and a referential Chinese approach for global chronic disease prevention and the development of localized health industries such as"dietotherapy-driven agriculture."

The relationship between psychological capital,empathic ability and humanistic care ability of midwifery students in higher vocational colleges

Abstract:Objective:To explore the current status of psychological capital,empathy ability and humanistic care ability of midwifery students in higher vocational colleges,and to analyze the intrinsic connections among the three.Methods:By using the convenience sampling method,a total of 248 midwifery students from two higher vocational colleges in Hunan Province were selected as the research subjects in September 2024.The General Information Questionnaire,Positive Psychological Capital Questionnaire(PPQ),Jefferson Scale of Empathy for Nursing Students(JSPE-S),and the Caring Ability Inventory(CAI)were used for assessment.Descriptive statistics,Pearson correlation analysis and multiple stepwise regression analysis were conducted using SPSS 25.0 software.Results:The scores of humanistic care ability,psychological capital and empathy ability of the research subjects were(165.25±14.87),(118.50±18.03)and(97.43±13.84)respectively.Correlation analysis indicated that the total scores of psychological capital and empathy ability were significantly positively correlated with the total score of humanistic care ability(P<0.01).Multiple regression analysis showed that optimism,resilience,self-efficacy,emotional care and perspective taking together explained 50.5%of the variance in humanistic care ability(R2=0.505).Among them,perspective taking(β'=0.253)and self-efficacy(β'=0.249)had the most prominent predictive effects.Conclusion:The overall humanistic care ability of midwifery students in higher vocational colleges needs to be improved,and it is closely related to psychological capital and empathy ability.Educators should attach importance to the cultivation of students'empathy ability and the development of positive psychological capital,so as to enhance their humanistic care ability and professional quality.

Influencing factors of central line-associated bloodstream infection in ICU patients:a scoping review

Abstract:Objective:To conduct a scoping review of studies regarding the influencing factors of central line-associated bloodstream infection in ICU patients.Methods:According to the scoping review framework developed by Arksey and O'Malley and following the"5S"pyramid model of evidence-based resources,a top-down search was conducted across the following databases:BMJ Best Practice,the Guidelines International Network(GIN),the National Guideline Clearinghouse(NGC),the National Institute for Health and Care Excellence(NICE)guidelines repository,the Scottish Intercollegiate Guidelines Network(SIGN),the Registered Nurses'Association of Ontario(RNAO),Medlive,the Joanna Briggs Institute(JBI)Evidence-Based Healthcare Center,PubMed,Web of Science,the Cochrane Library,CINAHL,Embase,China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Chinese Science and Technology Journal Database,and the China Biology Medicine disc(CBM).The literature types included clinical practice guidelines,evidence summaries,best practice recommendations,practice recommendations,systematic reviews,and original studies.The search period was set from January 1,2019,to September 30,2024.Both Chinese and English literature were searched and screened,followed by data extraction and synthesis of the included studies.Results:A total of 22 studies from 22 countries were adopted,including 4 guidelines,1 practice recommendation,2 expert consensuses,1 evidence summary,3 systematic reviews and 11 original studies.Literature analysis indicates that the factors influencing the occurrence of central line-associated bloodstream infections in ICU patients include:demographic characteristics;disease-,treatment-,and hospitalization-related factors;catheter insertion factors;aseptic techniques during catheter maintenance;and the operational practices of healthcare staff during maintenance,etc.Conclusions:The influencing factors of central line-associated bloodstream infection in ICU patients were found to include 6 aspects,which are complex and diverse.In the future,medical institutions can optimize the monitoring indicators for central line-associated bloodstream infection based on the above influencing factors.Among these factors,the operational practices of healthcare staff are relatively more amenable to intervention.Hospitals should enhance supervision,standardize catheter operation procedures,and improve the skill level and professional competence of ICU medical personnel to reduce the incidence of central line-associated bloodstream infections in ICU patients.
