Association between dietary quality and sarcopenic obesity:based on the NHANES databaseAbstract:Objective To explore the association between dietary quality and sarcopenic obesity(SO).Methods Based on data from the National Health and Nutrition Examination Survey(NHANES)in the United States from 2011 to 2018,participants in four survey cycles were selected as the study subjects.Weighted multivariate Logistic regression analysis was used to explore the association and interaction between Healthy Eating Index 2020(HEI-2020)and SO,and the mediating effects of inflammation,metabolism,and nutritional status on the relationship between HEI-2020 and SO were analyzed.Results A total of 8,602 participants were included,of whom 504 had SO,with a weighted prevalence of approximately 5.33%,The SO group had lower HEI-2020 scores.Multivariate Logistic regression analysis showed that,after adjusting for age,sex,lifestyle,related comorbidities,and energy intake,compared with low HEI-2020 scores,medium HEI-2020 scores[OR=0.73,95%CI(0.54,0.99)]and high HEI-2020 scores[OR=0.50,95%CI(0.35,0.72)]were negatively correlated with SO.Subgroup analysis showed that,except for low-income groups and those with sedentary time≥8 hours,high HEI-2020 scores were negatively correlated with SO.No interaction was found between age,sex,education level,income level,or sedentary time on the association between HEI-2020 and SO.Mediation analysis showed that systemic inflammatory response index(SIRI),glycated hemoglobin,non-HDL cholesterol to HDL cholesterol ratio(NHHR),albumin,and plasma vitamin D negatively mediated the relationship between HEI-2020 and SO.Conclusion Dietary quality is negatively correlated with SO.HEI-2020 can affect SO through the mediating effects of SIRI,glycated hemoglobin,NHHR,albumin,and plasma vitamin D.We should strengthen dietary health education,improve the dietary quality of residents,and promote healthy aging.
Causal relationship between cathepsins and autoimmune thyroid diseases:insights from a Mendelian randomization studyAbstract:Objective To explore the causal relationship between cathepsins and autoimmune thyroid diseases(AITDs)using Mendelian randomization(MR)study,and to provide genetic evidence for the association between cathepsins and the risk of AITDs.Methods The pooled datasets of nine cathepsins(B,E,F,G,H,L2,O,S and Z)and four AITDs were selected from a publicly available genome-wide association study(GWAS)website.The generalized summary data-based MR(GSMR),univariate bidirectional MR,and multivariate MR(MVMR)were used to analyze the causal relationship between them and the independent effects of specific risk factors.Plasma protein quantitative trait loci(pQTL)genetic instrumental variables were obtained from the Ferkingstad study to verify causality at the protein level.The inverse variance weighted method(IVW)was used as the primary analytical approach of univariate bidirectional MR Analysis,supplemented by MR-Egger,weighted median method,simple model,and weighted model methods.Horizontal pleiotropy and heterogeneity were evaluated,and sensitivity analysis was performed to ensure the robustness of the results.Results GSMR and forward MR analysis showed that cathepsin F(CTSF)significantly reduced the risk of Graves disease(GD).Reverse MR Analysis showed no reverse causality between the GD and CTSF.After adjusting for the effects of other cathepsins,MVMR analysis confirmed that the correlation between CTSF and GD was still significant.Furthermore,the causal effect between CTSF and GD was verified by cis-pQTL MR.Conclusion There is a causal relationship between CTSF and the reduced risk of GD,and CTSF is a potential protective factor against GD.
Progress of oncolytic virus therapy in urological cancersAbstract:Oncolytic virus(OV)therapy is an emerging anti-tumor therapeutic approach in which the virus can selectively replicate and destroy tumor cells without damaging normal cells.The virus stimulates the anti-tumor immunoreactive responses by releasing antigens and activating inflammatory responses in the tumor microenvironment.Despite recent advancements in the treatment of advanced urinary tract tumors,subsequent-line treatment options remain limited,and the disease faces clinical challenges such as high recurrence rates and drug resistance.Therefore,there is an urgent need to develop safer and more effective novel treatment strategies and drugs.This article reviews the latest research progress on OV therapy in the treatment of urinary tract tumors,systematically outlining their application prospects,current challenges,and future development directions.
The research progress of the diagnosis and treatment of Raynaud phenomenonAbstract:Raynaud phenomenon(RP)is a disease of intermittent vascular hypercontraction of fingers and toes caused by cold or pressure.Its etiology is closely related to genetic and hormonal factors,though the precise mechanisms remain unclear.The clinical treatment of RP is mainly to alleviate symptoms and prevent tissue damage.This article reviews the current research status of the RP,with a focus on its clinical diagnosis and treatment,aiming to provide scientific basis and guidance for clinical practice.
Prevalence and influencing factors of intraoperative hypothermia in pediatric patients:a Meta-analysisAbstract:Objective To systematically review the current status and influencing factors of intraoperative hypothermia(IOH)in pediatric patients.Methods CNKI,WanFang,VIP,CBM,PubMed,Web of Science,Embase,The Cochrane Library,CINAHL and other databases were searched from database inception to March 7,2025.After literature screening,quality assessment and data extraction,RevMan 5.4 was used for Meta-analysis.Results A total of 22 articles involving 7,846 patients were included.The results of Meta-analysis showed that the incidence of IOH in pediatric patients was 45%[95%CI(34%,55%)].Age≤1 year[OR=2.85,95%CI(1.66,4.87)],weight[SMD=-0.34,95%CI(-0.59,-0.09)],preoperative temperature[MD=-0.26,95%CI(-0.36,-0.15)],anesthesia duration[SMD=0.55,95%CI(0.24,0.85)],ASA grade≥Ⅲ[OR=2.16,95%CI(1.33,3.51)],surgery duration[SMD=0.41,95%CI(0.11,0.71)],surgery duration≥2 h[OR=2.07,95%CI(1.52,2.80)],intraoperative infusion volume[SMD=0.52,95%CI(0.31,0.73)],intraoperative blood loss[SMD=0.45,95%CI(0.17,0.73)],intraoperative blood transfusion[OR=3.18,95%CI(2.29,4.43)],intraoperative blood transfusion volume[SMD=0.66,95%CI(0.29,1.03)],and emergency surgery[OR=1.47,95%CI(1.05,2.04)]were significant influencing factors of IOH in pediatric patients.Conclusion The incidence of IOH in children is relatively high with many influencing factors.The screening ability of the surgical team should be improved to identify high-risk groups early,and collaborate with multiple disciplines to formulate individualized nursing interventions to prevent the occurrence of IOH and ensure the safety of children during surgery.
Efficacy and safety of Allium covered metallic ureteral stent placement for maintenance treatment of ureteral stricture caused by idiopathic retroperitoneal fibrosisAbstract:Objective To evaluate the safety and efficacy of Allium covered metallic ureteral stent placement as a maintenance therapeutic strategy for ureteral stricture secondary to idiopathic retroperitoneal fibrosis(iRPF)through a multicenter retrospective observational study.Methods A retrospective analysis was conducted on clinical data of patients diagnosed with iRPF-induced ureteral stricture,treated at Peking University People's Hospital,Aerospace Center Hospital,and Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine between June 2020 and June 2024.All patients underwent Allium covered metallic ureteral stent implantation.Regular postoperative follow-ups were performed to assess technical success rates,stent patency rates,renal function(serum creatinine levels),improvement in the degree of hydronephrosis,and associated complications.Results In this study,a total of 45 stents were implanted in 27 patients(40 renal units),achieving a technical success rate of 100%.The median follow-up duration was 44 months.At the final follow-up,the median serum creatinine level decreased significantly from 121.8 μmol/L preoperatively to 100.0 μmol/L(P<0.001).The median renal pelvic width also exhibited a significant reduction from 1.7 cm preoperatively to 1.0 cm(P<0.001).During the follow-up period,the primary complication was stent migration(1 case,3.7%),which was successfully managed endoscopically.The primary patency rate was 97.5%,and the secondary patency rate following re-intervention was 100%.Conclusion The implantation of Allium covered metallic ureteral stents constitutes a safe and effective minimally invasive therapeutic modality for managing ureteral stricture caused by iRPF.It significantly improves renal function,relieves obstruction,and demonstrates favorable long-term patency outcomes.Stent migration represents the principal complication;however,its incidence is low and it can be successfully addressed through endoscopic intervention.
Research progress on the correlation between PICC insertion site and catheterized pericardial effusion in premature infantsAbstract:Peripherally inserted central catheter(PICC)is safe,stable and durable,and has been widely used in neonatal intensive care units in recent years.However,there are still risks associated with thrombosis and arrhythmia in the process of application.PICC-associated pericardial effusion(PICC-PE)is characterized by a low incidence rate but high mortality rate,and it represents one of the most serious complications of central venous catheters in neonates.The stability of the PICC tip varies among different insertion sites in preterm infants,and the incidence of infection and other related complications also varies.This article describes the latest progress regarding the correlation between PICC insertion sites and PICC-PE in preterm infants,including its pathogenesis,influencing factors,prevention and care strategies.The aim is to provide a reference for the subsequent development of an individualized PE risk prediction model,thereby reducing the risk of this complication and improving the clinical safety of PICC.
Association with PM2.5 exposure and metabolic syndrome:a cross-sectional study in rural areas in three provinces of ChinaAbstract:Objective To evaluate the association between indoor PM2.5 exposure and metabolic syndrome(MetS)in rural areas of three provinces in China.Methods This cross-sectional study was conducted across 240 villages in three Chinese provinces.Participants aged≥30 years were randomly selected through gender-and age-stratified sampling in each village.Sociodemographic characteristics,lifestyle habits,medical history,and residential environment features were collected via standardized questionnaires.The prevalence of MetS was assessed based on blood pressure,waist circumference,and fasting venous blood biochemical indices.Indoor PM2.5 concentrations were assessed using PM2.5 monitoring devices that provided continuous 24-hour monitoring over seven days.Multivariable Logistic regression models were constructed to analyze the relationship between PM2.5 exposure levels and MetS risk,with dose-response relationships explored using restricted cubic spline models.Subgroup analysis and sensitivity analysis were also conducted simultaneously.Results A total of 958 participants were included in the study,with a prevalence of MetS of 54.91%.After adjusting for confounders,participants exposed to the highest levels of PM2.5 exhibited significantly elevated MetS risk compared to the lowest exposure[OR=1.62,95%CI(1.16,2.26)].Subgroup analyses revealed heightened susceptibility to high PM2.5 exposure among females[OR=2.28,95%CI(1.39,3.75)],individuals aged 50~<60 years[OR=2.08,95%CI(1.14,3.80)],low level of education[OR=1.99,95%CI(1.33,3.52)],low level of income[OR=2.26,95%CI(1.20,4.25)],exposure to household second-hand smoke[OR=1.58,95%CI(1.10,2.27)],those cooking for 1~2 h/d[OR=1.78,95%CI(1.12,2.82)]and individuals engaged in light physical labor[OR=1.55,95%CI(1.07,2.26)].Sensitivity analyses confirmed the robustness of these findings.Conclusion Elevated indoor PM2.5 exposure is associated with increased MetS risk in rural China,highlighting the need for targeted interventions to address household air pollution and improve indoor air quality.
Analysis of the disease burden of neonatal jaundice in China from 1990 to 2021Abstract:Objective This study analyzed the burden and changing trends of neonatal jaundice(NNJ)in China from 1990 to 2021.Methods Data on the burden of NNJ in China from 1990 to 2021 were collected from the 2021 Global Burden of Disease Study database.Descriptive analysis was performed using indicators such as prevalence,mortality rate,and disability-adjusted life years(DALY)rate.Joinpoint software was used to calculate the average annual percentage change(AAPC)and 95%confidence interval(CI)of the prevalence,mortality rate,and DALY rate,to analyze the temporal trends.Results In 2021,the prevalence,mortality rate,and DALY rate of NNJ in China were 60.17 per 100,000 people,42.06 per 100,000 people,and 3,804.15 per 100,000 person-years.From 1990 to 2021,the prevalence,mortality rate,and DALY rate of NNJ in China all showed a downward trend,with AAPCs of[-1.57%,95%CI(-1.62%,-1.53%)],[-7.01%,95%CI(-7.26%,-6.77%)],and[-7.00%,95%CI(-7.24%,-6.76%)],respectively.In 2021,male infants had higher NNJ prevalence,mortality rate,and DALY rate than female infants;in terms of age,infants aged 0~6 days had higher mortality and DALY rates than infants aged 7~27 days,but there was no significant difference in prevalence.There were significant global inequalities in the burden of NNJ across regions with different Socio-demographic Index(SDI)levels.Low-SDI regions bear the heaviest burden and show the slowest improvement,while China is at an intermediate level.Conclusion The prevalence,mortality rate,and DALY rate of NNJ in China showed a downward trend.The disease burden was higher in male infants than in female infants,and higher in newborns aged 0~6 days than in newborns aged 7~27 days.
Research on the awareness and needs of scientific and technological achievements transformation in tertiary public hospitals in ShanghaiAbstract:Objective To assess the awareness and needs of healthcare professionals and researchers in tertiary public hospitals regarding technology transfer.Methods A self-administered electronic questionnaire on technology transfer awareness and demand was distributed to staffacross seven tertiary public hospitals in Shanghai.Descriptive analysis and multivariate Logistic regression were employed for data processing.Results Among 2,101 valid responses collected,results revealed generally low awareness of technology transfer among hospital staff,with suboptimal research output quality and poor market alignment of most innovations.Different genders,occupations and patent applications experience were the influencing factors of technology transformation awareness levels(P<0.05).The main factors restricting the transformation of scientific and technological achievements are the lack of service capabilities in third-party technology transfer agencies,a shortage of time and energy,and the lack of innovation,advancement,and maturity in the outcomes.In promoting the transformation of scientific and technological achievements,medical staffand researchers have the strongest demand for financial support(83.4%),followed by the provision of professional technology transfer services(82.8%),specialized training(79.2%),and the establishment of platforms for medical-engineering and medical-enterprise cooperation(79.2%).Conclusion Hospital administrations should implement regular technology transfer training programs,cultivate specialized management personnel,establish institutional commercialization funds,and develop end-to-end management mechanisms to systematically enhance technology transfer efficiency.
Application and research progress of nanomaterials in ischemic stroke therapyAbstract:Ischemic stroke(IS)is a leading cause of death and disability worldwide,with complex pathophysiological mechanisms posing significant challenges to clinical treatment.In recent years,nanotechnology has shown broad prospects in the treatment of IS due to its excellent biocompatibility,high specific surface area,and outstanding drug loading capacity.This review systematically summarizes the progress of nanomaterials in IS treatment,focusing on their functional characteristics,targeting strategies,and controlled drug release technologies,aiming to optimize the design and functionality of nanomaterials to facilitate their transition from laboratory studies to clinical applications.
Construction and validation of a prediction model for postmenopausal asymptomatic endometrial thickeningAbstract:Objective To analyze the risk factors of asymptomatic endometrial thickening after menopause and construct a prediction model.Methods A retrospective study was conducted on postmenopausal women who visited the Department of Gynecology,the Zhangjiagang First People's Hospital from March 2022 to February 2024.Patients were divided into a test set and a validation set at a ratio of 7∶3.Patients in the test set were further divided into an endometrial thickening group(endometrial thickness≥5 mm)and a normal group(endometrial thickness<5 mm)based on ultrasound results.Multivariate Logistic regression analysis was performed to analyze the risk factors for asymptomatic endometrial thickening in postmenopausal women,and a predictive model was constructed.The predictive efficacy of the model was evaluated using the receiver operating characteristic(ROC)curve,area under the curve(AUC),calibration curve,and decision curve.Results A total of 410 patients were enrolled,with 287 in the training set and 123 in the validation set.Multivariate Logistic regression analysis revealed that high BMI[OR=2.71,95%CI(1.30,5.67)],longer duration of menopause[OR=7.17,95%CI(3.27,15.75)],presence of hypertension[OR=3.05,95%CI(1.43,6.50)],elevated fasting blood glucose level[OR=3.10,95%CI(2.23,4.32)],and high estradiol level[OR=1.22,95%CI(1.11,1.35)]were independent risk factors for asymptomatic endometrial thickening in postmenopausal women.The calibration curves for both the training and validation sets demonstrated high consistency with the ideal curve and exhibited significant net benefit.The AUC were all 0.88 for the training and validation sets,respectively.Conclusion BMI,menopausal years,hypertension,fasting blood glucose,and estradiol are all independent risk factors for asymptomatic endometrial thickening after menopause.The prediction model constructed based on these factors shows a high level of predictive and clinical decision-making effectiveness,which helps to evaluate the risk of asymptomatic endometrial thickening after menopause and guide the development of targeted intervention strategies accordingly.
Global disease burden of severe periodontitis from 1990 to 2021Abstract:Objective To analyze the global disease burden and its changing trend of severe periodontitis from 1990 to 2021.Methods Data from the Global Burden of Disease Study 2021(GBD 2021)database were obtained to analyze the incidence,prevalence,year lived with disability(YLD)of severe periodontitis by regional,socio-demographic index(SDI),age and sex group from 1990 to 2021.The change of age-standardized rate was estimated by calculating the estimated annual percentage change.Results In 2021,the global age-standardized incidence rate(ASIR)of severe periodontitis was 1,069.44 per 100,000,the age-standardized prevalence rate(ASPR)was 12,498.3 per 100,000,and the age-standardized YLD rate(ASYR)was 80.89 per 100,000 person years.From 1990 to 2021,there was no significant changing trend in the global ASIR[EAPC=0.03%,95%CI(-0.04%,0.09%)],ASPR[EAPC=0.08%,95%CI(-0.02%,0.18%)],and ASYR[EAPC=0.08%,95%CI(-0.02%,0.17%)]of severe periodontitis.The relatively high ASIR,ASPR,and ASYR were recorded in South-East Asia Region,Eastern Mediterranean Region,and Region of the Americas in 2021,respectively.ASIR,ASPR,and ASYR for severe periodontitis were higher in regions with lower SDI.In 2021,the global incidence,prevalence,and YLD rate of severe periodontitis increased with age in the 15-54 age group.The incidence rate was higher in men than in women in the 15-49 age group,and the prevalence and YLD rate were higher in men across all age groups.Conclusion The global burden of severe periodontitis varies across geographic regions,levels of socioeconomic development,sex and age.In the future,appropriate prevention and treatment strategies should be developed for low-income areas,young men and the elderly.
Association of sleep duration and sleep quality with metabolic associated fatty liver disease among Wuhan subway workersAbstract:Objective To investigate the association of sleep duration and sleep quality with metabolic associated fatty liver disease(MAFLD)among subway workers.Methods The study was a cross-sectional study,which was conducted based on workers from the Wuhan Metro Group Co.,Ltd..Sleep duration and sleep quality were obtained by the Pittsburgh Sleep Quality Index(PSQI).Logistic regression model was used to investigate the association of sleep duration and sleep quality with MAFLD.Mediation analysis was employed to evaluate the potential mediation effect of BMI on the association between sleep duration and MAFLD.Results A total of 8,650 subjects were enrolled,including 2,420 cases in the MAFLD group and 6,230 cases in the non-MAFLD group,resulting in MAFLD prevalence rate of 27.98%.Compared with participants with a sleep duration of 7~<8 h,those slept for 6~<7 h had a 19%increased risk of MAFLD[OR=1.19,95%CI(1.02,1.04)],and those slept for≥9 h had a 19%decreased risk of MAFLD[OR=0.81,95%CI(0.69,0.95)]after adjusted for multiple covariates.For each 1-hour increase in sleep duration,the risk of MAFLD decreased by 8%[OR=0.92,95%CI(0.88,0.96)].No significant association was observed between sleep quality and MAFLD.Restricted cubic spline analyses did not find a significant nonlinear association of sleep duration and sleep quality with MAFLD.Subgroup analyses showed that the association between sleep duration and MAFLD was more pronounced among non-current drinkers,a significant interaction was found between sleep duration and drinking status(Pinteraction=0.009).Mediation analysis indicated that the average causal mediation effect of BMI in the association between sleep duration and MAFLD was-0.04,with a mediation proportion of 93.8%.Conclusion Among subway employees,compared to those who sleep 7~<8 h,a sleep duration of 6~<7 h was associated with a higher risk of MAFLD prevalence,while≥9 h was associated with a lower risk.BMI mediated the association between sleep duration and MAFLD.There was no significant association between sleep quality and MAFLD.
The pathogenesis model research of alopecia areata:from animal experiments to mathematical prediction modelsAbstract:Alopecia areata(AA)is a common clinical hair disorder characterized by sudden,round or oval-shaped non-scarring alopecic patches.In severe cases,it may involve the entire scalp,nails,and eyebrows.Animal models play an indispensable role in exploring the pathogenesis of AA and developing therapeutic strategies.This article systematically reviews 13 types of AA animal models and 2 categories of AA mathematical models.It comprehensively analyzes spontaneous models(such as C3H/HeJ mice,DEBR rats and B6.KM-AA mice),induced models(including skin grafts,lymph node cell injections,and drug-induced methods),humanized models,and genetically engineered models.Additionally,it evaluates the current applications of both mechanism-driven and data-driven mathematical models and provides a reference for future research and the construction of ideal experimental systems in AA research.
A Meta-analysis of Seminar teaching versus lecture-based learning for surgery teaching in clinical medical studentsAbstract:Objective To systematically evaluate the effect of Seminar teaching versus lecture-based learning(LBL)in surgical clinical teaching for medical students.Methods A literature search was conducted to identify the studies on the teaching method of Seminar versus LBL in surgery clinical teaching published before November 2024.The quality of the literature was evaluated according to the Cochrane evaluation criteria.RevMan 5.4 software was used for Meta-analysis.Results A total of 17 studies involving 1,194 participants were included in this study.Meta-analysis indicated that the Seminar group was more efficient than the LBL group in basic theory scores[SMD=6.13,95%CI(5.16,7.11)],clinical practice skills score[SMD=6.91,95%CI(5.34,8.48)],overall satisfaction[OR=4.35,95%CI(2.73,6.93)],increasing the interesting of learning[SMD=9.81,95%CI(7.10,12.52)],improving clinical practice ability[OR=4.80,95%CI(2.09,11.05)],mastering the knowledge[OR=4.05,95%CI(1.81,9.06)]and communication skills[SMD=4.45,95%CI(1.91,6.98)].Conclusion The Seminar teaching method can improve students'performance in both basic theory and clinical practice skills.It is beneficial for enhancing students'interest in learning,clinical application abilities,and communication skills.Additionally,it helps with knowledge acquisition and improves teaching satisfaction.
Meta-analysis of the efficacy and safety of long-term low-dose clarithromycin in the treatment of chronic rhinosinusitisAbstract:Objective To systematically evaluate the efficacy and safety of long-term low-dose clarithromycin in the treatment of chronic rhinosinusitis(CRS).Methods Randomized controlled trials(RCTs)of low-dose clarithromycin for CRS were collected from CNKI,WanFang,VIP,PubMed,Embase,Web of Science,The Cochrane Library databases,and the clinical trial registration platform from database inception to January 21,2025.Meta-analysis was performed by RevMan 5.4.1 software.Results A total of 31 RCTs were included.The treatment group showed superior results compared to the control group in terms of overall effectiveness[OR=4.31,95%CI(3.30,5.63)],SNOT-20 scores[MD=-1.64,95%CI(-3.20,-0.08)],Lund-Kennedy scores[MD=-1.17,95%CI(-1.55,-0.79)],Lund-Mackay score[MD=-1.38,95%CI(-1.84,-0.92)],and VAS scores[MD=-1.11,95%CI(-1.50,-0.72)],with statistically significant differences(P<0.05).However,there was no statistically significant difference between the two groups in terms of adverse event rate[OR=1.12,95%CI(0.74,1.70)]and recurrence rate[OR=0.73,95%CI(0.39,1.37)](P>0.05).Conclusion Current evidence suggests that long-term low-dose clarithromycin treatment for CRS has good clinical efficacy,though further validation through high-quality,large-scale RCTs remains necessary.
The effect of multidisciplinary team intervention on nursing care for patients with colorectal cancer:a Meta-analysisAbstract:Objective To systematically evaluate the application efficacy of multidisciplinary team(MDT)nursing compared with conventional nursing in postoperative patients with colorectal cancer(CRC).Methods PubMed,Embase,Web of Science,CNKI,WanFang Data,and VIP Database were electronically searched to collect randomized controlled trials(RCTs)comparing the nursing efficacy of MDT nursing and conventional nursing in postoperative patients with CRC.The search period was from the inception to November 1,2024.Two researchers independently conducted literature screening,data extraction,and bias risk assessment of the included studies.Meta-analysis was performed using RevMan 5.3 software.Results A total of 19 RCTs involving 1,612 patients were finally included.The results of Meta-analysis showed that,compared with the conventional nursing group,patients in the MDT group exhibited increased satisfaction with care and self-care capability scores,alongside reduced depression and anxiety scores.Perioperative indicators,including time to ambulation,length of hospital stay,duration of intraoperative haemorrhage,wound healing time,and time to first flatus,were all shortened,with a concurrent decrease in complication rates.All differences were statistically significant.Conclusion The implementation of MDT collaborative nursing intervention in postoperative patients with CRC can effectively improve patients'negative emotions,enhance nursing satisfaction,reduce the incidence of complications,and play a role in optimizing perioperative indicators,demonstrating significant clinical promotion value.
Establishment and validation of a prognostic survival prediction model for colorectal cancer patients with liver metastasis:based on SEER databaseAbstract:Objective To explore the relevant factors affecting patients with liver metastasis of colorectal cancer,and to establish a nomogram prediction model to provide evidence for individualized diagnosis and treatment.Methods To retrospectively analyze the case information of colorectal cancer liver metastasis patients in the SEER database from January 2010 to December 2019.Lasso regression and Cox regression model were used to analyze independent risk factors affecting patient prognosis and establish the prediction nomogram model.The discrimination and consistency of the model were evaluated based on the receiver operating characteristic curve and its area under the curve(AUC),calibration curve and decision curve analysis(DCA).Results A total of 5,121 patients with colorectal cancer liver metastases were included,comprising 3,631 cases in the modelling group and 1,490 cases in the validation group.Undifferentiated tumour grade[HR=4.29,95%CI(2.74,6.71)],positive neural infiltration tumour[HR=1.32,95%CI(1.16,1.49)],lymph node ratio(LNR)>0.6[HR=1.70,95%CI(1.28,2.28)]were risk factors for patients with colorectal cancer liver metastases.Age>60 years old[HR=0.85,95%CI(0.75,0.95)]and surgery for metastatic lesions[HR=0.85,95%CI(0.75,0.96)]were protective factors for the prognosis of colorectal cancer patients with liver metastasis.A survival prediction model was established by nomogram.The AUC values for the modeling group and validation group were 0.722 and 0.732,respectively.The calibration curve approximates the ideal curve,and the DCA curve demonstrates that the model possesses good clinical validity.Conclusion This study is based on the SEER database to construct a survival prediction nomogram model,which has high discrimination,consistency,and clinical effectiveness,demonstrating the practicality of the prediction model in clinical practice.
Perioperative nursing of a patient with giant hemangioma of upper limb complicated with elevated D-dimerAbstract:Hemangioma is a benign tumor formed by abnormal proliferation of vascular endothelial cells.Hemangiomas with a diameter greater than 10 cm are defined as giant hemangiomas,with an incidence rate of about 0.40%.Once trauma or spontaneous rupture occurs,it can lead to massive bleeding.This article retrospectively reports a case of a patient with a huge upper limb hemangioma accompanied by elevated D-dimer levels.After periodic perioperative nursing model,the patient was successfully discharged.During the follow-up period,the affected limb exhibited good function and no symptoms related to thrombosis,suggesting that periodic perioperative nursing model has strong clinical applicability.
