Application of health-promoting behavior education based on Internet platform in the daily life of liver cancer patients outside the hospital

Abstract:Objective:To explore the application of health-promoting behavior education based on Internet platforms in the daily life of liver cancer patients outside the hospital.Methods:A total of 105 liver cancer patients who visited a tertiary Grade A hospital in Shanxi province from October 2024 to March 2025 were selected as the study subjects.They were randomly divided into a control group and an test group by using a random number table method.The control group was received conventional nursing care and outpatient guidance.The test group received health-promoting behavior education based on internet platform in addition to the control group's interventions.The self-management ability,general self-efficacy,quality of life,cancer-related fatigue,physical activity,and sleep duration were compared between the two groups.Results:After the intervention,the general self-efficacy,self-management ability,quality of life,cancer-related fatigue,and physical activity of the test group were all better than those of the control group(P<0.05).Conclusions:Health-promotion behavior education based on Internet platform could improve the self-management ability,general self-efficacy,quality of life,and physical activity of liver cancer patients in their daily life outside hospital,and alleviate cancer-related fatigue of patients.

Development of a long-term care needs assessment scale for patients with systemic lupus erythematosus and its reliability and validity test

Abstract:Objective:To develop a long-term care needs assessment scale for patients with systemic lupus erythematosus(SLE)and test its reliability and validity.Methods:Based on Maslow's hierarchy of needs theory,the initial draft of the scale was determined through literature review,semi-structured interviews,expert consultation,and a preliminary survey.Using random sampling,a total of 512 systemic lupus erythematosus patients from a tertiary grade A hospital in Wuhan city were selected as the research subjects.The preliminary draft scale was used for the investigation to test its reliability and validity level.Results:The long-term care needs assessment scale for patients with systemic lupus erythematosus consisted of 5 dimensions(symptom management needs,psychological needs,healthcare service needs,information needs,and social support needs)and 27 items.The overall Cronbach's α coefficient of the scale was 0.935,with subscale coefficients ranging from 0.707 to 0.888.The Guttman split-half reliability was 0.783,with subscale split-half reliabilities ranging from 0.721 to 0.885.The scale-level content validity index was 0.888,the item-level content validity index were ranged from 0.720 to 0.960.Exploratory factor analysis extracted five common factors,with a cumulative variance contribution rate of 63.674%.Confirmatory factor analysis results showed that all model fitting indicators were acceptable.Conclusions:The long-term care needs assessment scale for patients with systemic lupus erythematosus developed in this study comprehensively covered the content of long-term care needs for systemic lupus erythematosus patients,with good reliability and validity,and could be used as an assessment tool for long-term care needs of patients with systemic lupus erythematosus.

Influence of dietary diversity on the intrinsic capacity of the elderly

Abstract:Objective:To investigate the prevalence of intrinsic capacity decline among older adults in China and to explore the associated factors,with a specific focus on the impact of diet on their intrinsic capacity.Methods:Based on national data from the Chinese Longitudinal Healthy Longevity Survey(CLHLS),Logistic regression analysis was employed to examine the effects of food variety and dietary diversity on the intrinsic capacity of older adults.Confounding factors were controlled for in the model construction.Results:A total of 3 338 older adults aged 65 years and above were included in the study.The detection rate of intrinsic capacity decline among Chinese the elderly was 67.2%.Demographic characteristics(age,living arrangement,marital status,family income,educational level,medical insurance type),lifestyle factors(smoking and drinking),and health-related indicators(body mass index,number of comorbidities,and number of oral medications taken)were significantly associated with the intrinsic capacity of older adults(all P<0.05).After adjusting for confounding factors,Logistic regression analysis revealed that fish,garlic,nuts and dietary diversity were significantly correlated with the risk of intrinsic capacity decline in the elderly(all P<0.05).Conclusion:Increased frequency of fish,garlic,nuts consumption and high dietary diversity could effectively reduce the risk of intrinsic capacity decline in the elderly.

Research on the coupling coordination and spatial relationship between the allocation of nursing human resources allocation and economic development level in China

Abstract:Objective:To explore the coupling coordination relationship and spatial distribution status between the nursing human resources allocation and the economic development level in China,and to provide reference basis for promoting the coordinated development of nursing human resource allocation and economic level.Methods:The entropy method and coupling coordination degree model were used to study the allocation of nursing human resources,economic development level and coupling coordination degree in China.The spatial autocorrelation method was used to study the spatial layout and changing trend of the coupling coordination degree in 2019 and 2023.Results:There were significant regional differences in the allocation of nursing human resources and economic development level in China.The average coupling coordination degree of the two systems was in the stage of approaching imbalance.There was a spatial aggregation phenomenon in the coupling coordination degree,and the Moran's I index increased over time,with the degree of aggregation continuously deepening.The local aggregation presented a spatial pattern of"high in the east and low in the west".Conclusions:The coupling and coordination degree of nursing human resources allocation and economic development in our country is not high and there are obvious regional differences.Therefore,each province should scientifically plan the plan of nursing human resources based on its own economic situation and regional medical needs,and promote the coordinated development of economic level and the nursing human resources allocation in a coordinated manner.

Systematic review of post-stroke cognitive impairment assessment tools based on COSMIN guidelines

Abstract:Objective:Based on the consensus-based standards for the selection of health measurement instruments(COSMIN)guidelines,this study systematically evaluated the methodological quality and measurement properties of assessment tools for post-stroke cognitive impairment(PSCI),aiming to identify high-quality evaluation tools.Methods:A computerized search was conducted in PubMed,Web of Science,MedLine,CINAHL,EMbase,Cochrane Library,Scopus,CNKI,WanFang Database,VIP,and the China Biology Medicine(CBM)database for literature related to the psychometric evaluation of PSCI assessment tools.The retrieval period spanned from the establishment of the database to March 6,2025.Two researchers independently screened the literature and extracted data.Methodological risk of bias and quality of measurement properties were assessed using the COSMIN guidelines.The GRADE evidence grading system was applied to classify evidence levels,ultimately formulating recommendation levels.Results:A total of 22 studies were included,covering 10 PSCI assessment tools:the Oxford Cognitive Screen(OCS),Mini-Mental State Examination(MMSE),Cognitive Assessment Scale for Post-Stroke Patients(CASP),Birmingham Cognitive Screen(BCoS),Montreal Cognitive Assessment(MoCA),National Institute of Neurological Disorders and Stroke-Canadian Stroke Network(NINDS-CSN),Brief Assessment of Impaired Cognition(BASIC),Mild Vascular Cognitive Impairment(MVCI)assessment tool,Barrow Neurological Institute Screen for Higher Cerebral Functions(BNIS)and Telephone Interview for Cognitive Status-Modified(TICS-M).MMSE received grade A recommendation,while the remaining 9 tools were classified as grade B recommendations.Conclusions:The MMSE was classified as a grade A recommendation and could be temporarily recommended until better evidence emerges.However,its methodological quality requires improvement,and its measurement properties need further validation.

Machine learning model predicts risk of postoperative delirium in total hip replacement patients

Abstract:Objective:To predict the risk of postoperative delirium in patients undergoing total hip arthroplasty using machine learning models.Methods:A total of 622 patients who underwent total hip arthroplasty in Ganzhou People's Hospital were selected as research subjects from January 2020 to December 2024.The Confusion Assessment Method(CAM)was used to assess postoperative delirium.The Boruta algorithm was employed to screen for important feature variables associated with postoperative delirium risk.Patients were randomly divided into training set(442 cases)and testing set(180 cases)at 7∶3 ratio.Nine machine learning models were constructed,trained,and validated using ten-fold cross-validation.The area under the curve(AUC)of receiver operator characteristic was used to evaluate model performance and identify the best machine learning model.Decision curve analysis was used to assess the clinical utility of the model.The SHapley additive explanations(SHAP)method,including bar plots,summary plots,dependence plots,and force plots,was used to interpret and visualize the machine learning models.Results:The incidence of postoperative delirium among the 622 patients undergoing total hip arthroplasty was 30.87%.The Boruta algorithm identified nine important postoperative delirium risk feature variables.Based on the feature importance scores(Z-values),the ranking from highest to lowest was C-reactive protein(CRP),anesthesia duration,albumin(ALB),age,total bilirubin(TB),blood glucose,intraoperative blood loss(IBL),history of diabetes,and cerebrovascular disease(CSD).Multivariate Logistic regression analysis showed that age,ALB,TB,blood glucose,CRP,and anesthesia duration were independent influencing factors for postoperative delirium in patients undergoing total hip arthroplasty(all P<0.05).The XGBoost model demonstrated excellent performance in both the training and test sets,exhibiting the strongest robustness and predictive efficacy for estimating the risk of postoperative delirium in patients undergoing total hip arthroplasty.Interpretation and visualization of the XGBoost model using SHAP revealed that the model could predict postoperative delirium risk in patients undergoing total hip arthroplasty with high accuracy.Conclusions:Age,ALB,TB,blood glucose,CRP,anesthesia duration are independent influencing factors for postoperative delirium in patients undergoing total hip arthroplasty.The XGBoost model demonstrated high predictive value for postoperative delirium in patients undergoing total hip arthroplasty.

Study on the relationship between the neutrophil-to-lymphocyte ratio and postoperative pneumonia in elderly patients with hip fracture

Abstract:Objective:To investigate the relationship between the neutrophil-to-lymphocyte ratio(NLR)and postoperative pneumonia in elderly patients with hip fracture.Methods:A total of 1 444 elderly patients with hip fracture admitted to Shenzhen Second People's Hospital from January 2012 to December 2021 were selected as the study subjects.Clinical data were collected.Univariate and multivariate Logistic regression analyses were used to explore influencing factors of postoperative pneumonia in elderly patients with hip fracture.A generalized additive model was employed for smooth curve fitting to examine the relationship between NLR and postoperative pneumonia in elderly hip fracture patients.Results:The incidence of postoperative pneumonia in elderly hip fracture patients was 6.30%(91/1 444).The NLR was significantly higher in the pneumonia group compared with the non-pneumonia group(P<0.001).Multivariate Logistic regression analysis revealed that NLR was an important influencing factor of postoperative pneumonia in elderly patients with hip fracture(P=0.001).Curve fitting results indicated a linear correlation between NLR and the occurrence of postoperative pneumonia(P<0.05).Conclusions:NLR has predictive value for the occurrence of postoperative pneumonia in elderly patients with hip fracture.

Construction and evaluation of nursing knowledge question answering system based on deep learning

Abstract:Objective:To construct a nursing knowledge question answering system based on deep learning technology and to evaluate its application effectiveness in clinical nursing.Methods:A knowledge base in the nursing domain was constructed using deep learning methods.The pre-trained language models was taken such as Bidirectional encoder representations from transformers(BERT)as the core to develop the nursing knowledge question answering system.The technical performance and clinical application effects of the system were evaluated.Results:The constructed nursing knowledge question answering system achieved an accuracy of 86.8%on nursing professional questions.The average response time was 1.2 seconds.Clinical application results showed that the system effectively reduced the workload of nurses.The per shift time spent for patients'answering common questions in nurses was decreased by 93.4%.The satisfaction score of the nurse was 4.3.Conclusions:The nursing knowledge question answering system based on deep learning can effectively enhance nursing work efficiency and provide reliable knowledge support for nurses.

Development of the perioperative missed nursing care scale and its reliability and validity test

Abstract:Objective:To develop the perioperative missed nursing care scale and to test its reliability and validity.Methods:The initial version of the perioperative missed nursing care scale was developed through literature review and qualitative interviews.The scale was revised by Delphi expert consultations and a pre-survey.A total of 754 operating room nurses from 22 provinces,autonomous regions,and municipalities in China were selected as survey subjects.The reliability and validity of the scale were tested.Results:The questionnaire response rates for the two rounds of expert consultations were 100.0%and 94.4%,respectively.The expert authority coefficients were 0.911 and 0.935,respectively.Kendall's W coefficients for subscale A were 0.279 and 0.295(both P<0.001).Kendall's W coefficients for subscale B were 0.220 and 0.185(both P<0.001).Exploratory factor analysis extracted four common factors each for both subscale A and subscale B,with cumulative variance contribution rates of 76.41%and 75.71%,respectively.Confirmatory factor analysis results showed that all fit indices were within acceptable ranges.For subscale A,the average variance extracted values for the four dimensions ranged from 0.430 to 0.529,and composite reliability values ranged from 0.880 to 0.966.For subscale B,the average variance extracted values for the four dimensions ranged from 0.519 to 0.700,and composite reliability values ranged from 0.873 to 0.972.The item-level content validity index(I-CVI)for the perioperative missed nursing care scale ranged from 0.857 to 1.000.The scale-level content validity index(S-CVI)was 0.914.The overall Cronbach's α coefficient for the scale was 0.967.The Cronbach's α coefficients for the dimensions of subscale A ranged from 0.885 to 0.965.The Cronbach's α coefficients for the dimensions of subscale B ranged from 0.866 to 0.972.The overall split-half reliability was 0.924 for subscale A and 0.898 for subscale B.Conclusions:The perioperative missed nursing care scale demonstrates scientific rigor and practical utility.It could be used to assess missed nursing care items and their causes in the perioperative period.

Symptom transition patterns and influencing factors in patients with acute pancreatitis before and after discharge

Abstract:Objective:To analyze the symptom transition patterns and influencing factors in patients with acute pancreatitis(AP)before and after discharge.Methods:A total of 204 AP patients admitted to the First Affiliated Hospital of Naval Medical University were selected as study subjects by a convenience sampling method from October 2022 to October 2024.Data were collected using a general information questionnaire and Memorial Symptom Assessment Scale-Chinese Version.Latent transition analysis was performed to evaluate symptoms on the 1st to 7th day after admission and the 3rd month after discharge.Results:The symptom profiles of the 204 patients before and after discharge were both categorized into two groups,namely low symptom burden and high symptom burden.The probability of transitioning from the low symptom burden group before discharge to the high symptom burden group after discharge was 61.8%.The probability of transitioning from the high symptom burden group to the low symptom burden group was 52.9%.Logistic regression analysis showed that urban employee medical insurance(OR=0.076)and mild disease severity(OR=0.044)were associated with a decreased probability of transitioning from low to high symptom burden,while the monthly household income per capita<3 000 yuan(OR=2.648)increased this probability.Additionally,age<60 years(OR=2.074)increased the likelihood of transitioning from high to low symptom burden,while the monthly household income per capita<3 000 yuan(OR=0.286)decreased this probability.Conclusions:Symptom profiles among AP patients exhibit group heterogeneity before and after discharge.Healthcare professionals should implement individualized management strategies based on symptom transition patterns.

Latent profile analysis and influencing factors of anxiety of the existential meaninglessness in elderly patients with chronic diseases

Abstract:Objective:To analyze the categories and influencing factors of anxiety of the existential meaninglessness in elderly patients with chronic diseases.Methods:A total of 493 elderly patients with chronic diseases from six tertiary grade A hospitals in Hunan province were selected as research subjects by convenience sampling from July to September 2024.A questionnaire survey was conducted using the General Information Questionnaire,Anxiety Measure of the Existential Meaninglessness Scale,Chinese Self-Transcendence Scale,and Health empowerment scale for elderly patients with chronic disease.Latent profile analysis and multinomial Logistic regression were used to explore the categories and influencing factors of anxiety of the existential meaninglessness in elderly patients with chronic diseases.Results:Anxiety of the existential meaninglessness in elderly patients with chronic diseases could be divided into three categories,namely low existential meaninglessness anxiety group(48.48%),moderate existential meaninglessness anxiety mixed group(43.41%),and high existential meaninglessness anxiety group(8.11%).Logistic regression analysis showed that economic resources,monthly family income,attitude towards life,type of medical insurance,and health empowerment were influencing factors for the categories of anxiety of the existential meaninglessness in elderly patients with chronic diseases(all P<0.05).Conclusions:Anxiety of the existential meaninglessness in elderly patients with chronic diseases exhibits group heterogeneity.Targeted interventions based on different categories could be implemented in clinical nursing to reduce the anxiety of existential meaninglessness in elderly patients with chronic diseases.

Longitudinal study of core symptom clusters in postoperative breast cancer patients undergoing chemotherapy based on network analysis

Abstract:Objective:To construct a network model of symptom clusters and quality of life in postoperative breast cancer patients undergoing chemotherapy.To identify the core symptom clusters and the core domains of quality of life,and to determine the predictive relationships between them,thereby providing scientific guidance for improving patients'quality of life.Methods:A total of 260 breast cancer patients receiving chemotherapy in the oncology department of a tertiary grade A hospital in Yinchuan city were selected as the survey subjects using convenience sampling from May to December 2024.A longitudinal survey was conducted to track their symptoms and quality of life before,during,and after chemotherapy.Data were analyzed using exploratory factor analysis,partial correlation networks,and cross-lagged panel networks.Results:Exploratory factor analysis and partial correlation network analysis revealed four stable symptom clusters in postoperative breast cancer patients undergoing chemotherapy,namely the sickness behavior symptom cluster,the psychosocial symptom cluster,the body image symptom cluster,and the gastrointestinal symptom cluster.The centrality of fatigue symptoms remained the top five across all three periods(rs1=3.02,rs2=4.21,rs3=4.10).Cross-lagged panel network analysis further indicated that the body image symptom cluster was the core symptom cluster in postoperative breast cancer patients undergoing chemotherapy,and physical well-being was the core domain of quality of life.The body image symptom cluster at the previous time point positively predicted physical well-being at the subsequent time point across the three periods(β=1.24 and β=1.49,respectively).Conclusions:The core symptom cluster in postoperative breast cancer patients undergoing chemotherapy is the body image symptom cluster.Physical well-being is the core domain of quality of life.The body image symptom cluster positively predicts improvements in quality of life.Healthcare professionals should develop interventions based on core symptom clusters during treatment to reduce the transmission effect of the body image symptom cluster within the symptom network,indirectly promote the improvement of physical well-being,and thereby enhance quality of life.

Chinesization of Ankle Instability Instrument and its reliability and validity test

Abstract:Objective:To translate Ankle Instability Instrument(AII)into Chinese and to test its reliability and validity.Methods:The AII was translated and cross-culturally adapted.The Chinese version of AII was formed.A total of 1850 university students from six institutions in Henan province were surveyed by convenience sampling.The reliability and validity of the scale were tested.Results:The Chinese version of AII comprised 8 items divided into three dimensions,namely injury history and management,functional instability,and stair/complex scenario instability.The cumulative variance contribution rate was 66.20%.The three-factor model demonstrated a good fit.The composite reliability of the three dimensions ranged from 0.80 to 0.85.The average variance extracted ranged from 0.53 to 0.59.The square root of the average variance extracted for each dimension(0.73-0.77)was greater than its correlation coefficients with other dimensions(0.37-0.55).The heterotrait-monotrait ranged from 0.49 to 0.68.The total score and dimension scores of the Chinese AII were negatively correlated with scores on the Foot and Ankle Ability Measure and the Cumberland Ankle Instability Tool,and positively correlated with scores on the Visual Analogue Scale(all P<0.05).The Cronbach's α coefficient of the Chinese version of AII was 0.88,the split-half reliability was 0.86,and the retest reliability was 0.87(P<0.01).Conclusions:The Chinese version of AII demonstrates good reliability and validity.It could be used as a tool for measuring functional ankle instability.

Design and development of an AI-based smart care platform for patients with mild cognitive impairment

Abstract:Objective:To design and develop a smart care platform for patients with mild cognitive impairment(MCI)that integrates artificial intelligence(AI)technologies.Methods:A total of 554 MCI patients and their family caregivers were recruited from three community health service centers in Liuzhou city,Guangxi Zhuang Autonomous Region from January 2022 to June 2024.Semi-structured interviews were conducted to clarify the functional requirements of the platform.The platform was developed using speech recognition,data analysis,and personalized recommendation technologies,which integrated functions such as intelligent schedule management,health monitoring,cognitive training,and data sharing.Patients'care compliance,cognitive training completion rates,occurrence of forgetfulness events,nursing information documentation completeness,and incidents of wandering or sudden health events before and after using the smart care platform were evaluated.Caregivers'care burden,anxiety levels,and the burden of high care workload before and after using the platform were evaluated.Results:After using the platform,MCI patients showed higher care compliance,cognitive training completion rates,and nursing information record integrity,while experiencing lower incidence of memory lapse events,wandering or sudden health incidents compared to before platform use.Family caregivers demonstrated lower care burden scores,anxiety levels,and proportion reporting high care workload after platform use(P<0.001).Conclusions:The smart care platform can provide personalized and continuous intelligent care support for MCI patients.It contributes to improve care service quality and keep better maintenance of patients'functional levels.

Abstract:Objective:To translate the Iceland-Family Perceived Support Questionnaire(ICE-FPSQ),and to test its reliability and validity among family members of hospitalized children.Methods:Based on the Brislin translation model,the ICE-FPSQ was translated to form the Chinese version(ICE-FPSQ-CN).A total of 340 caregivers of hospitalized children in two tertiary grade A hospitals in Wuhan city were selected as participants by convenience sampling from July 2024 to January 2025.They were surveyed to test the ICE-FPSQ-CN reliability and validity.Results:The ICE-FPSQ-CN consisted of 14 items across two dimensions,namely emotional support and cognitive support.Both the item level content validity index(I-CVI)and scale level content validity index(S-CVI)were 1.000.Exploratory factor analysis extracted two common factors,with a cumulative variance contribution rate of 64.946%.Results from the modified confirmatory factor analysis showed that χ2/df=2.412,RMSEA=0.077,IFI=0.939,TLI=0.921,CFI=0.938,GFI=0.909,indicating a good model fit.The composite reliability for the emotional support and cognitive support dimensions were 0.899 and 0.794,respectively.Average variance extracted values were 0.502 and 0.444,respectively.The square roots of the average variance extracted were greater than the corresponding correlation coefficients.The overall Cronbach's α coefficient of the questionnaire was 0.934.The split-half reliability was 0.848.The test-retest reliability was 0.897.Conclusions:The ICE-FPSQ-CN demonstrates good reliability and validity.It can be used as an assessment tool for perceived family support in China.
