Effect of Repetitive Transcranial Magnetic Stimulation Combined with Stage-wise Rehabilitation Training on Cognitive Function and Self-care Ability Recovery in Middle-aged and Elderly Patients After Minimally Invasive Surgery for Hypertensive Intracerebral Hemorrhage
Abstract:Objective To explore the effect of repetitive transcranial magnetic stimulation (rTMS) combined with stage-wise rehabilitation training on the recovery of cognitive function and self-care ability in middle-aged and elderly patients after minimally invasive surgery for hypertensive intracerebral hemorrhage (HICH). Methods A total of 92 middle-aged and elderly HICH patients who underwent minimally invasive surgery at Suixi County Hospital from January 2020 to December 2023 were selected and randomly divided into a control group (46 cases) and an observation group (46 cases) according to the random number table method. Both groups received routine nursing interventions. The control group received stage-wise rehabilitation training on top of routine care, while the observation group received rTMS treatment on top of the control group's treatment. Both groups received continuous treatment for 6 weeks. The therapeutic effects of the two groups were compared. The NIHSS scores, cognitive function [MoCA scores, MMSE scores] before and after treatment were compared between the two groups. The upper limb motor function assessment scale (UL-FMA), lower limb motor function assessment scale (FMA-L), self-care ability [FIM scores, ADL scores] before and after treatment were also compared. Results After treatment, the total effective rate of the observation group was 91.30%, which was higher than that of the control group (71.74%), with a statistically significant difference (χ²=5.846, P<0.05). After treatment, the NIHSS score of the observation group [(10.75±1.27) vs. (13.42±1.86)] was lower than that of the control group, while the MoCA score [(21.67±3.11) vs. (16.67±2.38)], MMSE score [(20.81±3.67) vs. (18.24±3.15)] were higher than those of the control group, with statistically significant differences (t=8.040, 8.659, 3.604, P<0.05). The UL-FMA score [(38.44±4.46) vs. (32.57±3.73)], FMA-L score [(29.84±4.53) vs. (24.76±4.12)], FIM score [(93.25±9.32) vs. (85.12±8.67)], and ADL score [(78.31±6.95) vs. (67.39±5.75)] of the observation group were all higher than those of the control group, with statistically significant differences (t=6.848, 5.627, 4.332, 8.211, P<0.05). Conclusion The application of rTMS combined with stage-wise rehabilitation training in middle-aged and elderly HICH patients after minimally invasive surgery has a significant therapeutic effect, effectively improving the patient's cognitive function, motor ability, and neurological function, and enhancing their self-care ability and quality of life.
