Mechanisms of Wuzi Yuye Decoction in the treatment of diabetic oligoasthenozoospermia:Based on network pharmacology

Abstract:Objective The aim of this study is to systematically analyze the therapeutic mechanisms of Wuzi Yuye Decoction in diabetic oligoasthenozoospermia based on network pharmacology and animal experiments.Methods Active components and therapeutic targets of Wuzi Yuye Decoction were screened by integrating multiple databases,and their inter-section with diabetic oligoasthenozoospermia-related targets was obtained.Common targets were identified using the Venny platform,and protein-protein interaction(PPI)networks were constructed via the STRING database.And key targets were screened through topological analysis with Cytoscape.Gene Ontology(GO)functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway analyses were performed using the DAVID platform.A mouse model of diabetic oligoasthenozoospermia was established by high-fat diet combined with streptozotocin induction,followed by intervention with Wuzi Yuye Decoction.Body weight,testicular index,and sperm quality were assessed,while histopathological changes were evaluated by HE staining and transmission electron microscopy(TEM).In addition,ELISA,oxidative stress assays and Western blot were used to verify the changes in key pathway-related proteins.Results A total of 36 common targets were identified,including key nodes such as TNF,IL6 and IL1B.Enrichment analysis indicated that these targets were sig-nificantly involved in inflammatory responses,oxidative stress,autophagy-apoptosis regulation and reproduction-related pathways.Animal experiments confirmed that Wuzi Yuye Decoction markedly improved testicular mass and sperm motility in diabetic mice,reduced the levels of AGE,TNF-α,IL-6 and IL-1β in testicular tissues,alleviated oxidative stress damage,and restored autophagic flux and blood-testis barrier function by inhibiting the RAGE/NF-κB/mTOR signaling axis.Conclusion Wuzi Yuye Decoction improves the pathological progression of diabetic oligoasthenozoospermia by synergistically modulating multiple targets,including inflammation inhibition,antioxidant effects,autophagy regulation,and related pathways.

Application of mindfulness-based stress reduction on the patients treated with image fusion-guided prostate biopsy

Abstract:Objective:To evaluate the application effect of mindfulness-based stress reduction(MBSR)therapy on the patients treated with image fusion-guided transperineal prostate biopsy.Methods:A total of 160 patients who underwent image fusion-guided transperineal prostate biopsy in the Urology Department from April 2023 to April 2024 were included.Patients were randomly assigned to a control group and an observation group,with 80 cases in each group.The control group received routine care,while the observa-tion group received combined MBSR on the basis of routine care.The surgical indicators,pain levels,psychological states,nursing satisfaction,and postoperative complication rates of both groups were compared.Results:There was no statistically significant differ-ence in general personal information and clinical data between the two groups(P>0.05).The surgery duration,secondary fusion rate,and postoperative complication rate in the observation group were all lower than those in the control group([23.54±2.07]min vs[26.25±1.69]min,P<0.05;8.75%vs 22.50%,P=0.017;17%vs 29%,P=0.036),and nursing satisfaction was higher in the observation group than in the control group(77%vs 69%,P=0.025).The VAS scores biopsy(5.11±0.93 vs 6.27±1.32,P=0.041),discharge(0.74±0.67 vs 1.85±0.95,P=0.004),and scores of SDS(47.76±2.06 vs 50.46±2.07,P=0.009)and SAS(46.89±2.68 vs 49.75±2.83,P=0.031)in the observation group were all lower than those in the control group.Conclusion:The application of MBSR in image fusion-guided prostate biopsy can synergistically utilize the advantages of minimally in-vasive technology,significantly optimize surgical indicators,and improve patients' psychological experiences,which is worthy of clinical application and promotion.

Abstract:Multimodal precision nursing intervention enhances cognitive function and quality of life through multisensory collabo-rative stimulation by integrating multidimensional data of patients.As one of the most common complications after radical prostatecto-my,urinary incontinence seriously affects the quality of life of prostate cancer patients after surgery which has a negative impact on multidimensional health in the"physiological-psychological-social"aspects.This paper summarizes the clinical manifestations,influ-encing factors,and nursing interventions of urinary incontinence after radical prostatectomy,and focuses on the key points of nursing based on multimodal theory,in order to provide nursing strategies for improving urinary incontinence after prostate cancer surgery.

Impact of sutureless dorsal venous complex combined with robotic-assisted laparoscopic prostatectomy on sexual function in patients with prostatic cancer

Abstract:Objective:To evaluate the impact of sutureless approach to the dorsal venous complex(DVC)combined with ro-botic-assisted laparoscopic prostatectomy on sexual function of patients with prostatic cancer.Methods:This study included 114 prostate cancer patients who underwent robot-assisted laparoscopic radical prostatectomy at our hospital from January 2021 to January 2024.The patients were randomly divided into a control group(n=57)and an observation group(n=57).The control group re-ceived conventional"figure-of-eight"suture ligation of the dorsal venous complex(DVC),while the observation group underwent di-rect DVC transection using monopolar electrocautery scissors after increasing pneumoperitoneum pressure.Surgical duration,intraopera-tive blood loss,positive surgical margin rates,and positive apical margin rates were recorded.The continence rates and rates of morn-ing erections at 1,3 and 6 months after the operation were compared between groups.Sexual function was assessed pre-operatively and at 6 months post-operation by using the IIEF-5 and PEDT.Results:The operative time in the observation group was significantly lon-ger than that in the control group([115.71±19.42]min vs[103.42±12.78]min,P<0.05).While no significant differences were observed in intraoperative blood loss,positive surgical margin rate,or positive apical margin rate between the two groups(P>0.05).At 3 and 6 months after the operation,the observation group exhibited higher urinary continence rates and morning erection re-covery rates compared to the control group(P<0.05).Furthermore,at 6 months postoperatively,the observation group demonstrated significantly higher IIEF-5 scores and lower PEDT scores than those of control group(P<0.05).Conclusion:The use of a suture-less DVC technique in robotic-assisted laparoscopic radical prostatectomy protects the post-operative sexual function in patients.

Seminal plasma miR-26a-5p influences sperm DNA integrity by targeting and regulating the PTEN gene

Abstract:Objective:By analyzing the differential miRNA in seminal plasma between individuals with normal and abnormal sperm DNA fragmentation index(DFI),we aim to identify miRNA that may impact sperm DNA integrity and target genes,and attempt to analyze their potential mechanisms of action.Methods:A total of 161 study subjects were collected and divided into normal con-trol group,DFI-medium group and DFI-abnormal group based on the DFI detection values.Differential miRNA were identified through miRNA chip analysis.Through bioinformatics analysis and target gene prediction,miRNA related to DFI and specific target genes were identified.The relative expression levels of differential miRNA and target genes in each group were compared to explore the impact of their differential expression on DFI.Results:Through miRNA chip analysis,a total of 11 differential miRNA were detected.Bioin-formatics analysis suggested that miR-26a-5p may be associated with reduced sperm DNA integrity.And gene prediction indicated that PTEN was a specific target gene of miR-26a-5p.Compared to the normal control group,the relative expression levels of miR-26a-5p in both the DFI-medium group and the DFI-abnormal group showed a decrease,while the relative expression levels of PTEN showed an in-crease.The relative expression levels of miR-26a-5p in all groups were negatively correlated with DFI values,while the relative expres-sion levels of PTEN showed a positive correlation with DFI values in the DFI-medium group and the DFI-abnormal group.The AUC of miR-26a-5p in the DFI-medium group was 0.740(P<0.05),with a sensitivity of 73.6%and a specificity of 71.5%;the AUC of PTEN was 0.797(P<0.05),with a sensitivity of 76.5%and a specificity of 78.4%.In the DFI-abnormal group,the AUC of miR-26a-5p was 0.848(P<0.05),with a sensitivity of 81.3%and a specificity of 78.1%.While the AUC of PTEN was 0.763(P<0.05),with a sensitivity of 77.2%and a specificity of 80.2%.Conclusion:miR-26a-5p affects the integrity of sperm DNA by regulating the expression of PTEN negatively.The relative expression levels of seminal plasma miR-26a-5p and PTEN have good diag-nostic value for sperm DNA integrity damage,which can help in the etiological diagnosis and prognosis analysis of abnormal DFI.This provides a diagnostic and treatment approach for the study and diagnosis of DFI abnormalities without clear etiology.

Tonifying kidney and activating blood therapy for the treatment of diabetic erectile dysfunction:A systematic review and meta-analysis

Abstract:Objective:To systematically evaluate the clinical efficacy and safety of Tonifying kidney and activating blood thera-py for the treatment of diabetic mellitus erectile dysfunction.Methods:China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP,Chinese Biomedical Database(CBM),PubMed,Cochrane Library,Embase and Web of Science were searched from incep-tion until October 20th of 2024,for randomized controlled trials of Tonifying kidney and activating blood therapy for the treatment of dia-betic erectile dysfunction.Literature screening,quality evaluation,and data extraction were carried out in accordance with relevant standards.The software of RevMan5.4 was used for the analysis of publication bias.And meta-analysis was conducted to assess the im-pact of this therapy on IIEF-5,total effective rate,adverse reactions.The evidence levels according to the analysis results were evalua-ted.Results:Totally 19 RCTs were included,involving 1 612 patients.The result of meta-analysis indicated that Tonifying kidney and activating blood therapy had advantages on the improvement of IIEF-5 scores(MD=3.59,95%CI[2.14,5.03],P<0.01),total effective rate(OR=4.30,95%CI[3.29,5.32],P<0.000 01).However,there was no statistically significant difference in the inci-dence of adverse reactions(OR=0.98,95%CI[0.48,2.01],P=0.96)between the two groups.Conclusions:Tonifying kidney and activating blood therapy can improve the clinical efficacy and IIEF-5 score for the patients with diabetic erectile dysfunction.But considering the limited quantity of included studies,more high-quality studies still be needed to validate the therapeutic effect.

Influencing factors of lower urinary tract symptoms in patients after radical prostatectomy and nursing strategy

Abstract:Objective:This study aims to analyze the influence factors of lower urinary tract symptoms(LUTS)in patients re-ceiving radical prostatectomy for prostate cancer,and to explore effective nursing strategy in order to provide a theoretical basis for im-proving the postoperative quality of life of patients.Methods:A retrospective study was conducted on 103 elderly male patients who underwent radical prostatectomy for prostate cancer in the Department of Urology at General Hospital of Eastern Theater Command from August 2022 to August 2024.The patients were categorized into two groups based on whether LUTS occurred.Demographic and clinical characteristics,perioperative parameters,follow-up data,and participation in pelvic floor muscle training were analyzed to identify risk factors associated with postoperative LUTS.Results:The incidence of postoperative LUTS in the patients with LUTS before the oper-ation was significantly higher than that in the patients without LUTS before the operation(68.42%vs 32.61%,P=0.001).Addition-ally,the use of larger catheters(22F)was closely associated with an increased incidence of postoperative LUTS(P<0.01).Pelvic floor exercises demonstrated a significant protective effect,with patients who engaged in pelvic floor exercises exhibiting a lower inci-dence of postoperative LUTS(38.60%vs 60.87%,P=0.040).Regression analysis further revealed that pelvic floor exercises was the protective factor for postoperative LUTS(OR=0.215,95%CI:0.091-0.508,P<0.01).Conclusion:Preoperative LUTS and catheter size are significant risk factors for the occurrence of postoperative LUTS following radical prostatectomy.Pelvic floor mus-cle exercise after surgery has a protective effect.Postoperative personalized nursing interventions are necessary for different patients to achieve optimal recovery outcomes.

Predictive efficacy of serum hepcidin,ferritin,and q-Dioxn MRI for upgrading,upstaging,and biochemical recurrence in prostate cancer patients:A comparative study

Abstract:Objective:The aim of this study is to explore the correlation among serum hepcidin,ferritin,and q-Dioxn MRI with upgrading,upstaging and biochemical recurrence in prostate cancer(PCa)patients.Methods:A total of 103 PCa patients di-agnosed by biopsy were selected for this study.All patients underwent q-Dixon MRI prior to biopsy for T2*value measurement.Then serum hepcidin and ferritin were measured before receiving radical prostatectomy.Pathological grading and staging were conducted both preoperatively and postoperatively.The correlations among hepcidin,ferritin,T2*values,and postoperative upgrading,upstaging,biochemical recurrence were subsequently analyzed.Results:The hepcidin level of PCa patients was measured at(123.51±23.03)ng/mL,while the ferritin level was recorded at(239.80±79.59)ng/mL,and the T2* value was(41.07±6.37)ms.A total of 49 and 36 cases were observed with upgrading and upstaging in postoperative pathology,respectively.The median follow-up duration was 28.0 months(6.0-38.0 months),during which biochemical recurrence was observed in 12 cases.For upgrading,hep-cidin and ferritin demonstrated the predictive efficacy,with areas under the ROC curve of 0.777 and 0.642,respectively,whereas T2*values did not show sufficient predictive power.For upstaging,hepcidin,ferritin,and T2*exhibited predictive efficacy,with areas under the ROC curve of 0.806,0.696,and 0.655,respectively.Multivariate Logistic regression analysis indicated that hepcidin served as an independent risk factor for both upgrading(OR 1.055,95%CI 1.027-1.085,P<0.001)and upstaging(OR 1.094,95%CI 1.040-1.152,P<0.001).Cox regression analysis showed that hepcidin(95%CI 1.000-1.052,P=0.049)was a sig-nificant risk factor for predicting biochemical recurrence.Conclusion:Hepcidin could serve as a predictor for pathological upgra-ding,upstaging and biochemical recurrence after radical prostatectomy,which provides a novel potential index for risk stratification and prognostic evaluation of PCa patients.

Expression and function of CDYL-interacting protein MYH9 in mouse testis

Abstract:Objective:To identify the CDYL-interacting proteins in murine testis and investigate the mechanism of CDYL in-volved in spermatogenesis.Methods:CDYL-interacting partners in testis were identified using co-immunoprecipitation coupled with liquid chromatography-tandem mass spectrometry(LC-MS/MS).Expression pattern of CDYL-interacting protein MYH9 was analyzed through immunohistochemistry(IHC),confocal immunofluorescence(IF)and Western blot(WB)in mouse testicular cells.The effect of the Cdyl conditional knockout(CdylcKO)in spermatogenic cell on Myh9 expression was quantified via RT-qPCR,WB and IF imaging in both spermatids and spermatozoa from cauda epididymides.Results:Direct interaction between MYH9 and CDYL was confirmed in murine testis.During spermiogenesis,MYH9 exhibited co-localization with CDYL at the manchette structure,and binding to F-ACTIN,the component of manchette.In cauda epididymal spermatozoa,MYH9 signal concentrated on acrosomal region and con-tinuously distributed along the tail length.Conditional deletion of Cdyl in spermatogenic cell resulted in the transcriptional downregula-tion of Myh9.In spermatids,CdylcKO led to reduced but retained MYH9 localization to the disorganized manchette structure.In sperma-tozoa from CdylcKO mice,abnormalities of MYH9 localization were observed,including attenuation of acrosomal signal and/or partial vanishment/enhancement of tail signal.Conclusion:In murine spermatids,MYH9 protein is localized to the manchette structure,with its expression and subcellular distribution is affected by CDYL protein.CDYL-MYH9 interaction is essential for the spermiogenesis.

Application of predictive nursing to the rehabilitation of patients after endoscopic surgery for prostate under the concept of enhanced recovery after surgery

Abstract:Objective:To explore the efficacy of predictive nursing on the recovery of patients after endoscopic surgery for prostate under the concept of enhanced recovery after surgery(ERAS).Methods:A total of 82 patients with benign prostatic hyper-plasia who underwent surgery from February 2022 to February 2023 were divided into control group(n=41)with traditional care and the observation group(n=41)with predictive care based on the difference in nursing methods.And the clinical data of the two groups were compared.Results:The observation group showed lower incidence rates than the control group for all individual complications(urinary tract infection[2.44%vs 4.88%],hemorrhage[2.44%vs 7.32%],bladder spasm[0%vs 4.88%],and hypostatic pneumonia[0%vs 2.44%]),though none reached statistical significance(P>0.05).However,the total complication rate was sig-nificantly lower in the observation group(4.88%vs 19.51%,P<0.05).Notably,the observation group demonstrated significantly lower IPSS scores(5.49±1.53 vs 10.35±1.77,P<0.05)and shorter hospital stays([5.26±0.38]dvs[9.95±0.84]d,P<0.05).Additionally,nursing satisfaction was markedly higher in the observation group(92.68%vs 78.95%,P<0.05).Conclusion:Theapplication of ERAS-guided anticipatory nursing in postoperative rehabilitation for patients with benign prostatic hy-perplasia can significantly improve quality of life,reduce complication rates,shorten hospital stays,and enhance patient satisfaction.

RNA-binding protein ELAVL1 regulates SOX4 stability and promotes hormone-sensitive prostate cancer proliferation through m6A-dependent regulation

Abstract:Objective:To investigate the expression of RNA binding protein ELAVL1 in prostate cancer(PCa),especially hor-mone-sensitive prostate cancer(HSPC),and its relationship with tumor proliferation.This study further aims to reveal the molecular mechanism by which ELAVL1 promotes HSPC proliferation by stabilizing SOX4 mRNA in an m6A-dependent manner.Methods:The expression of ELAVL1 in PCa tissues and its relationship with prognosis were analyzed in the Cancer Genome Atlas(TCGA)data-base,and the differences in HSPC and hormone-resistant prostate cancer(HRPC)were compared.And its relationship with prognosis were analyzed in the Cancer Genome Atlas(TCGA)database,and the differences in HSPC and hormone-resistant prostate cancer(HRPC)were compared.Western blot was used to detect ELAVL1 protein expression in PCa cell lines.After ELAVL1 knockdown by siRNA,cell proliferation was evaluated using CCK-8 assays,and changes in downstream target genes were detected by RT-qPCR.Tumor xenograft experiments in nude mice were performed to further assess the impact of ELAVL1 on tumor growth.The interaction be-tween ELAVL1 and SOX4 mRNA was verified by RIP-seq.And the mRNA and protein levels of SOX4 after knockdown of ELAVL1 were detected by RT-qPCR and Western blot,respectively.CCK-8 was used to evaluate the effect of SOX4 knockdown on cell prolifer-ation.MeRIP-qPCR was used to detect the m6A modification level of SOX4 and the effect of knocking down METTL3.RNA pull-down experiments verified the interaction between SOX4 RNA fragments and ELAVL1 protein.RNA stability experiments evaluated the effect of ELAVL1 knockdown on SOX4 mRNA stability.Results:The expression of ELAVL1 in PCa cells was higher than that in normal prostate epithelial cells.The prognosis of patients with high expression of ELAVL1 was significantly worse than that of patients with low expression.In the GSE32269 dataset,the expression level of ELAVL1 in HSPC was significantly higher than that in HRPC.After knocking down of ELAVL1 in LNCaP and VCaP cells,CCK-8 experiments showed that the cell proliferation ability was significantly af-fected after knocking down ELAVL1,and overexpressed ELAVL1 promoted the proliferation of HSPC cells.The results of in vivo studies showed that knockdown of ELAVL1 significantly inhibited the tumorigenic capacity of LNCaP cells and resulted in a marked reduction in xenograft tumor mass.The levels of SOX4 mRNA and protein in LNCaP and VCaP cells were significantly higher than those in nor-mal prostate epithelial cells RWPE-1.RIP-qPCR confirmed the interaction between ELAVL1 protein and SOX4 mRNA.After knocking down of ELAVL1,the expression levels of SOX4 mRNA and protein were significantly decreased.After knocking down of SOX4,the proliferation ability of LNCaP and VCaP cells was significantly inhibited.Conclusion:ELAVL1 is highly expressed in HSPC.High expression of ELAVL1 is associated with the proliferation of HSPC.SOX4 is a downstream molecule of ELAVL1 which promotes the proliferation of HSPC.ELAVL1 enhances the stability of SOX4 mRNA through an m6A-dependent mechanism.

Clinical effect of Huotujiji prescription on patients with asthenospermia of spleen-kidney yang-deficiency type

Abstract:Objective:To observe the effectiveness and safety of Huotujiji prescription for patients with asthenospermia of spleen-kid-ney yang-deficiency.Methods:Patients with spleen-kidney-yang deficiency type of asthenospermia were divided into two groups by ran-domized control method.The patients in the control group were treated with L-carnitine oral solution alone.And the patients in experimental group were treated with Huotujiji prescription.Semen volume,sperm concentration,PR,TCM symptom score and spouse pregnancy were compared between the two groups before and after treatment.Results:A total of 115 patients were included in the study,with 55 in the ex-perimental group and 60 in the control group.In the experimental group,the sperm concentration before and after treatment were(160.71±53.7)× 106/mL,(187.19±41.89)× 106/mL,and PR were(20.37±9.42)%,(26.7±6.92)%,respectively.PR+NP were(32.06±8.49)%and(34.89±7.25)%.After the treatment,the sperm motility of both groups significantly improved compared to pre-treatment lev-els(P<0.05).And the experimental group's sperm concentration also showed a significant increase after the treatment(P<0.05).The pregnancy rate of spouses in the control group was 11.67%,which was 29.09%in the experimental group.The pregnancy rate of the spouses in the two groups showed a statistically significant difference(P<0.05).After 12weeks of treatment,the Traditional Chinese Medicine(TCM)symptom scores in the control group did not decrease significantly compared to those before treatment(P>0.05).In contrast,the TCM symptom scores in the experimental group decreased significantly after treatment(P<0.05).Moreover,compared with the control group,the TCM symptom scores of the experimental group decreased significantly after treatment(P<0.05).Conclusion:Huotujiji pre-scription can safely and effectively improve the sperm quality and vitality of patients with spleen-kidney-yang deficiency type of asthenosper-mia,improve the symptoms of patients,and increase the pregnancy rate of their spouses,which is worthy of clinical promotion.

Abstract:Objective:To evaluate the association between erectile dysfunction(ED)and myocardial infarction(MI)using two sample Mendelian randomization.Methods:A Mendelian randomization study was conducted using comprehensive data on ED and MI from extensive genome-wide association data.Using inverse variance weighted analysis for causal relationships,and correct for confounding factors using multivariate Mendelian randomization,the potential mediating effects were evaluated as well.Based on Gene-card data,the genes related to ED and MI were identified.Molecular docking was used to reveal spontaneously bound drug molecules.Results.Our study found that exposure to ED was a risk factor for MI(OR:1.001 0,95%CI:1.000 2-1.001 8,P=0.017 7),which also held true in the validation dataset(OR:1.028 5,95%CI:1.005 0-1.052 6,P=0.017 2).No statistically significant heterogeneity or horizontal pleiotropy was found.The results of reverse Mendelian randomization analysis showed any reverse causal re-lationship between ED and MI.In multivariate Mendelian randomization analysis,after excluding confounding factors(excluding tri-glycerides and high-density lipoprotein),the P-value remained less than 0.05,and the OR ranged from 1.000 1 to 1.000 7,indica-ting that ED was still a risk factor for MI.In the mediation analysis,it was found that the current mediation ratio of smoking to MI was 13.06%.In summary-data-based mendelian randomization analysis,it was found that the gene PTPN11 was a common target gene for MI and ED(OR=0.990,P<0.001).Subsequent molecular docking with sildenafil,clopidogrel,and dapoxetine could spontaneous-ly bind to the PTPN11 gene receptor.Conclusion:There is a causal relationship between ED and MI,with smoking as a potential mediating factor,and the gene PTPN11 being a co-target gene.

Effect of 3D laparoscopic radical prostatectomy on urinary control and sexual function of patients with prostate cancer

Abstract:Objective:To investigate the effects of 3 D laparoscopic radical resection prostatectomy(LRP)on urinary control and sexual function of patients with prostatic cancer.Methods:A total of 268 patients who were treated with LRP in the Fifth People's Hospital of Huai an City from January 2019 to May 2022 were selected and divided into 2 groups according to surgical methods,with 134 cases in each group.The patients in the control group were treated with traditional LRP,and the 3D LRP was used in the observa-tion group.The clinical effects of the two groups were compared.Results:The patients in observation group had less blood loss([135.62±13.58]mL vs[143.18±14.89]mL)and shorter indwelling catheter time([8.26±1.47]d vs[9.78±1.73]d)compared with control group,but the operation time([160.52±10.78]min vs[154.47±10.41]min)was longer than that in the control group(P<0.05).The recovery of sexual function and urinary control in the observation group was better than that in the con-trol group after 3,6 and 12 months of the surgery(P<0.05).After one month of surgery,the scores of ICIQ-SF and 1 h urine pad test were lower than those of the control group.The EPIC-UIN score([72.63±6.85]points vs[67.15±5.09]points)of the observation group was higher than that of the control group(P<0.05).The patients in the observation group had lower level of residual urine volume([60.26±6.63]mL vs[76.89±7.89]mL),higher maximum urine flow rate([7.52±0.46]mL/s vs[6.17±0.43]mL/s)and detrusor pressure([85.19±7.18]mL vs[76.29±6.85]mL)at maximum urine flow rate compared with the control group(P<0.05).There was no difference in the incidence of complications between the observation group and the control group(5.97%vs 8.27%,P>0.05).The recurrence rate of tumor 3 years after operation in the observation group was lower than that in the control group(11.94%vs 29.10%,x2=12.102,P<0.05).Conclusion:3D LRP has obvious advantages in surgical clarity and preci-sion,which reduces the risk of postoperative complications and improves urinary control ability and sexual function of patients.

Research progress of circular RNA in male reproductive disorders

Abstract:Male reproductive disorders have emerged as a global issue.Infertility affects 8%to 12%of couples of childbearing age.The sperm concentration and total sperm count of men have shown a significant downward trend over the past four decades,with a decrease of more than 50%.Male reproductive disorders are related to multiple factors.Circular RNA(circRNA)is a type of non-cod-ing RNA with covalently closed circular structures.It is involved in a variety of biological processes,including gene expression regula-tion,protein function regulation and epigenetic regulation.Studies have shown that there are differences in the expression of circRNA in the testicles and semen between infertile patients and healthy people,suggesting that circRNA is involved in the process of spermato-genesis,and its abnormal expression is associated with male infertility.This review takes the biological functions of circRNA as the starting point and summarizes the research progress of circRNA in male reproductive disorders.CircRNA has the potential to serve as a novel biomarker due to its conservative,special structure and tissue specificity,which provides a new strategy for the clinical diagnosis of male reproductive disorders.

Therapeutic mechanism of Compound Xuanju Capsule on erectile dysfunction

Abstract:Objective:To investigate the pharmacological mechanism of Compound Xuanju Capsule in the treatment of erectile dysfunction(ED)by using network pharmacology and molecular docking technology.Methods:The active ingredients and targets of Compound Xuanju Capsule were screened using Traditional Chinese Medicine Systematic Pharmacology Database and Analysis Platform(TCMSP).TTD,OMIM,DrugBank and GeneCards databases were used to obtain genes related to ED,and the union of the results was taken as the disease genes of ED.The common target of drug and disease was taken as the potential target of Compound Xuanju Capsule in ED,and the drug-disease interaction network was constructed by using Cytoscape software.The protein-protein interaction(PPI)network was constructed by using String database,which was then imported into Cytoscape to identify the key target.Based on the drug-disease intersection genes,GO and KEGG enrichment analyses were performed to predict the relevant signaling pathways and molecular mechanisms of Compound Xuanju Capsule for the treatment of ED.Autodock software was used to perform molecular docking between the active ingredients and the core targets.Results:Forty chemical components of Compound Xuanju Capsule were screened,and 239 predicted targets were obtained.A total of 1 907 ED-related genes were screened,and 97 common targets were identified between Compound Xuanju Capsule and ED,among which the core targets included EGFR,ESR1,HIF1A,PTGS2,and STAT3.The signaling pathways obtained by KEGG enrichment analysis included calcium signaling pathway,HIF-1 signaling pathway,PI3K-Akt signaling pathway,cGMP-PKG signaling pathway,relaxin signaling pathway,Serotonergic synapse signaling pathway.The molecular docking results showed that there were molecular binding sites between the key active ingredients and the core targets with strong binding activity.Conclusion:Compound Xuanju Capsule may treat ED through multi-target pathways such as anti-inflamnmato-ry and improving cellular oxidative stress.

Application of 3D-printed navigation for genital nerve regulation in male with lower urinary tract symptoms

Abstract:Objective:To investigate the efficacy of 3D-printed navigation guided pudendal lead implantation on nervous regula-tion of lower urinary tract symptoms(LUTS)in male patients.Methods:Twenty-eight male patients who underwent perineal nervous regulation treatment for LUTS in Gongli Hospital of Pudong New Area from October 2021 to October 2023 were randomly divided into observation group and control group.The technology assisted with 3D-printed navigation to regulate the genital nerves was used in ob-servation group.And the patients in control group were treated with regulation of the genital nerves by routine puncture.Operation time of puncture,number of surgical punctures,and stimulator debugging time compared between the two groups.The improvement of post-operative symptoms and surgical complications of patients in the observation group were recorded as well.Result:A total of 12 male LUTS patients were included in the observation group,with an average age of 36.5±6.5 years,including 7 cases of frequent micturi-tion,3 cases of perineal pain,and 2 cases of dysuria.Four patients showed no significant improvement in symptoms,including two pa-tients with pain and two cases of frequent micturition who did not undergo secondary surgery.While the other eight patients showed sig-nificant improvement in symptoms.The average time for successful puncture in control group was(21.13±4.53)minutes,which was longer than that of the 3D-printed navigation group([10.32±3.42]min)significantly(P<0.05).The average number of punctures in the ordinary puncture group was 5.62±1.43,which was significantly higher than that in the 3 D-printed navigation group(1.5±0.56).There was no statistically significant difference in the average time for stimulator debugging between the two groups of patients.The conversion rate of the 3D-printed navigation group in the second phase was 66.7%,which was higher than that(37.5%)significantly(P<0.05).Conclusion:3D printing navigation of pudendal nerve electrode wire implantation can improve the accuracy of electrode implantation and the conversion rate to a certain extent,which has the advantages of reducing the difficulty of surgery.

Effect analysis of innovative model on perioperative pain management in prostate cancer patients with hematuria undergoing prostatic artery embolization

Abstract:Objective:To investigate the effect of innovative perioperative pain management on prostate cancer patients with he-maturia undergoing prostatic artery embolization(PAE).Methods:A total of 60 patients undergoing PAE in the Interventional Ther-apy Department of General Hospital of Eastern Theater Command from May 2024 to January 2025 were selected by convenience sam-pling method and randomly divided into the intervention group and the control group,with 30 patients in each group.The control group received traditional pain management of nursing.An innovative perioperative pain management was performed in intervention group in-cluding preoperative"body-mind-pain"holistic assessment and preparation,intraoperative humanistic care and real-time support,post-operative multimodal analgesia and rehabilitation,dynamic monitoring and closed-loop feedback.The pain degree after 6 hours,1 day,3 days and 1 week of the operation,and the quality of life after 1 week of operation,as well as nursing satisfaction at discharge were compared between the two groups.Results:The VAS scores of the intervention group were significantly lower than those of the con-trol group after 6 hours,1 day,3 days and 1 week of operation(P<0.05).One week after the operation,the quality of life in the ob-servation group was higher than that of the control group significantly(P<0.05).The nursing satisfaction of the observation group was significantly higher than that of the control group at discharge(P<0.05).Conclusion:The application of innovative perioperative pain management can alleviate pain of patients with PAE,which improves the quality of life and nursing satisfaction of patients,and is conducive to the rehabilitation of patients.

Varicocele ligation with mobile phone microscope:Report of 5 cases and literature review

Abstract:Objective:To investigate the feasibility of varicocele ligation with mobile phone microscope.Methods:The high-performance mobile phone and mobile phone stand were combined to act as a mobile phone microscope.And the varicocele ligation was performed under the mobile phone microscope.Results:All five patients successfully underwent varicocelectomy under the guidance of a mobile phone microscope.The average operation time was(112.8±52.2)with ranged from 74.0 to 195.0 minutes.Three pa-tients completed the follow-up after the operation with the proportion of improved sperm quality reaching 100.0%(3/3).Conclusion:High-performance mobile phone microscope can be used for varicocele ligation.
