Complex decongestion therapy of lymphedema after breast cancer operation

Abstract:Objective To investigate the clinical effect of complex decongestion therapy(CDT) in treating upper extremity lymphedema after breast cancer operation.Methods A retrospective study was performed on 32 patients with lymphedema after breast cancer operation who were admitted from January 2016 to December 2016.Patients were randomly divided into the control group and the CDT group,with 16 cases ineach group.From the second day after diagnosis of lymphedema,the patients in both groups began to receive the treatment,respectively.Patients in the control group received the partial functional exercises;the others in the CDT group received the CDT treatment combined with functional exercises.The curative effects were compared between the two groups after 3 months.Results There was statistically significant difference between the two groups in the grade of local pain,arm circumference and the grade of edema improved rate(P<0.05).After the treatment,on jaws of death,5 cm above wrist horizontal grain,10 cm under elbow horizontal grain,10 cm above elbow horizontal grain and axillary different diameters,there was statistically significant difference(P<0.05);the difference of diameters of different parts in the CDT group before and after the treatment had statistically significance(P<0.05);5 com above wrist horizontal grain in the control group before and after the treatment had statistically significant difference(P<0.05).No serious adverse reaction occurred in both groups.Conclusion CDT is effective and safe in treatment of upper extremity lymphedema after breast cancer operation.

Hepatic transcatheter arterial chemoembolization combined with stereotactic radia-tion therapy of primary liver cancer

Abstract:Objective To explore the effect of hepatic transcatheter arterial chemoembolization( TACE) combined with ster-eotactic radiation therapy in primary liver cancer. Methods A retrospective study was performed on 56 patients with large liver cancer admitted in No. 463 Hospital of PLA from March 2010 to March 2014. Patients were divided into the combina-tion therapy group(n=29)and the radiation therapy group(n=27). Patients in the combination therapy group were given TACE combined with stereotactic radiation therapy,and the others in the radiation therapy group were given single stereotac-tic radiation therapy. The short-term effect and long-term survival rate before and after the treatment were compared between the two groups. Results The short-term total effective rate of combination therapy group was 58. 6%,which was obviously higher than that of radiation therapy group ( 44. 4%) , the differences between the two groups were statistically significant (P<0. 05). The survival rate in the combination therapy group and radiation therapy group at 6,12,18 months were 82. 8% versus 66. 7%,58. 6% versus 44. 4%,44. 8% versus 29. 6%,the differences between the two groups were statisti-cally significant(P<0. 05). There were slightly adverse reactions,and all the cases recovered after conventional treatments. Conclusion TACE combined with stereotactic radiation therapy can improve the short-term effect and long-term survival rate of patients with primary liver cancer,and has higher safety.

External application of Chinese medicine combined with lymphatic drainage on upper limb edema of postoperative breast cancer

Abstract:Objective To investigate the clinical effect of external application of Chinese medicine combined with lymphatic drainage on postoperative upper limb edema of breast cancer.Methods A retrospective study was performed on 80 cases of postoperative upper limb edema who were admitted from January 2016 to December 2016.The patients were randomly divided into the combination group and the conventional group,with 40 cases in each group.The patients in the convention group were treated with lymphatic drainage technique combined with local limb functional exercise;the patients in the combination group were treated with external application of Chinese medicine combined with lymphatic drainage and local limb function.After 3 weeks of treatment,the adverse effects were evaluated by measuring the difference in the arms,the degree of pain,and the quality of life of the patients.Results After 3 weeks of treatment,the difference between left and right arm of the combination group was significantly lower than that of the conventional group(P<0.05);the VAS score of the combination group was significantly lower than that of the conventional group after treatment(P<0.05);the score of quality of life in the combination group was significantly higher than that in the conventional group(P<0.05);the total effective rate of the combination group(95.00%) was significantly higher than that of the conventional group(82.5%),all the differences had statistical significance(P<0.05).Conclusion The clinical effect of traditional Chinese medicine combined with lymphatic drainage on the treatment of postoperative upper limb edema of breast cancer is remarkable and deserves clinical promotion.

Clinical analysis of elderly patients with atrial fibrillation clinical characteristics and anticoagulation therapy:965 cases report

Abstract:Objective To investigate the characteristics of elderly patients with atrial fibrillation( AF) and analysis of antico-agulant therapy with existing problems. Methods A retrospective study was performed on 965 cases of elderly patients with AF received from January 2010 to October 2015. The thrombosis and bleeding risks were assessed,anticoagulant medications and adverse reactions were analyzed. Results There were more complications in elderly patients with AF. The thrombosis risk classification(CHA2DS2-VASc)scores and the anticoagulant bleeding scores(HASBLED)were higher. All patients′(CHA2DS2-VASc)score≥2,anticoagulant therapy should be performed. The HASBLED score≥3 in 522 cases(54. 1%), and there were high proportion of the patients with higher risk in anticoagulant bleeding thrombosis. At present,the clinical anticoagulant therapy ratio in elderly patients with AF is still low,the fluctuation since 2010 to 2015 was around 40% with-out apparent increases. There were statistically differences in different departments,the proportions of anticoagulant theropy in cardiology and the cadre ward were higer. Anticoagulant therapy changes were following guideline,Dabigatran and Rivar-oxaban were applied gradually. Conclusion The education of AF needs to be further popularized in order to improved un-derstanding of AF in doctors and patients,thus increase proportion of anticoagulant therapy in patients with AF. The clinical experience of new oral anticoagulant drugs in elderly patients is needed to be constantly accumulated and the dose is still needed to be further studied.
