胸腰椎结核保守与手术治疗的合理选择

摘要：Spinal cord injury, progressive kyphosis deformity and tuberculosis focus with sequstrum or abscess are severe complications of spinal tuberculosis. In order to select the appropriate treatment, the following factors should be taken into consideration: the cause of spinal cord injury, the severity of spinal cord injury and the adverse prognostic factors of the spinal cord injury; the prediction of the kyphosis deformity angle and its development;the prognosis of the tuberculosis lesions after anti-tuberculosis drugs treatment. Patients indicated for conservative treatment must meet all the following 3 criteria: ( 1 ) The spinal cord dysfunction with Frankel grade > C, with pus as the compressive factor and with no indexes for adverse prognostic factors of the spinal cord injury; ( 2 ) The kyphosis deformity angle < 30°, with no “high risk factors” of progressive kyphosis deformity; ( 3 ) The symptoms of patients are significantly improved after anti-tuberculosis drugs therapy and the local focus is also improved gradually indicated by radiology imaging. If patients fail to reach all these criteria, then surgery should be considered. However, we should deliberate whether we should perform the surgery when there are adverse prognostic factors of the spinal cord injury.
