Abstract:Objective: To explore the correlation between monocyte to high-density lipoprotein cholesterol (MHR) and apolipoprotein B/apolipoprotein A1 (ApoB/ApoA1) with the severity of coronary artery stenosis in patients with acute coronary syndrome (ACS). Methods: A total of 237 ACS patients who were hospitalized for chest pain and underwent coronary angiography from June 2022 to March 2023 were selected. According to the tertiles of Gensini score, the patients were divided into three groups: mild stenosis group (<12.3 points, 79 cases), moderate stenosis group (12.3–44.0 points, 82 cases), and severe stenosis group (>44.0 points, 76 cases). The differences in MHR and ApoB/ApoA1 among the groups were compared. Multivariate logistic regression analysis was used to identify factors affecting the severity of coronary artery stenosis in ACS patients. Receiver operating characteristic (ROC) curves were used to evaluate the predictive value of MHR and ApoB/ApoA1 for the severity of coronary artery stenosis. Spearman correlation analysis was performed to assess the correlation between MHR, ApoB/ApoA1, and Gensini scores. Results: MHR and ApoB/ApoA1 were higher in the moderate and severe stenosis groups compared to the mild stenosis group, with statistically significant differences (P < 0.05). In the severe stenosis group, MHR and ApoB/ApoA1 were significantly higher than in the moderate stenosis group (P < 0.001). MHR and ApoB/ApoA1 showed a positive correlation with Gensini scores (r = 0.376, P < 0.05; r = 0.343, P < 0.05). Multivariate logistic regression analysis found that MHR and ApoB were risk factors for severe coronary artery stenosis in ACS patients. The area under the curve (AUC) for predicting severe coronary artery stenosis using MHR combined with ApoB/ApoA1 was 0.743 [95% CI (0.678, 0.809), P < 0.05], with a sensitivity of 80.3% and a specificity of 56.5%. Conclusion: MHR and ApoB/ApoA1 are positively correlated with the severity of coronary artery stenosis in ACS patients, and their combination can enhance the diagnostic value for the condition.
