Summary of the Best Evidence for Out-of-hospital Follow-up in Patients with Chronic Heart Failure

Abstract:Objective: To summarize the best evidence for outpatient follow-up of patients with chronic heart failure. Methods: Using the '6S' evidence model, a bidirectional search in Chinese and English was conducted to systematically retrieve guidelines, consensus statements, best clinical practices, recommended practices, and systematic reviews related to outpatient follow-up of patients with chronic heart failure. The search period was from the establishment of the database to May 2023. The quality of included literature was ensured using relevant literature evaluation tools, and evidence was extracted from the literature that met the quality requirements. Results: A total of 12 articles were included, including 4 guidelines, 2 clinical decision-making documents, 1 systematic review, 4 expert consensuses, and 1 randomized controlled trial. Finally, 31 pieces of best evidence were summarized in seven aspects: follow-up team, follow-up timing, follow-up methods, follow-up assessment, follow-up content, follow-up records, and health education during follow-up. Conclusion: This study can provide theoretical guidance for the development of outpatient follow-up service plans for patients with chronic heart failure.
