The Distribution of TCM Syndrome Types in Convalescent Patients with Cerebral Infarction and Its Relationship with Platelet Activation Status and Parameters

Abstract:Objective:To explore the relationship between traditional Chinese medicine(TCM)syndrome type distribution and platelet activation status and parameters in convalescent patients with cerebral infarction.Methods:A total of 80 patients in the convalescent stage of cerebral infarction were selected.According to the four diagnostic information of patients,a systematic cluster analysis of TCM syndrome types was conducted,along with a corresponding analysis of TCM syndrome types and cerebral infarction subtypes.The clinical data,platelet activation status and parameters of patients with different TCM syndromes were compared.Logistic regression was used to analyze the correlation between TCM syndrome types and platelet activation status as well as parameters.Results:The convalescence of cerebral infarction was divided into five types:liver-Yang hyperactivity syndrome,wind-phlegm-stasis syndrome,phlegm-heat fu-organs syndrome,Yin-deficiency wind-movement syndrome,and Qi-blood stasis syndrome.Among them,wind-phlegm-stasis syndrome was most commonly associated with atherosclerotic cerebral infarction,and Qi-deficiency blood-stasis syndrome was most commonly associated with watershed cerebral infarction.There were significant differences in platelet-activating factor(PAF),platelet aggregation rate(PAG),maximum aggregation rate(MAR),platelet membrane glycoprotein(GMP-140),platelet glycoprotein GP Ⅱ b/Ⅲ a complex fibrinogen receptor(PAC-1),P-selectin glycoprotein(CD62P),lysosomal protein(CD63),platelet count(PLT),plateletcrit(PCT),mean platelet volume(MPV),and platelet distribution width(PDW)among different syndrome types(P<0.05).Further Logistic regression analysis showed that GMP-140,PAC-1,and CD62Pwere positively correlated with liver-Yang hyperactivity syndrome;PAC-1,CD62P,and MPV were positively correlated with wind-phlegm and blood stasis syndrome;CD62P,CD63,and MPV were positively correlated with phlegm-heat fu-organs syndrome;and PAG,GMP-140,PAC-1,and CD62P were negatively correlated with Yin deficiency wind movement syndrome.PAG,PAC-1,CD62P,CD63,and MPV were negatively correlated with Qi deficiency and blood stasis syndrome.Conclusion:Cerebral infarction convalescent patients can be categorized into the following syndromes:liver-Yang hyperactivity syndrome,wind-phlegm-stasis syndrome,phlegm-heat fu-organs syndrome,Yin deficiency wind movement syndrome,and Qi deficiency blood stasis syndrome.Among them,patients with wind-phlegm and blood stasis syndrome are more likely to develop atherosclerotic thrombotic cerebral infarction,whereas those with Qi deficiency and blood stasis syndrome are more prone to watershed cerebral infarction.PAG,GMP-140,PAC-1,CD62P,CD63,and MPV were correlated with the distribution of TCM syndrome types,which can be used to accurately assess patients'conditions and enable targeted treatment.
