The Clinical Efficacy and Safety of Drug-coated Balloons in the Treatment of New-onset Large Vessel Disease in Non-ST-segment Elevation Acute Coronary Syndrome

Abstract:Objective: To explore the clinical efficacy and safety of drug-coated balloon (DCB) in treating new large vessel lesions in patients with non-ST-segment elevation acute coronary syndrome (NSTE-ACS). Methods: A total of 186 patients with NSTE-ACS admitted to Shanxi Medical University Affiliated Jincheng Da Hospital from January 2020 to December 2022 were selected and divided into a DCB treatment group (DCB group, 58 cases) and a drug-eluting stent (DES) treatment group (DES group, 128 cases) based on their actual surgical conditions. The follow-up period was 9 months, during which the incidence of major adverse cardiovascular events (MACE) was recorded. After 9 months, coronary angiography and echocardiography were performed to observe late lumen loss (LLL), changes in cardiac systolic and diastolic function in both groups. Results: At 9-month follow-up, the LLL of the target lesion in the DCB group was -0.02 (-0.28, 0.23) mm, while that in the DES group was -0.21 (-0.42, -0.06) mm. There was a statistically significant difference between the two groups (P < 0.05). There were no statistically significant differences in left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), and the ratio of early diastolic flow velocity (E peak) to late diastolic flow velocity (A peak) (E/A) before and 9 months after treatment between the two groups (P > 0.05). During the 9-month follow-up, the MACE incidence rate was 25.9% in the DCB group and 23.4% in the DES group, with no statistically significant difference between the two groups (P > 0.05). Conclusion: DCB alone shows better late lumen loss (LLL) outcomes than DES for the target lesions in NSTE-ACS, with better safety.
