Abstract:Objective: To observe the effect of Yugeng Tongyu Decoction combined with aspirin and clopidogrel on serum endothelial cell micro-particles (EMPs), thromboxane B2 (TXB2), and endothelin (ET) levels in patients with acute myocardial infarction (AMI). Methods: A total of 114 AMI patients treated at Hebei No.7 People's Hospital from January 2022 to December 2023 were selected and randomly divided into a Western medicine group and a traditional Chinese medicine group, with 57 patients in each group. All patients received percutaneous coronary intervention (PCI) and routine treatment. The Western medicine group was treated with aspirin and clopidogrel bisulfate, while the traditional Chinese medicine group received Yugeng Tongyu Decoction in addition to the Western medicine treatment. Both groups were treated for 2 weeks. The clinical efficacy, TCM syndrome scores, microcirculation indicators, myocardial injury indicators, serum EMPs, TXB2, ET levels, and safety indicators were compared between the two groups. Results: After 2 weeks of treatment, the total effective rate of the traditional Chinese medicine group was higher than that of the Western medicine group, with a statistically significant difference (89.47% vs. 73.68%, P < 0.05). After 2 weeks of treatment, the coronary flow reserve index (CFR) increased in both groups, with the traditional Chinese medicine group showing a higher value than the Western medicine group. In both groups, the TCM syndrome scores, microcirculation resistance index (IMR), serum creatine kinase isoenzyme (CK-MB), troponin I (cTnI), serum EMPs, TXB2, and ET levels decreased, with the traditional Chinese medicine group showing lower values than the Western medicine group, and the differences were statistically significant (P < 0.05). During the treatment period, there was no statistically significant difference in the incidence of adverse reactions between the traditional Chinese medicine group and the Western medicine group (15.79% vs. 12.28%, P > 0.05). Conclusion: Adding Yugeng Tongyu Decoction to PCI combined with aspirin and clopidogrel can effectively relieve clinical symptoms in patients with AMI, reduce myocardial injury, improve microcirculation and vascular endothelial function, thereby enhancing the therapeutic effect.
