Abstract:Objective: To analyze the diagnostic value of serum D-dimer (D-D) and soluble vascular cell adhesion molecule-1 (sVCAM-1) in predicting the occurrence of venous thromboembolism (VTE) in elderly inpatients. Methods: A retrospective analysis was conducted on the clinical data of 120 elderly inpatients from April 2020 to April 2022. The patients were divided into a VTE group (32 cases) and a non-VTE group (88 cases) based on whether they developed VTE during hospitalization. The general information and levels of serum D-D and sVCAM-1 were compared between the two groups. Multivariate analysis was used to identify risk factors for VTE in inpatients, and the receiver operating characteristic (ROC) curve was used to evaluate the predictive value of serum D-D and sVCAM-1 for VTE in elderly inpatients. Results: The proportions of patients aged ≥80 years, hospitalized for ≥5 days, with cardiovascular disease, alcohol history, abnormal coagulation tests, respiratory failure, liver and kidney diseases, as well as the levels of D-D and sVCAM-1 were higher in the VTE group than in the non-VTE group, with statistically significant differences (P < 0.05). Multivariate logistic regression analysis showed that age ≥80 years, hospitalization duration ≥5 days, cardiovascular disease, alcohol history, abnormal coagulation tests, respiratory failure, liver and kidney diseases, high levels of D-D and sVCAM-1 were all risk factors for VTE in elderly inpatients (P < 0.05). ROC analysis confirmed that the areas under the ROC curves for D-D, sVCAM-1 alone, and their combination in predicting VTE in elderly inpatients were 0.869, 0.866, and 0.900, respectively. Conclusion: Serum D-D and sVCAM-1 are highly expressed in elderly inpatients with VTE and can serve as reference indicators for monitoring VTE occurrence in elderly inpatients, with high diagnostic value when used in combination.
