Literature Review on the Objectification of Traditional Chinese Medicine Syndromes in Heart Failure

Abstract:Objective:To review the research literatures and achievements on the objectification of traditional Chinese medicine syndromes of heart failure,to provide references for the research and application of syndromes of heart failure.Methods:The database was searched for literature related the objectification of traditional Chinese medicine syndromes in heart failure from the establishment of the database to 30 June,2023,and the results were analysed to summarise the research findings on the objectification of heart failure syndromes.Results:A total of 219 literatures were included,with 32 779 patients,involving 218 objective indicators,and the total frequency of the indicators was 1 015 times.The types of diseases included chronic heart failure(130 articles,59.36%),coronary heart failure(26 articles,11.87%),heart failure with preserved ejection fraction(9 articles,4.11%).Objective research was mainly conducted based on syndrome type differentiation(202 articles,92.24%)or syndrome element differentiation(17 articles,7.76%).The 218 objectified indicators by diagnostic methods included clinical laboratory indicators(97 items,44.50%),echocardiographic indicators(43 items,19.72%),molecular biology indicators(24 items,11.01%),and other auxiliary examination indicators(54 items,24.77%).Multiple indicators showed different syndromes of heart failure.Left ventricular ejection fraction(LVEF),brain natriuretic peptide(BNP),and N-terminal pro-brain natriuretic peptide(NT-proBNP)were positively correlated with the degrees of Qi deficiency and Yang deficiency in patients with heart failure.The left ventricular end-diastolic diameter(LVEDD)showed certain reference value in the diagnosis of water retention syndrome and Yang deficiency syndrome.The changes in cardiac diastolic function indicators such as peak blood flow in the early diastole(E)/peak blood flow in the late diastole(A)related Yin deficiency and Qi deficiency.Conclusion:The overall level of objective study on the syndromes of heart failure in traditional Chinese medicine showed continuous improvement,which was reflected in the refinement of clinical classification of heart failure,the continuous expansion of research indicators,and the comprehensive research techniques.The current research showed problems such as inconsistent diagnosis of heart failure,complex indicators,insufficient promotion,and application.
