Abstract:Objective: To investigate the risk factors for postoperative delirium in patients with acute ST-segment elevation myocardial infarction (STEMI) after percutaneous coronary intervention (PCI), construct a nomogram model, and validate it. Methods: A total of 400 patients with STEMI who underwent PCI at Weihai Central Hospital from September 2021 to August 2023 were selected as the study subjects, and were divided into a modeling group (September 2021 to December 2022, 300 cases) and a validation group (January 2023 to August 2023, 100 cases) based on admission time. Data including general information, comorbidities upon admission, laboratory indicators, anxiety scores, pain scores, and delirium assessments were collected. The influencing factors of post-PCI delirium were analyzed, and a nomogram model was established and validated. Results: Multivariate logistic regression analysis showed that age, State-Trait Anxiety Inventory (STAI) score, living alone, alcohol consumption, diabetes, operation time, hypoxemia, and pain were risk factors for postoperative delirium. Based on these factors, the constructed nomogram model had an area under the receiver operating characteristic curve (AUC) of 0.848 [95% CI (0.795, 0.902)] in the modeling group and 0.797 [95% CI (0.695, 0.899)] in the validation group. The calibration curves of the modeling and validation groups showed good consistency between predicted and actual probabilities, and the decision curve analysis (DCA) showed a satisfactory clinical net benefit. Conclusion: Age, STAI score, living alone, alcohol consumption, diabetes, operation duration, hypoxemia, and pain are risk factors for post-PCI delirium in patients with STEMI. The nomogram risk assessment model constructed based on these factors has good predictive ability and consistency.
