The Correlation between Serum NHR,BAFF,POSTN,and Myocardial Injury and MACE in Patients with H-type Hypertension Complicated with AMI

Abstract:Objective: To analyze the correlation between serum neutrophil-to-high-density lipoprotein cholesterol ratio (NHR), B-cell activating factor (BAFF), and periostin (POSTN) levels and myocardial injury and major adverse cardiovascular events (MACE) in patients with H-type hypertension (HHT) combined with acute myocardial infarction (AMI). Methods: A total of 137 patients with HHT combined with AMI were selected as the HHT+AMI group from February 2021 to August 2022. According to whether MACE occurred after 12-month follow-up, they were divided into the MACE group (30 cases) and the non-MACE group (107 cases). Another 80 patients with HHT without AMI were selected as the HHT group, and 140 healthy individuals were selected as the healthy group. The serum levels of BAFF, POSTN, and NHR were compared among the groups. Pearson correlation analysis was used to evaluate the correlation, multivariate logistic regression analysis was used to identify factors influencing MACE in patients with HHT combined with AMI, and receiver operating characteristic (ROC) curves were used to assess the diagnostic value of serum NHR, BAFF, and POSTN levels for MACE in these patients. Results: The serum levels of NHR, BAFF, POSTN, creatine kinase isoenzyme (CK-MB), cardiac troponin I (cTnI), and N-terminal pro-B-type natriuretic peptide (NT-proBNP) increased sequentially in the healthy group, HHT group, and HHT+AMI group (P < 0.05). The serum levels of NHR, BAFF, and POSTN were higher in the MACE group than in the non-MACE group (P < 0.05). The left ventricular ejection fraction and cardiac index were lower in the MACE group, while the left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), CK-MB, cTnI, and NT-proBNP levels were higher in the MACE group (P < 0.05). Serum NHR, BAFF, and POSTN levels were positively correlated with CK-MB, cTnI, and NT-proBNP levels (P < 0.05). NHR, BAFF, and POSTN were independent risk factors for MACE in patients with HHT combined with AMI (P < 0.05). The area under the ROC curve (AUC) for the combined diagnosis of NHR, BAFF, and POSTN was 0.929, which was better than individual diagnoses (ZNHR-Combined = 3.237, P = 0.001; ZBAFF-Combined = 0.778, P = 0.047; ZPOSTN-Combined = 2.616, P = 0.009). The sensitivity and specificity of the combined detection were 90.00% and 86.92%, respectively. Conclusion: Patients with HHT combined with AMI have higher serum levels of NHR, BAFF, and POSTN, which are closely related to myocardial injury and the occurrence of MACE.
